56. Reconsidering Fresh Frozen Plasma Availability to Reduce Blood Product Waste During Massive Transfusion Events in Trauma

Background Within component therapy of massive transfusion protocol (MTP) in trauma, thawed plasma is particularly susceptible to expiring without use given its short 5-day shelf life. Optimizing the number of thawed products without compromising safety is important for hospital resource management. The goal is to examine thawed plasma utilization rates in trauma MTP events and optimize the MTP cooler content at our Level I trauma center.Methods Trauma MTP activations from 01/2019 to 12/2022 were retrospectively reviewed. During the study period, blood products were distributed in a 12:12:1 ratio of packed red blood cells (pRBC): plasma: platelets per cooler, with up to 4 additional units of low-titer, group O whole blood (LTOWB) available. The primary measure was percent return of unused, thawed plasma.Results There were 367 trauma MTP activations with a median (IQR) activation call-to-first cooler delivery time of 8 (6-10) minutes. 73.0% of thawed plasma was returned to the blood bank unused. In one third of MTP activations, all dispensed plasma was returned. The majority (74.1%) of patients required 6 or fewer units of plasma. In 81.5% of activations, 10 or fewer units of plasma and 10 or fewer units of pRBC were used.Discussion The majority of trauma MTP requirements may be accommodated with a reduced cooler content of 6 units pRBC, 6 units plasma, and 1 pheresis platelets, buffered by up to 4 units LTOWB (approximates 4 units of pRBC/4 units plasma), in conjunction with a sub-10min cooler delivery time. Follow-up longitudinal studies are needed.

- Abstract
We describe the case of an elderly woman who acquired a Mycobacterium marinum infection following skin exposure to the bacteria through a small wound on her right ring finger, obtained while preparing fish. The resultant sporotrichoid nodules of the right hand and the distal forearm, refractory to the initial therapy with doxycycline and rifampicin, were successfully treated with oral regimen of clarithromycin.

B-cell chronic lymphocytic leukemia (B-CLL) is a low-grade lymphoproliferative disease. Cutaneous involvement of B-CLL is limited and, in most cases, it represents non-specific manifestations related to an impaired immune system. Leukemic skin infiltrates (leukemia cutis) occur in 4–20% of patients. Herein we report the case of a 65-year-old woman with B-CLL presenting with papular, nodular, and plaque skin infiltrates affecting the nose, mimicking granulomatous rosacea. We discuss several aspects of rare cutaneous manifestations of B-CLL involving the face.

- Abstract
Arrhythmic storm is a clinical emergency associated with high mortality, which requires multi-disciplinary management. Reprogramming of the implantable cardiac defibrillator (ICD) aimed at reducing shocks, adrenergic blockade using beta-blockers, sedation/anxiolysis, and blockade of the stellate ganglion represent the first simple and effective manoeuvres, but further suppression of arrhythmias with antiarrhythmics is often required. A low-risk patient (e.g. monomorphic ventricular tachycardia, functioning ICD, and haemodynamically stable) should be managed with a beta-blocker (possibly non-selective) plus amiodarone, in addition to sedation with a benzodiazepine or dexmedetomidine; in patients at greater risk (high burden and haemodynamic instability), autonomic modulation with blockade of the stellate ganglion and the addition of a second antiarrhythmic (lidocaine) should be considered. In patients refractory to these measures, with advanced heart failure, general anaesthesia with intubation and the establishment of a haemodynamic circulatory support should be considered. Ablation, performed early, appears to be superior in terms of mortality and reduction of future shocks compared with titration of antiarrhythmics.

83. On the Low-Cost Production of Tissue-Mimicking Skin Phantoms Up to 40 GHz

- Abstract
Objectives: In this paper, a simple methodology for making skin phantoms is presented. In addition, the first millimeter (mm)-wave images in scenarios including the skin phantoms are shown. Technology or Method: Two mixtures based on the use of simple and inexpensive materials are produced and dielectrically characterized in the [0.5--40] GHz frequency range. Cole-Cole parameters are derived by the least-squares method. An inexpensive polystyrene mold to obtain morphologically compatible skin thicknesses is presented. Finally, these phantoms are used for experimental mm-wave imaging tests in two scenarios, with targets with different size and reflectivity in air and in an oil-based phantom. Results: The dielectric characteristics of the produced skin phantoms are compared with those of human skin, showing excellent agreement over the entire spectrum. Realistic and uniform thicknesses from 1 to 2.5 mm were obtained. The experimental images show the possibility of detecting targets with different reflectivity below the skin at mm- waves. Conclusions: two phantoms based on safe and low-cost materials emulating dielectric characteristics (up to 40 GHz) and morphological characteristics of the skin were proposed. One of these phantoms has been used for a number of mm-wave imaging system tests in different scenarios, and the potential of mm-waves to detect non-superficial targets even in the presence of skin is demonstrated. Clinical or Biological Impact: the results presented in this paper provide a replicable methodology for skin phantom realization and show the potential feasibility of mm-wave imaging for early detection of breast cancer.

They produce many of the chemical elements necessary for life to exist and their remnants-neutron stars and black holes-are interesting astrophysical objects in their own right. However, despite millennia of observations and almost a century of astrophysical study, the explosion mechanism of core-collapse supernovae is not yet well understood. Hyper-Kamiokande is a next-generation neutrino detector that will be able to observe the neutrino flux from the next galactic core-collapse supernova in unprecedented detail. We focus on the first 500 ms of the neutrino burst, corresponding to the accretion phase, and use a newly-developed, high-precision supernova event generator to simulate Hyper-Kamiokandeʼs response to five different supernova models. We show that Hyper-Kamiokande will be able to distinguish between these models with high accuracy for a supernova at a distance of up to 100 kpc. Once the next galactic supernova happens, this ability will be a powerful tool for guiding simulations toward a precise reproduction of the explosion mechanism observed in nature.

- Abstract
Floating-point division involves the computation of the ratio (1 + Mx)/(1 + My), where Mx and My represents the mantissas of the input values. In this paper, we propose a new method for approximating this operation using a linear function of Mx, with coefficients that depend on My. The coefficients are calculated to minimize the Mean Relative Error Distance (MRED) of the approximation. To this end, the range of My is partitioned in N sub-intervals where the minimization of MRED is formulated as a linear programming problem, whose solution gives optimal coefficient values. The hardware implementation requires a small lookup table, two multipliers and an adder. An aggressive coefficients quantization is exploited to further optimize the design. - Abstract
Aortic intramural hematoma (IMH) is characterized by an aortic wall hematoma without intimal flap and it is a variant of acute aortic syndromes (AAS). This entity may represent 10%-25% of the AAS involving the ascending aorta and aortic arch (Stanford Type A) in 10%-30% of cases and the descending thoracic aorta (Stanford Type B) in 60%-70% of cases. IMH impairs the aortic wall and may progress to either inward disruption of the intima, which finally induces typical dissection or outward rupture of the aorta. The literature describes some clinical reports where Type A aortic dissection mimics a pulmonary embolism but is not described as a case provoked by IMH with outward rupture of the aorta. - Abstract
Wireless technologies spanning at the higher bands of microwave range (i.e., Ka-band) are gaining increasing importance in everyday life. Most recent wireless power transfer (WPT) applications work in the microwave and millimeter wave (mmW) frequencies, overlapping with the new frequency bands of the fifth-generation (5G) mobile technology. The spread of such novel electromagnetic (EM) sources raised the need to investigate their possible effects on population’s health. Numerical dosimetry is fundamental to assess the exposure of the human body. In the range 24–28 GHz, i.e., the low-band spectrum of the mmW, and bridge between microwaves and mmW, there is poor consensus on the best strategy to model the human body. This article proposes a comparison between the two numerical methodologies typically adopted for this frequency range: the use of multilayer planar slabs and realistic anthropomorphic numerical models. The aim is to highlight the advantages and limits of each method, by comparing EM exposure results obtained on the Virtual Population (ViP) model Duke with several multilayer planar slabs. The differences between the two methods are non-negligible, suggesting the need of further studies and the necessity of improving both modeling approaches, depending on the frequency of work and the investigated application.

- Abstract
Since its outbreak in early 2020, the COVID-19 pandemic has diverted resources from non-urgent and elective procedures, leading to diagnosis and treatment delays, with an increased number of neoplasms at advanced stages worldwide. The aims of this study were to quantify the reduction in surgical activity for indeterminate thyroid nodules during the COVID-19 pandemic; and to evaluate whether delays in surgery led to an increased occurrence of aggressive tumours.
