Abstract

In this paper we describe our design, implementation, and initial results of a prototype connected-phonem-based speech recognition system on the Cell Broadband Engine[TM] (Cell/B.E.) processor. Automated speech recognition decodes speech samples into plaintext (other representations are possible) and must process samples at real-time rates. Fortunately, the computational tasks involved in this pipeline are highly data parallel and can receive significant hardware acceleration from vector-streaming architectures such as the Cell/B.E. Architecture. Identifying and exploiting these parallelism opportunities is challenging and critical to improving system performance. From our initial performance timings, we observed that a single Cell/B.E. processor can recognize speech from thousands of simultaneous voice channels in real time--a channel density that is orders of magnitude greater than the capacity of existing software speech recognizers based on CPUs (central processing units). This result emphasizes the potential for Cell/B.E. processor-based speech recognition and will likely lead to the development of production speech systems using Cell/B.E. processor clusters.

Abstract

A circularly and a linearly polarized XeCl laser beam was used to study stimulated rotational and vibrational Raman scattering in H2. The effect of focusing geometry on the threshold for rotational and vibrational scattering was also analyzed. The presence of a circularly polarized pump beam was shown to allow a high-angle focusing geometry to get rotational Raman scattering alone, while low-angle focusing geometry provides the vibrational counterpart.

Abstract

In this study, the performance of two types of source apportionment models was evaluated by assessing the results provided by 40 different groups in the framework of an intercomparison organised by FAIRMODE WG3 (Forum for air quality modelling in Europe, Working Group 3). The evaluation was based on two performance indicators: z-scores and the root mean square error weighted by the reference uncertainty (RMSEu), with pre-established acceptability criteria. By involving models based on completely different and independent input data, such as receptor models (RMs) and chemical transport models (CTMs), the intercomparison provided a unique opportunity for their cross-validation. In addition, comparing the CTM chemical profiles with those measured directly at the source contributed to corroborate the consistency of the tested model results. The most commonly used RM was the US EPA- PMF version 5. RMs showed very good performance for the overall dataset (91% of z-scores accepted) while more difficulties were observed with the source contribution time series (72% of RMSEu accepted). Industrial activities proved to be the most difficult sources to be quantified by RMs, with high variability in the estimated contributions. In the CTMs, the sum of computed source contributions was lower than the measured gravimetric PM10 mass concentrations. The performance tests pointed out the differences between the two CTM approaches used for source apportionment in this study: brute force (or emission reduction impact) and tagged species methods. The sources meeting the z-score and RMSEu acceptability criteria tests were 50% and 86%, respectively. The CTM source contributions to PM10 were in the majority of cases lower than the RM averages for the corresponding source. The CTMs and RMs source contributions for the overall dataset were more comparable (83% of the z-scores accepted) than their time series (successful RMSEu in the range 25% - 34%). Abstract

Background Frailty assessment has acquired an increasing importance in recent years and it has been demonstrated that this vulnerable profile predisposes elderly patients to a worse outcome after surgery. Therefore, it becomes paramount to perform an accurate stratification of surgical risk in elderly undergoing emergency surgery. Study design 1024 patients older than 65 years who required urgent surgical procedures were prospectively recruited from 38 Italian centers participating to the multicentric FRAILESEL (Frailty and Emergency Surgery in the Elderly) study, between December 2016 and May 2017. A univariate analysis was carried out, with the purpose of developing a frailty index in emergency surgery called "EmSFI". Receiver operating characteristic curve analysis was then performed to test the accuracy of our predictive score. Results 784 elderly patients were consecutively enrolled, constituting the development set and results were validated considering further 240 consecutive patients undergoing colorectal surgical procedures. A logistic regression analysis was performed identifying different EmSFI risk classes. The model exhibited good accuracy as regard to mortality for both the development set (AUC = 0.731 [95% CI 0.654-0.772]; HL test chi(2) = 6.780; p = 0.238) and the validation set (AUC = 0.762 [95% CI 0.682-0.842]; HL test chi(2) = 7.238; p = 0.299). As concern morbidity, our model showed a moderate accuracy in the development group, whereas a poor discrimination ability was observed in the validation cohort. Conclusions The validated EmSFI represents a reliable and time-sparing tool, despite its discriminative value decreased regarding complications. Thus, further studies are needed to investigate specifically surgical settings, validating the EmSFI prognostic role in assessing the procedure-related morbidity risk.

74. Verapamil: prevention and treatment of cardio-renal syndromes in diabetic hypertensive patients?

Abstract

Patients with diabetes mellitus (DM) often present other chronic comorbidities including arterial hypertension (AH), chronic kidney disease (CKD), ischemic heart disease (IHD) and heart failure with preserved ejection fraction (HFpEF). The frequent association of the latter conditions is considered part of the spectrum of cardio-renal syndromes (CRS), a group of disorders of the heart and kidneys whereby acute or chronic dysfunction in one organ may induce acute or chronic dysfunction of the other. Verapamil is a non-dihydropyridine calcium channel blocker (CCB) widely used in the treatment of hypertension, chronic stable angina, secondary prevention of reinfarction, paroxysmal supra-ventricular tachycardia and for rate control in atrial fibrillation/flutter. In addition to its antihypertensive and anti-ischemic actions verapamil exerts favorable effects also on glycemic control, proteinuric diabetic nephropathy, left ventricular diastolic dysfunction and sympathetic nervous system overactivity which may potentially benefit patients with DM and CRS. In this narrative review, we summarize the current evidence on the potential role of verapamil in the prevention and treatment of CRS in diabetic hypertensive patients. [ABSTRACT FROM AUTHOR]

80. CHEST TUBE REMOVAL: COMPARISON OF TWO PAIN MANAGEMENT IN PEDIATRIC CARDIAC SURGERY

Abstract

Background and aim: Cardiac surgery requires the insertion of drainage into the pleural or mediastinal spaces. These drains are generally removed within 24 to 48 hours after surgery. Removal of the cardiac/thoracic drainage/tube (Chest Tube Removal: CTR) is associated with moderate to severe pain. During CTR, separating the thoracic tube from tissue causes pain in patients because,while the drainage isin place, the endothelium covering the thoracic cavity adheres to the tube. The tensile force of the tube removal can tear these adhesions and cause pain. Literature highlights how early removal can reduce the perception of the painful stimulus of this procedure. However, this is not always possible; indeed, in some cases a tube remains inside of a patient for several days and its removal may take place outside of intensive care, in the “general” ward. Pain from CTR can cause inadequate lung expansion; secretion retention due to ineffective cough; and immobility that promotes hypoxemia, atelectasis, and pneumonia. The use of analgesics is the most common method to relieve pain induced by CTR, but this maneuver is often performed without the use of analgesics drugs or other analgesic techniques. Furthermore,the use of specific analgesics outside of environments considered protected, such as intensive care, is still some what a taboo. A recent literature review emphasizes how current pain management protocols associated with CTR, using a variety of pharmacological and non-pharmacological techniques, are either non-existent or unsatisfactory. Yet the control of pain is a specific competence and requirementofour profession—even legally—for all ages. Through the use of validated tools, our study compares a new analgesic protocol (Group A: drugs Atropine mg/kg, Midazolam mg/kg, andKetamine mg/kg) versus Paracetamol mg/Kg (Group B), the drug mostly used as an pediatric analgesic. Methods: A multi-center comparative study. Results: During the period between January 9, 2018 and March 31, 2018, we observed 32 participants in two centers. The average age of participants was five and a half years. T-tests for dependent and independent samples were performed. In the standardized samples, pre- and post-CRT pain evalutations were performed. The pre-procedure evaluation between the two samples shows no statistically significant difference (p=0.154). At post-evaluation, the protocol GroupA showed a significant difference (p=0.001) compared to the protocol Group B. Conclusion: Preliminary results show a greater efficacy with protocol Group A compared to Paracetamol for the management of pain also outside of the pediatric intensive care unit, when CTR is actuated in a ward of pediatric cardiac surgery

81. DELIRIUM IN ICU PATIENTS FOLLOWING CARDIAC SURGERY: AN OBSERVATIONAL STUDY

Abstract

Aims and objectives. To observe the clinical and structural factors that can be associated with the post-operative onset of delirium in patients who have undergone heart surgery. Background. Several risk factors could contribute to the development of delirium, such as the use of some sedative drugs and a patient’s history with certain types of acute chronic disease. However, in the literature, there is little knowledge about the association between delirium in patients who have undergone cardio-surgical intervention and their clinical and environmental predictors. Design. We used an observational design. Methods. We enrolled 89 hospitalised patients in the ICU. Patients were first evaluated using the Richmond Agitation Sedation Scale (RASS) and subsequently using the Confusion Assessment Method for the ICU (CAM-ICU). A linear model of regression was used to identify the predictors of delirium in patients. Results. The patients had an average age of 89 years (SD=6.9), were predominantly male (84.3%) and were mostly married (79.8%). The majority of patients had been subjected to bypass (80.9%), while 19.1% had undergone the intervention of endo prosthesis. The logistic regression model showed that patient age, the duration of VAM, continuous exposure to artificial light and the presence of sleep disorders were predictors of the onset of delirium. Conclusion. This study further confirms that clinical aspects such as insomnia and one’s circadian rhythm as well as structural elements such as exposure to artificial light are variables that should be monitored in order to prevent and treat the onset of severe post-operative delirium. Relevance to clinical practice. Identifying the possible factors that predispose a patient to the onset of delirium during intensive therapy following cardiac surgery, it is fundamental to implement interventions to prevent this syndrome.

The charateristics of stimulated Brillouin scattering in cyclohexane and methanol was investigated through broad-band pumping technique. The reflectivity and phase conjugation (PC) of SBS mirrors with regard to pump intensity were studied under different pump bandwidths using a xenic chloride laser system. It was found that the the pump spectral width and intensity have significant effects on the PC fidelity.

Abstract

[Laboratory Medicine: specialists of tomorrow] Laboratory Medicine rides the wave of technological progress, the metamorphosis of information systems and data management. The Young Specialist is not a mere observer, but rather takes a leading role in this change, taking advantage of the opportunities offered by “omics” technologies, capturing new ideas and innovative stimuli that lead to a new concept of work and research oriented to health and prevention. Thanks to the support of international web platforms, training and exchange programs supported by the International Scientific Societies and Federations that favor professional and scientific growth, Young Scientists work in a global context. In this scenario, the SIBioC Young Scientists Study Group, with the auspices of SIBioC, EFLM and IFCC, organized a meeting on "Laboratory Medicine: Specialists of tomorrow" with the aim of discussing and highlighting some of the most important challenges, such as technological progress, training and internationalization of young people. Finally, the future of laboratory medicine looks at a multidisciplinary approach that leads to integrated diagnosis, identification of the frail patient, the use of the Point of Care Testing as an indispensable tool in crisis areas, making the dialogue between physician and laboratory specialist a fundamental step for the diagnosis and treatment with the final aim of a better outcome for the patient.

Abstract

INTRODUCTION: Glaucoma is often a misdiagnosed pathology and is one of the main causes of blindness in the world population.Glaucoma frequentlycauses limitations in theactivities of daily livingand changes the patient's quality of life. AIM: The purpose of our study is to describe the life experiences of subjects in southern Italy suffering from glaucoma. METHOD: Phenomenological study RESULTS: From the analysis of interviews six main themes emerged:(1) a profound change in lifestyle, (2) casual diagnosis,(3) independent research,(4) fear of blindness,(5) fear of being a burden to family, and (6) fear of familial pathology. Two additional themes appeared: the subjects felt lucky not to have a deadly disease and were uncertain about the future. CONCLUSION: The negative aspects prevail over any positive facets. A deep understanding of these experiences can help health care professionals to deliver correct holistic support. Abstract

OBJECTIVE: The patients with re- paired Tetralogy of Fallot (rToF) are a growing population due to the improvement of surgical management in neonatal age. However, the sig- ni cant pulmonary regurgitation, consequent to the repair, is the most frequent sequelae and leads to a progressive right ventricle dilation over time. The latter, in turn, is responsible for the possible dysfunction of right and/or left ven- tricle and an increased risk of dangerous ven- tricular arrhythmias. Therefore, right ventricle monitoring is necessary for rToF patients and a 3D method is required due to its three anatomi- cal and functional subunits. Magnetic resonance imaging (MRI) has become the 3D modality of choice in the evaluation of both cardiac anatomy and ventricular volumes in rToF patients since it is able to evaluate both the pathophysiology and anatomy, it is free of radiation and, when strict- ly necessary, it uses a non-iodinated contrast agent. Cardiac CT should be considered in the evaluation of the sequelae in rToF only in select- ed cases, given that it implies a radiation dose and iodinated contrast, in addition to not evalu- ating the pathophysiology as MRI.

Abstract

Background: According to quality specifications required by international guidelines, the evaluation of the 99th URL value is a very difficult task that is usually beyond the capacity of a single laboratory. The aims of this article are to report and discuss the results of a multicenter study concerning the evaluation of the 99th percentile URL and reference change (RCV) of the ADVIA Centaur High-Sensitivity Troponin I (TNIH), recently distributed to the Italian clinical laboratories. Materials and methods: The reference population evaluated with ADVIA XPT method for the calculation of cTnI reference distribution parameters consisted of 1325 healthy adults subjects (age range from 18 to 86 years), including 653 women (mean age 50.7 years, SD 14.5 years) and 672 men (mean age 50.9 years, SD 13.8 years), well matched for both age (P =.8112) and sex (F/M = 0.97). Results: cTnI distribution values of reference population was highly skewed, while log-transformed cTnI values roughly approximated a log-normal distribution. Men have higher cTnI values than women throughout all the adult lifespan. Moreover, the subjects with age ≤ 55 years had significantly lower cTnI values than those with age > 55 years (p <.0001). Of note, 62% of women and 77% of men had equal or higher than cTnI values than the LoD value of the method (i.e., 2.2 ng/L). Conclusions: The results of the present study demonstrate that the ADVIA Centaur High-Sensitivity Troponin I using the XPT automated platform fits both the criteria and quality specifications required by the most recent international guidelines for high-sensitivity methods for cTnI assay.

Abstract

Background: The Italian Society of Clinical Biochemistry (SIBioC) and the Italian Section of the European Ligand Assay Society (ELAS) have recently promoted a multicenter study (Italian hs-cTnI Study) with the aim to accurately evaluate analytical performances and reference values of the most popular cTnI methods commercially available in Italy. The aim of this article is to report the results of the Italian hs-cTnI Study concerning the evaluation of the 99th percentile URL and reference change (RCV) values around the 99th URL of the Access cTnI method. Materials and methods: Heparinized plasma samples were collected from 1306 healthy adult volunteers by 8 Italian clinical centers. Every center collected from 50 to 150 plasma samples from healthy adult subjects. All volunteers denied the presence of chronic or acute diseases and had normal values of routine laboratory tests (including creatinine, electrolytes, glucose and blood counts). An older cohort of 457 adult subjects (mean age 63.0 years; SD 8.1 years, minimum 47 years, maximum 86 years) underwent also ECG and cardiac imaging analysis in order to exclude the presence of asymptomatic cardiac disease. Results and conclusions: The results of the present study confirm that the Access hsTnl method using the DxI platform satisfies the two criteria required by international guidelines for high-sensitivity methods for cTn assay. Furthermore, the results of this study confirm that the calculation of the 99th percentile URL values are greatly affected not only by age and sex of the reference population, but also by the statistical approach used for calculation of cTnI distribution parameters.

Abstract

Background: The study aim is to compare cTnI values measured with three high-sensitivity (hs) methods in apparently healthy volunteers and patients admitted to emergency department (ED) with acute coronary syndrome enrolled in a large multicentre study. Methods: Heparinized plasma samples were collected from 1511 apparently healthy subjects from 8 Italian clinical institutions (mean age: 51.5 years, SD: 14.1 years, range: 18-65 years, F/M ratio:0.95). All volunteers denied chronic or acute diseases and had normal values of routine laboratory tests. Moreover, 1322 heparinized plasma sample were also collected by 9 Italian clinical institutions from patients admitted to ED with clinical symptoms typical of acute coronary syndrome. The reference study laboratory assayed all plasma samples with three hs-methods: Architect hs-cTnI, Access hs-cTnI and ADVIA Centaur XPT methods. Principal Component Analysis (PCA) was also used to analyze the between-method differences among hs-cTnI assays. Results: On average, a between-method difference of 31.2% CV was found among the results of hs-cTnI immunoassays. ADVIA Centaur XPT method measured higher cTnI values than Architect and Access methods. Moreover, 99th percentile URL values depended not only on age and sex of reference population, but also on the statistical approach used for calculation (robust non-parametric vs bootstrap). Conclusions: Due to differences in concentrations and reference values, clinicians should be advised that plasma samples of the same patient should be measured for cTnI assay in the same laboratory. Specific clinical studies are needed to establish the most appropriate statistical approach to calculate the 99th percentile URL values for hs-cTnI methods.
