Centenarians are rare and exceptional individuals characterized by a peculiar phenotype. They are the best example of healthy aging in humans as most of them have escaped or substantially delayed the onset of major age-related diseases. Within this scenario, the purpose of the present work was to understand if immune status is associated with survival and health status in centenarians. To this aim, 116 centenarians were concomitantly characterized for their immunological, health and functional status, and followed-up for five-year survival. On the basis of previous knowledge we focused on a core of fundamental and basic immune parameters (number of leukocytes, monocytes, total lymphocytes, CD3(+) T lymphocytes, CD4(+) helper T lymphocytes, CD8(+) cytotoxic T lymphocytes, CD19(+) B lymphocytes and plasma levels of IgM), and the most important findings can be summarized as follows: i. a hierarchical cluster analysis was able to define Cluster1 (88 centenarians) and Cluster2 (28 centenarians) characterized by low and high values of all these immune parameters, respectively; ii. centenarians of Cluster2 showed a statistically longer five-year survival and more favorable values of other important immune (naïve, activated/memory and effector/memory T cells) and metabolic (glycemia, insulin and HOMA-IR) parameters, in accord with previous observations that centenarians have a peculiar immune profile, a preserved insulin pathway and a lower incidence of type 2 diabetes; and iii. unexpectedly, parameters related to frailty, as well as functional and cognitive status, did not show any significant correlation with the immune clustering, despite being capable per se of predicting survival. In conclusion, high values of basic immunological parameters and important T cell subsets correlate with five-year survival in centenarians, independent of other phenotypic characteristics. This unexpected biological scenario is compatible with the general hypothesis that in centenarians a progressive disconnection and loss of biological coherence among the different functions of the body occur, where survival/mortality result from the failure of any of these domains which apparently follow an independent age-related trajectory. more...

According to the findings of some recent studies, the centenarians' offspring appear to represent a promising model for research on longevity and healthy aging. This study compares the health status and the functional status of three groups of subjects: 1. individuals with two long-lived parents (one of whom centenarian), 2. individuals with only one long-lived (centenarian) parent, and 3. individuals with no long-lived parents. The goal is to verify whether the centenarians' offspring display any advantage over the offspring of both non-long-lived parents and to evaluate whether the longevity of the non centenarian parent provides a further advantage. A total of 374 subjects (mean age approximately 70 years) was examined. A threshold for longevity was established for non-centenarian parents through demographic data available for Italy (males surviving to at least 81 years of age and females to 87 years). The participants were assessed for their health and functional status by means of a standardized questionnaire and tests of physical performance. Data were analyzed using multivariate regression models adjusted for socio-demographic characteristics and risk factors for age-related pathologies. The results of the study show that centenarians' offspring have a better functional status, a reduced risk for several age-related pathologies and reduced drug consumption than the offspring of non-long-lived parents. In addition, the health status of centenarians' offspring does not appear to be influenced by the longevity of the second parent. It therefore seems possible to conclude that at ages around 70 years the genetic contribution to health status deriving from having one centenarian parent is not substantially improved if the other parent is also long-lived. more...

Background: Agnosia for objects is often overlooked in neuropsychology, especially with respect to rehabilitation. Prosopagnosia has been studied more extensively, yet there have been few attempts at training it. The lack of training protocols may partially be accounted for by their relatively low incidence and specificity to sensory modality. However, finding effective rehabilitations for such deficits may help to reduce their impact on the social and psychological functioning of individuals. Objective: Our aim in this study was to provide clinicians and researchers with useful information with which to conduct new studies on the rehabilitation of object agnosia and prosopagnosia. To accomplish this, we performed a systematic and comprehensive review of the effect of neuropsychological rehabilitation on visual object and prosopagnosia. Methods: The Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines were followed. In addition, the Single-Case Experimental Design (SCED) and the Critical Appraisal Skills Programme (CASP) scales were used to assess the quality of reporting. Results: Seven articles regarding object agnosia, eight articles describing treatments for prosopagnosia, and two articles describing treatments for both deficits were included. Conclusions: In the light of the studies reviewed, treatments based on analysis of parts seem effective for object agnosia, while prosopagnosia appears to benefit most from treatments relying on holistic/configural processing. However, more attempts at rehabilitation of face and object agnosia are needed to clarify the mechanisms of these processes and possible rehabilitations. Moreover, a publication bias could mask a broader attempt to find effective treatments for visual agnosia and leaving out studies that are potentially more informative. more...

A typical feature of ageing is a chronic, low-grade inflammation characterized by a general increase in the production of pro-inflammatory cytokines and inflammatory markers ("inflamm-ageing"). This status may slowly damage one or several organs, especially when unfavorable genetic polymorphisms and epigenetic alterations are concomitant, leading to an increased risk of frailty together with the onset of age-related chronic diseases. The contribution of different tissues (adipose tissue, muscle), organs (brain, liver), immune system and ecosystems (gut microbiota) to age-related inflammation ("inflamm-ageing") will be discussed in this review in the context of its onset/progression leading to site-restricted and systemic effects. Background: The aim of this study was to achieve a consensus on the minimum set of outcome measures and predictors to be used in the neurosurgical practice and on the timing of outcome assessment. Methods: A consensus building approach was employed. All neurosurgical departments in Lombardy (Italy) were invited to participate by the Carlo Besta Neurologic Institute IRCCS Foundation. Three workshops were organized during which a multidisciplinary group called Neurosurgical Outcome Network (NEON) was created and the methodology to select outcome measures, predictors, and timing of outcome assessment was established. Eight working groups were created for the different neurosurgical diseases (neuro-oncological, skull base, vascular, traumatic, spinal, peripheral nervous system, malformation, functional) and 8 workshops were organized to identify the outcome measures and predictors specific for each of the neurosurgical diseases based on the experts' clinical practice and the existing literature. Results: A total of 20 neurosurgical departments participated in this study. Specific outcome measures, predictors and the timing of outcome assessment were identified for each of the 8 neurosurgical diseases. Moreover, a list of variables common to all pathologies were identified by the NEON group as further data to be collected. Conclusions: A consensus on the minimum set of outcome measures and predictors and the timing of outcome assessments for 8 neurosurgical diseases was achieved by a group of neurosurgeons of the Lombardy region, called NEON. Background: The standardization of outcome measures is needed for comparing studies and using common measures in clinical practice. We aimed to identify cognitive and patient-reported outcomes and timing of assessment for glioma, meningioma, and vascular surgery. Method: A consensus study was conducted. Participants selected cognitive and patient-reported measures among a list of instruments identified through a literature search. Results: Seventeen cognitive tests for the glioma and meningioma’s evaluation, 8 for the vascular diseases, and one questionnaire on quality of life and one on emotional distress were identified. The timing of outcome assessment selected was before surgery, at discharge, and after 3 and 12 months for glioma; before surgery and after 3 months for meningioma; before surgery, at discharge, and after 6 months for vascular diseases. Conclusion: The identification of common outcome measures is the first step toward a shared data collection improving the quality and comparability of future studies. more...

Background Prison mental health care is a significant topic which has been already studied and described in literature, particularly because of important implications both in the prison and in the health care system. It's not uncommon that inmates suffering from mental disorders are referred to high security forensic services (HSFS) but, to date, studies assessing factors associated with relevant referrals to these services are missing. So, the aim of our study is to investigate socio-demographic, criminological, psychopathological and toxicological variables among those who were referred to HSFS as compared to their non-referred counterpart. Methods We conducted a cross-sectional study recruiting 159 subjects receiving prison inpatient care in an Italian jail, between January 2010 and August 2015. No subjects were excluded from the study. The mean age was 39. Results About half of included prisoners suffered from personality disorder while one-third from psychotic disorders. >60% of the subjects had comorbid substance use disorders. The odds of being referred to HSFS were related to previous admission (odds ratio [OR] = 5.34, 95% confidence interval [CI] 1.66–17.16), diagnosis of psychosis (OR = 2.79, 95% CI 1.11–7.04) and cannabis use disorder (OR = 2.68, 95% CI 1.14–6.28). Personality disorder was inversely associated to the referral to forensic facilities (OR = 0.37, 95% CI 0.14–0.97). Conclusions Mental health services should improve preventive measures for vulnerable prisoners in order to reduce criminal recidivism and forensic readmission. more...

Il recente sviluppo delle tecnologie informatiche ha consentito agli istituti di valutazione di migliorare il processo di assemblaggio dei test tramite l’automated test assembly (ATA). Una struttura generale per ATA consiste nell’adottare modelli di programmazione intera-mista. Questi modelli sono pensati per essere risolti dasolver commerciali che, nonostante il loro successo nella gestione della maggiorparte dei problemi noti, non sono sempre in grado di risolvere problemi di ATA molto vincolati o di grandi dimensioni. Inoltre, tutti i parametri sono considerati fissi e noti, un’ipotesi che non vale per le stime dei parametri di item response theory (IRT). In questo lavoro proponiamo un modello chance-constrained per affrontarel’incertezza nei modelli di ATA senza aumentarne la complessità The recent development of computer technologies enabled test institutes to improve the test assembly process by automated test assembly (ATA). A general framework for ATA consists in adopting mixed-integer programming models. These models are intended to be solved by common commercial solvers which, notwithstanding their success in handling most of the known problems, are not always able to find solutions for highly constrained and large-sized ATA problems. Highlights • Local visual processing in ASD were investigated with EEG during a crowding task. • Target discrimination in presence of interfering flankers predicted ASD symptoms. • ASD group did not show beta desynchronization for resolving visual crowding scenes. • Altered beta response reflected atypical occipital–inferotemporal connectivity. • Altered beta connectivity in ASD may reflect abnormal long-range interactions., Sensory and perceptual anomalies may have a major impact on basic cognitive and social skills in humans. Autism Spectrum Disorder (ASD) represents a special perspective to explore this relationship, being characterized by both these features. The present study employed electroencephalography (EEG) to test whether detail-oriented visual perception, a recognized hallmark of ASD, is associated with altered neural oscillations and functional connectivity in the beta frequency band, considering its role in feedback and top-down reentrant signalling in the typical population. Using a visual crowding task, where participants had to discriminate a peripheral target letter surrounded by flankers at different distances, we found that detail-oriented processing in children with ASD, as compared to typically developing peers, could be attributed to anomalous oscillatory activity in the beta band (15–30 Hz), while no differences emerged in the alpha band (8–12 Hz). Altered beta oscillatory response reflected in turn atypical functional connectivity between occipital areas, where the initial stimulus analysis is accomplished, and infero-temporal regions, where objects identity is extracted. Such atypical beta connectivity predicted both ASD symptomatology and their detail-oriented processing. Overall, these results might be explained by an altered feedback connectivity within the visual system, with potential cascade effects in visual scene parsing and higher order functions. more...

Inflamm-aging, that is the age-associated inflammatory status, is considered one of the most striking consequences of immunosenescence, as it is believed to be linked to the majority of age-associated diseases sharing an inflammatory basis. Nevertheless, evidence is emerging that inflamm-aging is at least in part independent from immunological stimuli. Moreover, centenarians who avoided or delayed major inflammatory diseases display markers of inflammation. In this paper we proposed a reappraisal of the concept of inflamm- aging, suggesting that its pathological effects can be independent from the total amount of pro-inflammatory mediators, but they would be rather associated with the anatomical district and type of cells where they are produced and where they primarily act. more...

La recentissima approvazione della normativa di superamento degli Ospedali Psichiatrici Giudiziari (OPG) ha posto il clinico psichiatra che opera in carcere di fronte a nuovi interrogativi e a nuove sfide sul trattamento intra ed extra-murario dei detenuti affetti da patologia psichiatrica grave (SMI). Gli studi Italiani condotti sull’argomento sono pochi e quelli internazionali di riferimento riportano percentuali in crescita di detenuti affetti da SMI ed una crescente difficoltà dell’istituzione a farsene carico. Lo studio apre ad indagini esplorative tese ad individuare un modello di caratteristiche di fragilità intrinseca alla sotto-popolazione di detenuti inviati al Reparto di Osservazione Psichiatrica per l’accertamento delle infermità psichiche. Nello specifico sono state ricercate delle caratteristiche associate all‘invio in strutture alternative alla detenzione ordinaria per quei detenuti con esito positivo dell’osservazione.Lo studio è stato condotto in maniera retrospettiva sui dati di tutti i 159 detenuti transitati presso il Reparto di Osservazione Psichiatrica della C.C. di Monza tra il 1 gennaio 2010 e il 31 agosto 2015 e valutati dagli psichiatri del Servizio di Psichiatria Penitenziaria dell’ Ospedale San Gerardo operante all’interno dell’istituto. Sono state raccolte ed analizzate una serie di variabili relative a caratteristiche socio-demografiche, criminologiche, tossicologiche cliniche e di modello comportamentale. Sui dati, raccolti in uno specifico database sono state condotte le analisi statistiche.Le evidenze maggiormente significative nella lettura dei risultati sono state le correlazioni tra l’esito positivo dell’osservazione psichiatrica con la diagnosi di psicosi in asse I, i gesti autolesivi e suicidari compiuti prima dell’ingresso in carcere e un’anamnesi positiva per pregressi accessi in l’Ospedale Psichiatrico Giudiziario. Non si sono evidenziate significatività invece nelle categorie di variabili tossicologiche né in quelle legate al campo d’imputazione e al grado di giudizio. Nel complesso i risultati ottenuti vanno letti tenendo in considerazione alcuni limiti del nostro studio: in particolare l’impossibilità di effettuare (retrospettivamente) una diagnosi con scale standardizzate, ma anche i bias di informazione e di recall propri del metodo di raccolta dati, oltre che il limite derivante dalla numerosità del campione che non ha permesso alcune sotto-analisi esplorative di interesse. I risultati dello studio aprono a nuove prospettive di ricerca: pur nell’impossibilità di effettuare un confronto diretto con le evidenze scientifiche presenti in letteratura (a causa della difficoltà di reperire studi analoghi dovuta, in parte, alle differenze tra i sistemi giuridici e terapeutici dei vari paesi), sono emerse variabili significative, affidabili, facilmente reperibili e altamente standardizzabili. Questi risultati, se confermati da ulteriori studi, potrebbero aiutare il clinico, ma anche il legislatore, a pensare, per i detenuti con determinate caratteristiche, a specifici percorsi/luoghi di cura/detenzione evitando così l’insorgere o l’acuirsi di una sofferenza psichica e dei concomitanti problemi di gestione e custodia in strutture non idonee come le sezioni ordinarie degli istituti di pena. more...

Aging is considered the major risk factor for cancer, one of the most important mortality causes in the western world. Inflammaging, a state of chronic, low-level systemic inflammation, is a pervasive feature of human aging. Chronic inflammation increases cancer risk and affects all cancer stages, triggering the initial genetic mutation or epigenetic mechanism, promoting cancer initiation, progression and metastatic diffusion. Thus, inflammaging is a strong candidate to connect age and cancer. A corollary of this hypothesis is that interventions aiming to decrease inflammaging should protect against cancer, as well as most/all age-related diseases. Epidemiological data are concordant in suggesting that the Mediterranean Diet (MD) decreases the risk of a variety of cancers but the underpinning mechanism(s) is (are) still unclear. Here we review data indicating that the MD (as a whole diet or single bioactive nutrients typical of the MD) modulates multiple interconnected processes involved in carcinogenesis and inflammatory response such as free radical production, NF-κB activation and expression of inflammatory mediators, and the eicosanoids pathway. Particular attention is devoted to the capability of MD to affect the balance between pro- and anti-inflammaging as well as to emerging topics such as maintenance of gut microbiota (GM) homeostasis and epigenetic modulation of oncogenesis through specific microRNAs. more...

Objective To examine the association between posttraumatic stress disorder (PTSD) and obesity in the literature to date. Data sources We systematically searched PubMed, Embase, Scopus, Web of Science, and ProQuest from database inception till September 2013. Search phrases combining the terms Obesity and Post-Traumatic Stress Disorder were used. Study selection We selected observational studies estimating obesity prevalence in samples of people with PTSD, as well as in comparison groups without PTSD. Data extraction Obesity rates as well as demographic, clinical, and methodological variables were extracted from each publication or obtained directly from its authors. Results A total of 113, 395, 59, 115, and 400 records were generated from PubMed, Embase, Scopus, Web of Science, and ProQuest, respectively. Thirteen studies were eligible according to inclusion criteria. The pooled crude odds ratio (OR) and 95% confidence interval (CI) for obesity among people with PTSD, based on 589,781 subjects, was 1.55 (1.32-1.82). A large heterogeneity was found (I(2) = 90%), and risk of publication bias was statistically significant (P = .002). However,subgroup and sensitivity analyses including only studies with most accurate methods to assess obesity (OR = 1.35; 95% CI,1.05-1.74; I(2) = 47%) and PTSD (OR = 1.82; 95% CI, 1.33-2.50; I(2) = 75%) also confirmed the association between PTSD and obesity. Conclusions Despite some limitations, individuals suffering from PTSD seem more likely, relative to controls, to suffer from obesity. As such, individuals with this comorbidity should be targeted for intensive prevention and treatment focused on both disorders. International audience; The development of a chronic, low grade, inflammatory status named "inflammaging" is a major characteristic of ageing, which plays a critical role in the pathogenesis of age-related diseases. Inflammaging is both local and systemic, and a variety of organs and systems contribute inflammatory stimuli that accumulate lifelong. The NU-AGE rationale is that a one year Mediterranean whole diet (considered by UNESCO a heritage of humanity), newly designed to meet the nutritional needs of the elderly, will reduce inflammaging in fully characterized subjects aged 65-79 years of age, and will have systemic beneficial effects on health status (physical and cognitive). Before and after the dietary intervention a comprehensive set of analyses, including omics (transcriptomics, epigenetics, metabolomics and metagenomics) will be performed to identify the underpinning molecular mechanisms. NU-AGE will set up a comprehensive database as a tool for a systems biology approach to inflammaging and nutrition. Introduction: An analysis of the literature on eating disorders (ED) indicates that Eds are often comorbid with the diagnosis of Personality Disorder (PD). This seems to be a psychopathological feature, characteristic of this patient population. However, the double classification seems to be often insufficient when describing a very complex psychopathological picture. These observations have informed our intention to investigate these two distinct nosographic groups by means of a single construct. Aims: The aim of this study is to identify and describe (through tests that evaluate defense mechanisms) the defensive styles that characterize the two groups of patients, in order to outline a defensive profile that can assimilate or differentiate the psychopathological groups. Materials and methods: The study was carried at the Department of Psichiatry of the San Gerardo Hospital in Monza. The Defence Style Questionnaire-40 (Farma Cortinovis, 2000) was administered and analysed. The sample consisted of 40 participants with eating disorders, recruited among the outpatients for eating disorders, and 40 patients with personality disorders recruited from two Psychiatric Outpatient Services. Results and conclusions: The data analysis showed a statistical significance in the use of Immature defense mechanisms and Neurotic defence mechanisms in both clinical samples.The patients with PD used a significantly higher number of psychotic defense mechanisms than patients with ED. With aging, an increased prevalence of a clustering of metabolic abnormalities has been observed. These abnormalities include obesity, dyslipidemia, hypertension, and insulin resistance and are collectively known as metabolic syndrome (MetS), a low-grade, systemic, inflammatory condition associated with an increased risk of cardiovascular disease, diabetes, and other adverse health outcomes. A number of studies have demonstrated that centenarians’ offspring have a significant survival advantage and a lower risk of developing the most important age-related diseases. They therefore represent one of the best models with which to study the familiar component of human longevity. The aim of this study was to determine if the offspring of centenarians (n = 265 subjects) showed a different prevalence of MetS in comparison to the offspring of non-long-lived parents (controls, n = 101 subjects). In addition, we assessed whether centenarians’ offspring showed particular features of MetS and a distinct regulation of circulating adipokines, cytokines, and metabolic mediators. Although the prevalence of MetS was quite similar both in the offspring of centenarians and the controls, MetS-affected centenarians’ offspring seemed healthier, more functionally fit, and had lower resistin levels. MetS prevalence did not change in centenarians’ offspring across resistin, IGF-1, and resistin/IGF-1 ratio tertiles. On the other hand, in controls, MetS prevalence strongly increased across resistin tertiles and in the third resistin/IGF-1 ratio tertile, indicating a dramatic increase in MetS prevalence when the ratio between these two factors is unbalanced, with high levels of resistin and low levels of IGF-1. more...

Introduction: Since we know the difficulties in the “talking cure” of patients with eating disorders (Farrell, 2000) and the importance of the concept of alexithymia (Taylor, 1996; Bruch, 1973), we present the first results of the study “Alexithymia and emotional regulation in the psychotherapy of EDs patients” taking place at the Specialist services for eating disorders, San Gerardo Hospital, Monza. This project, in the field of psychotherapy research, aims to evaluate, in a series of 8 patients with EDs, changes in emotional expression through the language using the application of Italian computer measures of referential process-I-DAAP (Maskit, Bucci, et al., 2004) - developed on the Bucci’s Multiple Code Theory of Bucci (Bucci, 1997). Through an evaluation of the outcome of the therapeutic process and a qualitative analysis of the same, we will examine the modification and/or the influence of the alexithymic condition in the course of psychotherapy of anorexic-bulimic frameworks. Method: The effectiveness of the therapeutic process will be measured by repeated administration of self-report scales related to symptoms, subjective experience, cognitive and relational aspects. The qualitative study will be made with the I-DAAP which allows to evaluate the expression of emotional content through the overall organization of the sentence and the semantic contents. The sample consists of 8 patients with eating disorders in psychotherapy with three experienced therapists. The collection of data (self-report scales and verbatim transcribed of 8 sessions per patient) will be carried out in three stages. In this first analysis on the collected material at time T1 (psychometric scales and 25 transcripts), we have calculated the indices of correlation (r Pearsons) among the variables tested with the self-report instruments (TAS20 for alexithymia, EDI2 for the characteristics of the eating disorder, SCL90 for general psychopathology, PBI for the representation of parental bonding, BUT for body image disorder, WDQ for brooding, MPS for perfectionism, DES for dissociative episodes, WAI for therapeutic alliance), and between these ones and the average narrative indexes of I-DAAP (RA indices - MIWRAD2, MHIWRAD2, HPIWRAD2 -, Reflection - MIRef, Disfluency - MIDIF, Affect - MIAffP, MIAffN, MIAffZ -, Sensorysomatic MISenS). For all values we have also evaluated the correlation with the index of high-functioning of SWAP200 (AF PD_T, AFQ-T), applied to assess the personality of the patients. Data processing was performed using the statistical program STATA. Results and Discussion: The correlations between the variables measured by self-report scales confirm some expected and confirmed by the literature results. We found: Negative correlation between TAS20 and Maternal Care (PBI) (p =.04) and positive correlation between TAS20 and maternal overprotection (p =.03), confirming the relationship between alexithymia and the parenting style. Because of the important role of the caregiver in the process of emotional regulation, both neglect and overprotection seem to contribute to difficulties of identification and expression of one's emotional experience. This is in agreement with the work of Thorberg (2011) and De Panfilis (2008). A positive correlation between TAS20 and EDI2 Drive for thinness (p =.04) and Interoceptive Awareness (p =.03), confirming the link between lack of emotional expression and the symptoms of eating disorders, as well as a tendency to express misunderstood stimuli by means of the body. This finding is consistent with the one reported by Taylor (1996), Schmidt (1993). At present, individuals can live up to 80–120 years, a time much longer than that of our ancestors, as a consequence of the improvements in life conditions and medical care. Thus, the human immune system has to cope with a lifelong and evolutionarily unpredicted exposure to a variety of antigens, which are at the basis of profound age-related changes globally indicated as immunosenescence, a multifaceted phenomenon that increases morbidity and mortality due to infections and age-related pathologies. The major changes occurring during immunosenescence are the result of the accumulation of cellular, molecular defects and involutive phenomena (such as thymic involution) occurring concomitantly to a hyperstimulation of both innate and adaptive immunity (accumulation of expanded clones of memory and effector T cells, shrinkage of the T cell receptor repertoire, progressive activation of macrophages), and resulting in a low-grade, chronic state of inflammation defined as inflammaging. It is unknown whether inflammaging, which represents a risk factor for most age-related pathologies, is a cause or rather an effect of the aging process. In this complex scenario, the role of genetic background likely represents a fundamental variable to attain successful aging and longevity. Accordingly, centenarians seem to be equipped with gene variants that allow them to optimize the balance between pro- and anti-inflammatory molecules, and thus to minimize the effects of the lifelong exposure to environmental insults and stressors.
