Abstract

The global widespread mortality after the emergence of SARS-CoV-2 infection in China, has become a critical concern all around the world. Convalescent plasma (CP) therapy is one of the methods elevating the survival rate for COVID-19 infection cases. This technique, as a practicable therapy, was used in previous viral outbreaks including influenza, SARS and MERS. In CP therapy, the blood plasma is collected from persons rehabilitated from that specific infection in order to develop a passive immunity in other patients. Therefore, this review aimed to point out the role of CP therapy in aforementioned viral infections and illustrate different factors influencing the efficacy of CP therapy.

Abstract

Problem statement: At present study Constant Rate of Strain (CRS) consolidation under Non-Darcy condition was investigated. Approach: In this study a governing conditions on pore water flow were investigated using proposed equation and experimental tests. CRS experiments under different rates of strain were conducted on different soil samples. Results: Results of these experiments and there comparison with proposed equation showed that flow of pore water flow in the most part of each test was Non-Darcy and changed to Darcy condition in the final almost one forth of tests. Conclusion: According to the results the threshold where Non-Darcy flow changes to Darcy was dependent on variations of relative pore water pressure versus total strain and it can be determined based on variations in inclination of relative pore water pressure-total stress curve. Abstract

After introducing IL-1/IL-6 inhibitors, some patients with Still and Still-like disease developed unusual, often fatal, pulmonary disease. This complication was associated with scoring as DReSS (drug reaction with eosinophilia and systemic symptoms) implicating these inhibitors, although DReSS can be difficult to recognize in the setting of systemic inflammatory disease.

Abstract

Background and Objectives: One of the decentralized models that has been formed based on universal health coverage is the model of health complexes in Tabriz. This study was conducted to evaluate the model. Methods: This case study was conducted in 2017. Beside observation, 28 individuals, including informed experts in the field of establishment of health complexes in Tabriz University of Medical Sciences, were purposefully selected and semi-structured interviews were also conducted. To analyze the interviews and the results of observations, the framework analysis was used based on the components of the Primary Care Evaluation Tool (PCET) model. Results: The strengths of the health complex model are decentralized planning, strengthening the private sector participation, have more appropriate methods for assessing the performance of the health team, creating a sense of delivering effective services in providers, using electronic information registration system, using the prospective payment method, strengthening the referral system, enhancing service continuity, and facilitating financial and geographical access, especially in marginalized areas. However, there are some problems such as lack of some facilities and equipment, lack of some workforces and high workload, financial instability, and lack of insurance organizations cooperation with the plan. Conclusion: The health complex model has improved organizational, financial, and geographic access to health services. It seems that if the resources are more stable and by cooperation of basic insurances, the results of this project will be better.

Abstract

Background: Echinococcosis is a zoonotic parasitic disease caused by the larval stage of Echinococcus granulosus. Several native and recombinant antigens, derived from different stages of E. granulosus life cycle, have been used for vaccine trials. In vitro reared adult worms are good candidates for vaccination as they do not produce fertile egg/s and do not have any risk of contamination for researchers. Objective: To evaluate different antigens derived from in vitro reared E. granulosus adult worms for the immunization of BALB/c mice against secondary hydatidosis. Methods: Viable protoscoleces (PCSs) of sheep hydatid cyst were cultivated in S.10E.H media. Excretory secretory (E/S) and crude antigens were prepared from reared adult worms. A total of fifty BALB/c mice, each 8-weeks-old, were divided into 5 groups of 10 mice. Three groups were subcutaneously immunized with crude, E/S and immunodominant antigens on days 1 and 28. The fourth group received only PBS and the fifth group had no injection. Three weeks following the second immunization, all groups were challenged, intraperitoneal, with viable PSCs. After the autopsy of the mice and opening their abdominal wall, cysts were counted and measured followed by histopathological observations. Results: The highest protective immunity (98.7%) against hydatidosis was induced by crude antigen, followed by E/S and immunodominant antigens. Conclusion: Antigens (crude antigens in particular) derived from in vitro reared E. granulosus adult worms, and their different protein components are suitable candidates for the vaccination of intermediate hosts against hydatidosis.

Abstract

As part of the AVATAR and Mexnext projects, this study compares several methods used to derive lifting line variables from CFD simulations of the MEXICO rotor in yawed inflow. The results from six partners within the AVATAR/Mexnext consortium using five different methods of extraction were compared. Overall comparison of the induced velocities at the mid and tip parts of blade shows fairly good agreement between the tested methods, where the derived angle of attack differs within 1°, within the linear range this accounts to ˂ 10% uncertainty on the aerodynamic forces. The presented comparison shows inadequate agreement between the methods for application towards the root.

Abstract

Since the investigation of van Ingen et al., attempts were undertaken to search for laminar parts within the boundary layer of wind turbines operating in the lower atmosphere with much higher turbulence levels than seen in wind tunnels or at higher altitudes where airplanes usually fly. Based on the results of the DAN-Aero experiment and the Aerodynamic Glove project, a special work package Boundary Layer Transition was embedded in IAEwind Task 29 MexNext 3rd phase (MN3). Here, we report on the results of the application of various CFD tools to predict transition on the MEXICO blade. In addition, recent results from a comparison of thermographic pictures (aimed at detecting transition) with 3D transitional CFD are included as well. The MEXICO (2006) and NEW MEXICO (2014) wind tunnel experiments on a turbine equipped with three 2.5 m blades have been described extensively in the literature. In addition, during MN3, high-frequency Kulite data from experiments were used to detect traces of transitional effects. Complementary, the following set of codes were applied to cases 1.1 and 1.2 (axial inflow with 10 m/s and 15 m/s respectively) – elsA, CFX, OpenFOAM (with 2 different turbulence/transitional models), Ellipsys, (with 2 different turbulence models and eN transition prediction tool), FLOWer and TAU – to search for detection of laminar parts by means of simulation. Obviously, the flow around a rotating blade is much more complicated than around a simple 2D section. Therefore, results for even integrated quantities like thrust and torque are varying strongly. Nevertheless, visible differences between fully turbulent and transitional set-ups are present. We discuss our findings, especially with respect to turbulence and transition models used.

Abstract

This paper presents final results from the EU project AVATAR in which aerodynamic models are improved and validated for wind turbines on a scale of 10 MW and more. Special attention is paid to the improvement of low fidelity engineering (BEM based) models with higher fidelity (CFD) models but also with intermediate fidelity free vortex wake (FVW) models. The latter methods were found to be a good basis for improvement of induction modelling in engineering methods amongst others for the prediction of yawed cases, which in AVATAR was found to be one of the most challenging subjects to model. FVW methods also helped to improve the prediction of tip losses. Aero-elastic calculations with BEM based and FVW based models showed that fatigue loads for normal production cases were over predicted with approximately 15% or even more. It should then be realised that the outcome of BEM based models does not only depend on the choice of engineering add-ons (as is often assumed) but it is also heavily dependent on the way the induced velocities are solved. To this end an annulus and element approach are discussed which are assessed with the aid of FVW methods. For the prediction of fatigue loads the so-called element approach is recommended but the derived yaw models rely on an annulus approach which pleads for a generalised solution method for the induced velocities.

The latter methods were found to be a good basis for improvement of induction modelling in engineering methods amongst others for the prediction of yawed cases, which in AVATAR was found to be one of the most challenging subjects to model. FVW methods also helped to improve the prediction of tip losses. Aero-elastic calculations with BEM based and FVW based models showed that fatigue loads for normal production cases were over predicted with approximately 15% or even more. It should then be realised that the outcome of BEM based models does not only depend on the choice of engineering add-ons (as is often assumed) but it is also heavily dependent on the way the induced velocities are solved. To this end an annulus and element approach are discussed which are assessed with the aid of FVW methods. For the prediction of fatigue loads the so-called element approach is recommended but the derived yaw models rely on an annulus approach which pleads for a generalised solution method for the induced velocities.

93. Prevalence of Orofacial Clefts and Associated Factors in Infants.

Abstract

BACKGROUND AND OBJECTIVE: Orofacial clefts are the most common congenital facial defect. Genetic and environmental factors play a role in their development. The prevalence of orofacial clefts in Qom province has not been studied so far. The aim of this study was to evaluate the prevalence of orofacial clefts and associated factors in infants born in Qom province during 2010-2018. METHODS: In this cross-sectional study, during nine years, the medical files of all infants with orofacial clefts and the files of their mothers in all hospitals of Qom province were examined. The files of 476 healthy infants were also evaluated to investigate the associated factors. Healthy infants were selected one number before and one after the case number of sick infants. Infant-related variables (weight, blood type, and other illnesses) and parent-related factors (age, smoking, alcohol, and medications, illness, consanguineous marriage, and number of children) were extracted through reviewing the files or telephone interview. FINDINGS: Of the 171,270 infants, 238 were found with orofacial clefts (1.39 per 1,000 births). Isolated cleft palate with 39.5% was the most common type of anomaly. Among the studied factors, the percentage of smoking in fathers in the case group (34.5%) was higher than the control group (24.2%) (p=0.004). Moreover, the history of maternal high blood pressure in the case group (7.6%) was higher than the control group (3.4%) (p=0.013). The incidence of diabetes in mothers of the case group (18.9%) was higher than the control group (8.2%) (p=0.001). CONCLUSION: The results of the study showed that the prevalence of orofacial cleft is 1.39 per 1000 births and smoking, diabetes and high blood pressure are the associated risk factors.
