55. Pathways from street network design to symptoms of depression among emerging adults in China

Abstract

Streets comprise over 80% of all urban public space, while previous studies associated street network attributes with traffic and transport model choice, they did not examine network design in conjunction with symptoms of depression. This paper developed a path analysis model to examine the direct and indirect effects of street network designs on symptoms of depression among undergraduates. Road network density, road intersection density and public transit density were measured within 1 km buffers centered on university campuses. A survey that included the 9-item patient health questionnaire (PHQ9) addressed these effects and measured the incidence of symptoms of depression among a random sample of 22,060 Chinese undergraduates. After controlling for individual- and campus-level covariates, the results revealed that exposure to PM2.5, poor sleep quality and unhealthy dietary pattern (excluding transport-related physical activity) mediated the relationship between specific street network attributes and symptoms of depression. Higher road density was found to alleviate symptoms of depression by increasing exposure to PM2.5. Greater road connectivity tended to alleviate symptoms of depression by reducing exposure to PM2.5 but to exacerbate symptoms of depression by worsening sleep quality and increasing the incidence of unhealthy dietary patterns. Better access to public transit inclined to ameliorate symptoms of depression by improving sleep quality but to exacerbate symptoms by increasing PM2.5 exposure. These findings emphasize the need for strategies aimed at improving street network designs to include assessments of the aggregate effects on campus environs and the associated impacts on undergraduate mental health.

65. 12 years of observations of wind and airglow brightness in the northern polar cap mesopause region

Abstract

Observations of wind and airglow brightness in the northern polar cap mesopause region using an all-sky imager (irradiance images) and a field-widened Michelson interferometer (E-Region Wind Interferometer (ERWIN) - wind and irradiance) have taken place at the Polar Environment Atmospheric Research Laboratory (PEARL – Eureka, Nu, 80 N) since 2008. These observations provide insights into solar cycle, seasonal and diurnal variations in airglow brightness and winds. The sodium and oxygen green line airglow brightness correlates strongly with the solar cycle in spite of the observation site being in polar night. The brightness varies strongly during the winter months with several peaks. The cause of this variability is not obvious. The seasonally average meridional winds are generally southward (northward would be expected based on the residual circulation) over the 12 years with maximum southward amplitudes of ~ 10 m/s in the 2014/2016 period. Seasonally averaged zonal winds are generally eastward. The monthly mean zonal winds maximize in January at 10 m/s. Abstract

This article discusses a case study of a 46-year-old woman who presented with corneal nerve thickening (CNT), a characteristic feature of multiple endocrine neoplasia (MEN) type 2B. However, genetic testing revealed that she had a new lineage of MEN2A, which is typically associated with medullary thyroid carcinoma (MTC), phaeochromocytoma, and primary hyperparathyroidism. The article highlights that while CNT is commonly associated with MEN2B, it can also be present in MEN2A, emphasizing the importance of considering both syndromes when CNT is detected. Abstract

Background: Few mobility-based studies have investigated the associations between multiple environmental exposures, including social exposures, and mental health. Objective: To assess how exposure to green space, blue space, noise, air pollution, and crowdedness along people's daily mobility paths are associated with anxiety symptoms. Methods: 358 participants were cross-sectionally tracked with Global Positioning System (GPS)-enabled mobile phones. Anxiety symptoms were measured at baseline using the Generalized Anxiety Disorder-7 (GAD-7) questionnaire. Green space, blue space, noise, and air pollution were assessed based on concentric buffers of 50 m and 100 m around each GPS point. Crowdedness was measured by the number of nearby Bluetooth-enabled devices detected along the mobility paths. Multiple linear regressions with full covariate adjustment were fitted to examine anxiety-environmental exposures associations. Random forest models were applied to explore possible nonlinear associations and exposure interactions. Results: Regression results showed null linear associations between GAD-7 scores and environmental exposures. Random forest models indicated that GAD-7-environment associations varied nonlinearly with exposure levels. We found a negative association between green space and GAD-7 scores only for participants with moderate green space exposure. We observed a positive association between GAD-7 scores and noise levels above 60 dB and air pollution concentrations above 17.2 μg m−3. Crowdedness was positively associated with GAD-7 scores, but exposure-response functions flattened out with pronounced crowdedness of >7.5. Blue space tended to be positively associated with GAD-7 scores. Random forest models ranked environmental exposures as more important to explain GAD-7 scores than linear models. Conclusions: Our findings indicate possible nonlinear associations between mobility-based environmental exposures and anxiety symptoms. More studies are needed to obtain an in-depth understanding of underlying anxiety-environment mechanisms during daily life.

78. The Impact of Health Checks for People with Intellectual Disabilities: A Systematic Review of Evidence

Abstract

Background: Health checks for people with intellectual disabilities (ID) have been recommended as one component of health policy responses to the poorer health of people with ID. This review summarises evidence on the impact of health checks on the health and well-being of people with ID. Methods: Electronic literature searches and email contacts were used to identify literature relevant to the impact of health checks for people with ID. Results: A total of 38 publications were identified. These involved checking the health of over 5000 people with ID from a range of countries including a full range of people with ID. Health checks consistently led to detection of unmet health needs and targeted actions to address health needs. Conclusions: Health checks are effective in identifying previously unrecognised health needs, including life-threatening conditions. Future research should consider strategies for optimising the cost-effectiveness or efficiency of health checks.

Abstract

Background: Limited information is available regarding intraoperative ventilator settings and the incidence of postoperative pulmonary complications (PPCs) in patients undergoing neurosurgical procedures. The aim of this post-hoc analysis of the 'Multicentre Local ASsessment of VEntilatory management during General Anaesthesia for Surgery' (LAS VEGAS) study was to examine the ventilator settings of patients undergoing neurosurgical procedures, and to explore the association between perioperative variables and the development of PPCs in neurosurgical patients. Methods: Post-hoc analysis of LAS VEGAS study, restricted to patients undergoing neurosurgery. Patients were stratified into groups based on the type of surgery (brain and spine), the occurrence of PPCs and the assess respiratory risk in surgical patients in Catalonia (ARISCAT) score risk for PPCs. Results: Seven hundred eighty-four patients were included in the analysis; 408 patients (52%) underwent spine surgery and 376 patients (48%) brain surgery. Median tidal volume (VT) was 8 ml [Interquartile Range, IQR = 7.3-9] per predicted body weight; median positive end-expiratory pressure (PEEP) was 5 [3 to 5] cmH20. Planned recruitment manoeuvres were used in the 6.9% of patients. No differences in ventilator settings were found among the sub-groups. PPCs occurred in 81 patients (10.3%). Duration of anaesthesia (odds ratio, 1.295 [95% confidence interval 1.067 to 1.572]; p = 0.009) and higher age for the brain group (odds ratio, 0.000 [0.000 to 0.189]; p = 0.031), but not intraoperative ventilator settings were independently associated with development of PPCs. Conclusions: Neurosurgical patients are ventilated with low VT and low PEEP, while recruitment manoeuvres are seldom applied. Intraoperative ventilator settings are not associated with PPCs.

Abstract

Background It is uncertain whether the association of the intraoperative driving pressure (ΔP) with postoperative pulmonary complications (PPCs) depends on the surgical approach during abdominal surgery. Our primary objective was to determine and compare the association of time–weighted average ΔP (ΔPTW) with PPCs. We also tested the association of ΔPTW with intraoperative adverse events. Methods Posthoc retrospective propensity score–weighted cohort analysis of patients undergoing open or closed abdominal surgery in the ‘Local ASsessment of Ventilatory management during General Anaesthesia for Surgery’ (LAS VEGAS) study, that included patients in 146 hospitals across 29 countries. The primary endpoint was a composite of PPCs. The secondary endpoint was a composite of intraoperative adverse events. Results The analysis included 1128 and 906 patients undergoing open or closed abdominal surgery, respectively. The PPC rate was 5%. ΔP was lower in open abdominal surgery patients, but ΔPTW was not different between groups. The association of ΔPTW with PPCs was significant in both groups and had a higher risk ratio in closed compared to open abdominal surgery patients (1.11 [95%CI 1.10 to 1.20], P P P TW with intraoperative adverse events was also significant in both groups but had higher odds ratio in closed compared to open abdominal surgery patients (1.13 [95%CI 1.12– to 1.14], P P P Conclusions ΔP is associated with PPC and intraoperative adverse events in abdominal surgery, both in open and closed abdominal surgery. Trial registration LAS VEGAS was registered at clinicaltrials.gov (trial identifier NCT01601223).

Abstract

Background Limited information is available regarding intraoperative ventilator settings and the incidence of postoperative pulmonary complications (PPCs) in patients undergoing neurosurgical procedures. The aim of this post-hoc analysis of the ‘Multicentre Local ASsessment of VEntilatory management during General Anaesthesia for Surgery’ (LAS VEGAS) study was to examine the ventilator settings of patients undergoing neurosurgical procedures, and to explore the association between perioperative variables and the development of PPCs in neurosurgical patients. Methods Post-hoc analysis of LAS VEGAS study, restricted to patients undergoing neurosurgery. Patients were stratified into groups based on the type of surgery (brain and spine), the occurrence of PPCs and the assess respiratory risk in surgical patients in Catalonia (ARISCAT) score risk for PPCs. Results Seven hundred eighty-four patients were included in the analysis; 408 patients (52%) underwent spine surgery and 376 patients (48%) brain surgery. Median tidal volume (VT) was 8 ml [Interquartile Range, IQR = 7.3–9] per predicted body weight; median positive end–expiratory pressure (PEEP) was 5 [3 to 5] cmH20. Planned recruitment manoeuvres were used in the 6.9% of patients. No differences in ventilator settings were found among the sub-groups. PPCs occurred in 81 patients (10.3%). Duration of anaesthesia (odds ratio, 1.295 [95% confidence interval 1.067 to 1.572]; p = 0.009) and higher age for the brain group (odds ratio, 0.000 [0.000 to 0.189]; p = 0.031), but not intraoperative ventilator settings were independently associated with development of PPCs. Conclusions Neurosurgical patients are ventilated with low VT and low PEEP, while recruitment manoeuvres are seldom applied. Intraoperative ventilator settings are not associated with PPCs.

Abstract

This chapter reviews the morphodynamics of open-ocean barrier systems, synthesizing classic studies, current scientific knowledge, and future research directions regarding 14 barrier provinces worldwide. Within a coastal tectonic framework, it addresses: (1) Amero-trailing-edge coasts (USA’s New England coast, Mid-Atlantic Bight coast, North Carolina Outer Banks, Georgia Bight coast, Florida Atlantic coast; Brazil’s Santa Catarina coast; Europe’s German Bight coast; Australia’s southern and western coasts); (2) marginal-sea coasts (USA’s Florida Gulf Coast; Gulf Coast of Louisiana, Mississippi, and Alabama; Texas Gulf Coast; Australia’s eastern coast); and (3) collision coasts (USA’s Alaskan Pacific coast, Japan’s and New Zealand’s coasts). This chapter also provides a glossary, important conclusions, specific recommendations regarding future coastal research, and a robust, current set of references.

Abstract

Generic risk assessment (RA) tools can be applied to assess the incursion risk of multiple animal diseases, allowing for a rapid response to a variety of newly emerging or re-emerging diseases. To explore the opportunities for cross-validation of generic RA tools, seven tools were used to assess the incursion risk of African swine fever (ASF) for the Netherlands and Finland for the 2017 situation and for two hypothetical scenarios in which ASF cases were reported in wild boar and/or domestic pigs in Germany. The generic tools ranged from qualitative risk assessment tools to stochastic spatial risk models but were all parameterised using the same global databases. The tools had different outputs and endpoints and therefore the cross-validation focused on the relative risks across countries and scenarios. All tools evaluated the risk to the Netherlands to be higher than Finland for the live animal trade pathway. The risk to Finland was the same or higher as the Netherlands for the wild boar pathway, but the tools were inconclusive on the animal products pathway. All tools agreed that the hypothetical presence of ASF in Germany increased the risk for the Netherlands, but not for Finland. The case study illustrated that conclusions on the ASF risk were similar across the generic RA tools, despite differences observed in calculated risks. Hence, it was concluded that the cross-validation contributed to the credibility of their results.

Abstract

© 2015 Elsevier Inc. Using an error correction model, we document strong evidence of Granger causality in mean from the S&P option market to the sovereign CDS market in 98% of the 56 sovereigns we investigate. Tests under conditional heteroskedasticity provide further evidence of the risk spillover effect from the S&P index option market to the CDS market in mean, variance, and value-at-risk. The strong spillover effect during the recent financial crisis implies that global shocks first affect the S&P option market and then spill over to the sovereign CDS market. We demonstrate that our results are quite robust.

Abstract

In the local universe, OH megamasers (OHMs) are detected almost exclusively in infrared-luminous galaxies, with a prevalence that increases with IR luminosity, suggesting that they trace gas-rich galaxy mergers. Given the proximity of the rest frequencies of OH and the hyperfine transition of neutral atomic hydrogen (H i), radio surveys to probe the cosmic evolution of H i in galaxies also offer exciting prospects for exploiting OHMs to probe the cosmic history of gas-rich mergers. Using observations for the Looking At the Distant Universe with the MeerKAT Array (LADUMA) deep H i survey, we report the first untargeted detection of an OHM at z > 0.5, LADUMA J033046.20-275518.1 (nicknamed "Nkalakatha"). The host system, WISEA J033046.26-275518.3, is an infrared-luminous radio galaxy whose optical redshift z ≈ 0.52 confirms the MeerKAT emission-line detection as OH at a redshift z OH = 0.5225 ± 0.0001 rather than H i at lower redshift. The detected spectral line has 18.4σ peak significance, a width of 459 ± 59 km s-1, and an integrated luminosity of (6.31 ± 0.18 [statistical] ± 0.31 [systematic]) × 103 L ⊙, placing it among the most luminous OHMs known. The galaxy's far-infrared luminosity L FIR = (1.576 ±0.013) × 1012 L ⊙ marks it as an ultraluminous infrared galaxy; its ratio of OH and infrared luminosities is similar to those for lower-redshift OHMs. A comparison between optical and OH redshifts offers a slight indication of an OH outflow. This detection represents the first step toward a systematic exploitation of OHMs as a tracer of galaxy growth at high redshifts.

Abstract

This ERIC digest discusses ways of effectively banning smoking in schools so that controversies do not continue after implementation of the policy. By advocating a process approach, the document cites steps taken by the Lake Forest School Board to prohibit smoking in and around school grounds. Step one involved committee planning involving representatives from all the smoking and nonsmoking adult school publics. Step two involved feedback from groups potentially affected by a smoke-free schools policy. A trial balloon or run to test the policy's effectiveness comprised step three. Step four was a survey of smoker and nonsmoker attitudes toward the new policy. Step five involved official policy implementation through school board approval. In addition, this article reviews successfully implemented antismoking legislation in public places and in schools and lists the medical risks involved in continued smoking in public places and in schools.
