Background: Patient safety incidents are unavoidable and nurses, as parties involved, become second victims due to the incident itself and the way it is handled. In China, reconstructing the course of events is a crucial step in the aftermath of the incident; however, its impact on the emotional well-being of the second victim remains unclear. Purpose: The purpose of this study is to gain insight into the psychological experiences and current conditions of nurses who act as second victims during the process of reconstructing the sequence of events. Additionally, the study aims to provide justifications for supporting these individuals. Methods: An exploratory mixed research method was adopted to understand the emotional experience of the second victim when reconstructing the passage of the incident through qualitative research. Fourteen nurses with experience as second victims were selected for semi-structured interviews using purposive sampling according to the maximum difference sampling strategy. Through quantitative research, we explored the negative psychology and support needs of the second victims when they reverted to the incident, and a self-developed questionnaire (the Cronbach's alpha coefficient was 0.895) was used to survey 3,394 nurses with experiences as second victims in 11 tertiary hospitals in Shanxi Province. Results: In the qualitative part of the study, the emotional experience of the second victim's reconstruction of the course of events after a patient safety incident could be categorized into 3 themes: negative views as initial psychological impact, avoidance as part of psychological impact, and expectations and growth in overcoming negative psychological impact. The quantitative part of the study revealed that the emotions of guilt and self-blame accounted for the highest percentage after a patient safety incident. The second victim presented a high score of 39.58 ± 5.45 for support requirements. Conclusion: This study provides a better understanding of the true emotional experiences and the need for support of the second victim in the process of reconstructing the course of events. Following a patient safety incident, nursing administrators and healthcare institutions should consider the adverse psychological effects on the second victim, prioritize their support needs during the incident's reconstruction, create a positive safety culture, and reduce the risk of secondary victimization for these individuals. Purpose: The aim of this article is to investigate the impact of the frequency of safety training on the formation of a safety culture in a Polish company. Design/methodology/approach: The study was conducted in the first half of 2024 in a Polish company. Data were collected on the organisation of health and safety training courses and the reporting of near misses. Statistical and qualitative methods were used in the analysis to establish a correlation between the frequency of training and the number and type of reported incidents. Findings: Studies have shown that frequent and well-structured safety training significantly reduces the number of potential accident incidents. However, this effect diminishes over time, suggesting the need for regular reminders. Research limitations/implications: The study was limited to one company, which may affect the limited generalisability of the results. Future studies could cover different industries and organisations to verify the results in a broader context. Practical implications: The results suggest that companies should increase the frequency and variety of training to maintain a high level of employee safety awareness. It is also crucial to foster a positive culture of hazard reporting, which enables problems to be identified at an early stage. Originality/value: The article provides new insights into the long-term effectiveness of safety training and highlights the importance of the frequency and variety of such training in developing an effective safety culture. The establishment of Indonesia’s new capital, Ibu Kota Nusantara, was a massive project that created significant risks during the construction phase, such as construction accidents. In response, total construction safety culture was developed to make a belief and implement strategies for minimizing risks. This research aimed to recommend strategies based on a structural equation model of total construction safety culture to improve safety performance. Using structural equation modeling with a partial least square approach, strategies were categorized into two aspects, covering the macro impact of construction accidents (national scope) and the micro and meso impacts (company and project scope). The macro strategy recommended the creation concept of nomenclature and criteria within government regulations related to construction safety The suggestion for the government regualation would cover construction safety ecosystem in Indonesia. Meanwhile, the micro and meso strategies concept included practical steps such as technology transformation, tacit knowledge, and improved supervision methods. By transformationing technology in construction safety such as using movement sensor and Building Information Modeling, it will be helpfull for the contractors to monitor all of the manpower and create safer working enivronment. This research examines the influence of safety culture on aviation safety through the implementation of the Safety Management System (SMS) in Papua. The focus of this study is on safety risk management and crew training at airports in Papua, which face unique challenges such as extreme weather and difficult terrain. The research method used is quantitative analysis by collecting data through surveys and interviews. The results indicate that the implementation of a strong and effective safety culture can significantly improve aviation safety. Additionally, well-implemented SMS helps in identifying and managing risks and enhancing the quality of training and personnel qualifications. The study also emphasizes the importance of involving all stakeholders, including local communities, to create an environment that supports aviation safety. The construction industry holds remarkable benchmarks of aptitude and creativity; however, it is among the most lethal industries when it comes to occupational health and safety (OHS). This study incorporates a detailed analysis of the key OHS issues within the construction sector with a special regard for developing countries such as Nigeria. The paper discusses different physical, chemical, biological, and psychological risks within the industry and highlights the importance of compliance with OHS requirements aimed at avoiding mishaps, incidents, and even fatalities. Moreover, it discusses the issues relating to poor training, funding limitations, and project pressures while highlighting the significance of maintaining a strong safety culture, workers' awareness, and compliance in enhancing OHS results. In addition to that, the review also examines current safety measures, identifies gaps, and suggests improvements, drawing on global best practices to inform future policies and strategies. Background: To enhance patient safety in healthcare, it is crucial to address the underreporting of issues in Critical Incident Reporting Systems (CIRSs). This study aims to evaluate the effectiveness of generative Artificial Intelligence and Natural Language Processing (AI/NLP) in reviewing CIRS cases by comparing its performance with human reviewers and categorising these cases into relevant topics. Methods: A case–control feasibility study was conducted using CIRS cases from the German CIRS-Anaesthesiology subsystem. Each case was reviewed by a human expert and by an AI/NLP model (ChatGPT-3.5). Two CIRS experts blindly assessed these reviews, rating them on linguistic quality, recognisable expertise, logical derivability, and overall quality using six-point Likert scales. Results: On average, the CIRS experts correctly classified 80% of human CIRS reviews as created by a human and misclassified 45.8% of AI reviews as written by a human. Ratings on a scale of 1 (very good) to 6 (failed) revealed a comparable performance between human- and AI-generated reviews across the dimensions of linguistic expression (p = 0.39), recognisable expertise (p = 0.89), logical derivability (p = 0.84), and overall quality (p = 0.87). The AI model was able to categorise the cases into relevant topics independently. Conclusions: This feasibility study demonstrates the potential of generative AI/NLP in analysing and categorising cases from the CIRS. Purpose: This paper aims to assess the influence of nursing leadership style on error management culture (EMC). Design/methodology/approach: This scoping review was conducted following the integrative review methodology of the Joanna Briggs Institute (JBI) and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). PubMed, CINAHL, Scopus, Web of Science, Embase and EBSCO databases were systematically searched to identify studies on nursing leadership, error management and measurement, and error management culture. The studies' methodological quality was then assessed using the JBI Critical Appraisal Checklist for Analytical Cross-Sectional Studies. Findings: Thirteen manuscripts were included for review. The analysis confirmed that nursing leadership plays an important role in EMC and nurses' intention to report errors. Three emerging themes were identified: 1) leadership and EMC; 2) leadership and the intention to report errors; and 3) leadership and error rate. Research limitations/implications: A major limitation of the studies is that errors are often analyzed in a transversal way and associated with patient safety, and not as a single concept. Practical implications: Healthcare managers should promote training dedicated to head nurses and their leadership style, for creating a good work environment in which nurses feel free and empowered to report errors, learn from them and prevent their reoccurrence in the future. Originality/value: There is a positive relationship between nursing leadership and error management in terms of reduced errors and increased benefits. Abstract Safety culture is a critical determinant of organisational performance, particularly in high-risk industries especially in oil and gas. Understanding stakeholder preferences is essential for developing effective strategies that enhance safety culture. This study utilised the Analytic Hierarchy Process (AHP) to prioritise stakeholder preferences, identifying key elements of safety culture in Malaysia's oil and gas sector. This study employed a structured methodology to evaluate safety culture within the oil and gas industry, focusing on 18 sub-elements across three key domains: psychological, behavioural, and situational factors. A diverse sample of industry experts was recruited using purposeful and snowball sampling to ensure a comprehensive representation of stakeholder views. The AHP framework was applied to analyse the data, utilizing structured questionnaires and multicriteria decision-making techniques to prioritize the identified safety culture elements. The AHP analysis identified distinct priorities among different professional groups within the oil and gas sector. Safety and Health Practitioners emphasized practical elements such as safety rules and management commitment, while academicians prioritized knowledge and training. Management personnel highlighted the importance of safety ownership and communication, whereas policymakers focused on broader, policy-oriented aspects. The findings suggest that safety culture improvement initiatives should be tailored to address the specific needs and priorities of each professional group. A nuanced understanding of stakeholder preferences is crucial for developing comprehensive strategies that integrate observable behaviours, situational conditions, and psychological factors, ultimately fostering a robust safety culture in the oil and gas industry.

Background: One of the most important effective factors in increasing patient satisfaction and improving health and treatment results is proper communication between physician and nurse, and the patient. The present study was conducted with the aim of determining the relationship between the communication skills of the medical staff and patient’s safety culture. Methods: This was an analytical study conducted among 277 working medical staff including nurses, paramedics, full-time geographical employment doctors and anesthesia, and operating room technicians in Shahid Sadoughi Hospital in Yazd by stratified random sampling in 2022. The instrument used was the safety culture standard questionnaire made by the health care quality and research agency and communication skills of Barton J. Data analysis was done with SPSS 25 software, Mann-Whitney, Kruskal-Wallis, and multiple linear regression tests. Results: The overall mean and standard deviation of patient safety culture was 3.04 ± 0.23, which indicates the average status of safety culture in the studied hospital, and the overall mean of communication skills among medical staff was 3.36 ± 0.40. Which reports a relatively strong level of this skill. The results of linear regression showed that 2 components of communication skills (listening and feedback) have a positive and significant effect on the improvement of patient safety culture, so that they are able to explain 33 percent of the variance of patient safety culture changes. Conclusion: Based on the findings of the research, the communication skills of the medical staff have a significant effect on patient's safety culture. Therefore, in order to improve the patient’s safety culture among them, special attention should be paid to communication skills because effective communication is considered a vital element in effective care, and in the medical system it has a significant effect on the quality of care and consequently increases the satisfaction of patients.

In this article, an attempt is made to indicate the role and importance of ecosystem services as advantages that are provided to us by the environment. The authors demonstrate selected concepts of ecosystem services and their classification, but only to introduce the reader to pilot studies of the urban ecosystem. The present article aims to define the factors that influence the culture of utilizing ecosystem services in urbanized areas. In this article, a fragment of an urban ecosystem, specifically the Old Town of Gdańsk (which features anthropogenic elements and whose functioning is dependent on ecological and natural systems), is observed through a researcher’s eye. This area is predominantly focused on using international experiences in interdisciplinary research, a dynamically developing new science discipline called “city ecology.” The key research problem of this study is as follows: how can the safety culture be efficiently developed and integrated with the management of ecosystem services to ensure their persistence and resistance to environmental and anthropogenic risks?

Background: Nowadays, safety is one of the most important organization issues, and without this, it's not possible industrial organizations operating. Health and safety performance affected by various individual and managerial variables, which their evaluation depends on the organization under investigation and could be influenced by the background variable's of the organization. Therefore, this study was conducted to investigate the impact of organizational safety measures on organizational safety and health performance, emphasizing moderator role of internal and external motivation and safety culture in TaghBist Company. Methods: In this descriptive study, 110 workers whom employees at TaghBist Company were selected. Also, validity and reliability of the questionnaires were confirmed according to the methods described by Zhang et al. (2022), Brborović et al. (2022), Li et al. (2022), Buck (2011) and Fey (2005). Also, all statistical analyses of the obtained data were performed using SPSS and Smart PLS-3 softwares. Results: Based on the results obtained, 'extrinsic motivation', 'intrinsic motivation' and 'safety culture' did not have a moderating role on the relationship between the organization's safety measures and TaghBist organizational safety and health performance, but the organization's safety measures, 'intrinsic motivation' and 'safety culture' directly and positively affected on organizational health and safety performance in TaghBist Company. Also, the effect of 'extrinsic motivation' on organizational health and safety performance was not significant. Conclusion: In general, it can be acknowledged that in TaghBist Company, it is possible to directly improve the organizational health and safety performance through the organization's safety measures, intrinsic motivation and safety culture.

The survey assessed various dimensions of safety culture and incident reporting behaviors. Statistical analysis included correlation matrices, regression models, and comparative assessments across different types of hospital settings.Results: The study revealed significant associations between perceived safety culture and incident reporting behaviors (p < 0.01). Specifically, management (B = 0.64, p < 0.01), working conditions (r = 0.51, p < 0.01), and job satisfaction (r = 0.52, p < 0.01) were identified as crucial for improvement. The study highlighted the importance of fostering a blame-free culture and establishing clear reporting guidelines to enhance reporting frequencies.Conclusion: Enhancing the perception of patient safety within healthcare settings positively influences the likelihood of incident reporting. Abstract Background Measures of patient safety culture and patient experience are both commonly utilised to evaluate the quality of healthcare services, including hospitals, but the relationship between these two domains remains uncertain. In this study, we aimed to explore and synthesise published literature regarding the relationships between these topics in hospital settings. Methods This study was performed using the five stages of Arksey and O’Malley’s Framework, refined by the Joanna Briggs Institute. Searches were conducted in the CINAHL, Cochrane Library, ProQuest, MEDLINE, PsycINFO, SciELO and Scopus databases. Further online search on the websites of pertinent organisations in Australia and globally was conducted. Data were extracted against predetermined criteria. Results 4512 studies were initially identified; 15 studies met the inclusion criteria. Several positive statistical relationships between patient safety culture and patient experience domains were identified. Communication and teamwork were the most influential factors in the relationship between patient safety culture and patient experience. Managers and clinicians had a positive view of safety and a positive relationship with patient experience, but this was not the case when managers alone held such views. Qualitative methods offered further insights into patient safety culture from patients’ and families’ perspectives. Conclusion The findings indicate that the patient can recognise safety-related issues that the hospital team may miss. However, studies mostly measured staff perspectives on patient safety culture and did not always include patient experiences of patient safety culture. Further, the relationship between patient safety culture and patient experience is generally identified as a statistical relationship, using quantitative methods. Further research assessing patient safety culture alongside patient experience is essential for providing a more comprehensive picture of safety. This will help to uncover issues and other factors that may have an indirect effect on patient safety culture and patient experience.

The mental health and overall quality of seafarers are critical to the safety and efficiency of maritime operations. This study investigates the factors influencing the quality of Indonesian seafarers, with a particular focus on mental health as a mediating variable. Using Structural Equation Modeling (SEM) with Partial Least Squares (PLS), we analyzed data from 280 respondents, including skippers and crew members working on Indonesian flagged ships. The study examines the impact of working conditions, recruitment and placement, training and development, and safety culture on seafarer quality, both directly and indirectly through mental health. Our findings indicate that mental health significantly mediates the relationship between these factors and the quality of seafarers. Training and development emerged as the most influential factor, followed by safety culture, recruitment and placement, and working conditions. The results emphasize the importance of enhancing mental health support to improve working conditions, recruitment processes, training initiatives, and safety culture, thereby increasing productivity and the overall quality of seafarers. This research addresses an empirical gap by providing practical strategies to improve Indonesian seafarers' quality and job stability in the global maritime industry. Future research should expand the geographic scope and explore additional factors affecting seafarer quality and mental health.

Abstract Background The use of personal protective equipment (PPE) should be a culture of a workplace, and deeply rooted in worker behavior and attitude during their practice. According to the recent studies only 64% of the workers use PPE properly. The present study aims to investigate the utilization of PPE among workers in small and medium-sized enterprises (SMEs), and its relationship with knowledge, attitude, performance, and safety culture among workers. Methods This cross-sectional study was carried out using a questionnaire tool across SMEs in Kashan city in year 2023. The used tool included three questionnaires: demographic, safety culture, and knowledge, attitude and performance. Study papulation was 529 SMEs. Totally, the sample size was 369 persons and questionnaires were distributed among the workers of SMEs. Finally, SPSS software was used for statistical analysis and structural equation modeling. Various statistical tests including T-Test, ANOVA, RMSEA, CFI, TLI, and the chi-square ratio were employed. Results The mean values (standard deviation) of age and work experience were 35.19 (12.33), and 15.60 (1.69) years, respectively. Among the 369 participants, 267 participants (72.4%) indicated that they use some PPE, although not all types. However, 102 individuals (27.7%) do not employ any PPE. The lowest score for safety culture dimension was attributed to safety training at 1.58. The results of the final model indicate that the assumed relationships between variables, as outlined in the study objectives, were well established, with all connections proving statistically significant. Conclusion It can be concluded that the missing of inadequate legal supervision for small industries exists. Therefore, it can be inferred that if supervision and regulation are enhanced for safety training and implementation that may lead to increased usage of PPE.

Objective. The purpose of the theoretical research of the article is to formulate and describe cases in which it is possible to conduct correlation and other types of analysis for some situations to quantify the impact of the human factor on safety of technical systems using plausible patterns and statistical distributions, to consider some problems and reliability issues that arise in the course of solving such problems.Method. Some cases of possible application of the chi-square distribution to assess the impact of the human factor on safety of systems with the participation of people in technological operations are presented.Result. Approach on using of agreement criterium of chi-square, explanations and recommendations on the implementation of expert research in this area are offered for analysis and assessment.Conclusion. The article considers the conditions and describes the possibilities for carrying out some types of analysis in assessing the impact of the human factor on the functioning of technical systems.

Introduction Methods Results Conclusion Safety and quality improvement are essential to clinical practice in radiation therapy as planning and treatment increase in complexity and sophistication. An incident learning system (ILS) is a safety and quality improvement tool that can aid risk mitigation to improve patient safety and quality of care. The aim of this study was to quantify the impact of implementing a new e‐ILS, Learning In Radiation ONcology (LIRON), on reporting and safety culture within a local health district (LHD).The ILS (LIRON) was implemented in 2020 with the intent of tracking actual incidents, near misses and procedural non‐compliances for analysis of root causes and contributing factors. A survey was conducted after 12 months of LIRON use, and distributed to radiation oncologists, radiation therapists and radiation oncology medical physicists within the LHD. Results were compared with the responses to a pre‐ILS implementation survey, to review changes in staff perceptions of safety culture, barriers to reporting and ILS understanding.Survey response rates were similar at baseline and at the 12‐month follow‐up, 64% and 63%, respectively. Findings showed increased ILS participation (49–71%), increased perception of no barriers to reporting (34–43%) and increased encouragement to report (37–43%). Greater confidence in the department's ability to learn from the ILS was evident (24–46%).Initial findings of LIRON implementation show positive impact but warrant further long‐term review for greater understanding of its impact on staff perceptions, safety culture and improving departmental processes. Background/Objectives: The stress levels in emergency services are enormous. The resulting stress can range from psychological irritation to burnout. This study examines the importance of resilience in the German EMS and its significance for the risk of irritation and burnout among EMS personnel. Methods: A quantitative cross-sectional online survey was conducted among 285 EMS personnel in Germany. Resilience was measured by the RS-13 Scale, irritation by the Irritation Scale (IS), and burnout by the Maslach Burnout Inventory (MBI). Sociodemographic and job-related data were also collected. A classification into resilient groups was used to compare stress levels. Results: More than one-third (39%) of the participants had a low level of resilience. EMS personnel with high levels of resilience had significantly lower scores on the cognitive and emotional irritation dimensions, as well as on the burnout dimensions of emotional exhaustion and cynicism. Conclusions: Resilience plays an important role in the safety culture of emergency services. The results support the hypothesis that high levels of resilience lead to less stress and help people cope better with stress. Almost two-fifths of the participants had lower resilience, underscoring the need for safe communication and targeted measures to strengthen resilience. Regular training, a supportive work environment, and promoting team cohesion and social support can improve emergency responders' mental health and job performance. Future research should develop specific intervention strategies and evaluate their effectiveness to ensure the long-term health of emergency responders and improve the quality of emergency care. Within the dynamic, complex, and often safety-critical operations of many process industries, the integration of technology and human elements has given rise to sociotechnical systems (STSs), where the interaction between people and technology plays a pivotal role. To thrive in this complex environment, organizations must adopt adaptive error management strategies and cultivate organizational resilience. This approach involves managing the unexpected and designing systems to embrace disorder by organizational learning from errors in STSs. The main objective of this article was to present empirical data of error-causing elements in STSs based on the Dirty Dozen concept, their underlying structure, and implications for error causation screening and adaptive error management systems. A sample of 544 workers employed in seven process industries, such as automotive, chemicals, defense, metal, and timber, participated in this study. The results revealed a three-factor model of human error causation in STSs. Based on these results, an adaptive error management system (AEMS), which includes evidence-based interventions to manage causes of human errors and mitigate their risky consequences, was presented. One of the most important values in human life is health and safety. This is reflected in the Sustainable Development Goals (SDG3, SDG8), which were presented by the United Nations in 2015. These goals emphasize the importance of ensuring healthy lives, promoting well-being for all, and fostering inclusive and sustainable economic growth through decent work and safe working conditions. Taking into account the importance of issues related to safety, including occupational safety and human health, research was carried out to examine the impact of the development of a safety culture during employees' work on improving public health. In order to conduct the research, a survey questionnaire was developed, consisting of 20 questions relating to 10 attributes of safety culture. The research surveys were carried out among 363 employees in a production enterprise, and an interview questionnaire was administered to 3 employees of the Occupational Health and Safety Service of this enterprise. The results revealed that the enterprise's employees rate the level of safety culture very highly. Along with the improvement of working conditions in the enterprise, the level of safety increases, the number of accidents at work decreases, and thus the protection of employee health is higher. Therefore, a high level of safety culture is correlated with a high value attached to the health and life of employees, and this, in turn, affects public health. Moreover, fostering a strong safety culture aligns with the principles of sustainable development, contributing to long-term societal well-being and aligning with global goals for responsible and ethical growth. Medical error is a serious public health concern and undermines healthcare organizations' commitment to drive safe, high-quality patient care. Voluntary error reporting (VER) is one key solution to address this concern because it is through conducting root cause analysis that constructive retrospective learning can take place to improve future practice. Nurses form the largest health workforce and are key stakeholders contributing to the institutional error management culture. While the significance of VER and nurses' role in driving this initiative cannot be further emphasized, studies revealed that nurses failed to engage in VER due to less positive experiences towards VER. Nurses' negative attitude towards VER can be attributed to unsupportive organizational responses to their act of VER, underpinned by the endorsement of blame, shame, and punitive culture consistent with the human approach to error management. This induces fear of speaking up for error among nurses, creating a culture of silence. This paper examines and discusses current policies underpinning the error management system and identifies the contemporary factors that challenge these policies, followed by proposing recommendations to support these policies to drive nurses' commitment to VER and improve the overall error management system. Background: Patient safety climate constitutes an important element for quality improvement. Its current evidence base has been generated in hospital settings in developed countries. Studies in dentistry are limited. Objective: To systematically explore the evidence regarding assessing patient safety climate in dentistry. Methods: We developed a search strategy to explore MEDLINE, SCOPUS, and Web of Science databases from January 1st, 2002, to December 31st, 2022, to include observational studies on patient safety culture or patient safety climate assessment. Methodological features and item data concerning the dimensions employed for assessment were extracted and thematically analyzed. Reported scores were also collected. Results: Nine articles out of 5584 were included in this study. Most studies were generated from high‐income economies. Our analysis revealed methodological variations. Non‐randomized samples were employed (ranging from 139 to 656 participants), and response rates varied from 28% to 93.7%. Three types of measurement instruments have been adapted to assess patient safety climate. These mainly consisted of replacing words or rewording sentences. Only one study employed an instrument previously validated through psychometric methods. In general, patient safety climate levels were either low or neutral. Only one study reported scores equal to or greater than 75. Discussion: Despite diverse assessment tools, our two‐decade analysis reveals a lag compared with medicine, resulting in methodological variations for assessing patient safety climate. Collaboration is vital to elevate standards, prioritize patient safety across oral healthcare services, and advocate for integrating safety climate into local and national quality and patient safety strategies. Background: Measures of patient safety culture and patient experience are both commonly utilised to evaluate the quality of healthcare services, including hospitals, but the relationship between these two domains remains uncertain. In this study, we aimed to explore and synthesise published literature regarding the relationships between these topics in hospital settings. Methods: This study was performed using the five stages of Arksey and O'Malley's Framework, refined by the Joanna Briggs Institute. Searches were conducted in the CINAHL, Cochrane Library, ProQuest, MEDLINE, PsycINFO, SciELO and Scopus databases. Further online search on the websites of pertinent organisations in Australia and globally was conducted. Data were extracted against predetermined criteria. Results: 4512 studies were initially identified; 15 studies met the inclusion criteria. Several positive statistical relationships between patient safety culture and patient experience domains were identified. Communication and teamwork were the most influential factors in the relationship between patient safety culture and patient experience. Managers and clinicians had a positive view of safety and a positive relationship with patient experience, but this was not the case when managers alone held such views. Qualitative methods offered further insights into patient safety culture from patients' and families' perspectives. Conclusion: The findings indicate that the patient can recognise safety-related issues that the hospital team may miss. However, studies mostly measured staff perspectives on patient safety culture and did not always include patient experiences of patient safety culture. Further, the relationship between patient safety culture and patient experience is generally identified as a statistical relationship, using quantitative methods. Further research assessing patient safety culture alongside patient experience is essential for providing a more comprehensive picture of safety. This study analyzes the effect of safety culture on employee safety performance at a Coal Fired Power Plant (CFPP) in Java, as well as the mediating role of safety communication and moderating role of service years. This study aims to investigate the direct and indirect effects between the variables of safety culture, safety communication and employee safety performance. In addition, the study also explored the moderating role of service year in the relationship between safety culture and employee safety performance. Data was obtained from distributing questionnaires to CFPP employees using a stratified sampling method. SmartPLS SEM. The findings reveal that safety culture does not have a significant direct effect on employee safety performance. However, safety communication significantly mediates the relations between safety culture and employee safety performance. The service year does not moderate the relationship between safety culture and employee safety performance This indicates that other factors such as continuous training, reward system, and work environment may be more influential in this context. In Australia, acute inpatient units within public mental health services have become the last resort for mental health care. This research explored barriers and facilitators to safe, person‐centred, recovery‐oriented mental health care in these settings. It utilised participant observations conducted by mental health nurses in acute inpatient units. These units were located in three distinct facilities, each serving different areas: a large metropolitan suburban area in a State capital, a mid‐sized regional city, and a small city with a large rural catchment area. Our findings highlighted that, in the three inpatient settings, nurses tended to avoid common areas they shared with consumers, except for brief, task‐related visits. The prioritisation of administrative tasks seemed to arise in a situation where nurses lacked awareness of alternative practices and activities. Consumers spent prolonged periods of the day sitting in communal areas, where the main distraction was watching television. Boredom was a common issue across these environments. The nursing team structure in the inpatient units provided a mechanism for promoting a sense of psychological safety for staff and were a key element in how safety culture was sustained. The main purpose of crew resource management (CRM) is to ensure safe flights by preventing possible errors with the effective use of non-technical skills. The aim of the current study is to examine the effects of CRM on flight safety culture (FSC) with the help of the structural equation model with 451 airline pilots. As a result of the analysis, it was determined that there was a significant correlation between CRM and FSC and that CRM has a significant positive effect on FSC. It has been demonstrated that if CRM awareness and skills are used effectively, the perception of FSC will also improve. Furthermore, these findings indicate that there is a need to progress to the corporate CRM phase, i.e. CRM 7.0, to ensure that organisation-wide FSC awareness is established through CRM awareness. This article incorporates the various diversified challenges faced by industries which results in major incidents. These incidents impact the performance and brand image of the organization in the competitive market. To understand the challenges one of Asia's biggest Heating, Ventilation & Air conditioning (HVAC) industry has been selected, existing safety culture was analysed and based on that a 3 phase action plan was devised to bring cultural shift. First phase consisted of immediate action plan to bring the current situation under control, the second phase was intermediate action plan to establish the safety management systems and the third phase was a long term action plan to implement the cultural tools and sustain them. With this a practical approach to resolve safety, health and environment (SHE) challenges helped the organization to change the culture which resulted in improvement of major parameters like training man-hours, reporting & closure of observations, reporting of Near Miss and Audits. It also helped in reduction of lagging parameters like Loss Time Injury (LTI's) - a lost time injuryis an injury sustained on the job by an employee that results in the loss of productive work time, Loss Time Injury Frequency Rate (LTIFR) - the number of lost time injuries that occurred during the reporting period. The surgical team works collaboratively to prevent the occurrence of retained surgical items (RSIs). The purpose of this quality improvement project was to increase compliance with facility policies and improve teamwork skills to prevent the occurrence of RSIs. The project team implemented an evidence‐based communication protocol, updated hospital network policies, introduced just‐in‐time job aids, and facilitated leader support through a daily huddle to address identified practice gaps. The TeamSTEPPS Teamwork Attitudes Questionnaire was used to measure the change in staff members' attitudes about teamwork before and after project implementation. Additional process and outcome measures included the number of near misses and actual RSIs, compliance with the daily huddle, and completion of the communication training. Background: The use of personal protective equipment (PPE) should be a culture of a workplace, and deeply rooted in worker behavior and attitude during their practice. According to the recent studies only 64% of the workers use PPE properly. The present study aims to investigate the utilization of PPE among workers in small and medium-sized enterprises (SMEs), and its relationship with knowledge, attitude, performance, and safety culture among workers. Methods: This cross-sectional study was carried out using a questionnaire tool across SMEs in Kashan city in year 2023. The used tool included three questionnaires: demographic, safety culture, and knowledge, attitude and performance. Study papulation was 529 SMEs. Totally, the sample size was 369 persons and questionnaires were distributed among the workers of SMEs. Finally, SPSS software was used for statistical analysis and structural equation modeling. Various statistical tests including T-Test, ANOVA, RMSEA, CFI, TLI, and the chi-square ratio were employed. Results: The mean values (standard deviation) of age and work experience were 35.19 (12.33), and 15.60 (1.69) years, respectively. Among the 369 participants, 267 participants (72.4%) indicated that they use some PPE, although not all types. However, 102 individuals (27.7%) do not employ any PPE. The lowest score for safety culture dimension was attributed to safety training at 1.58. The results of the final model indicate that the assumed relationships between variables, as outlined in the study objectives, were well established, with all connections proving statistically significant. Conclusion: It can be concluded that the missing of inadequate legal supervision for small industries exists. Therefore, it can be inferred that if supervision and regulation are enhanced for safety training and implementation that may lead to increased usage of PPE. [ABSTRACT FROM AUTHOR]

A student Laboratory Safety Officer (LSO) program formalizes a position often found within academic institutions. Through clearly defined responsibilities and effective communication between an LSO and their principal investigators, environmental health and safety office, and department administrators, this program can be effectively used to establish a baseline for safety standards. This article outlines pathways for an institution to establish or strengthen a student LSO program. The strength within this program goes beyond defined responsibilities, where the program's initiatives can deepen a department-wide community while improving access to resources and advancing leadership skills. The common hurdles faced by these programs are discussed at length for new and existing programs to gain insights into how to address those obstacles while also elucidating the pathways to empower the program. Faculty and student buy-in is fundamental for the success of this program, which often is best addressed by giving visibility to the program itself and its outstanding participants. This article gives an overview of laboratory safety teams (LSTs), a separate graduate student-led laboratory safety initiative, and how an LST can be used to support and even serve as an LSO program. The implementation of a student LSO program has the potential to foster a greater sense of community and safety culture within a department, while giving students the opportunity to enhance their leadership and laboratory skills. This study aims to assess the perceptions of nurses regarding safety culture in primary healthcare settings in Saudi Arabia. A positive safety culture, characterized by trust, communication, transparency, and accountability, plays a significant role in preventing harm and ensuring the well-being of patients. Nurses, as frontline healthcare workers, significantly contribute to maintaining a safety culture through effective communication, adherence to safety protocols, and active participation in continuous training. This study highlights the importance of fostering a safety culture that promotes the reduction of medical errors, supports the well-being of healthcare professionals, and ultimately enhances the quality of care provided to patients. A comprehensive diagnosis of an organization's preparedness for a crisis requires an examination of its climate beyond the influences from the management. This study emphasizes the crucial role employees play in establishing and maintaining a climate that will survive crises by examining the function of promotive and prohibitive voice in fostering a culture of safety. The current study proposed a model in which employee promotive and prohibitive voices impact organizational crisis preparedness through the mediation of organizational safety culture. 329 working adults participated in the study through answering an online questionnaire on MTurk. The data were consistent with the proposed path model, demonstrating the critical role that employee voice plays in creating a culture of safety, which ultimately leaves an organization adequately or inadequately prepared for a potential crisis event. In this article, an attempt is made to indicate the role and importance of ecosystem services as advantages that are provided to us by the environment. The authors demonstrate selected concepts of ecosystem services and their classification, but only to introduce the reader to pilot studies of the urban ecosystem. The present article aims to define the factors that influence the culture of utilizing ecosystem services in urbanized areas. In this article, a fragment of an urban ecosystem, specifically the Old Town of Gdańsk (which features anthropogenic elements and whose functioning is dependent on ecological and natural systems), is observed through a researcher's eye. This area is predominantly focused on using international experiences in interdisciplinary research, a dynamically developing new science discipline called "city ecology."
