Abstract

Knee and hip arthroplasty procedures requiring prosthesis selection are both common and costly. Prosthesis selection is a substantial cost driver and potential source of unwarranted variation for hip and knee arthroplasties. According to the Royal Australasian College of Surgeons (2015), there is substantial variation in cost of hip prosthesis which has previously been reported to range from $4908 to $16, 178 with a median of $10, 727. Additionally, a significant inconsistency has been observed in the cost of prosthesis for knee and hip replacements between public and private health systems in Australia, with orthopaedic prosthesis costs potentially twice as high in the private sector than the public. It is also noteworthy that Australia pays a higher price per prosthesis than other OECD countries. There are early indications in the literature that it is possible to influence prostheses selection choices in ways that promote high value care and good outcomes for patients. However, little is known regarding what factors influence prosthesis selection by surgeons/clinicians. This review aims to provide a list of factors that may influence prosthesis selection by surgeons/clinicians for knee and hip arthroplasties in high income/OECD countries. In order to compose this list, all factors that may influence prosthesis selection will be extracted from the selected literature and divided into overarching categories. For each category, the number of studies that mention this factor will be counted.

Abstract

Objectives: Most of the paediatric quality‐of‐life instruments in oral health research are not preference‐based measures, thus cannot be used in economic evaluations. The Early Childhood Oral Health Impact Scale (ECOHIS) is one such instrument which assesses oral health impact on children's quality of life among three‐ to five‐year‐olds. With increasing demands for more resource allocation in oral health care, there is a need for an outcome metric which can be used as the outcome in economic evaluations. The aim of this study was to develop a preference‐based, health‐state classification system from the existing ECOHIS instrument. Methods: The 13‐item ECOHIS instrument was applied to the carers of 280 preschoolers who participated in a clinical trial in metropolitan Perth, Western Australia. Exploratory factor analysis was conducted to assess the dimensionality of the ECOHIS. Rasch analysis was used to eliminate and select items per determined dimensions and to reduce the number of response options for each item, because six levels were not amenable to valuation. The final classification system was determined through a combination of psychometric and expert advice. Results: Items loaded on to four dimensions with the factor analysis. Using Rasch analysis, three items were selected from the first dimension (child functional and psychological impact) and one item from the second dimension (child social interaction impact). None were selected from the third dimension (parental distress and family function impact) as it is not an appropriate proxy method of measuring the level of impact on a child. The final classification system has four dimensions with three levels in each, and so it has been named the ECOHIS‐4D. Conclusions: The ECOHIS‐4D classification system is a new preference‐based instrument derived from the existing ECOHIS instrument. Future valuation studies will develop a utility value set for the health states defined by this classification system, and this will guide economic evaluations related to oral health interventions. Abstract

To describe some selected characteristics of immune thrombocytopenia (ITP) such as presentation, the disease pattern, treatment options, and response to therapy and side effects to common therapeutic options.A group of 72 patients diagnosed as ITP were studied over a period of 5 years in two main centers in Sri Lanka. Characteristics of the disease such as presentation, the disease pattern, treatment options, and response to therapy and side effects to common therapeutic options were studied.The ages of the patients ranged from 12 to 71 years but the majority (64.2%) were between 20 and 45 years. In 47 (65.3%) the average platelet count at presentation was below 30 × 10(9)/l and in these patients there was a significant increase in bleeding manifestations. Sixty-nine patients were given steroids as the initial treatment and all had responded initially. Time to respond ranged from 3 to 30 days with an average of 11 days. Twenty-four patients developed steroid-related side effects. In 24 patients second-line treatment options such as dapsone, azathioprine, and danazole were given but a satisfactory response to them could not be demonstrated.The heterogeneous behavioral patterns of the disease with patients who are having the same platelet count (30 × 10(9)/l) were observed and this revealed that ITP needs an individualized approach regardless of the platelet count of the patient. A close follow-up of the patients revealed that the patient's understanding of the disease and ability to identify the warning signs resulted in less intervention with drugs which lead to many harmful long-term side effects.
