Variations of the inferior alveolar artery are seemingly quite rare, especially with regard to its origin from the maxillary artery. We present an unusual case of an inferior alveolar artery that originated from the external carotid artery. To the best of our knowledge, our case is one of only two reports of the inferior alveolar artery arising from the external carotid artery. The clinician who deals with the mandibular region should be aware of such a variation in the arterial architecture. [ABSTRACT FROM AUTHOR]

Background: There is still no standard of care to manage thoracolumbar burst fractures. With all the recent advances, posterior approaches are still one of the mainstays of treatment. On the other hand, while spinal canal decompression in neurological impaired patients is an important goal of treatment, its technique remains controversial.This study compared the effects of direct laminectomy decompression against ligamentotaxis/indirect canal decompression on neurological and radiographic improvements., Methods: A prospective double-blind randomized clinical trial was conducted on 60 thoracolumbar burst-fracture patients meeting our inclusion and exclusion criteria. They were randomized into 2 treatment arms: (1) direct decompression using laminectomy and (2) indirect decompression using ligamentotaxis/distraction. Each patient was observed for 6 months, and their neurological and radiographical data were collected prospectively. Statistical analysis was done by the Student t test, Friedman test, Mann Whitney-U test, Wilcoxon ranked test, and 1-way analysis of variance., Results: Among 60 patients enrolled in our study, each treatment arm had an improvement in Frankel scores but there was no difference between the groups at any given time. After 6 months of surgery, local sagittal kyphosis improved in both groups (from 32.2 to 7.43 and 29.93 to 8.77 for the indirect and direct groups, respectively), as well as anterior vertebral height ratio (from 57.73 to 70.7 and 62.17 to 66.27 for the indirect and direct group, respectively) and posterior vertebral height ratio (from 61.17 to 74.87 and 64 to 67.5 for the indirect and direct group, respectively). For between-group comparisons after 6 months, there was a significant difference only for posterior vertebral height ratio ( P = 0.040)., Conclusion: Posterior approaches with ligamentotaxis have shown to be safe and may present the same outcome as direct decompression techniques using wide laminectomy., Competing Interests: The authors declare that they have no competing interests., (© 2023 Iran University of Medical Sciences.)

Introduction and Objectives: Most of the studies evaluating the effect of cross links on spinal stability are performed in vitro on porcine or human spine segments and there is limited data regarding clinical benefits of cross link augmentation in traumatic injuries. In this study we aimed to evaluate the effects of cross-links insertion between rods on the fusion rates and post-surgical patients' satisfaction among patients with traumatic thoracolumbar fractures who underwent posterior spinal fixation with pedicle screws., Materials and Methods: This study was conducted as a randomized clinical trial on 60 patients suffering from traumatic thoracolumbar vertebrae fractures. Patients were randomized into three groups: A (without any cross-link), B (One cross-link insertion) and C (two cross-links insertion). Six months after surgery outcomes were evaluated: fusion rates (plain X-ray and CT scan), Back pain (Visual Analog Scale) and patient satisfaction (fair, good, excellent)., Results: In group A 13 (65%) patients had structured bone fusion, but in 7 (35%) patients bone fusion was not observed. In both groups B and C, 19 patients (95%) had bone fusion, but only in 1 patient (5%) fusion failed (p=0.009). In group A, fair satisfaction has the highest rate (8 patients (40%)) compared to the other groups. The highest reported severity of back pain was observed in group A while the lowest reported intensity of back pain was related to group B (p=0.001)., Conclusions: Adding cross link to posterior spinal fixations of patients with traumatic thoracolumbar fractures can be associated with better final fusion results and patients' satisfaction. Background: Citicoline, a neuroprotective drug, has been suggested to improve level of consciousness, mitigating secondary to brain damage and ectopic vascular calcification, following post-traumatic neurogenesis and angiogenesis, inducing calcification modulators, like fetuin-A and matrix Gla-protein (MGP). This study aimed to investigate effects of citicoline on levels of consciousness, serum levels of fetuin-A and MGP in patients with severe traumatic brain injury., Methods: This double blind randomized controlled trial (RCT) was conducted on patients with diagnosis of diffuse axonal injury (DAI) and GCS≤8. The cases were treated with citicoline (500 mg every 6 hours) intravenously for fifteen days. Daily GCS assessment and intermittent blood sampling were done for both cases and controls., Results: Fifty-eight patients were included in the study and during the study period, mean GCS levels improved in both groups; however, the difference was inconsiderable (p>0.05). Serum levels of fetuin-A, a negative phase reactant, increased in the group treated with citicoline (p=0.012), while these changes were insignificant for the controls (p=0.455). Serum levels of MGP, a calcification inhibitor, increased in the cases (p=0.046). The alterations were inconsequential in the control group (p=0.405)., Conclusion: The findings of this study suggest neutral effects of citicoline on level of consciousness and GCS. Through increasing levels of fetuin-A and MGP, citicoline may have protective effects against inflammatory damage and vascular calcification secondary to head trauma.

Objective: There is paucity of information regarding the specific anatomy and clinical significance of ossified ligaments near the foramen ovale (e.g., pterygospinous and pterygoalar ligaments). The present study was undertaken to define this anatomy in more detail and to review the literature regarding these anatomic variations., Methods: One hundred fifty-four adult human dry skulls were analyzed for the presence of ossified ligaments of pterygospinous (ligament of Civinini) and pterygoalar (ligament of Hyrtl). Measurements were made of these bony structures and observations made of their relationships to the inferior aspect of the foramen ovale and neighboring structures., Results: Two ossifications each (2.6%) of the ligaments of Civinini and Hyrtl were found. One of each of these (1.3%) was completely ossified, thereby resulting in 2 complete foramina (i.e., 1 foramen of Civinini and 1 foramen of Hyrtl). A significant correlation was found between the left and right sides, with either complete or incomplete ossification of these ligaments being found on left sides (75%) (incomplete Civinini on right side and all others on left side). The complete foramen of Civinini was found to have an area of 16.7 mm, and the complete foramen of Hyrtl was found to have an area of 9.42 mm., Conclusion: Such anomalous bony obstructions could interfere with transcutaneous needle placement into the foramen ovale or distort anatomic relationships during approaches to the cranial base.

The diencephalon, upper brain stem and other basal brain structures are supplied chiefly by penetrating branches of the cerebral arteries. We examined the retrochiasmatic space between the superior border of the pons and posterior edge of the optic chiasm in six randomly selected adult fresh brain specimens. Lateral or anterolateral to the mamillary bodies, two small quadrangular spaces (2.5 x 3.5 mm) were found that were limited laterally by the junction of the optic tract and crus cerebri. These spaces were pierced on each side by 1 to 5 small penetrating branches (premamillary arterial complex) of the posterior communicating artery. A single, large and obliquely oriented penetrating branch of the posterior communicating artery (the so-called premamillary, thalamotuberal or mamillothalamic artery) was found to pierce this area in all specimens. Based on our findings, the above-mentioned vessels of this perforating substance supply the floor of third ventricle, hypothalamus and ventral thalamic nuclei. Hence, special attentions should be made during surgery in this area such as third ventriculostomy for hydrocephalus.

Variations of the muscles of the anterior forearm are common. We report the cadaveric findings of an unusual variant of flexor digitorum superficialis (FDS). The deep part of the FDS was found to be split and showed two distinct fusiform muscle bellies. The medial belly originated from the common flexor tendon from the medial condyle of the humerus and continued as a thin tendon at the middle of the forearm to insert onto the fifth digit. The lateral belly blended proximally with the deep surface of the superficial portion of the FDS and ended in a relatively thicker tendon, which was inserted onto the second digit. The median nerve passed between the medial and lateral bellies of the deep part of the FDS. To our knowledge, this is only the second report of a split FDS in the extant literature. A review of neuromuscular variations of the anterior forearm is presented.

Herniation of the hindbrain outside of the setting of intracranial hypertension, trauma, and brain tumors is an uncommon phenomenon with estimated incidence of less than 1%. In the late 1890's, Hans Chiari, a German pathologist, classified hindbrain herniation into three forms. This classification was then extended to include six types. We reviewed the current literature for the proposed embryological theories as well as the potential genetic mutations/syndromes associated with the hindbrain herniation or Chiari malformation. The review is illustrated by a unique cadaver with Chiari type I malformation (i.e. herniation of the cerebellar tonsils through the foramen magnum). Finally, it seems that no single theory could explain all forms of the Chiari malformation, and that this malformation might be a heterogeneous entity.
