52. Insuring Good Health: Outcomes and Acceptability of a Participatory Health Insurance Literacy Intervention in Diverse Urban Communities

Abstract

Background: The U.S. uninsured rate has dropped significantly since the passage of the Affordable Care Act (ACA), yet insurance coverage remains lower in historically marginalized communities than in the overall population. New consumer engagement approaches that involve these populations are needed. The purpose of this study was to evaluate the effectiveness of the Insuring Good Health Intervention, a website and video series designed to improve engagement with health insurance and ACA reforms. The study was designed and implemented using a community-based participatory research approach. Method: We conducted a lagged-control cluster randomized controlled trial to evaluate Insuring Good Health with racially and ethnically diverse adults seeking services in medically underserved areas within the Detroit, Michigan metropolitan area. Outcomes were assessed at baseline and at 6 and 9 months postintervention, and including self-efficacy, knowledge, beliefs, and intention to seek help with insurance navigation and care. Results: Among 243 participants, mean age was 43.4 (SD = 13) years, and all participants met federal guidelines for poverty. The study had an 86% response rate at 9-month follow-up (n = 209). Compared with the lagged-control group, intervention participants had more positive beliefs concerning preventive care (estimate 0.51, standard error 0.16; p < .01), and intention to seek help with insurance navigation and care (estimate 0.43, standard error 0.17; p < .001) at 9-month follow-up. Hispanic participants benefitted the most from the intervention, including improved knowledge of health insurance eligibility over 9 months compared with other racial/ethnic groups (estimate -0.97, standard error 0.40, p < .01). Discussion: Insuring Good Health was associated with increased intention to seek help with insurance navigation and care, and improved beliefs around preventive care. Conclusions: Insuring Good Health has the potential to be an important consumer engagement tool to reduce disparities in health insurance coverage and care seeking.

65. Signal extraction and breakpoint identification for array CGH data using robust state space model

Abstract

Array comparative genomic hybridization(CGH) is a high resolution technique to assess DNA copy number variation. Identifying breakpoints where copy number changes will enhance the understanding of the pathogenesis of human diseases, such as cancers. However, the biological variation and experimental errors contained in array CGH data may lead to false positive identification of breakpoints. We propose a robust state space model for array CGH data analysis. The model consists of two equations: an observation equation and a state equation, in which both the measurement error and evolution error are specified to follow t-distributions with small degrees of freedom. The completely unspecified CGH profiles are estimated by a Markov Chain Monte Carlo(MCMC) algorithm. Breakpoints and outliers are identified by a novel backward selection procedure based on posterior draws of the CGH profiles. Compared to three other popular methods, our method demonstrates several desired features, including false positive rate control, robustness against outliers, and superior power of breakpoint detection. All these properties are illustrated using simulated and real datasets.

Abstract

Mediation analysis aims to assess if, and how, a certain exposure influences an outcome of interest through intermediate variables. This problem has recently gained a surge of attention due to the tremendous need for such analyses in scientific fields. Testing for the mediation effect (ME) is greatly challenged by the fact that the underlying null hypothesis (i.e. the absence of MEs) is composite. Most existing mediation tests are overly conservative and thus underpowered. To overcome this significant methodological hurdle, we develop an adaptive bootstrap testing framework that can accommodate different types of composite null hypotheses in the mediation pathway analysis. Applied to the product of coefficients test and the joint significance test, our adaptive testing procedures provide type I error control under the composite null, resulting in much improved statistical power compared to existing tests. 88. Novel antenna design for future mobile systems

Abstract

User requirements for access to a diversified range of wireless telecommunication services have resulted in a rapid technological push to unify these several different systems on a common platform. The objective of this work is to develop novel antenna design solutions not only for existing systems but also any future generation wireless communication systems. This thesis titled "Novel Antenna Design for Future Mobile Systems" describes the work done for the period from 15th January 1998 to 14th January 2001, under the studentship agreement between British Telecommunications and the University of Birmingham. Three topics are investigated in this thesis. They are: firstly, the family of multiband planar inverted F antenna (PIFA); secondly, the gain enhancement and packaging concepts of electrically small antennas and finally, the family of multiband fractal antennas. In the first topic, two novel PIFA antennas were designed. The first is a triple band antenna where a dual band single feed element using a reactive L shape spur line load is housed within a lower frequency antenna element. The frequency ratio for the reactively loaded dual band single feed antenna is tuneable between 1.28 to over 3.5. The second antenna is designed with two PBFA housed within a quarter wave patch antenna. The antenna is tuned for operation in the GSM, DECT and WLAN bands. The second includes a discussion of a method of integrating electrically small antenna with the transceiver chip on a single package, where the poor efficiency of small integrated antenna can be overcome by the use of a parasitic radiator. Experimental investigation of the characteristics of this parasite, including circular polarisation techniques, are addressed. The final topic is the family of multiband fractal antennas where five novel designs are proposed. The first two designs address feeding solutions for a perturbed Sierpinski gasket monopole antenna, using a microstrip and an angular feed. These techniques enable the closely spaced, tuned bands of the perturbed Sierpinski gasket to be matched without additional matching circuits. The third design is a shorted half sized Sierpinski gasket monopole antenna, similar to the design of the inverted L antenna development. Tuning of bands are also demonstrated by structural perturbation. The fourth design is a set of multiple rings, similar to the multi-level design of the Sierpinski gasket monopole antenna. The bandwidth of each band is well over 40%, which is much larger than the Sierpinski gasket monopole. Finally, a pair of Sierpinski carpet monopoles are fed in parallel and demonstrate a very large impedance bandwidth.
