Abstract

Reduced order modeling has gained considerable attention in recent decades owing to the advantages offered in reduced computational times and multiple solutions for parametric problems. The focus of this manuscript is the application of model order reduction techniques in various engineering and scientific applications including but not limited to mechanical, naval and aeronautical engineering. The focus here is kept limited to computational fluid mechanics and related applications. The advances in the reduced order modeling with proper orthogonal decomposition and reduced basis method are presented as well as a brief discussion of dynamic mode decomposition and also some present advances in the parameter space reduction. Here, an overview of the challenges faced and possible solutions are presented with examples from various problems.

Abstract

Background Atrial fibrillation (AF) promotes atrial remodeling that in turn promotes AF perpetuation. The aim of our study is to investigate the impact of AF history length on 1-year outcome of AF catheter ablation in a cohort of patients enrolled in the Atrial Fibrillation Ablation Registry. Methods We described the real-life clinical epidemiology, therapeutic strategies, and the short- and mid-term outcomes of 1948 patients (71.9% with paroxysmal AF) undergoing AF ablation procedures, stratified according to AF history duration (= 2 years). Results The mean AF history duration was 46.2 +/- 57.4 months, 592 patients had an AF history duration = 2 years (mean 75.5 +/- 63.5 months) (P < 0.001). Patients with AF history duration = 2 years (34.0%) (P = 0.037). AF history duration >= 2 years, overall ablation procedure duration, hypertension, and chronic kidney disease were all predictors of recurrences after the blanking period. Conclusions In this multicenter registry, performing catheter ablation in patients with an AF history >= 2 years was associated with higher rates of AF recurrences at 1 year. Since cumulative time in AF in not necessarily equivalent to AF history, its role remains to be clarified.

Abstract

An integrated investigation including geological, geomorphological, geophysical and structural survey, tephra analyses, 14C and 40Ar/39Ar dating, as well as paleoseismic trenching along the N-Matese fault system is presented. The study allowed the characterization of the tectonic mobility of this structure as well as the associated Bojano basin sedimentary-tectonic evolution since the early Middle Pleistocene, providing also new clues concerning the fault historical activity and the associated Mw > 6.5 earthquakes. We have found lines of evidence for > 1 mm/yr slip rate along the presently buried Bojano fault during the mid Middle Pleistocene, and similar rates for the main fault segments paralleling the Matese flanks. The buried Bojano fault significantly slowed down during the last 300 kyr, ceasing its activity before the Holocene. In turn, the segments outcropping along the Matese flanks reactivated at the onset of Late Pleistocene, after a long period of quiescence (480–110 ka), with robust slip rates that would seem even accelerating in post LGM times. Paleoseismic data suggest the occurrence of four Mw > 6.6 earthquakes in the past 3 ka, three of which match the little known 280 BC event, and the devastating 1456 and 1805 earthquakes. © 2017 Elsevier B.V.

Abstract

Primary ovarian insufficiency (POI) is defined as hypergonadotropic amenorrhea before the age of 40 yr. In 4-5% of patients with POI, an ovarian autoimmune process is present.Serum concentrations of antimüllerian hormone (AMH) have been determined in 26 women with POI due to steroidogenic cell autoimmunity (SCA-POI), 66 with nonautoimmune idiopathic POI (iPOI), 40 postmenopausal women (PMW), and 44 healthy fertile women (HW). SCA-POI was diagnosed according to presence of steroidogenic enzyme autoantibodies (17alpha-hydroxylase, side chain cleavage, and 21-hydroxylase autoantibodies).AMH concentrations were significantly higher in women with SCA-POI than women with iPOI (P = 0.018) or PMW (P = 0.03) but significantly lower than HW (P0.0001). AMH was detected in 11 of 26 women with SCA-POI (42%) and seven of 66 with iPOI (11%) (P = 0.002). Serum concentrations above the fifth percentile of the normal range (0.6 ng/ml) were detected in nine of 26 women with SCA-POI (35%) and four of 66 with iPOI (6%) (P = 0.001). Eight of 12 women with SCA-POI with less than 5 yr (67%) and one of 14 with longer disease duration (7%) had AMH concentrations within the normal range (P = 0.003). AMH concentrations correlated inversely with disease duration in women with SCA-POI (rho = -0.563, P = 0.003) but not women with iPOI. AMH correlated inversely with FSH serum concentrations in HW (rho = -0.584, P0.001) but not PMW or women with POI.Two thirds of women with recent-onset SCA-POI had normal AMH concentrations. Women with SCA-POI, differently from those with iPOI, present a preserved ovarian follicle pool for several years after diagnosis of ovarian insufficiency.

Safety of Catheter Ablation for Atrial Fibrillation. Introduction: Despite catheter ablation (CA) becoming an accepted treatment option for symptomatic, drug-resistant atrial fibrillation (AF), safety of this procedure continues to be cause for concern. Aim of the present multicenter registry was to assess the incidence of early CA complications and detect their predictors in a contemporary, unselected AF population Methods and Results: From January 1, 2011 to December 31, 2011, data from 2,323 consecutive patients who underwent CA (median age 60 [52–67]; 72.3% male) for AF in 29 Italian centers were collected. All major complications occurring to the patient from admission to 30th postprocedural day were recorded. No procedure-related death was observed. Major complications occurred in 94 patients (4.0%): 50 patients (2.2%) suffered vascular access complications; 12 patients (0.5%) developed cardiac tamponade; 14 patients (0.6%) presented with pericarditis; 5 patients (0.2%) had transient ischemic attack; 4 patients had stroke; 3 patients (0.1%) had phrenic nerve paralysis; 3 patients (0.1%) had hemothorax. Other isolated but serious adverse events were documented in 3 patients (0.1%). Female gender (OR 2.643; 95% CI 1.686–4.143; P < 0.0001) and longer procedural duration (OR 2.195; 95% CI 1.388–3.473; P < 0.001) independently predicted a higher risk of complications. Conclusion: Major complications occurred in 4.0% of the CA procedures for AF, with vascular access complications being the most frequent events.

All complications occurring to the patients from admission to 30th post-procedural day were recorded.Complications occurred in 94 patients (4.0%); of these 7 (0.30%) developed permanent sequelae. There was a significant trend toward a greater incidence of complications with increasing age-group. In particular, the incidence of complications was 35/1066 (3.3%) in patients60 year-old vs 59/1257 (4.7%) in those60 year-old (p=0.03). All 7 patients with permanent sequeale were older than 60. Females had a higher incidence of complications both among younger [13/231 (5.6%) vs 22/915 (2.5%), p=0.02] and older patients [32/405 (7.9%) vs 27/739 (3.5%) p=0.001]. In subjects older than 60, 5/405 (1.2%) females and 2/176 (0.3%) males (p=0.04) suffered from permanent sequelae.Older patients and females are a subgroup at higher risk of complications during AF ablation. A particular care should be taken when performing CA in this clinical setting.

Abstract

Patients with idiopathic megarectum (extreme dilatation of the rectum of unknown cause) with or without megacolon (extreme dilation of the colon) may develop symptoms in childhood or adulthood. The major symptoms are abdominal pain, constipation, soiling, and recurrent fecal impaction. Usually medical treatment is successful, but some patients may require surgery. The Duhamel operation, originally developed to treat megacolon that occurs in Hirschsprung's disease, removes the upper rectum and dilated colon, and reconnects the remaining normal segment of bowel at the upper end of the anal canal. A report is presented of the outcome of 20 patients with megarectum and megacolon who underwent the Duhamel operation. Patients were treated over a 17-year period by one surgeon, and there were 14 males and six females. The average age of onset of constipation was three years; the average age at the time of surgery was 25 years. There were many complications soon after surgery; five patients required further surgery for constipation. Most patients had a subjective feeling of improved well-being after surgery, however, soiling, straining, abdominal pain, and distention were common after surgery. Ten patients had improvement in bowel frequency. Given these mixed results, it is recommended that patients with moderately dilated rectum or colon who require surgery undergo colectomy with ileorectal anastomosis.
