55. Community Pop‐Up Clinic: Cascade of Care and HCV Treatment of Vancouver's Inner‐City PWID Populations.

Abstract

ABSTRACT Elimination of HCV infection as a public health concern by the end of this decade will require a concerted effort in all target populations, including drug‐users in the inner‐city. Several strategies have been proposed to identify, engage and provide HCV‐infected residents with antiviral therapy and maximise treatment and cure achievement. This study aims to assess the effectiveness of a multidisciplinary approach in delivering HCV treatment to people who inject drugs (PWID) within Vancouver's inner city. We have evaluated a novel approach, the Community Pop‐Up Clinic, for its ability to promote access to care and uptake of HCV therapy, with additional analyses of HCV reinfection and opioid‐related mortality. From January 2021 to August 2023, we evaluated 1968 individuals. 620 (31.5%) were found to carry HCV antibodies and of these, 474 (76.5%) were found to be viremic. Treatment engagement has been secured in 387 (81.6%). 326 (84.2%) have started treatment, 60 in the pre‐treatment phase and 1 died of an overdose in pre‐treatment. Of 326, 302 completed treatments, 18 are currently on treatment and 1 died of an overdose. Of 302 who completed treatment, 286 confirmed as cured (SVR 12), 16 are awaiting SVR 4, 2 had documented virologic relapse and 1 was reinfected. Three patients withdrew from treatment. By mITT, the cure rate is 286/288 (99.3%). We documented 2 overdose deaths over 326 PY. The data presented validates multidisciplinary programs such as ours aimed at treating HCV in inner‐cities and highlights societal benefits that could be achieved including lower overdose death rates. Abstract

SummaryHumans and yeast are separated by a billion years of evolution, yet their conserved core histones retain central roles in gene regulation. Here, we “reset” yeast to use core human nucleosomes in lieu of their own, an exceedingly rare event which initially took twenty days. The cells adapt, however, by acquiring suppressor mutations in cell-division genes, or by acquiring certain aneuploidy states. Robust growth was also restored by converting five histone residues back to their yeast counterparts. We reveal that humanized nucleosomes in yeast are positioned according to endogenous yeast DNA sequence and chromatin-remodeling network, as judged by a yeast-like nucleosome repeat length. However, human nucleosomes have higher DNA occupancy and reduce RNA content. Adaptation to new biological conditions presented a special challenge for these cells due to slower chromatin remodeling. This humanized yeast poses many fundamental new questions about the nature of chromatin and how it is linked to many cell processes, and provides a platform to study histone variants via yeast epigenome reprogramming.Highlights- Only 1 in 107 yeast survive with fully human nucleosomes, but they rapidly evolve- Nucleosome positioning and nucleosome repeat length is not influenced by histone type- Human nucleosomes remodel slowly and delay yeast environmental adaptation- Human core nucleosomes are more repressive and globally reduce transcription in yeast

Abstract

Australian governments, academia, and law enforcement agencies have recognized the need to improve intelligence capabilities in order to adapt to the increasingly complex criminal and security environments. In response, the Australian Criminal Intelligence Commission (ACIC), the Australian Federal Police (AFP) and other Australian policing agencies have adopted several reform measures to improve intelligence capability support. While some have focused on developing specific criminal intelligence doctrine, others have sought to improve more challenging aspects of intelligence capability such as analytical and field collection workforce planning. The complexity of the current and emerging criminal environment and a growing professionalization of policing practice more broadly has resulted in a uniquely new strategic approach to developing the analytical and field collection workforce. This article surveys the development of an Australian Criminal Intel Training and Development Continuum (CITDC). The continuum is an end-to-end continuing professional development framework for criminal intelligence analysts and field intelligence officers that monitor proficiency, competence, and knowledge achievement through pre-entry aptitude testing, rigorous class room, and workplace mentoring. The continuum is designed at the post-graduate level and articulates with Charles Sturt University's MA (Intelligence Analysis). The article argues that both the philosophy of rigorous standards and the learning underpinning the continuum are having demonstrable and positive outcomes for intelligence practitioners and the investigative workforce they support. Abstract

Calciphylaxis is a rare and potentially fatal disease that affects the subcutaneous layer of the skin. It is a calcific vasculopathy induced by a systemic process that causes occlusion of small blood vessels. The mortality rate for individuals diagnosed with calciphylaxis is estimated between 52% and 81% with sepsis being the leading cause of death. Uraemic calciphylaxis and its known effective treatments are well documented in the literature. Unfortunately, there is no known effective treatment for non‐uraemic calciphylaxis. Most of the current treatments for non‐uraemic calciphylaxis are derived from uraemic calciphylaxis treatment protocols. We report a case of a 75‐year‐old female with calciphylaxis on the right lower extremity who was successfully treated with four pamidronate infusions in addition to local wound care. This case represents a non‐uraemic calciphylaxis wound successfully treated with pamidronate infusions and standard wound care, and suggests that IV pamidronate can be an effective treatment option.
