Abstract

Le rivaroxaban (Xarelto®) fait partie des nouveaux anticoagulants oraux. Il présente de nombreux avantages potentiels par rapport aux anti-vitamines K. Il possède un effet anticoagulant prévisible et ne nécessite pas à priori de monitoring biologique régulier. Il se caractérise également par un profil d’interactions médicamenteuses et alimentaires nettement plus favorable. Néanmoins, en raison des risques majeurs associés à des sur- ou sous-dosages de ces produits, leur utilisation optimale par les patients est indispensable. L’objectif de cet article est de proposer au pharmacien des recommandations concises sur l’utilisation pratique du Xarelto® quelle que soit son indication. Ce document est adapté du guide d’utilisation pratique du rivaroxaban qui a fait l’objet d’un consensus d’un groupe indépendant de spécialistes impliqués dans le domaine de la thrombose et de l’hémostase au niveau belge

52. Perioperative fluid balance in children: an observational study for elective surgery in a daycare setting.

Abstract

Introduction The Holliday's and Segar's 4-2-1 ml/kg/h rule remains the commonly used formula for calculating the volume of perioperative fluid maintenance in children. In this observational study, our goal is to determine differences between the administered amount of fluid during elective surgery and the current guidelines. Material and methods Children (0-6 years) undergoing elective surgery for urologic or otolaryngologic procedures with need for an IV-line placement, were enrolled between February and April 2017. At start of the procedure, the fasting period and body weight was assessed to calculate the theoretical IV fluid. At discharge of the recovery, the amount of the given fluid was determined. PAED scores (pediatric anesthesia emergence delirium scores) were taken at emergence of anesthesia and at dismissal out of the hospital. Other outcomes like nausea and vomiting were recorded throughout the process. Results 121 children were included during this period. They had an average fasting period of 13 hours. The total amount of fluids (375 ml; IQR: 256-500) given to children was less compared to the theoretical amount of fluids (795ml; IQR: 625-920) (p<0.0001). Moreover, Bland-Altman analysis revealed that the difference between the theoretical and the given IV fluid increases with increasing average values. No differences were observed for the total amount of IV fluids depending on body weight or PAED score. None of the patients demonstrated signs of nausea. Conclusions We conclude an underestimation of the IV fluid balance in comparison with the theoretical calculated IV fluid according to the current guidelines. [ABSTRACT FROM AUTHOR]

Abstract

Background: Contrast‐enhanced ultrasound examination (CEUS) is a functional imaging technique allowing noninvasive assessment of tissue perfusion. Studies in humans show that the technique holds great potential to be used in the diagnosis of chronic kidney disease (CKD). However, data in veterinary medicine are currently lacking. Objectives: To evaluate renal perfusion using CEUS in cats with CKD. Animals: Fourteen client‐owned cats with CKD and 43 healthy control cats. Methods: Prospective case‐controlled clinical trial using CEUS to evaluate renal perfusion in cats with CKD compared to healthy control cats. Time‐intensity curves were created, and perfusion parameters were calculated using off‐line software. A linear mixed model was used to examine differences between perfusion parameters of cats with CKD and healthy cats. Results: In cats with CKD, longer time to peak and shorter mean transit times were observed for the renal cortex. In contrast, a shorter time to peak and rise time were seen for the renal medulla. The findings for the renal cortex indicate decreased blood velocity and shorter total duration of enhancement, likely caused by increased vascular resistance in CKD. Increased blood velocity in the renal medulla has not been described before and may be because of a different response to regulatory factors in cortex and medulla. Conclusions and Clinical Importance: Contrast‐enhanced ultrasound examination was capable of detecting perfusion changes in cats with CKD. Further research is warranted to assess the diagnostic capabilities of CEUS in early stage of the disease process. [ABSTRACT FROM AUTHOR]

Abstract

Because of the concurrence, in members of one family, of different autoimmune disorders [rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), psoriasis (PS), and inflammatory bowel disease (IBD)], we investigated the genotypes of each member and compared the results with current knowledge of genetic inheritance of rheumatic and other autoimmune diseases.Sera and peripheral blood lymphocytes of 16 family members were collected for HLA typing and investigation.All patients with RA were DR4 positive ad 4 of 5 showed the maternal A2B51DR4 haplotype. Two of 3 siblings with the genotype HLA-A2/3, B8/35, DR2/3 had SLE. Patients with IBD and psoriasis shared the haplotype HLA-A3, B35 CW4 DR2.The results show that the important role of patients, sex and confirm the association between HLA haplotype and RA or SLE. They support the hypothesis that autoimmunity is a mendelian dominant trait, and that secondary genes, including these of the major histocompatibility complex, confer phenotypic specificity.
