Abstract

AIM: To identify the risk factors for postoperative proliferative vitreoretinopathy (PVR) in patients with primary rhegmatogenous retinal detachment (RRD) and develop a nomogram for predicting postoperative PVR-free probability. METHODS: A total of 741 patients (741 eyes) diagnosed with primary RRD who underwent first surgery in the same hospital were retrospectively reviewed and randomly assigned with 521 to the training set and 220 to the validation set. Univariate and multivariate logistic regression analyses were performed in the training cohort to determine risk factors to construct a nomogram for predicting the 3-, 4-, 5-, and 6-month postoperative PVR-free probabilities. Nomogram performance was estimated by the concordance index (C-index), calibration plot, and the area receiver operating characteristic (ROC) curve. RESULTS: A nomogram was constructed based on the preoperative PVR, silicone oil tamponade time (SOTT), photocoagulation energy (PE), retinal tear size (RTS), and hypertension. In the training set, the C-index of the nomogram was 0.896, 0.936, 0.961, and 0.972 at 3, 4, 5, and 6mo, respectively. The C-index values in the validation set were 0.860, 0.936, 0.951, and 0.965 at 3, 4, 5, and 6mo, respectively. Decision-curve analysis indicated that only the 4-, 5-, and 6-month nomograms had significant net benefits over a large threshold probabilities interval. CONCLUSION: Preoperative PVR, SOTT, PE, RTS, and hypertension are significant risk factors for postoperative PVR formation in patients with primary RRD. The proposed nomogram can effectively predict the 4-, 5-, and 6-month PVR-free probabilities after surgery and assist in making clinical decisions during follow-up.

75. KlothoNull Mutation Involvement in Adenosine A2B Receptor–Related Skeletal Muscle Degeneration

Abstract

Klothois known for its age-suppressing function and has been implicated in sarcopenia pathology. It has recently been proposed that the adenosine A2B receptor plays a crucial role in skeletal muscle energy expenditure. However, the association between Klothoand A2B remains elusive. In this study, Klothoknockout mice, aged 10 weeks, and wild-type mice, aged 10 and 64 weeks, were used for comparison in indicators of sarcopenia (n= 6 for each group). PCR was performed to confirm the mice genotypes. Skeletal muscle sections were analyzed using hematoxylin and eosin staining as well as immunohistochemistry staining. The skeletal muscle cross-sectional area was significantly reduced in Klothoknockout mice and wild-type mice, aged 64 weeks, when compared with wild-type mice, aged 10 weeks, with a decreased percentage of type IIa and IIb myofibers. Likely impaired regenerative capacity, as reflected by the reduction of paired box 7 (Pax7)– and myogenic differentiation protein 1 (MyoD)–positive cells, was also observed in Klothoknockout mice and aged wild-type mice. 8-Hydroxy-2-deoxyguanosine expression was enhanced with Klothoknockout and aging, indicating higher oxidative stress. Adenosine A2B signaling was impaired, with a lower expression of the A2B receptor and the cAMP-response element binding protein in Klothoknockout and aged mice. The present study provides the novel finding that sarcopenia involves adenosine signaling under the influence of Klothoknockout.

76. Liver-function parameters are associated with incident hypertension in a large Taiwanese population follow-up study

Abstract

Previous studies demonstrated inconsistent results regarding the association between liver function and hypertension. In addition, large cohort follow-up studies are lacking. Therefore, this longitudinal study aimed to investigate the association between liver function and incident hypertension using data from the Taiwan Biobank (TWB). We evaluated liver biomarkers, including aspartate aminotransferase (AST), alanine aminotransferase (ALT), albumin, alpha-fetoprotein (AFP), total bilirubin, and gamma-glutamyl transferase (GGT) in this study. A total of 21,293 participants without hypertension at baseline were analyzed. During the mean 3.9-year follow-up, 3002 participants developed hypertension (defined as incident hypertension). Multivariable analysis revealed that high AST (odds ratio [OR], 1.004; 95% confidence interval [CI], 1.001–1.007; p= 0.014), high ALT (OR, 1.004; 95% CI, 1.002–1.006; p< 0.001), high albumin (OR, 1.897; 95% CI, 1.573–2.286; p< 0.001), and high GGT (OR, 1.004; 95% CI, 1.003–1.005; p< 0.001) were significantly associated with incident hypertension in all study participants. In subgroup analysis of the participants with an ALT level ≤2 times the normal limit (80 u/l) (n= 20,983), multivariable analysis demonstrated that high ALT (OR, 1.009; 95% CI, 1.005–1.012; p< 0.001) and high GGT (OR, 1.005; 95% CI, 1.003–1.006; p< 0.001) were significantly associated with incident hypertension. In conclusion, we found that elevated AST, ALT, albumin, and GGT were associated with incident hypertension in a large Taiwanese cohort. A greater understanding of potential risk factors for hypertension may help to reduce the burden of hypertension in this Taiwanese population.

89. Effect of FADS1rs174556 Genotype on Polyunsaturated Fatty Acid Status: A Systematic Review and Meta-Analysis

Abstract

PUFA status is highly implicated in cognitive development and metabolic disorder-related diseases. Genetic variants of FADSgenes encoding enzymes that catalyze the rate-limiting steps of PUFA biosynthesis appear to be associated with n-3 and n-6 PUFA contents. Therefore, we conducted the first systematic review and meta-analysis to explore the association of the A-allele carriers of the FADS1rs174556 with PUFA status. The PRISMA guidelines were followed. The literature search was conducted up to November 2022 in PubMed, Web of Science, Embase, Cochrane Library, Airiti Library, and CINAHL. The Joanna Briggs Institute checklists were used to assess the methodological quality. The correlation with 95% CIs was determined by a random-effect meta-analysis. Eleven studies that met the inclusion criteria and acceptable quality were included in this systematic review. The data on PUFA contents were collected when they were mainly analyzed using blood samples and breast milk. Results of the meta-analysis on eight studies (one randomized controlled trial, one cohort study, and six cross-sectional studies) showed that the A-allele carriers of rs174556 were significantly negatively correlated with the concentrations of AA (P= 0.001), EPA (P= 0.004), and DHA (P= 0.025). However, ALA and LA were not associated with the A-allele carriers. To clarify the discrepancy, we further divided the studies into blood samples and breast milk subgroups. The subgroup analysis revealed that the A-allele carriers of rs174556 were significantly positively correlated with LA (P= 0.031) and negatively correlated with AA (P= 0.001), EPA (P= 0.036), and DHA (P< 0.001) in the blood sample group, but not in the breast milk group. The current meta-analysis proved that the A-allele carriers of the FADS1rs174556 appeared to be highly associated with lower concentrations of AA, EPA, and DHA but higher LA in the blood samples. Abstract

The GM5 game is designed to help children overcome the challenges of computational thinking and decision making. Thinking, calculation, and finding ways to avoid defeat by opponents can be deeply systemically configured in the minds of students through step-by-step analysis. There are five time-limited steps in this game: Selecting cards, analysing card combinations' risk vectors, assessing opponents, planning allocations, and tricking opponents. The game's chain reaction of logic analysis, mathematical operations, and strategic/tactical moves is quite complicated. As a result, this game has the potential to significantly improve the player's analytical strategic thinking and decision-making abilities. Nonetheless, a systematic learning scheme and assistance tools are required to achieve more profound educational goals. As a result, this research develops an AI program to assist students to continuously rethink and improve their game strategies in a reflexive loop and adopts a 5-E learning cycle for students to develop systemic game-playing abilities, experiences and mindsets. For those teachers who want to train students to learn independently, this will be a very good teaching method.
