The pathophysiology of right heart function is complex, as right ventricular (RV) function is easily affected by preload and afterload. Purpose To test the hypothesis that machine learning would detect heterogeneity in right heart function and improve risk stratifications in a heart failure population Methods This study included 403 heart failure patients who had a history of hospitalization for decompensation. Patients were excluded if they had primary heart valve disease or pericardial disease or a previous cardiac surgery. Hierarchical clustering was undertaken on right heart variables (RV strain, right ventricular systolic pressure (RVSP), vena contracta of tricuspid regurgitation (TR) and diameter of inferior vena cava) to identify homogenous groups of patients with similar profiles of the variables. Cox hazard analysis was used to elucidate the benefit of clustering over each variable for prognosticating heart failure. Endpoint was hospitalization for worsening heart failure. Results Cluster analysis identified three groups with distinct right heart function. Cluster 1 (n = 191) represented patients with preserved RV function and low RVSP (figure A). On the other hand, cluster 2 (n = 144) had reduced RV function and low RVSP, while cluster 3 (n = 68) had preserved RV function and high RVSP, associated with severe TR and high central venous pressure. The latter 2 clusters carried worse outcome than cluster 1 (p Conclusion Cluster analysis identified two distinct phenotypes of right heart failure that were associated with adverse outcomes. This data-driven phenotyping can help in categorizing right heart failure and better prognosticating heart failure. Abstract Figure.

To reach fusion conditions and control the plasma configuration in ITER, the next step in tokamak fusion research, two neutral beam injectors (NBIs) will supply 16.5 MW each, by neutralizing accelerated negative hydrogen or deuterium ions. The requirements of ITER NBIs (40A/1 MeV D- ions for ≤1 h, 46A/870 keV H- ions for ≤1000 s) have never been simultaneously attained. So in the Neutral Beam Test Facility (NBTF, Consorzio RFX, Italy) the operation of the full-scale ITER NBI prototype (MITICA) will be tested and optimised up to full performances, focussing on accelerator (including voltage holding), beam optics, neutralisation, residual ion removal. The NBTF includes also the full-scale prototype of the ITER NBI source with 100 keV particle energy (SPIDER), for early investigation of: negative ion production and extraction, source uniformity, negative ion current density and beam optics. This paper will describe the main results of the first two years of SPIDER operation, devoted to characterizing plasma and beam parameters, including investigation of RF-plasma coupling efficiency and magnetic filter field effectiveness in reducing co-extracted electrons. SPIDER is progressing towards the first caesium injection, which aims at increasing the negative ion density. A major shutdown, planned for 2021, to solve the issues identified during the operation and to carry out programmed modifications, will be outlined. The installation of each MITICA power supply and auxiliary system is completed; in-vessel mechanical components are under procurement by Fusion for Energy (F4E). Integration, commissioning and test of the power supplies, procured by F4E and QST, as the Japanese Domestic Agency (JADA), will be presented. In particular, 1.0MV insulating tests were carried out step-by-step and successfully completed. In 2020 integrated tests of the power supplies on the accelerator dummy load started, including the assessment of their resilience to accelerator grid breakdowns using a short-circuit device located in vacuum. The aggressive programme, to validate the NBI design at NBTF and to meet ITER schedule (requiring NBIs in operation in 2032), will be outlined. Unfortunately, in 2020 the coronavirus disease infection affected the NBTF activities. A solution to proceed with integrated power tests despite the coronavirus is presented.

The objective of this study is to clarify the effects of web position on the shear resistance mechanism of Ultra High Strength Fiber Reinforced Concrete (UFC) beams. Four-point bending tests were conducted on total three beams with different web positions, i.e. centered web, two webs arranged at the quarter of the flange and two webs at the edge of the flange. The results revealed the structural performances such as shear capacity, flexural rigidity, displacement at the shear failure and post peak behavior differed in each specimen. The different structural performances can be explained by the difference in the area resisting the shear force, which is determined by the existence of flange overhang and whether webs were distributed in the flange.

We report searches for B0→invisible and B0→invisible+γ decays, where the energy of the photon is required to be larger than 0.5 GeV. These results are obtained from a 711 fb-1 data sample that contains 772×106BB¯ pairs and was collected near the ϒ(4S) resonance with the Belle detector at the KEKB e+e- collider. We observe no significant signal for either decay and set upper limits on their branching fractions at 90% confidence level of B(B0→invisible)

To compare the radiation dose, diagnostic accuracy, and the resultant ablation procedures using 80 and 120-kVp cardiac computed tomography angiography (CCTA) protocols with the same contrast-to-noise ratio in patients scheduled for atrial fibrillation (AF) ablation.This retrospective study was performed following institutional review board approval. We divided 140 consecutive patients who had undergone CCTA using a 64-MDCT scanner into two equal groups. Standard deviation (SD) of the CT number was set at 25 Hounsfield units (HU) for the 120-kVp protocol. To facilitate a reduction in radiation dose it was set at 40 HU for the 80 kVp protocol. We compared the two protocols with respect to the radiation dose, the diagnostic accuracy for detecting left atrial appendage (LAA) thrombi, matching for surface registration, and the resultant ablation procedures.At 120 kVp, the dose length product (DLP) was 2.2 times that at 80 kVp (1269.0 vs 559.0 mGy cm, p 0.01). The diagnostic accuracy for thrombus detection was 100% using both protocols. There was no difference between the two protocols with respect to matching for surface registration. The protocols did not differ with respect to the subsequent time required for the ablation procedures and the ablation fluoroscopy time, and the radiation dose (p = 0.54, 0.33, and 0.32, respectively).For the same CNR, the DLP at 80 kVp (559.0 mGy cm) was 56% of that delivered at 120 kVp (1269.0 mGy cm). There was no reduction in diagnostic accuracy.Maintaining CNR allows for a reduction in the radiation dose without reducing the image quality.

Objectives Plasma atrial natriuretic peptide (ANP) levels have been reported to be elevated in cats with cardiomyopathy. We investigated the diagnostic accuracy of plasma ANP concentration as an indicator of the severity of cardiomyopathies. Animals This study included 78 control cats and 83 cats with various types of cardiomyopathy. Methods This was a prospective multicentre study. Control cats were determined to have a normal heart, and diseased cats were diagnosed by echocardiography. Diseased cats were divided into asymptomatic cats without left atrial dilation (LAD), asymptomatic cats with LAD, and cats with heart failure. Plasma C-terminal ANP concentrations were measured using chemiluminescence. Results The median plasma ANP concentration in controls was 43.3 (interquartile range, 33.0–56.3) pg/mL. Plasma ANP values were significantly higher in the cardiomyopathic cats with LAD and heart failure, but the values in cats without LAD were comparable to those in control cats. To distinguish cats with cardiomyopathy from controls, a plasma ANP concentration >77.5 pg/mL afforded sensitivity of 66.3% and specificity of 84.6%. Use of plasma ANP concentration >110.9 pg/mL to identify cats with LAD had a sensitivity of 73.6% and specificity of 93.5%. The areas under the receiver-operating characteristic curve were 0.80 and 0.86. Conclusions Plasma ANP concentrations were higher in cats with more advanced cardiomyopathy. Although assaying the ANP concentration alone may not help to diagnose cardiac disease, measuring provides additional information that is useful for assessing the severity of cardiomyopathies.

This study clarified the structure in which virtual social capital (SC) affects the quality of life (QOL) and promotes urban resilience in a metropolitan area. It has been determined that assistance from neighboring residents and acquaintances is essential for community resilience and is considered one of the critical factors in urban systems' resilience. These social ties among residents are called SC. In today's metropolises, social ties with others are shifting from face-to-face relationships to virtual relationships. In this study, virtual SC is defined as a weak virtual network formed among people through social media, email, telephones, letters, which is less overlapped with networks formed in face-to-face networks. Also, direct communication online positively affects psychological and personal well-being (QOL). The case study in this paper examines the Tokyo metropolitan region and Bangkok metropolitan region. A web-based survey was administered among those in the younger generation who communicate daily using online tools, and 193 valid responses were collected. Structural Equation Modeling was used to analyze. This study's hypothesis model is the structure that virtual SC affects urban resilience through the quality of life. The difference in structure was identified as both ordinary time and the period of emergency declaration. Virtual SC affected urban resilience through QOL during ordinary times. On the other hand, Virtual SC directly worked towards urban resilience during the period of an emergency declaration. Moreover, the use of media with bridging actors forms rich virtual SC and improves urban resilience. Finally, some recommendations of virtual SC use for urban resilience in a metropolitan area were discussed.

Infection during pregnancy is common and the developing fetal brain is vulnerable to vertical transmission due to immaturity of the fetal immune system. Infection is a major cause of multiple organ abnormalities, including the neuraxis, due to the neurotropism of the infectious agents. This review sets out to give an overview of fetal infection, review the general principles of the nature and timing of the infectious insult with respect to outcomes, review the neuroimaging of infection by ultrasound and magnetic resonance imaging (MRI), and review the various pathogens involved, including the two most common, cytomegalovirus (CMV) and Toxoplasma, and also other common viral and nonviral infections.

We present the first measurement of the Michel parameters η ‾ and ξκ in the radiative leptonic decay of the τ lepton using 703 fb-1 of data collected with the Belle detector at the KEKB e + e − collider. The Michel parameters are measured by an unbinned maximum likelihood fit to the kinematic information of e + e − → τ + τ − → ( π + π 0 ν ‾ ) ( l − ν ν ‾ γ ) ( l = e or μ ) . The preliminary values of the measured Michel parameters are η ‾ = − 2.0 ± 1.5 ± 0.8 and ξ κ = 0.6 ± 0.4 ± 0.2 , where the first error is statistical and the second is systematic.

The surface of a painted steel material is periodically repainted to maintain its corrosion resistance by removing the rust and remaining paint films on the steel sheet using surface preparation. However, there are several unknown factors governing the relation between the surface state of steel sheet and its corrosion resistance. This study aims to investigate the effect of surface preparation on the corrosion resistance of a steel sheet. Herein, three types of steel sheets treated with different surface preparations were painted and their corrosion resistances were investigated by combined cyclic corrosion tests. The differences in the corrosion resistance among the painted sheets were investigated from the viewpoint of the surface free energy. Results show that the corrosion resistance of a painted steel sheet prepared using a zinc-plated steel wheel wire brush is better than those for a steel wheel wire brush and nickel-plated steel wheel wire brush. This finding suggests that the difference in the corrosion resistance based on the plated metal arises from the polar components at the steel surface.

Background/Introduction Oral anticoagulants are widely used for the treatment and second prevention of venous thromboembolism (VTE) and stroke prevention in atrial fibrillation (AF). VTE and AF are common diseases and these sometimes might coexist. However, there are few reports about the relationship between VTE and AF. Purpose We sought to evaluate the clinical characteristics and outcomes in VTE patients with AF. Methods The COMMAND VTE Registry is a multicenter registry enrolling consecutive 3027 patients with acute symptomatic VTE objectively confirmed by imaging examination or by autopsy among 29 centers in Japan between January 2010 and August 2014. The current study population consisted of 129 patients with AF (AF group) and 2898 patients without AF (non-AF group). We compared the clinical characteristics, management strategies and long-term outcomes between the 2 groups. Results The AF group was older (mean age: 75.3 vs. 66.8 years, P The outcomes of VTE patients with AF Conclusions The risks for recurrent VTE between patients with AF and those without AF were not significantly different, although patients with AF received longer-term anticoagulation therapy, whereas the risks for major bleeding tended to be higher in patients with AF. This paper presents the models for several components needed for GaN IC design. MIT virtual source GaNFET (MVSG) model works very well in modelling the high-voltage p-GaN gate power HEMTs and low-voltage GaN logic transistors. The 2DEG resistor can be modelled either using MVSG model or empirical spice model. Low ohmic resistance can be simply modelled with empirical equation. Circuit simulations and measurements demonstrate the convergence and accuracy of the models.

InAlN-MIS-High Electron Mobility Transistors (HEMTs) with 90 nm gate length were fabricated on a Si substrate. The devices, with pad electrodes, exhibited on-resistance of $1.38\ \Omega \mathrm{mm}$ , contact resistance of $0.35\ \Omega \mathrm{mm}$ , transconductance of 460 mS/mm, and fT/fmax of 100/245 GHz when biased at $\mathrm{Vd} =10\mathrm{V}$ . In addition, the MIS structures reduced the forward current by 2-digits in comparison to MES structures, which were fabricated on the same wafer.

Understanding and modeling the large-scale transport of trace gases and aerosols is important for interpreting past (and projecting future) changes in atmospheric composition. Here we show that there are large differences in the global-scale atmospheric transport properties among the models participating in the IGAC SPARC Chemistry–Climate Model Initiative (CCMI). Specifically, we find up to 40 % differences in the transport timescales connecting the Northern Hemisphere (NH) midlatitude surface to the Arctic and to Southern Hemisphere high latitudes, where the mean age ranges between 1.7 and 2.6 years. We show that these differences are related to large differences in vertical transport among the simulations, in particular to differences in parameterized convection over the oceans. While stronger convection over NH midlatitudes is associated with slower transport to the Arctic, stronger convection in the tropics and subtropics is associated with faster interhemispheric transport. We also show that the differences among simulations constrained with fields derived from the same reanalysis products are as large as (and in some cases larger than) the differences among free-running simulations, most likely due to larger differences in parameterized convection. Our results indicate that care must be taken when using simulations constrained with analyzed winds to interpret the influence of meteorology on tropospheric composition.
