Purpose: The amount of coronary artery calcification (CAC) is a strong and independent predictor of cardiovascular disease (CVD) events. In clinical practice, CAC is manually identified and automatically quantified in cardiac CT using commercially available software. This is a tedious and time-consuming process in large-scale studies. Therefore, a number of automatic methods that require no interaction and semiautomatic methods that require very limited interaction for the identification of CAC in cardiac CT have been proposed. Thus far, a comparison of their performance has been lacking. The objective of this study was to perform an independent evaluation of (semi)automatic methods for CAC scoring in cardiac CT using a publicly available standardized framework. Methods: Cardiac CT exams of 72 patients distributed over four CVD risk categories were provided for (semi)automatic CAC scoring. Each exam consisted of a noncontrast-enhanced calcium scoring CT (CSCT) and a corresponding coronary CT angiography (CCTA) scan. The exams were acquired in four different hospitals using state-of-the-art equipment from four major CT scanner vendors. The data were divided into 32 training exams and 40 test exams. A reference standard for CAC in CSCT was defined by consensus of two experts following a clinical protocol. The framework organizers evaluated the performance of (semi)automatic methods on test CSCT scans, per lesion, artery, and patient. Results: Five (semi)automatic methods were evaluated. Four methods used both CSCT and CCTA to identify CAC, and one method used only CSCT. The evaluated methods correctly detected between 52% and 94% of CAC lesions with positive predictive values between 65% and 96%. Lesions in distal coronary arteries were most commonly missed and aortic calcifications close to the coronary ostia were the most common false positive errors. The majority (between 88% and 98%) of correctly identified CAC lesions were assigned to the correct artery. Linearly weighted Cohen's kappa for patient CVD risk categorization by the evaluated methods ranged from 0.80 to 1.00. Conclusions: A publicly available standardized framework for the evaluation of (semi)automatic methods for CAC identification in cardiac CT is described. An evaluation of five (semi)automatic methods within this framework shows that automatic per patient CVD risk categorization is feasible. Robust and fast solutions for anatomical object detection and segmentation support the entire clinical workflow from diagnosis, patient stratification, therapy planning, intervention and follow-up. Current state-of-the-art techniques for parsing volumetric medical image data are typically based on machine learning methods that exploit large annotated image databases. Two main challenges need to be addressed, these are the efficiency in scanning high-dimensional parametric spaces and the need for representative image features which require significant efforts of manual engineering. We propose a pipeline for object detection and segmentation in the context of volumetric image parsing, solving a two-step learning problem: anatomical pose estimation and boundary delineation. For this task we introduce Marginal Space Deep Learning (MSDL), a novel framework exploiting both the strengths of efficient object parametrization in hierarchical marginal spaces and the automated feature design of Deep Learning (DL) network architectures. In the 3D context, the application of deep learning systems is limited by the very high complexity of the parametrization. More specifically 9 parameters are necessary to describe a restricted affine transformation in 3D, resulting in a prohibitive amount of billions of scanning hypotheses. The mechanism of marginal space learning provides excellent run-time performance by learning classifiers in clustered, high-probability regions in spaces of gradually increasing dimensionality. To further increase computational efficiency and robustness, in our system we learn sparse adaptive data sampling patterns that automatically capture the structure of the input. Given the object localization, we propose a DL-based active shape model to estimate the non-rigid object boundary. Experimental results are presented on the aortic valve in ultrasound using an extensive dataset of 2891 volumes from 869 patients, showing significant improvements of up to 45.2% over the state-of-the-art. Accurate segmentation of the spinal canals in computed tomography (CT) images is an important task in many related studies. In this paper, we propose an automatic segmentation method and apply it to our highly challenging image cohort that is acquired from multiple clinical sites and from the CT channel of the PET-CT scans. To this end, we adapt the interactive random-walk solvers to be a fully automatic cascaded pipeline. The automatic segmentation pipeline is initialized with robust voxelwise classification using Haar-like features and probabilistic boosting tree. Then, the topology of the spinal canal is extracted from the tentative segmentation and further refined for the subsequent random-walk solver. In particular, the refined topology leads to improved seeding voxels or boundary conditions, which allow the subsequent random-walk solver to improve the segmentation result. Therefore, by iteratively refining the spinal canal topology and cascading the random-walk solvers, satisfactory segmentation results can be acquired within only a few iterations, even for cases with scoliosis, bone fractures and lesions. Our experiments validate the capability of the proposed method with promising segmentation performance, even though the resolution and the contrast of our dataset with 110 patient cases (90 for testing and 20 for training) are low and various bone pathologies occur frequently. Knowledge of left atrial (LA) anatomy is important for atrial fibrillation ablation guidance, fibrosis quantification and biophysical modelling. Segmentation of the LA from Magnetic Resonance Imaging (MRI) and Computed Tomography (CT) images is a complex problem. This manuscript presents a benchmark to evaluate algorithms that address LA segmentation. The datasets, ground truth and evaluation code have been made publicly available through the http://www.cardiacatlas.org website. This manuscript also reports the results of the Left Atrial Segmentation Challenge (LASC) carried out at the STACOM’13 workshop, in conjunction with MICCAI’13. Thirty CT and 30 MRI datasets were provided to participants for segmentation. Each participant segmented the LA including a short part of the LA appendage trunk and proximal sections of the pulmonary veins (PVs). We present results for nine algorithms for CT and eight algorithms for MRI. Results showed that methodologies combining statistical models with region growing approaches were the most appropriate to handle the proposed task. The ground truth and automatic segmentations were standardised to reduce the influence of inconsistently defined regions (e.g., mitral plane, PVs end points, LA appendage). This standardisation framework, which is a contribution of this work, can be used to label and further analyse anatomical regions of the LA. By performing the standardisation directly on the left atrial surface, we can process multiple input data, including meshes exported from different electroanatomical mapping systems. Purpose: Coarctation of the aorta (CoA) is a congenital heart disease characterized by an abnormal narrowing of the proximal descending aorta. Severity of this pathology is quantified by the blood pressure drop (ΔP) across the stenotic coarctation lesion. In order to evaluate the physiological significance of the preoperative coarctation and to assess the postoperative results, the hemodynamic analysis is routinely performed by measuring the ΔP across the coarctation site via invasive cardiac catheterization. The focus of this work is to present an alternative, noninvasive measurement of blood pressure drop ΔP through the introduction of a fast, image-based workflow for personalized computational modeling of the CoA hemodynamics. Methods: The authors propose an end-to-end system comprised of shape and computational models, their personalization setup using MR imaging, and a fast, noninvasive method based on computational fluid dynamics (CFD) to estimate the pre- and postoperative hemodynamics for coarctation patients. A virtual treatment method is investigated to assess the predictive power of our approach. Results: Automatic thoracic aorta segmentation was applied on a population of 212 3D MR volumes, with mean symmetric point-to-mesh error of 3.00± 1.58 mm and average computation time of 8 s. Through quantitative evaluation of 6 CoA patients, good agreement between computed blood pressure drop and catheter measurements is shown: average differences are 2.38 ± 0.82 mm Hg (pre-), 1.10± 0.63 mm Hg (postoperative), and 4.99 ± 3.00 mm Hg (virtual stenting), respectively. Conclusions: The complete workflow is realized in a fast, mostly-automated system that is integrable in the clinical setting. To the best of our knowledge, this is the first time that three different settings (preoperative--severity assessment, poststenting--follow-up, and virtual stenting-- treatment outcome prediction) of CoA are investigated on multiple subjects. • We propose an automatic end-to-end topology and width aware network for artery/vein classification, which, for the first time, integrates topology and vessel width information into the deep learning framework to boost the A/V classification performance. • A topology-aware module is proposed to increase the topological connectivity of the segmented artery and vein maps, which contains a topology-ranking discriminator and a topology preserving regularization module. • A width-aware module is designed to extract features related to the vessel width via predicting the width maps for the dilated/non-dilated ground truth A/V masks to enhance the model's perception of the vessel width, which is regularized by a width-aware loss. • The proposed framework is validated quantitatively and qualitatively on three publicly available datasets, including AV-DRIVE, INSPIRE-AVR and high-resolution fundus (HRF). [Display omitted] Automatic artery/vein (A/V) classification, as the basic prerequisite for the quantitative analysis of retinal vascular network, has been actively investigated in recent years using both conventional and deep learning based methods. The topological connection relationship and vessel width information, which have been proved effective in improving the A/V classification performance for the conventional methods, however, have not yet been exploited by the deep learning based methods. In this paper, we propose a novel Topology and Width Aware Generative Adversarial Network (named as TW-GAN), which, for the first time, integrates the topology connectivity and vessel width information into the deep learning framework for A/V classification. To improve the topology connectivity, a topology-aware module is proposed, which contains a topology ranking discriminator based on ordinal classification to rank the topological connectivity level of the ground-truth mask, the generated A/V mask and the intentionally shuffled mask. In addition, a topology preserving triplet loss is also proposed to extract the high-level topological features and further to narrow the feature distance between the predicted A/V mask and the ground-truth mask. Moreover, to enhance the model's perception of vessel width, a width-aware module is proposed to predict the width maps for the dilated/non-dilated ground-truth masks. Extensive empirical experiments demonstrate that the proposed framework effectively increases the topological connectivity of the segmented A/V masks and achieves state-of-the-art A/V classification performance on the publicly available AV-DRIVE and HRF datasets. Examinations of the spinal column with both, Magnetic Resonance (MR) imaging and Computed Tomography (CT), often require a precise three-dimensional positioning, angulation and labeling of the spinal disks and the vertebrae. A fully automatic and robust approach is a prerequisite for an automated scan alignment as well as for the segmentation and analysis of spinal disks and vertebral bodies in Computer Aided Diagnosis (CAD) applications. In this article, we present a novel method that combines Marginal Space Learning (MSL), a recently introduced concept for efficient discriminative object detection, with a generative anatomical network that incorporates relative pose information for the detection of multiple objects. It is used to simultaneously detect and label the spinal disks. While a novel iterative version of MSL is used to quickly generate candidate detections comprising position, orientation, and scale of the disks with high sensitivity, the anatomical network selects the most likely candidates using a learned prior on the individual nine dimensional transformation spaces. Finally, we propose an optional case-adaptive segmentation approach that allows to segment the spinal disks and vertebrae in MR and CT respectively. Since the proposed approaches are learning-based, they can be trained for MR or CT alike. Experimental results based on 42 MR and 30 CT volumes show that our system not only achieves superior accuracy but also is among the fastest systems of its kind in the literature. On the MR data set the spinal disks of a whole spine are detected in 11.5s on average with 98.6% sensitivity and 0.073 false positive detections per volume. On the CT data a comparable sensitivity of 98.0% with 0.267 false positives is achieved. Detected disks are localized with an average position error of 2.4mm/3.2mm and angular error of 3.9°/4.5° in MR/CT, which is close to the employed hypothesis resolution of 2.1mm and 3.3°. • We introduce a new task named Noisy Supervised Domain Adaptation (NSDA), which is both challenging and meaningful for real-world applications in medical image segmentation. • We propose a novel self-cleansing unsupervised domain adaptation (S-CUDA) method to address the NSDA problem for medical image segmentation, which trains two peer adversarial networks simultaneously in a co-training and co-cleansing style. For each peer network, we divide the training samples into the high-confidence clean labeled set (i.e., clean data) and noisy labeled set (i.e., noisy data) based on its per-sample loss. The identified clean data are then used to train the peer networks, which keep diverging to another one, to filter different type of errors and avoid confirmation bias in the self-training. • We propose to clean and reuse the identified high-confidence noisy labeled samples to provide more diversity and useful information to learn a robust model. For the identified high-confidence noisy data, we clean and refine their labels using the network's predictions as pseudo labels, which will be further reused in the following learning iterations. • We design a novel loss function to use both the original noisy labels and the updated cleaned labels from the peer network to break up the chain of "error propagation". The new loss function can help the cross-reviewing framework alleviate the "learning trap" problem exiting in self-training process (i.e., the quality of pseudo labels does not get improved and the new supervision from them becomes weaker and weaker). • We conduct extensive experiments on public datasets, i.e., two retinal fundus image datasets and a public spinal cord gray matter segmentation challenge dataset, to demonstrate the effectiveness of our proposed S-CUDA framework. The comprehensive results demonstrate the superior performance of our method compared with existing state-of-the-art methods. [Display omitted] Medical image segmentation tasks hitherto have achieved excellent progresses with large-scale datasets, which empowers us to train potent deep convolutional neural networks (DCNNs). However, labeling such large-scale datasets is laborious and error-prone, which leads the noisy (or incorrect) labels to be an ubiquitous problem in the real-world scenarios. In addition, data collected from different sites usually exhibit significant data distribution shift (or domain shift). As a result, noisy label and domain shift become two common problems in medical imaging application scenarios, especially in medical image segmentation, which degrade the performance of deep learning models significantly. In this paper, we identify a novel problem hidden in medical image segmentation, which is unsupervised domain adaptation on noisy labeled data, and propose a novel algorithm named "Self-Cleansing Unsupervised Domain Adaptation" (S-CDUA) to address such issue. S-CUDA sets up a realistic scenario to solve the above problems simultaneously where training data (i.e., source domain) not only shows domain shift w.r.t. unsupervised test data (i.e., target domain) but also contains noisy labels. The key idea of S-CUDA is to learn noise-excluding and domain invariant knowledge from noisy supervised data, which will be applied on the highly corrupted data for label cleansing and further data-recycling, as well as on the test data with domain shift for supervised propagation. To this end, we propose a novel framework leveraging noisy-label learning and domain adaptation techniques to cleanse the noisy labels and learn from trustable clean samples, thus enabling robust adaptation and prediction on the target domain. Specifically, we train two peer adversarial networks to identify high-confidence clean data and exchange them in companions to eliminate the error accumulation problem and narrow the domain gap simultaneously. In the meantime, the high-confidence noisy data are detected and cleansed in order to reuse the contaminated training data. Therefore, our proposed method can not only cleanse the noisy labels in the training set but also take full advantage of the existing noisy data to update the parameters of the network. For evaluation, we conduct experiments on two popular datasets (REFUGE and Drishti-GS) for optic disc (OD) and optic cup (OC) segmentation, and on another public multi-vendor dataset for spinal cord gray matter (SCGM) segmentation. Experimental results show that our proposed method can cleanse noisy labels efficiently and obtain a model with better generalization performance at the same time, which outperforms previous state-of-the-art methods by large margin. [Display omitted] Cervical cancer causes the fourth most cancer-related deaths of women worldwide. Early detection of cervical intraepithelial neoplasia (CIN) can significantly increase the survival rate of patients. World Health Organization (WHO) divided the CIN into three grades (CIN1, CIN2 and CIN3). In clinical practice, different CIN grades require different treatments. Although existing studies proposed computer aided diagnosis (CAD) systems for cervical cancer diagnosis, most of them are fail to perform accurate separation between CIN1 and CIN2/3, due to the similar appearances under colposcopy. To boost the accuracy of CAD systems, we construct a colposcopic image dataset for GRA ding cervical intraepithelial N eoplasia with fine-grained lesion D escription (GRAND). The dataset consists of colposcopic images collected from 8,604 patients along with the pathological reports. Additionally, we invite the experienced colposcopist to annotate two main clues, which are usually adopted for clinical diagnosis of CIN grade, i.e., texture of acetowhite epithelium (TAE) and appearance of blood vessel (ABV). A multi-rater model using the annotated clues is benchmarked for our dataset. The proposed framework contains several sub-networks (raters) to exploit the fine-grained lesion features TAE and ABV, respectively, by contrastive learning and a backbone network to extract the global information from colposcopic images. A comprehensive experiment is conducted on our GRAND dataset. The experimental results demonstrate the benefit of using additional lesion descriptions (TAE and ABV), which increases the CIN grading accuracy by over 10%. Furthermore, we conduct a human-machine confrontation to evaluate the potential of the proposed benchmark framework for clinical applications. Particularly, three colposcopists on different professional levels (intern, in-service and professional) are invited to compete with our benchmark framework by investigating a same extra test set—our framework achieves a comparable CIN grading accuracy to that of a professional colposcopist. Fully convolutional networks (FCNs) trained with abundant labeled data have been proven to be a powerful and efficient solution for medical image segmentation. However, FCNs often fail to achieve satisfactory results due to the lack of labelled data and significant variability of appearance in medical imaging. To address this challenging issue, this paper proposes a conjugate fully convolutional network (CFCN) where pairwise samples are input for capturing a rich context representation and guide each other with a fusion module. To avoid the overfitting problem introduced by intra-class heterogeneity and boundary ambiguity with a small number of training samples, we propose to explicitly exploit the prior information from the label space, termed as proxy supervision. We further extend the CFCN to a compact conjugate fully convolutional network (C 2 FCN), which just has one head for fitting the proxy supervision without incurring two additional branches of decoders fitting ground truth of the input pairs compared to CFCN. In the test phase, the segmentation probability is inferred by the learned logical relation implied in the proxy supervision. Quantitative evaluation on the Liver Tumor Segmentation (LiTS) and Combined (CT-MR) Healthy Abdominal Organ Segmentation (CHAOS) datasets shows that the proposed framework achieves a significant performance improvement on both binary segmentation and multi-category segmentation, especially with a limited amount of training data. Segmentation of medical images, particularly late gadolinium-enhanced magnetic resonance imaging (LGE-MRI) used for visualizing diseased atrial structures, is a crucial first step for ablation treatment of atrial fibrillation. However, direct segmentation of LGE-MRIs is challenging due to the varying intensities caused by contrast agents. Since most clinical studies have relied on manual, labor-intensive approaches, automatic methods are of high interest, particularly optimized machine learning approaches. To address this, we organized the 2018 Left Atrium Segmentation Challenge using 154 3D LGE-MRIs, currently the world's largest atrial LGE-MRI dataset, and associated labels of the left atrium segmented by three medical experts, ultimately attracting the participation of 27 international teams. In this paper, extensive analysis of the submitted algorithms using technical and biological metrics was performed by undergoing subgroup analysis and conducting hyper-parameter analysis, offering an overall picture of the major design choices of convolutional neural networks (CNNs) and practical considerations for achieving state-of-the-art left atrium segmentation. Results show that the top method achieved a Dice score of 93.2% and a mean surface to surface distance of 0.7 mm, significantly outperforming prior state-of-the-art. Particularly, our analysis demonstrated that double sequentially used CNNs, in which a first CNN is used for automatic region-of-interest localization and a subsequent CNN is used for refined regional segmentation, achieved superior results than traditional methods and machine learning approaches containing single CNNs. This large-scale benchmarking study makes a significant step towards much-improved segmentation methods for atrial LGE-MRIs, and will serve as an important benchmark for evaluating and comparing the future works in the field. Furthermore, the findings from this study can potentially be extended to other imaging datasets and modalities, having an impact on the wider medical imaging community. The current outbreak of coronavirus disease 2019 (COVID-19) has recently been declared as a pandemic and spread over 200 countries and territories. Forecasting the long-term trend of the COVID-19 epidemic can help health authorities determine the transmission characteristics of the virus and take appropriate prevention and control strategies beforehand. Previous studies that solely applied traditional epidemic models or machine learning models were subject to underfitting or overfitting problems. We propose a new model named Dynamic-Susceptible-Exposed-Infective-Quarantined (D-SEIQ), by making appropriate modifications of the Susceptible-Exposed-Infective-Recovered (SEIR) model and integrating machine learning based parameter optimization under epidemiological rational constraints. We used the model to predict the long-term reported cumulative numbers of COVID-19 cases in China from January 27, 2020. We evaluated our model on officially reported confirmed cases from three different regions in China, and the results proved the effectiveness of our model in terms of simulating and predicting the trend of the COVID-19 outbreak. In China-Excluding-Hubei area within 7 days after the first public report, our model successfully and accurately predicted the long trend up to 40 days and the exact date of the outbreak peak. The predicted cumulative number (12,506) by March 10, 2020, was only 3·8% different from the actual number (13,005). The parameters obtained by our model proved the effectiveness of prevention and intervention strategies on epidemic control in China. The prediction results for five other countries suggested the external validity of our model. The integrated approach of epidemic and machine learning models could accurately forecast the long-term trend of the COVID-19 outbreak. Purpose: For interventional cardiac procedures, anatomical and functional information about the cardiac chambers is of major interest. With the technology of angiographic C-arm systems it is possible to reconstruct intraprocedural three-dimensional (3D) images from 2D rotational angiographic projection data (C-arm CT). However, 3D reconstruction of a dynamic object is a fundamental problem in C-arm CT reconstruction. The 2D projections are acquired over a scan time of several seconds, thus the projection data show different states of the heart. A standard FDK reconstruction algorithm would use all acquired data for a filtered backprojection and result in a motion-blurred image. In this approach, a motion compensated reconstruction algorithm requiring knowledge of the 3D heart motion is used. The motion is estimated from a previously presented 3D dynamic surface model. This dynamic surface model results in a sparse motion vector field (MVF) defined at control points. In order to perform a motion compensated reconstruction, a dense motion vector field is required. The dense MVF is generated by interpolation of the sparse MVF. Therefore, the influence of different motion interpolation methods on the reconstructed image quality is evaluated. Methods: Four different interpolation methods, thin-plate splines (TPS), Shepard's method, a smoothed weighting function, and a simple averaging, were evaluated. The reconstruction quality was measured on phantom data, a porcine model as well as on in vivo clinical data sets. As a quality index, the 2D overlap of the forward projected motion compensated reconstructed ventricle and the segmented 2D ventricle blood pool was quantitatively measured with the Dice similarity coefficient and the mean deviation between extracted ventricle contours. For the phantom data set, the normalized root mean square error (nRMSE) and the universal quality index (UQI) were also evaluated in 3D image space. Results: The quantitative evaluation of all experiments showed that TPS interpolation provided the best results. The quantitative results in the phantom experiments showed comparable nRMSE of ≈0.047 ± 0.004 for the TPS and Shepard's method. Only slightly inferior results for the smoothed weighting function and the linear approach were achieved. The UQI resulted in a value of ≈ 99% for all four interpolation methods. On clinical human data sets, the best results were clearly obtained with the TPS interpolation. The mean contour deviation between the TPS reconstruction and the standard FDK reconstruction improved in the three human cases by 1.52, 1.34, and 1.55 mm. The Dice coefficient showed less sensitivity with respect to variations in the ventricle boundary. Conclusions: In this work, the influence of different motion interpolation methods on left ventricle motion compensated tomographic reconstructions was investigated. The best quantitative reconstruction results of a phantom, a porcine, and human clinical data sets were achieved with the TPS approach. In general, the framework of motion estimation using a surface model and motion interpolation to a dense MVF provides the ability for tomographic reconstruction using a motion compensation technique.
