The simultaneous association of epithelial and lymphoid tumour of the digestive tract is extremely rare it is most often a collision tumor combining the synchronization of adenocarcinoma end B lymphoma. We report the observation of a 45-year-old patient without a history who was hospitalized for management of adenocarcinoma of the sub-cardial region stomach. The patient was benefited on chemotherapy according to FLOT protocol and then had a total gastrectomy and Roux-en-Y oesophagojejunostomy on laparoscopic the result of the histological study showed a well differentiated adenocarcinoma and incidentally lymphoid proliferation has small nodular architecture cells whose immunohistochemistry study was in favour of gastric and ganglionic lymphoma type B. The patients postoperative course was uneventful. &nbsp

Adrenal sarcomatoid carcinoma (ASC) is a very rare aggressive variant of adrenocortical carcinoma showing carcinomatous and sarcomatous differentiation. It is a poorly differentiated carcinoma with poor prognosis. The diagnosis requires careful histological and immunohistochemical investigation. We describe a new case of ASC to raise awareness on this extremely rare entity. A 27-year-old woman presented with a right flank pain. Imaging revealed a tissular mass of the right adrenal gland without metastases. After adrenalectomy, histology revealed sheets of epithelioid cells that stained for synaptophysin and Melan-A; and spindled cells staining for S-100. We have reported the clinical and histopathological features of ACS’s case; as it is an extremely rare cancer with a challenging diagnosis. There is a need for a further understanding of ASC’s biology to improve it poor prognosis.

Introduction: Peutz-Jeghers syndrome (PJS) is a rare autosomal inherited polyposis syndrome, characterized by multiple hamartomatous polyps in the gastrointestinal tract, associated with mucocutaneous pigmentation. Polyps mainly occur in the small intestine. Patients with PJS have an increased risk of many cancers. Histologically, the presence of a smooth muscle arborization within the lamina propria is the hallmark of those polyps. Intussusceptions is a rare complications of gastrointestinal polyps. In the present study, we report the case of this rare entity with particular histological features in a young woman who developed a jejunal obstruction caused by an intussusception. Case Presentation: A 32-year-old woman presented to the emergency department with jejunal obstruction caused by an intussusception. She had a pigmented macules around the mouth and on the tongue. An urgent exploratory laparotomy was performed, revealing a distended invaginated ischemic part of proximal jejunum.Careful exploration of the abdominal cavity and intestine revealed multiple polyps inside the small intestine. On gross, one pedunculated polyp was found. Histollogically, it had a central core of smooth muscle showing a tree like branching with a pseudo-invasion like pattern due to the extension of glands in the subserousa. Conclusion: PJS is a rare syndrome, which often remains undiagnosed for many years. Early diagnosis, treatment, and regular follow-up are important for an excellent prognosis in individuals presenting with this rare syndrome.

Pseudomyxoma peritonei (PMP) is a clinicopathologic syndrome characterized by mucinous ascites and pools of mucin resulting in neoplastic mucinous epithelium in the peritoneal cavity. PMP is uncommon and it is characterized by clinical and unusual pathologic manifestations posing diagnostic and therapeutic problems. Involvement of abdominal viscera and lymph node metastases are rare and sporadic cases have been reported in the literature. We here report the case of a 56-year old patient who had undergone two operations for PMP of appendicular origin presenting with progressive abdominal pain five years after his last treatment. Scanner objectified a recurrence of peritoneal pseudo-myxoma with liver and splenic intraparenchymatous lesions. Anatomopathological examination showed intrasplenic and hepatic recurrence of low-grade peritoneal pseudo-myxoma confirming the metastases.Keywords: Peritoneal pseudo-myxoma, splenic metastasis, liver metastasis

Background: SEF is a rare variant of fibrosarcoma, mostly occurring in extraosseous sites. It is a clinically challenging entity as no standardized treatment regimens are available. SEF’s literature is limited, due to its rarity and recent recognition. The current study reports the case of a patient with SEF arising from the anterior chest wall. Case Presentation: A 26-year-old man presented with a 6 years history of a left superior chest mass. Imaging revealed an invasive tumor of the right pectoral region, without metastases. The patient undergone surgical resection of the tumor. Histology and immunohistochemistry confirmed the diagnosis and revealed a hypocellular tumor with large areas of hyalinized fibrous stroma. The patient was referred to the oncology department to undergo postoperative treatment (radiotherapy/chemotherapy).

Le blastome pulmonaire décrit surtout chez les enfants, est une tumeur rare. Elle représente 0.25 à 0.5% de toutes les tumeurs pulmonaires avec un pronostic très grave. Sur le plan histologique, il s'agit d'une tumeur à double composante: une composante épithéliale et une composante mésenchymateuse. Cliniquement, il s'annonce souvent par des douleurs thoraciques, de la toux, des hémoptysies et de la dyspnée, mais reste asymptomatique dans environ 40% des cas. Nous rapprtons le cas d'une femme de 25 ans, sans antécédents particuliers, qui se plaignait de dyspnée, de toux et de douleurs basithoraciques gauches. l'exploration radiologique avait montré une large masse basithoracique du poumongauche. La biopsie a été réalisée et avait ramené uniquement du matériel nécrotique. La pièce de résection était largement nécrosée, le tissu viable examiné au miroscope avait révélé un pattern biphasique, composée de tissu épithélial malin, associé à un tissu mésenchymateux malin, caractéristiques du pneumoblastome biphasique. La patiente a bénéficié d'une chimiothérapie puis radiothérapie. It accounts for 0.25-0.5% of all pulmonary tumors, with a very serious prognosis. Histologically, it is a tumor composed of two components: an epithelial component and a mesenchymal component. Clinically, it usually manifests as chest pain, cough, hemoptysis and dyspnea, but it is asymptomatic in approximately 40% of cases. We report the case of a 25 year old woman, with no previous medical history, who complained of dyspnoea, cough and left basithoracic pain. Radiological evaluation showed large basithoracic mass in the left lung. A biopsy was performed which only showed necrotic material. The surgical specimen was largely necrotic. The viable tissue was examined with the miroscope which showed biphasic pattern composed of malignant epithelial tissue associated with malignant mesenchymal tissue, typical of biphasic pneumoblastoma. The patient underwent chemotherapy and radiation therapy. Porokeratosis (PK) is a rare skin disease, of unknown etiology, probably due to a disorder of epidermal keratinization. We describe a 24 year-old male patient, with porokeratosis of Mibelli confirmed histopathologically. A young man of 24, from a non-consanguineous marriage, presented to our department with 4 years history of one hyperkeratotic lesion, symmetric, painless and non-pruritic on the nose. Clinical examination revealed the presence in the left wing of the nose, one annular lesion, measuring about 0.5 cm, the centres was atrophic and surrounded by a raised hyperkeratotic border.

We report an extremely rare tumor presenting with myoid features in the left cerebellar hemisphere in a 62- year-old man. This tumor consisted of medium to large round cells with focal lipomatous and myoid differentiation. Immunohistochemically, the tumor cells expressed synaptophysin, GFAP (glial fibrillary acidic protein) and focally desmin. From these findings, we concluded that this tumor was a liponeurocytoma with myoid features. To the best of our knowledge, this report constitutes the second described case of liponeurocytoma with myoid differentiation in the cerebellum.

Le lymphome médiastinal primitif à grandes cellules B (LBPM) est un lymphome survenant dans le médiastin antérieur à partir des cellules B de la zone médullaire thymique. Il s’agit d’une entité rare qui présente des particularités tant sur le plan épidémiologique, clinique et évolutif, que sur le plan anatomo-pathologique et immuno-histochimique. Nous rapportons une série de 5 patientes hospitalisées au service de pneumologie de l’hôpital Ibn Sina entre Janvier 2012 et Mai 2016, et chez qui on a retenu le diagnostic de LBPM. L’âge moyen était de 34 ans, le délai moyen de consultation était de 2 mois. Les symptômes rapportés étaient la dyspnée, la douleur thoracique, la toux sèche; deux patientes présentaient un syndrome cave supérieur. Le taux de LDH était élevé chez 4 patientes. L’imagerie thoracique montrait chez les 5 patientes un processus tissulaire médiastinal antérieur. Le diagnostic histologique était posé sur ponction biopsie transpariétale scanno-guidée chez les 5 patientes, et l’apport de l’immunohistochimie était déterminant dans tous les cas. Les patientes ont été adressées à l’institut national d’oncologie pour prise en charge thérapeutique. Le pronostic du LBPM est réservé, il survient volontiers chez des femmes jeunes, ce qui rend d’autant plus nécessaire une thérapeutique agressive afin d’améliorer le taux de survie.The Pan African Medical Journal 2016;24
