110. Detecting user experience issues from mHealth apps that support stroke caregiver needs:an analysis of user reviews

Abstract

Background: Existing research has demonstrated the potential of mHealth apps in improving the caregiving outcomes of stroke. Since most of the apps were published in commercially available app stores without explaining their design and evaluation processes, it is necessary to identify the user experience issues to promote long-term adherence and usage. Objective: The purpose of this study was to utilize published user reviews of commercially available apps to determine the user experience issues to guide future app development in stroke caregiving. Methods: User reviews were extracted from the previously identified 46 apps that support stroke caregiving needs using a python-scraper. The reviews were pre-processed and filtered using python scripts to consider English reviews that described issues faced by the user. The final corpus was categorized based on TF-IDF vectorization and k-means clustering technique, and the issues extracted from the various topics were classified based on the seven dimensions of user experience to highlight factors that may affect the usage of the app. Results: A total of 117,364 were extracted from the two app stores. After filtration, 13,368 reviews were included and classified based on the user experience dimensions. Findings highlight critical issues that affect the usability, usefulness, desirability, findability, accessibility, credibility, and value of the app that contribute to decreased satisfaction and increased frustration. Conclusion: The study identified several user experience issues due to the inability of the app developers to understand the needs of the user. Further, the study describes the inclusion of a participatory design approach to promote an improved understanding of user needs; therefore, limiting any issues and ensuring continued use.

Abstract

Background Globally, stroke is a leading cause of death and disability, with most care undertaken by caregivers who are generally family and friends without prior experience of care. The lack of experience or unpreparedness results in feelings of uncertainty, burnout, anxiety, burden, etc. Hence, it is necessary to identify the needs of caregivers to better support them in their caregiving journey and improve the quality of care delivered. Methods The study employed a grounded theory methodology that utilizes information gathered from literature reviews and social media to represent the needs and create a storyline visually. The storyline is further refined and evaluated using an online survey of 72 participants recruited through online stroke caregiving communities. Results The study identified four core categories of needs: (i) Information: sufficient information delivered in layman’s terms based on the individual situation of the caregiver and survivor through oral and hands-on demonstrations, (ii) Involvement: inclusion in the decision-making processes at different stages of recovery through face-to-face communication at the hospital, (iii) Self-care: ability to engage in work and leisure activities, (iv) Support: receive support in the form of resources, services and finances from different other stakeholders. Conclusions There is a need to create a caregiver-centered approach in stroke recovery to ensure limited obstruction to care and reduced uncertainty in stroke recovery. Moreover, through the inclusion of caregivers in stroke recovery, it may be possible to reduce the burden of care to the caregiver and ensure the satisfaction of the healthcare system throughout stroke recovery.

113. Online User and Power Allocation in Dynamic NOMA-Based Mobile Edge Computing

Abstract

This study tackles the online user allocation problem in mobile edge computing (MEC) systems powered by non-orthogonal multiple access. App vendors need to determine a proper wireless channel in a base station/edge server and sufficient transmit power for every user. We consider a stochastic MEC system where users arrive and depart over time. When an edge server runs out of computing resources, some users will have to wait until the resources become available again, which incurs an allocation delay cost. This cost is often not investigated in many studies, which also do not consider a multi-cell, multi-channel system as we do in this work, due to its complexity. We aim to minimize the allocation delay and transmit power costs, increasing the system's energy efficiency. To achieve this objective while guaranteeing users’ data rate requirements over time, we adopt the Lyapunov framework to convert this long-term optimization problem into a series of subproblems to be solved in every time slot. To solve the aforementioned subproblems efficiently, we present a distributed game theory-based approach. The proposed algorithm is theoretically evaluated and experimentally demonstrated to outperform several baseline and state-of-the-art methods, highlighting the significance of systematic consideration for both computation and communication aspects of this problem.

114. Evaluating the efficacy of a digital App to enhance patient‐centred nursing handover: A simulation study.

Abstract

Aim: The study aim was to evaluate the feasibility and efficacy of a digital App developed to enhance patient communication with nurses during bedside nursing handover at shift change. Methods: Six nurses and 11 patient actors/volunteers participated in 12 simulated nursing handovers across six simulation workshops. Over half the patients were aged 70+ years (55%); majority were female (82%). Handover video recordings were analysed using a structured observation tool and a revised Four Habits Coding Scheme to assess nurses' handover communication skills. Patient and nurse feedback was also sought. The STROBE checklist (Data S1) guided preparation of the study. Results: For all simulated handovers (n = 12): Nurses greeted the patient at commencement; nurses made eye contact with the patient; patients were given opportunity to ask questions; and all patient questions were answered. Nurses explained the handover process for less than half the handovers (42%). Familiarity with the patient's history was evident in every handover. Communication behaviours identified in most handovers included: good nonverbal behaviour; allowing time for the patient to absorb information; giving clear explanations; involving the patient in decisions; and exploring acceptability of the care plan. Patient and nurse feedback on the App included: The App was easy to navigate, features were well‐liked, with some improvements suggested. Conclusion: Patients and nurses provided positive feedback for the App during hospital stay and at handover. The App has the potential to enhance existing handover processes and increase safety of hospital care by using technology to educate and empower patients/carers to be active partners in communication with nurses during change‐of‐shift handover. Relevance to Clinical Practice: The App empowers and enables patients/carers to actively participate in nursing handover and allows patients to communicate concerns and provide information to their nursing team, facilitating a new approach. Patient or Public Contribution: Patients and carers were involved in the research from the original co‐design workshops that guided the development of the handover App. The research aims and outcome measures were informed by the experiences and preferences of patients/carers. Two patient representatives were involved in writing and submission of the grant application for the study to evaluate the efficacy of the App and were listed as co‐authors on this paper. Patient volunteers were involved in the current study to pilot test the handover App. Patient volunteers were recruited through a consumer representative and volunteer registry at the health service. They participated in simulated nursing handovers with two nurses to assess the feasibility and acceptability of the handover App and then provided feedback and suggestions for improvement. 130. Reduced Total Airway Count and Airway Wall Tapering after Three-Years in Ex-Smokers

Abstract

Computed tomography (CT) total-airway-count (TAC) and airway wall-thickness differ across chronic obstructive pulmonary disease (COPD) severities, but longitudinal insights are lacking. The aim of this study was to evaluate longitudinal CT airway measurements over three-years in ex-smokers. In this prospective convenience sample study, ex-smokers with (n = 50; 13 female; age = 70 ± 9 years; pack-years = 43 ± 26) and without (n = 40; 17 female; age = 69 ± 10 years; pack-years = 31 ± 17) COPD completed CT, 3He magnetic resonance imaging (MRI), and pulmonary function tests at baseline and three-year follow-up. CT TAC, airway wall-area (WA), lumen-area (LA), and wall-area percent (WA%) were generated. Emphysema was quantified as the relative-area-of-the-lung with attenuation < –950 Hounsfield-units (RA950). MRI ventilation-defect-percent (VDP) was also quantified. Differences over time were evaluated using paired-samples t tests. Multivariable prediction models using the backwards approach were generated. After three-years, forced-expiratory-volume in 1-second (FEV1) was not different in ex-smokers with (p = 0.4) and without (p = 0.5) COPD, whereas RA950 was (p < 0.001, p = 0.02, respectively). In ex-smokers without COPD, there was no change in TAC (p = 0.2); however, LA (p = 0.009) and WA% (p = 0.01) were significantly different. In ex-smokers with COPD, TAC (p < 0.001), WA (p = 0.04), LA (p < 0.001), and WA% (p < 0.001) were significantly different. In all ex-smokers, TAC was related to VDP (baseline: ρ = –0.30, p = 0.005; follow-up: ρ = –0.33, p = 0.002). In significant multivariable models, baseline airway wall-thickness was predictive of TAC worsening. After three-years, in the absence of FEV1 worsening, TAC diminished only in ex-smokers with COPD and airway walls were thinner in all ex-smokers. These longitudinal findings suggest that the evaluation of CT airway remodeling may be a useful clinical tool for predicting disease progression and managing COPD. Abstract

This paper proposes a hierarchical multi-stage approach based on a distributed level phasor measurement unit (µPMU) at local controllers and a phasor data concentrator (PDC) at the central control unit to restore system voltage when it exceeds the limits recommended by the IEEE 1547-2018 standard. The proposed algorithm does not need an accurate system model or employ optimization solutions. Therefore, it has less implementation complexity and would be popular among distribution network service providers (DNSPs) and distribution network operators (DNOs) as it does not suffer from cost and computational complexity limitations. A PMU-PDC-based communication platform has been developed as a more efficient alternative to the supervisory control and data acquisition (SCADA) system, and provides superior characteristics, including a higher sample rate, higher data resolutions, and faster communication. The proposed coordinated algorithm aims to postpone power generation curtailment in distributed energy resources (DERs) with overvoltage problems (local DERs) by incorporating all the DERs that are not subjected to voltage violation (remote DERs) by absorbing their maximum reactive power capacity. If the system voltage has not recovered after absorbing all of the reactive power capacity of all the DERs, a reduced active power curtailment proposed by the algorithm is then applied to the system to control the voltage. The proposed strategy has been simulated in MATLAB and applied to IEEE 13-bus and IEEE 33-bus radial distribution benchmark systems to validate the performance of the system, in terms of its ability to coordinate voltage control and the accuracy of the PMU-PDC-based communication interface. Abstract

Bacteria capable of biodegrading chlorpyrifos and cypermethrin were identified in agricultural drainage ditches in Kafr El Sheikh, Egypt, using an enrichment approach. Under laboratory circumstances, this work was carried out to isolate and molecularly identify microorganisms from water for chlorpyrifos and cypermethrin biodegradation in aquatic systems. Pseudomonas putida was identified based on morphological, biochemical, and 18S rDNA gene sequencing. Furthermore, the kinetics of chlorpyrifos and cypermethrin degradation by P. putida in an aqueous medium under laboratory conditions were studied. P. putida was cultivated in a minimal supplemented medium (MSM) with chlorpyrifos and cypermethrin as the only carbon sources. Within one, three, and seven days, the greatest capability of chlorpyrifos degradation (88.10 percent) was achieved at a concentration of 40 ppm, and the greatest capability of cypermethrin degradation (83.81 percent) was achieved at a concentration of 80 ppm and at 30 °C. Scanning Electron Microscopy (SEM) showed some localized degradation of chlorpyrifos and cypermethrin around the bacterial cells in the biofilm. As a result, P. putida can be effectively utilized for environmental remediation of agricultural wastewater polluted with high levels of chlorpyrifos, cypermethrin, and other pesticides. Nevertheless, treatment options for LMC due to metastatic epidermal growth factor receptor-mutant (EGFR-mutant) lung adenocarcinoma are expanding. We present a 52-year-old male patient with metastatic non-small-cell lung cancer (NSCLC). The patient was found to have L858R mutation in exon 21 of the EGFR gene. He was initially treated with erlotinib, followed by afatinib/cetuximab, followed by chemotherapy. Thereafter, his disease progressed to LMC. Although tissue biopsy did not show T790M-mutation, osimertinib (160 mg once daily) promptly induced clinical and radiological response that continued for five months.
