Abstract

Background Despite a crucial role played by obstetric care providers (OCPs) in providing pain relief methods during labour, the evidence suggests that there is a large number of women in Ethiopia who did not have access to pain relief methods. Moreover, to the best of the investigators' knowledge, evidence is scarce regarding the utilization of pharmacological labour pain management methods in the study area. Thus this study was carried out to determine the practice of pharmacological labour pain management methods among OCPs in Harari regional state health facilities in Ethiopia. Methods A facility-based cross-sectional study was conducted from 20 May to 10 June 2021 in Harari regional state health facilities. All (n=464) OCPs in Harari regional state health facilities were included in the study. Data were collected using a structured questionnaire adapted from previous studies. The data were entered into Epi-data version 3.1 statistical software. Statistical analysis was carried out using SPSS for Windows version 22. Bivariate and multivariate logistic regression analyses were employed to determine the association between independent variables and the outcome variable. A p-value <0.05 was used to establish statistical significance. Results In this study, 464 OCPs participated. The overall utilization of pharmacological labour pain relief methods was 50.9% (95% confidence interval [CI] 45.8 to 55.8). The main barriers to labouring mothers receiving pain relief include high patient flow (54.4%), the limited number of staff (51.9%) and a lack of knowledge and skill (32.9%). Being female (adjusted odds ratio [AOR] 2.07 [95% CI 1.31 to 3.26]), not having training (AOR 1.60 [95% CI 1.02 to 2.53]) and having an unfavourable attitude (AOR 9.1 [95% CI 5.34 to 15.35]) were the predictors of utilization of pharmacological labour pain management methods. Conclusions The study revealed that half of the OCPs utilized the pharmacological labour pain management method. In general, this study identified that variables like sex, training and the attitude of OCPs were factors associated with the utilization of pharmacological labour pain management methods. Abstract

Vulnerability to climate change impact is the most pressing issues for less developed countries whose economy mainly depends on the agricultural sector. The demand for food is growing swiftly whereas impacts of climate change on the global food production are increasing. More area specific research outputs and evidences-based policy directions are needed to tackle the ever changing climate and to reduce its impacts on the agricultural production. The aim of this study was to investigate subsistence farmer household’s vulnerability level to climate change impacts and its associations with household’s agricultural production. Then primary data was collected from 400 households from Kolla Temben District, Tigray Regional State, North Ethiopia. Multistage sampling techniques were applied to select households for interview from the district. In the first stage, 4 Kebelles (Kebelle - administration unit) were selected randomly out of 27 Kebelles and then400 households were selected for interview through systematic random sampling techniques (Figure 1). Multiple regressions were used to examine the associations between household’s vulnerability to climate change impacts and agricultural production. Grounded theory and content analysis techniques were use to analyze data from key informant interviews and focus group discussions. For every single unit increase in household vulnerability to climate change impacts, there was an average agricultural production decrease between 16.99 and 25.83 (Table 4). For single unit increase in household’s vulnerability to climate change impact, there was a decrease of total crop production, Total income, total livestock, total food consumption and food consumption per adult equivalent. Rainfall decrease, small farmland ownership, steep topography, frequent flood occurrences and large family size are among the major factors that negatively affect household’s agricultural production and total income. The more the vulnerable the households, the less in total annual crop production, total livestock size, total income from agricultural production and the more dependent on food aid). There is a negative association between household’s vulnerability level to climate change impacts and agricultural production (crop production, total livestock ownerships and total income from crop production). More access to irrigation and agricultural fertilizers, improved varieties of crops, small family size, improve farmland ownership size, more access to education and Agricultural Extension services are an effective areas of intervention to improve household’s resilient, reduce households vulnerability level to climate change impacts and increase household’s total agricultural production.

124. Genotype heterogeneity of high-risk human papillomavirus infection in Ethiopia.

Abstract

Cervical cancer is a vaccine-preventable sexually transmitted disease. In the year 2020, there were an estimated 604,000 new cases and 342,000 deaths worldwide. Although its incidence is global, it is much higher in sub-Saharan African countries. In Ethiopia, there is a scarcity of data about the prevalence of high-risk HPV infection and its association with cytological profiles. Therefore, this study was conducted to fill this information gap. A hospital-based cross-sectional study was conducted from April 26 to August 28, 2021, and enrolled 901 sexually active women. Sociodemographic and other relevant bio-behavioral and clinical data were collected using a standardized questionnaire. Visual inspection with acetic acid [VIA] was done as an initial screening method for cervical cancer. The cervical swab was then collected using L-Shaped FLOQSwabs in eNAT nucleic acid preservation and transportation medium. A Pap test was done to determine the cytological profile. Nucleic acid was extracted using STARMag 96 ProPrep Kit on SEEPREP32. A Real-time multiplex assay was performed to amplify and detect the HPV L1 gene used for genotyping. The data were entered into Epi data version 3.1 software and exported to STATA version 14 for analysis. A total of 901 (age range from 30 to 60 years, mean age = 34.8 years, and SD± 5.8) women were screened for cervical cancer using VIA and 832 women had a valid co-testing (Pap test and HPV DNA testing) results for further process. The overall prevalence of hr HPV infection was 13.1%. Out of 832 women, 88% of them had normal and 12% had abnormal Pap test results. The proportion of high risk HPV was significantly higher among women with abnormal cytology (X2  = 688.446, p < 0.001) and younger age (X2  = 15.3408, p = 0.018). Among 110 women with hr HPV, 14 genotypes (HPV-16, -18, -31, -33, -35, -39, -45, -51, -52, -56, -58, -59, -66, and -68) were identified while HPV-16, -31, -52, -58, and -35 genotypes were highly prevalent. The high risk HPV infection continues to be a significant public health problem among women 30–35 years old. The presence of high-risk HPV irrespective of genotypes is highly correlated with cervical cell abnormalities. Abstract

Objective: Although teenage pregnancy has declined in the last decade, it remains a major public health issue in Africa. Maternal mortality is common among teenagers due to their increased risk of obstetric and medical complications. In Africa, there is a lack of robust and comprehensive data on the prevalence and predictors of teenage pregnancy. As a result, this systematic review and meta-analysis were carried out to summarize evidence that will assist concerned entities in identifying existing gaps and proposing strategies to reduce teenage pregnancy in Africa. Methods: The review is registered by the international prospective register of systematic reviews (CRD42021275013). This search included all published and unpublished observational studies written in English between August 23, 2016, and August 23, 2021. The articles were searched using databases (PubMed, CINHAL [EBSCO], EMBASE, POPLINE, Google Scholar, DOAJ, Web of Sciences, MEDLINE, Cochrane Library, and SCOPUS). Data synthesis and statistical analysis were conducted using STATA version 14 software. Forest plots were used to present the pooled prevalence and odds ratio (OR) with a 95% confidence interval (CI) of metaanalysis using the random effect model. Results: A total of 43,758 teenagers (aged 13–19) were included in 23 studies. In Africa, the overall pooled prevalence of teenage pregnancy was 30% (95% CI: 17–43). Western Africa had the highest prevalence of teenage pregnancy 33% (95% CI: 10–55). Age (18–19) (OR = 2.99 [95% CI = 1.124–7.927]), wealth index (OR = 1.84 [95% CI = 1.384–2.433]), and marital status (OR = 6.02 [95% CI = 2.348–15.43]) were predictors of teenage pregnancy in Africa. Conclusion: In Africa, nearly one-third of teenagers become pregnant. Teenage pregnancy was predicted by age (18–19), wealth index, and marital status. Strengthening interventions aimed at increasing teenagers’ economic independence, reducing child marriage, and increasing contraceptive use among married teenagers can help to prevent teenage pregnancy. It plays a vital role in decreasing maternal mortality and morbidity rates. It is also an indicator of maternal health services quality of a country. CS practice has been rising regardless of the medical condition, age, race, or gestational age. However, it is associated with many complications compared with vaginal deliveries, particularly in developing countries. Therefore, the aim of this study was to assess the magnitude and determinants of primary cesarean section among women who gave birth in Suhul General Hospital, Shire town, Northern Ethiopia.Methods: Institutional cross-sectional study was conducted from September 19 to October 20, 2017, among women who gave birth between September and August 2016 in Suhul General Public Hospital. Data were entered into EPI Info version 7 and exported to SPSS version 25 for analysis. Bivariate and multivariate logistic regressions were examined to assess the association between outcome and independent variables. Finally, variables with p-value less than 0.05 on multivariate logistic regression analysis were considered as significantly associated.Results: The magnitude of primary CS was 20.2%. The main indication for CS was non-reassuring fetal heartbeat pattern (NRFHRP) (32.2%). Women who had been augmented were 3.14 times more likely to undergo primary CS than those who had not (adjusted odds ratio (AOR)= 3.14; 95% CI: 1.49, 6.57). Women who had pregnancy-induced hypertension were 3.1 times more likely to undergo primary CS than those who had not (AOR= 3.10; 95% CI: 1.23, 7.83). Additionally, women who gave birth of ≥ 4000 gm newborns were 11 times (AOR= 11; 95% CI: 2.30, 57.51) more likely to undergo primary CS than those who gave birth 2500– 3999 gm of newborns. Moreover, women with unknown gestational age were 5.83 times (AOR= 5.83; 95% CI: 2.37, 14.31) more likely to undergo primary CS than women whose gestational age was 37– 42 weeks.Conclusion: The magnitude of the primary cesarean section in this study was high. Gestational age, augmentations, PIH, and birth weight were significant determinants of primary CS. However, predictors of mortality among hospitalized COVID-19 patients have not been well investigated in Ethiopia. Therefore, this study aimed to identify the predictors of mortality among hospitalized COVID-19 patients at a tertiary care hospital in Ethiopia.Methods: A hospital-based retrospective cohort design study was implemented among hospitalized COVID-19 patients at a tertiary care hospital in Harar, Ethiopia from March 20 to August 20, 2021. Data of 531 admitted patients were entered using Epi-data 3.1 and exported to STATA 14.2 for analysis. Binary logistic regression was used to identify significant predictors of outcome variables with an adjusted odds ratio (AOR) with a 95% confidence interval.Results: Of the total 531 study participants, 101 deaths occurred. The mortality rate was 16.2 per 1000 person-days of observation with median survival time of 44 days with IQR [28, 74]. Smoking history [AOR=2.55, 95% CI (1.15, 5.65)], alcohol history [AOR=2.3, 95% CI (1.06, 4.97)], comorbidities [AOR=2.95, 95% CI (1.26, 6.91)], and increasing oxygen saturation [AOR=0.92, 95% CI (0.89, 0.95)], and lymphocyte count [AOR=0.90, 95% CI (0.88, 0.97)] were independent significant predictors of death from Covid-19.Conclusion: The incidence of mortality among hospitalized COVID-19 patients

Though FGM is declared as a human right violation, studies revealed it is being practiced throughout Ethiopia. Therefore, this study was conducted to assess the prevalence, trends, and predictors of FGM among reproductive-aged (1549 years) women in Ethiopia.Methods: Trends of FGM among reproductive-age women were estimated using the three Ethiopian Demographic and Health Survey (EDHS): 2000 (n=15,367), 2005 (n=14,070) and 2016 (n=7248) data. Multilevel logistic regression analysis was conducted to identify both individual- and community-level factors of FGM using the latest (2016) EDHS. To adjust potential confounders, the analysis was conducted considering sample weighting, clustering, and stratifications using STATA-14 software.Results: The prevalence of FGM among women of reproductive age in Ethiopia decreased from 79.91% in 2000 to 70.37% in 2016. Similarly, FGM among daughters of circumcised mothers decreased from 56.16% in 2000 to 16.76% in 2016. Being Muslim (adjusted odds ratio [AOR] 5.48; 95% confidence interval [CI]: 4.23, 7.09), attending higher education (AOR 0.40; 95% CI: 0.29, 0.54), 45– 49 years old (AOR 5.06; 95% CI: 3.38, 7.57), marriage at ≥ 18 years (AOR 0.80; 95% CI: 0.66, 0.96), not working (AOR 1.20; 95% CI: 1.02, 1.41), married (AOR 1.41; 95% CI: 1.12, 1.77) and residing in peripheral region (AOR 3.0.4; 95% CI: 1.96, 4.70) were determinants of FGM.Conclusion: Though the reduction of FGM among women of reproductive age in Ethiopia was minimal, it was encouraging among daughters of circumcised women over the last 16 years. Abstract

Background: Risky sexual behavior is defined as the behavior that increases the susceptibility of an individual to problems related to sexuality and reproductive health. The main aim of this study was to determine the pooled prevalence of risky sexual behavior and its associated factors in Ethiopia. Methods: Systematic review and meta-analysis (PRISMA) rules were used. During the searching period, MEDLINE, PUBMED, Cochrane Library, EMBASE, Google Scholar, and CINAHL were used with search terms. The STATA form 14 program was utilized to perform the meta-analysis. I2 statistics was used to test heterogeneity, and publication bias was assessed using Begg's and Egger's tests. Odds ratio (OR) with a 95% confidence interval (CI) was presented using forest plots. Results: There were 24 studies, and 13,440 study participants were included in this meta-analysis. The pooled prevalence of risky sexual behavior in Ethiopia was 40% (95% CI: 32%, 48%). The associated factors for risky sexual behavior were substance use [OR: 2.41 (95% CI: 1.49, 3.89)], watching pornography [OR: 2.59 (95% CI: 1.01, 6.69)], and night club visit, [OR: 2.53 (95% CI: 1.64, 3.90)].
