Objective: To evaluate the validity and reliability study of the Supportive Care Needs Survey for partners and caregivers of cancer patients in Turkish society (SCNS‐P&C‐T). Methods: This cross‐sectional survey followed by a test‐retest reliability and psychometric validation study was conducted with 270 participants. The research data were collected using a patient and caregiver demographic survey, the SCNS‐P&C‐T, the Caregiver Strain Index, and the Hospital Anxiety and Depression Scale. Results: Ten expert opinions were found to be consistent for content validity of the scale (I‐CVI = 0.993, S‐CVI = 0.956). The confirmatory factor analysis could not confirm the factor structure of the original scale. Therefore, an exploratory factor analysis was performed and the scale factor structure was determined. These factor structures are (a) psychological and emotional needs, (b) health care and information, (c) work and social needs, (d) communication and family needs. Conclusion: The SCNS‐P&C‐T is a valid and reliable tool which can be used to identify unmet needs among caregivers in Turkish populations. [ABSTRACT FROM AUTHOR]

Intravenous chemotherapy drugs used in the treatment of solid tumors are widely used because of the systemic response is rapid and effective. Extravasation is one of the important complications of chemotherapy. Extravasation is a tissue injury resulting in infiltration of the subcutaneous and subdermal space during infusion of the drug. Chemotherapy extravasation, which can be seen on different tabulations from soft tissue irritation to necrosis, is important for clinical practice. In this review, extravasation, classification and management were reviewed, Solid tümörlerin tedavisinde kullanılan intravenöz kemoterapi ilaçları sistemik yanıtın etkin ve hızlı olması nedeniyle yaygın olarak kullanılmaktadır. Kemoterapinin önemli komplikasyonlardan birisi ekstravazasyondur. Ekstravazasyon, ilacın infüzyonu sırasında subkutan ve subdermal alana infiltrasyonu sonucu gelişen doku hasarıdır. Yumuşak doku hasarından nekroz oluşumuna kadar farklı klinik tablolarda görülebilen kemoterapi ekstravazasyonu klinik uygulamalar açısından önemlidir. Objective: The aim of this study was to determine the perceived social support levels and quality of life of caregivers of cancer patients who were treated at the Akdeniz University Chemotherapy Unit.Material and Methods: The study data was collected from 235 patients’ caregivers who were diagnosed with cancer at the Akdeniz University Chemotherapy Unit between March and July of 2016. As a data collection tool, the patient and caregiver personal information form was used for socio-demographic features and the Edmonton Symptom Scale for determining patients’ symptoms, while the Multidimensional Perceived Social Support Scale and the Caregiver Quality of Life Index Cancer Scale were used for caregivers. Results: Caregivers included in the study were found to have a moderate level social support and the families were the most important supporter of caregivers. The quality of life of male caregivers was better than that of female caregivers and the difference was statistically significant. Moreover, it was found that the quality of life of caregivers decreased as the economic level decreased. Conclusion: Cancer is a chronic disorder and the required long-term treatment affects not only the patients’ life, but also the caregivers’. Because of holistic care approach, it is necessary to develop new interventions to increase the quality of life of cancer patients caregivers, Amaç: Bu çalışmanın amacı Akdeniz Üniversitesi Hastanesi Kemoterapi Ünitesi’nde tedavi gören kanser hastalarına bakım verenlerin algılanan sosyal destek düzeyleri ve yaşam kalitelerini belirlemektir. Gereç ve Yöntemler: Araştırma verileri Mart-Temmuz 2016 tarihleri arasında Akdeniz Üniversitesi Hastanesi Tıbbi Onkoloji bölümünde tedavi gören kanser hastalarına bakım veren 235 hasta yakınının çalışmaya katılmasıyla toplanmıştır. Veri toplama aracı olarak; sosyo-demografik özellikler için hasta ve bakım veren tanıtım formu, hasta semptomlarını belirlemek için Edmonton Semptom Tanılama Ölçeği, bakım verenler için Çok Boyutlu Algılanan Sosyal Destek Ölçeği ve Kanserli Hastalara Bakım Verenlerde Yaşam Kalitesi Ölçeği kullanılmıştır. Bulgular: Çalışma kapsamına alınan bakım verenlerin orta düzeyde sosyal desteğe sahip oldukları ve en büyük destekçilerinin aileleri olduğu belirlenmiştir. Erkek bakım verenlerin yaşam kalitesinin kadın bakım verenlerden daha iyi olduğu ve aralarındaki farkın istatistiksel olarak anlamlı olduğu görülmüştür. Ayrıca, ekonomik düzey azaldıkça bakım verenlerin yaşam kalitesinin de benzer bir şekilde azaldığı saptanmıştır. Sonuç: Kronik ve uzun süreli tedavi gerektiren kanser hastalığı yalnızca hastaları değil, bakım verenleri de etkilemektedir ve bakım verenlerin de güçlendirilmeye ihtiyacı vardır. Bu nedenle bütüncül bakımın gereği olarak kanser hastalarına bakım verenlerin yaşam kalitelerini artırmaya yönelik girişimlerin yapılması gerekmektedir
107. Determination of Sexual Perceptions and Sexual Care Needs of Cancer Patients Receiving Chemotherapy Treatment

Objective: The aim of this study was to determine sexuality perceptions and sexual needs of patients with colorectal and gynecological cancer who were treated at the Akdeniz University Day Chemotherapy Unit.Material and Methods: The study data was collected from 75 patients who were diagnosed with colorectal or gynecological cancer at Akdeniz University Day Chemotherapy Unit between October 2016 and April 2017. The obtained data were evaluated using percentage, mean, standard deviation values and the chi-square test.Results: While 66.7% of the participants were women, 33.4% were men; 42.7% were treated for gynecological cancer and 57.3% for colorectal cancer. The sexual activity of both women 76% and men 72% was negatively affected in the process of chemotherapy. During the chemotherapy period, 90% of the women and 96% of the men had a lower incidence of sexual intercourse. Most of the patients did not think that they could have a good sexual life during chemotherapy. During the chemotherapy period, 74.7% of the patients had a decrease in sexual desire. Some of the patients 33.3% stated that the frequency of sexual intercourse decreased after chemotherapy treatment, 28% had shortened duration of relationship and decreased quality, and 38.7% stated that no sexual intercourse took place after taking chemotherapy Conclusion: We observed that the sexual lives of both female and male patients are adversely affected in the course of chemotherapy. Sexual desire, frequency of sexual intercourse, and the spouse’s interest decreased with both types of cancer. The sexual life of patients with colorectal cancer was more restricted than that of gynecologic cancer patients during the treatment period., Amaç: Çalışma Akdeniz Üniversitesi Gündüz Kemoterapi Ünitesinde tedavi alan, kolorektal ve jinekolojik kanseri olan hastaların cinsellik algılarını ve cinsel bakım gereksinimlerini her iki kanser türü için ayrı ayrı belirlemek amacıyla yapılmıştır. Gereç ve Yöntemler: Araştırma verileri Ekim 2016-Nisan 2017 tarihleri arasında Akdeniz Üniversitesi Gündüz Kemoterapi Ünitesinde tedavi alan, kolorektal ya da jinekolojik kanser tanısı olan 75 hastadan toplanmıştır. Elde edilen veriler, yüzde, ortalama, standart sapma ve ki-kare testi kullanılarak değerlendirilmiştir. Bulgular: Katılımcıların %66,7’si kadın, %33,4’ü erkeklerden oluşmakta; %42,7’si jinekolojik kanser, %57,3’ü ise kolorektal kanser sebebiyle tedavi görmektedir. Kemoretapi sürecinde hem kadınların %76 , hem de erkeklerin %72 çoğunun cinsel yaşamının olumsuz etkilendiği belirlenmiştir. Kemoterapi sürecinde kadınların %90’ının, erkeklerin ise %96’sının cinsel ilişki sıklığı azalmıştır. Hastaların çoğu kemoterapi süresince kaliteli bir cinsel yaşamı olabileceğini düşünmemektedir. Kemoterapi sürecinde hastaların %74,7’sinde cinsel istekte azalma olmuştur. Hastaların %33,3’ü kemoterapi tedavisinden sonra cinsel ilişki sıklığının azaldığını, %28’i ilişki süresinin kısaldığını ve kalitesinin azaldığını, %38,7’si ise kemoterapi almaya başladıktan sonra hiç cinsel ilişkiye girmediğini belirtmiştir. Sonuç: Kemoterapi sürecinde hem kadın, hem erkek hastaların cinsel yaşamlarının olumsuz etkilendiği görülmüştür. Her iki kanser türünde de tedavi süreciyle cinsel isteklerinin, cinsel ilişki sıklıklarının, eşlerinin kendilerine olan ilgisinin azaldığı belirlenirken; tedavi sürecinde kolorektal kanseri olan hastaların cinsel yaşamlarının jinekolojik kanser hastalarına göre daha fazla kısıtlandığı görülmüştür

Trade borders are removing via regional trade agreements, bilateral agreements, etc. Some new strategies are created according to improve trade relations, for instance the EU neighbourhood policy. One of the major objectives of the EU neighbourhood policy is to develop trade integration between the enlarged EU and its neighbours. This paper is focuses on Euro - Mediterranean partnership from the scope of trade and energy. It is consisted of three main sections. In the first section it defines Euro- Mediterranean relation. In the second section it analyses intra and inter trade. We try seeing which commodity is the main argument in the trade, where energy is taken place and is this partnership is accelerated the Mediterranean economies. Section three concludes. - Abstract
Predictive prognostic scoring (PS) systems are not primarily applicable to elderly patients with classical Hodgkin lymphoma (cHL). The objective of this study was to develop a PS system for these patients. The derivation cohort (DC) was utilized for model development, consisting of 97 variables. The resulting algorithm was named as Hodgkin's Lymphoma Early Death in the Elderly within 12 months (HEDEL12). Internal and external validation cohorts (IVC and EVC) were employed for validation. A total of 286 patients were evaluated retrospectively. In DC 38 of 178 patients died within the first 12 months and overall survival (OS) at 12-month was 78.6%. Independent predictors of HEDEL12 were female sex, low albumin levels (<3.5 g/dL), and ECOG scores 2-4. According to HEDEL12 scores 0-1, OS at 12- months were 89.8% and 91.0% for IVC and EVC, respectively. The HEDEL12 scoring is useful in predicting the survival of advanced-stage cHL patients.

- Abstract
Background and Purpose:
To evaluate the long-term effects of the COVID-19 pandemic on seizure frequency and levels of mental distress in individuals with epilepsy and identify potential risk factors associated with increased seizure frequency.
., Methods:This is a cross-sectional study conducted in Türkiye in May 2021 by phone. Information on epilepsy syndromes, antiseizure medications, average seizure frequency, and drug resistance was obtained from medical records. A questionnaire was completed that included demographic and clinical characteristics and Kessler Psychological Distress Scale 10 (K-10). From people with epilepsy (PWE), seizure control in the month before the pandemic and perceived stress, sleep changes, changes in adaptation during this period, and whether there were changes in seizure control after the pandemic were questioned.
., Results:A total of 227 patients were included, and the K-10 score of 81.9% (186/227) of PWE was found to be ≥30. An increase in seizure frequency was detected in 34 (15%) patients. The factors affecting the increase in seizure frequency were analyzed using logistic regression analysis. In the univariate model hesitate to go to the emergency room despite having seizures during the pandemic (OR= 8.325; 95% CI: [2.943 - 23.551], p=<0.001) was evaluated as the parameter with the highest risk of increased seizure frequency. In multivariate analyses (enter model) only polytherapy (OR= 2.945; 95% CI: [1.152 – 7.532], p=0.024) was detected as the parameter with increased seizure frequency.
., Conclusion:One year after the declaration of the pandemic, we found that stress was still common, the frequency of seizures increased. In multivariate analyses, only polytherapy was detected as the parameter with increased seizure frequency.

- Abstract
Bilateral medial medullary infarction is a rare subtype of stroke.The typical heart-shaped appearance on magnetic resonance imaging is pathognomonic for bilateral medial medullary syndrome. Vertebrobasilar dolichoectasia is a condition characterized by tortuous dilatation and marked enlargement of the basilar and vertebral arteries, and it may cause posterior circulation infarction. We present the case of a 55-year-old female patient with complaints of speech disorder, regression in consciousness, and difficulty breathing. Diffusion-weighted imaging examination was normal on arrival. In the cranial imaging after 24 hours, acute infarction was observed in the bilateral medial medullary area. Time-of-flight magnetic resonance angiography revealed vertebrobasilar dolichoectasia. In this report, a case of bilateral medial medullary infarction with a unique radiological appearance accompanied by vertebrobasilar dolichoectasia, which is rarely reported in the literature, is presented. 113. The Diagnostic Value of Abnormal Bone Marrow Signal Changes on Magnetic Resonance Imaging: Is Bone Marrow Biopsy Essential?

- Abstract
Background: It is essential to determine whether bone marrow signal changes on magnetic resonance imaging (MRI) represent a physiological response or pathology; at present, the clinical significance of these signal changes is unclear. It is unknown whether a bone marrow biopsy is required when bone marrow signal changes are detected incidentally in individuals without suspected malignancy., Objective: The primary purpose of this study was to determine whether incidentally detected bone marrow signal changes on MRI performed for various reasons (at the time of admission or during follow-up) are clinically significant., Methods: We retrospectively evaluated the bone marrow biopsy clinical and laboratory findings of 42 patients with incidental bone marrow signal changes on MRI between September 2016 and January 2020. We also determined whether the patients were diagnosed with malignancy during admission or follow-up., Results: Of the 42 patients, three (7%) were diagnosed with hematological malignancies during admission, while two were diagnosed with multiple myeloma and one with B-cell acute lymphoblastic leukemia. Of the 42 patients, 35 had a mean follow-up of 40.6 ± 5.3 months. One patient was diagnosed with monoclonal gammopathy of undetermined significance four months after their first admission., Conclusions: In addition to MRI, detailed clinical and laboratory evaluations should be performed to inform the decision for bone marrow biopsy and exclude hematological malignancy. If there is any doubt, a bone marrow biopsy should be performed. 114. Retrospective Evaluation of the Results of Low-Dose Intravenous Thrombolytic Therapy in Acute Ischemic Stroke.

Objectives: This study aimed to investigate the clinical data of patients with acute ischemic stroke who received low-dose intravenous (IV) thrombolytic therapy (0.9 mg/kg; maximum 50 mg) for various reasons, compare the obtained results with those of patients who received standard-dose thrombolytic therapy, and discuss them in light of the literature., Methods: Patients who received IV thrombolytic therapy within 4.5 h of symptom onset between January 2015 and June 2018 were retrospectively reviewed. Patients were divided into the low-dose group (0.9 mg/kg; max. 50 mg) and the standard-dose group (0.9 mg/kg; max 90 mg) according to the thrombolytic therapy dose, after which demographic data and clinical results were analyzed., Results: A total of 109 patients receiving thrombolytic therapy (19 patients in the low-dose group and 90 patients in the standard-dose group) were included in the study. There was no significant difference between the two groups in terms of good outcome rates (47.4% vs. 52.2%). There was no statistically significant difference in terms of symptomatic and asymptomatic intracerebral hemorrhage rates., Conclusion: Our study showed similar efficacy and safety for low-dose IV thrombolytic therapy compared with standard-dose IV thrombolytic therapy administered within 4.5 h of symptom onset in patients with acute ischemic stroke., Competing Interests: None declared., (©Copyright 2023 by The Medical Bulletin of Sisli Etfal Hospital.)

Aim: To examine the effect of adiponectin administration on acute brain injury in an experimental model of cerebral ischemia/ reperfusion (I/R) in rats., Material and Methods: The study animals were divided into the following four groups: group I, sham (did not undergo surgical intervention and did not receive drugs); group II, the I/R model (received the intervention, but did not receive drugs); group III (I/Radiponectin) (the I/R model was used, and the animals were treated with 5 mg/kg adiponectin peritoneally 30 minutes after the ischemia); and group IV (I/R-tirofiban)( the I/R model was used, and the animals were treated with 0.5 mg/kg tirofiban peritoneally 30 minutes after the ischemia)., Results: Tumor necrosis factor-? (TNF-?) and interleukin (IL)-1? levels were statistically higher in the I/R group (group II) than in other groups. In the post-hoc (Tukey) test analysis, groups I, III, and IV had significantly lower TNF-? and IL-1? levels after treatment with both tirofiban and adiponectin than group II. No statistically significant difference was found between groups III and IV in terms of TNF-? levels. However, the decreased IL-1? level was more pronounced in group IV (tirofiban) than in other groups. The mean neurologic deficit scores were statistically significantly different among the groups. In the post-hoc (Tukey) test analysis, neurologic deficit scores were statistically significantly lower in groups III and IV than in group II., Conclusion: Adiponectin has anti-inflammatory and cerebral protective effects in experimental cerebral I/R injury.

- Abstract
This study report aimed to investigate the barriers to implementation of electronic health record systems from the perspective of nurses. The research data comprised responses from nurses working in a university hospital. Our data collection instruments were the Participant Information Form and EHR Nurse Opinion Questionnaire, which were developed by the researchers. Data analysis was presented as summary statistics, including mean values of variables, standard deviation, frequency, and percentages. A total of 160 nurses participated in the study. The mean age of participants was 30.94 ± 0.59 years, and 77.5% were university graduates. Barriers to adoption of the electronic health record system included high number of patients (82.8%), limited time (79%), lack of knowledge and skills for effective use of the system (22.9%), lack of user-friendly interface and inability to create a common language within the team (17.8%), and attachment to the traditional method (17.2%). Although most nurses thought that the electronic health record system offered some advantages, they reported that factors such as large numbers of patients, limited time, and lack of user-friendly interface hindered its adoption. Innovative strategies should be explored to develop user-friendly designs for electronic health records and to produce solutions for nursing shortages to increase the time allocated for patient care., (Copyright © 2021 Wolters Kluwer Health, Inc.
