Blood flow restriction training (BFRT) provides an alternative approach to traditional strength training. The purpose of this study was to determine differences in quadriceps muscle activation, subject reported pain, and perceived exertion between three exercise conditions: low-load resistance BFRT with (1) regulated and (2) standardized devices, and (3) high-load resistance exercise without BFRT. Thirty-four healthy subjects (18 male/16 female) each completed three randomized sessions of knee extensions using Delfi's Personalized Tourniquet System (R) at 30% of 1 repetition maximum (1RM), the B-Strong™ device (S) at 30% 1RM, and high-load resistance exercise (HL) at 80% 1RM. Quadriceps EMG activity, numeric pain rating scale (NPRS), and perceived exertion (OMNI-RES) were recorded. Average and peak EMG were greater in HL sessions than both S and R (p <.001). NPRS was greater in the R sessions compared to both S (p <.001) and HL (p <.001). OMNI-RES was greater in the R sessions compared to S (p <.02) and HL (p <.001). No differences (p >.05) in average or peak EMG activation were found between S and R sessions. Quadriceps EMG amplitude was greater during high-load resistance exercise versus low-load BFR exercise and there were no differences in EMG findings between BFRT devices. • Low load BFRT produced lower EMG activity in comparison to high-load resistance exercise. • Utilization of BFRT was better tolerated using a "standard" BFR device. • With a "regulated" BFR device, starting with an occlusion pressure below 80% of LOP may improve tolerance. • Men and women had similar quadriceps activity and perceptual responses to the respective exercise conditions. The present study intended to develop a climatic fire danger model for mid-hills zone of Himachal Pradesh using ten years weather data in relation with forest fire occurrence (2007-2016). Logistic regression technique was used to determine the relationship between fire occurrence and weather parameters viz., maximum temperature (°C), relative humidity (%), and wind speed (ms-1). The model was validated by calculating area under curve (AUC), coefficient of determination (R²) and root mean square Error (RMSE), with estimated values of 88.90%, 0.705 and 0.247, respectively. The fire danger model was verified with actual fire incidences In the study area during the year 2017. Wald's test was carried out to quantify Impact climatic parameters on forest fire. Wald's test value was highest for maximum temperature (40.07) followed by relative humidity (1.15) and wind speed (0.75), respectively. The approach to the radiographic diagnosis of bone tumors is much beyond the conventional radiographs in present era of multiplanar and functional imaging. Radiographs is still the most pertinent part of initial diagnosis of bone tumors, however, there are few limitations, like lesions in complex anatomy, marrow assessment, soft tissue resolution, which are important for staging. Diagnosis is just one aspect of the tumor evaluation, extent of marrow involvement by the tumor growth, involvement of overlying soft tissue, involvement of adjacent joint, and knowledge about skip lesions and metastasis are equally important for staging and treatment of the disease. Multimodality imaging like CT, MRI helps cover all these aspects. Emerging role of PETCT/PET MRI has further revolutionized the imaging of bone tumors by providing anatomical and morphological characteristics simultaneously and combining the whole body scan in same sitting. This article will discuss the role of various imaging modalities along with illustrative examples of few cases. Team work between radiologist with orthopedic oncologist and pathologist will help in deciding a road map for diagnosing and treatment of bone tumors. Purpose: To determine the publication rate of abstracts presented at the annual meetings of the Arthroscopy Association of North America (AANA) from 2011 through 2014 and to compare the level of evidence (LoE) between published and unpublished studies.Methods: A comprehensive search was performed using PubMed and Google Scholar for all abstracts given as podium presentations at the 2011 through 2014 AANA Annual Meetings. A publication rate was calculated from all presented abstracts. Each abstract was reviewed to determine the study's LoE (I-V or nonclinical). Among published studies, the average time from presentation to publication was calculated. The journals in which these studies were published were also noted.Results: A total of 290 abstracts were given as podium presentations at AANA Annual Meetings from 2011 through 2014. Of the 290 studies presented, 195 (195/290, 67%) were published in peer-reviewed journals. Of the 195 published studies, 184 (184/195, 94%) were published within 3 years of the meeting date. Studies were most frequently published in Arthroscopy (n = 59) and the American Journal of Sports Medicine (n = 48). The average time from presentation to publication was 12.2 months. Overall, there was no significant difference between published and unpublished studies in terms of LoE (P = .24).Conclusions: In recent years, studies presented at AANA Annual Meetings have achieved a high publication rate (67%). Based on other studies, this publication rate is comparable to recent American Orthopaedic Society for Sports Medicine (AOSSM) Annual Meetings and is higher than that of AANA Annual Meetings from previous years. The level of evidence of presented studies does not necessarily correlate with eventual publication.Clinical Relevance: The publication rate and level of evidence of podium presentations at AANA demonstrate the scientific impact the annual meeting has for peers pursuing orthopaedic research. Bullying is a significant international problem, and parent-teacher agreement on identifying perpetrators and victims is poor in general population studies. The goal of our study is to assess informant discrepancies in children with mental health disorders. Parents and teachers completed the Pediatric Behavior Scale as part of a diagnostic evaluation for 1,723 children (ages 2-16 years) referred to a psychiatry clinic over the past 10 years. Mother and father bullying and victimization ratings on the Pediatric Behavior Scale were similar, but parent-teacher agreement was poor. Half of parents considered their child a victim, twice the percentage for teachers. Parents were 1.2 times more likely than teachers to perceive their child as a bully. Most parents reported their child was a victim or bully, whereas most teachers reported the children were neither. For both parents and teachers, victim and bully percentages for our psychiatric sample were twice as high as in general population studies. Myopericytoma are uncommon, slow-growing benign perivascular neoplasms that show hemangiopericytoma-like vascular pattern. We report a 52-year-male patient with a painless palpable nodule in the left thumb for the past 9 months. The mass, on the thenar aspect, was 15x12mm in size. X-ray revealed a soft tissue swelling with no bony association. The excised nodule was a non-capsulated, well-circumscribed vascular neoplasm composed of proliferating spindle to ovoid bland cells with eosinophilic cytoplasm. A concentric perivascular arrangement of the cells was seen interspersed by thin-walled, branching, staghorn blood vessels. Nuclear atypia, mitotic figures and necrosis were not observed. Immunohistochemistry revealed diffuse positivity of the tumour cells for smooth muscle actin while staining negative for CD34 and desmin - features suggestive of origin from the perivascular myoid cell. Morphological features of myopericytoma are shared with hemangiopericytoma, glomus tumors, myofibroma and solitary fibrous tumour which form the important differential diagnoses. Background: As recently dictated by the American Medical Society, balance testing is an important component in the clinical evaluation of concussion. Despite this, previous research on the efficacy of balance testing for concussion diagnosis suggests low sensitivity (~30%), based primarily on the popular Balance Error Scoring System (BESS). The Balance Tracking System (BTrackS, Balance Tracking Systems Inc., San Diego, CA, USA) consists of a force plate (BTrackS Balance Plate) and software (BTrackS Sport Balance) which can quickly (<2 min) perform concussion balance testing with gold standard accuracy. Purpose: The present study aimed to determine the sensitivity of the BTrackS Balance Plate and Sports Balance Software for concussion diagnosis. Study Design: Cross-Sectional Study Methods: Preseason baseline balance testing of 519 healthy Division I college athletes playing sports with a relatively high risk for concussions was performed with the BTrackS Balance Tfest. Tfesting was administered by certified athletic training staff using the BTrackS Balance Plate and Sport Balance software. Of the baselined athletes, 25 later experienced a concussion during the ensuing sport season. Post-injury balance testing was performed on these concussed athletes within 48 of injury and the sensitivity of the BTrackS Balance Plate and Sport Balance software was estimated based on the number of athletes showing a balance decline according to the criteria specified in the Sport Balance software. This criteria is based on the minimal detectable change statistic with a 90% confidence level (i.e. 90% specificity). Results: Of 25 athletes who experienced concussions, 16 had balance declines relative to baseline testing results according to the BTrackS Sport Balance software criteria. This corresponds to an estimated concussion sensitivity of 64%, which is twice as great as that reported previously for the BESS. Disruptive mood dysregulation disorder (DMDD) is a controversial diagnosis introduced in the DSM-5 that is particularly relevant to autism and other disorders in which DMDD symptoms (irritable-angry mood and temper outbursts) are common. Mothers rated DMDD symptoms in 1593 children with autism, ADHD, and neurotypical development (6–16 years, IQ ≥ 80). Percentages with DMDD symptoms ( often or very often ) were autism 45%, ADHD-Combined type 39%, ADHD-Inattentive type 12%, and neurotypical 3%. Almost all (91%) with DMDD symptoms met DSM-5 criteria for ODD, and 79% with ODD had DMDD symptoms. Only 5% without ODD had DMDD symptoms, and most of these had autism. Children with autism had significantly higher DMDD scores than all other groups, even when the oppositional behavior score (excluding the two DMDD symptoms) was controlled. The findings suggest that DMDD and ODD are not meaningfully differentiated based on their symptoms and that DMDD symptoms are particularly common in autism, more so than expected by comorbid ODD alone. Aims The study was conducted to analyze the role of Cytomegalovirus (CMV), Rubella and Herpes Simplex Virus (HSV-2) as an etiological agent in congenital infections in infants. Material and methods The study was carried out at National Reference Centre i.e. NCDC, Delhi where samples are referred from various government hospitals of Delhi from the period of January 2013–December 2013. The samples were tested for CMV, Rubella and HSV specific IgM antibodies by μ capture ELISA (Enzyme linked Immunoassay). Results In children the overall positivity of CMV, Rubella and HSV-specific IgM antibodies was 20.7%, 5.4%, and 2.3% respectively. Conclusion The study indicated that maternal infection with CMV, Rubella and HSV-2 is a major contributory factor for congenital infections in neonates and infants and therefore screening for these viruses in early pregnancy should be made mandatory. Objectives: The aims of this study were to: 1) assess the feasibility of left ventricular (LV) vortex flow analysis using contrast echocardiography (CE); and 2) characterize and quantify LV vortex flow in normal subjects and patients with LV systolic dysfunction. Background: Vortices that form during LV filling have specific geometry and anatomical locations that are critical determinants of directed blood flow during ejection. Therefore, it is clinically relevant to assess the vortex flow patterns to better understand the LV function. Methods: Twenty-five patients (10 normal and 15 patients with abnormal LV systolic function) underwent CE with intravenous contrast agent, Definity (Bristol-Myers Squibb Medical Imaging, Inc., North Billerica, Massachusetts). The velocity vector and vorticity were estimated by particle image velocimetry. Average vortex parameters including vortex depth, transverse position, length, width, and sphericity index were measured. Vortex pulsatility parameters including relative strength, vortex relative strength, and vortex pulsation correlation were also estimated. Results: Vortex depth and vortex length were significantly lower in the abnormal LV function group (0.443 ± 0.04 vs. 0.482 ± 0.06, p < 0.05; 0.366 ± 0.06 vs. 0.467 ± 0.05, p < 0.01, respectively). Vortex width was greater (0.209 ± 0.05 vs. 0.128 ± 0.06, p < 0.01) and sphericity index was lower (1.86 ± 0.5 vs. 3.66 ± 0.6, p < 0.001) in the abnormal LV function group. Relative strength (1.13 ± 0.4 vs. 2.10 ± 0.8, p < 0.001), vortex relative strength (0.57 ± 0.2 vs. 1.19 ± 0.5, p < 0.001), and vortex pulsation correlation (0.63 ± 0.2 vs. 1.31 ± 0.5, p < 0.001) were significantly lower in the abnormal LV function group. Conclusions: It was feasible to quantify LV vorticity arrangement by CE using particle image velocimetry in normal subjects and those with LV systolic dysfunction, and the vorticity imaging by CE may serve as a novel approach to depict vortex, the principal quantity to assess the flow structure. Background: The correlation between spontaneous calcium oscillations (S-CaOs) and arrhythmogenesis has been investigated in a number of theoretical and experimental in vitro models. There is an obvious lack of studies that directly investigate how the kinetics of S-CaOs correlates with a specific arrhythmia in the in vivo heart. Objectives: The purpose of the study is to investigate the correlation between the kinetics of S-CaOs and arrhythmogenesis in the intact heart using an experimental model of ischemia/reperfusion (I/R). Methods: Perfused Langendorff guinea pig (GP) hearts were subjected to global I/R (10-15 minutes/10-15 minutes). The heart was stained with a voltage-sensitive dye (RH237) and loaded with a Ca2+ indicator (Rhod-2 AM). Membrane voltage (Vm) and intracellular calcium transient (CaiT) were simultaneously recorded with an optical mapping system of two 16 × 16 photodiode arrays. S-CaOs were considered to arise from a localized focal site within the mapped surface when these preceded the associated membrane depolarizations by 2-15 ms. Results: In 135 episodes of ventricular arrhythmias from 28 different GP experiments, 23 were linked to S-CaOs that were considered to arise from or close to the mapped epicardial window. Self-limited or sustained S-CaOs had a cycle length of 130-430 ms and could trigger propagated ventricular depolarizations. Self-limited S-CaOs that followed the basic beat action potential (AP)/CaiT closely resembled phase 3 early afterdepolarizations. Fast S-CaOs could remain confined to a localized site (concealed) or exhibit varying conduction patterns. This could manifest as (1) an isolated premature beat (PB), bigeminal, or trigeminal rhythm; (2) ventricular tachycardia (VT) when a regular 2:1 conduction from the focal site develops; or (3) ventricular fibrillation (VF) when a complex conduction pattern results in wave break and reentrant excitation. Conclusions: The study examined, for the first time in the intact heart, the correlation between the kinetics of focal S-CaOs during I/R and arrhythmogenesis. S-CaOs may remain concealed or manifest as PBs, VT, or VF. A “benign looking” PB during I/R may represent “the tip of the iceberg” of an underlying potentially serious arrhythmic mechanism.
