119. An audit of primary and secondary care services against the NHS Dental Conscious Sedation Commissioning Guidelines.

Abstract

Background Since 2017 primary care dental sedation providers have been required to undertake an annual service review against the NHS commissioning guideline checklist. This Is adapted from the SAAD safe sedation practice scheme checklist and aims to identify whether practices are missing essential safety procedures, information and/or equipment to facilitate service improvement, patient safety and satisfaction. Methods A Department of Community and Special Care Dentistry and a hospital-based Department of Oral Surgery undertook audits to improve services and to benefit from shared learning. A full audit cycle was conducted using this checklist in these primary and secondary care services. Results In the first cycle, both services identified multiple areas of improvement including the need to review patient experience through patient reported outcome measures, Action plans were informed by group discussion and were implemented to gain patient feedback and create assessment documentation. After the re-audit, there was improvement in compliance for both services, although further staff education was necessary. Conclusions This work highlights the utility of NHS commissioning guidelines as an audit tool for service improvement. The importance of regular audit and the role of collaboration in improving services, through sharing experience and knowledge with peers in other settings, is shown. [ABSTRACT FROM AUTHOR]

Abstract

Synthetic glucocorticoids (GC) are essential for the treatment of a broad range of inflammatory diseases. However, their use is limited by target related adverse effects on e.g. glucose homeostasis and bone metabolism. Starting from a non-steroidal GR ligand (4) that is a full agonist in reporter gene assays, we exploited key functional triggers within the receptor, generating a range of structurally diverse partial agonists. Of these, only a narrow subset exhibited full anti-inflammatory efficacy and a significantly reduced impact on adverse effect markers in human cell assays compared to prednisolone. This led to the discovery of AZD9567 (15) with excellent in vivo efficacy when dosed orally in a rat model of joint inflammation. Compound 15 is currently being evaluated in clinical trials comparing the efficacy and side effect markers with those of prednisolone.
