This study aimed to explore the interaction and antiaging characteristics of composite phase change materials in asphalt for long-term temperature adjustment of asphalt pavement. First, the epoxy resin/phase change materials (E-PCMs) modified asphalt binders were prepared by the melt blending method, and their microscopic, rheological, and temperature adjustment properties were tested. Finally, the long-term performance of the modified asphalt binders was evaluated by simulated aging. The results show that E-PCMs are well dispersed in the modified asphalt binders. E-PCMs can improve the temperature sensitivity and high-temperature stability of asphalt binders, and they could reduce the fluidity of asphalt molecules at high-temperature. The extreme temperature of the E-PCMs–modified asphalt binders can be reduced by 10.4°C and have an excellent temperature adjustment effect. The aged modified asphalt binders still have excellent temperature adjustment properties, and the survival rate of phase change enthalpy is 88%. E-PCMs could also prevent the oxidative aging of asphalt molecules and reduce the adverse effects of the photothermal environment on asphalt. It provides reference and basis for the large-scale promotion and application of temperature-adjusting pavement, which reduces the temperature disease of asphalt pavement and the near-surface thermal environment. National data on patient characteristics, treatment, and outcomes of critically ill coronavirus disease 2019 (COVID-19) solid organ transplant (SOT) patients are limited. We analyzed data from a multicenter cohort study of adults with laboratory-confirmed COVID-19 admitted to intensive care units (ICUs) at 68 hospitals across the United States from March 4 to May 8, 2020. From 4153 patients, we created a propensity score matched cohort of 386 patients, including 98 SOT patients and 288 non-SOT patients. We used a binomial generalized linear model (log-binomial model) to examine the association of SOT status with death and other clinical outcomes. Among the 386 patients, the median age was 60 years, 72% were male, and 41% were black. Death within 28 days of ICU admission was similar in SOT and non-SOT patients (40% and 43%, respectively; relative risk [RR] 0.92; 95% confidence interval [CI]: 0.70-1.22). Other outcomes and requirement for organ support including receipt of mechanical ventilation, development of acute respiratory distress syndrome, and receipt of vasopressors were also similar between groups. There was a trend toward higher risk of acute kidney injury requiring renal replacement therapy in SOT vs. non-SOT patients (37% vs. 27%; RR [95% CI]: 1.34 [0.97-1.85]). Death and organ support requirement were similar between SOT and non-SOT critically ill patients with COVID-19.

Blade coating offers a low-cost production method for fabricating perovskite thick-film x-ray detectors. However, due to the preferential nucleation at the gas–liquid interface of a perovskite solution, high-quality perovskite thick films are in such a dilemma, sacrificing either the density of the thick film by forming a porous structure or the thickness of the film through depositing a compact structure. The porous structure will lead to carrier recombination and ion migration while low thickness will cause insufficient x-ray absorption, which will increase the dark current or reduce the sensitivity of the corresponding x-ray detector. In the present work, a scalable ferroelectric dipole–methylamine lead iodide (MAPbI3) coupled x-ray detector is proposed, where the x-ray photoconductor in situ formed on indium tin oxide shows a dense fibrous mesoscopic structure oriented along the charge-carriers transport direction. It is demonstrated that ferroelectric dipoles promise not only strong coupling effects with MAPbI3, driving better crystallinity and denser assemblies, but also excellent passivation to MAPbI3 interfaces, decreasing trap-state densities, leading to improved direct x-ray detection performance. In addition, the enhanced operational stability of the corresponding device suggests a strategy of introducing the localized polarization field for stable perovskite direct x-ray detectors. Objectives. The tricuspid anterior leaflet is considered important in most repair techniques for Ebstein anomaly (EA). We aim to assess the anterior leaflet morphology using novel metrics and investigate the association of the morphology with recurrent moderately severe or greater tricuspid regurgitation (TR). Methods. Seventy-four paediatric patients with EA undergoing cone reconstruction (CR) between 2010 and 2021 were included. Anterior leaflet mobility (ALM) and anterior leaflet length (ALL) were remeasured on preoperative 2D echocardiography. The prediction accuracies of ALM and ALL-I (ALL indexed to body surface area) for recurrent TR were evaluated using receiver operating characteristic (ROC) curve analyses. Results. The median age of patients was 3.3 years (interquartile range, 1.9–7.1 years). Both ALM and ALL-I correlated with the Carpentier type and GOSH score. Nine patients (12.2%) developed recurrent TR during the one-year follow-up. By univariable logistic regression analyses, ALM (odds ratio [OR], 0.89; 95% CI [confidence interval], 0.82–0.96; p = 0.003) and ALL-I (OR, 1.39; 95% CI, 1.08–1.78; p = 0.011) were risk factors for recurrent TR. ROC curve analyses showed that ALM (AUC = 0.81) and ALL-I (AUC = 0.77) had better predictive performance for recurrent TR compared with the GOSH score (AUC = 0.68), the Carpentier type (AUC = 0.67), and preoperative TR severity (AUC = 0.58), and the combinations of ALM and ALL-I (AUC = 0.87) improved the predictive performance compared with ALM or ALL-I alone. Conclusions. ALM and ALL-I can help optimize evaluation in the anterior leaflet morphology and predict recurrent TR after CR in pediatric EA. [ABSTRACT FROM AUTHOR]

IMPORTANCE: Therapies that improve survival in critically ill patients with coronavirus disease 2019 (COVID-19) are needed. Tocilizumab, a monoclonal antibody against the interleukin 6 receptor, may counteract the inflammatory cytokine release syndrome in patients with severe COVID-19 illness. OBJECTIVE: To test whether tocilizumab decreases mortality in this population. DESIGN, SETTING, AND PARTICIPANTS: The data for this study were derived from a multicenter cohort study of 4485 adults with COVID-19 admitted to participating intensive care units (ICUs) at 68 hospitals across the US from March 4 to May 10, 2020. Critically ill adults with COVID-19 were categorized according to whether they received or did not receive tocilizumab in the first 2 days of admission to the ICU. Data were collected retrospectively until June 12, 2020. A Cox regression model with inverse probability weighting was used to adjust for confounding. EXPOSURES: Treatment with tocilizumab in the first 2 days of ICU admission. MAIN OUTCOMES AND MEASURES: Time to death, compared via hazard ratios (HRs), and 30-day mortality, compared via risk differences. RESULTS: Among the 3924 patients included in the analysis (2464 male [62.8%]; median age, 62 [interquartile range {IQR}, 52-71] years), 433 (11.0%) received tocilizumab in the first 2 days of ICU admission. Patients treated with tocilizumab were younger (median age, 58 [IQR, 48-65] vs 63 [IQR, 52-72] years) and had a higher prevalence of hypoxemia on ICU admission (205 of 433 [47.3%] vs 1322 of 3491 [37.9%] with mechanical ventilation and a ratio of partial pressure of arterial oxygen to fraction of inspired oxygen of
