106. Association Between Antibiotic Use and Outcome Among Metastatic Melanoma Patients Receiving Immunotherapy.

Abstract

Background Several observational studies have reported a decreased response to immune checkpoint inhibitors (ICI) following antibiotic use. ICI activity has been hypothesized to be impaired by antibiotic-induced gut dysbiosis. Methods Patients with advanced melanoma receiving an anti-PD-1 antibody as a first-line therapy between 2015 and 2017 in France were selected using the French Health Insurance database. We compared overall survival and time-to-treatment discontinuation according to antibiotic exposure in the 3 months prior to the initiation of anti-PD-1 antibody. To disentangle a causal effect of antibiotics from a confounding bias, we balanced characteristics of patients exposed and nonexposed to antibiotics using an overlap weighting method based on a propensity score. We also evaluated a control cohort of patients with advanced melanoma receiving first-line targeted therapy, as there is no rationale for decreased efficacy of targeted therapy following antibiotic treatment. Results The anti-PD-1 antibody cohort comprised 2605 individuals. Antibiotic exposure in the 3 months prior to anti-PD-1 antibody initiation was not associated with shorter overall survival (weighted hazard ratio = 1.01, 95% confidence interval = 0.88 to 1.17) or time-to-treatment discontinuation (weighted hazard ratio = 1.00, 95% confidence interval = 0.89 to 1.11). Consistent results were observed when the time frame of antibiotics was narrowed to 1 month prior to anti-PD-1 initiation or when exposure was restricted to antibiotics leading to more profound gut dysbiosis. Similar results were observed in the targeted therapy cohort. Conclusions In a large cohort of advanced melanoma patients, we showed that antibiotic use preceding anti-PD-1 antibody was not associated with worse outcome. Abstract

Staphylococcus aureus is the main bacterial pathogen encountered in mediastinitis after cardiac surgical procedures; it remains a devastating complication with a high mortality rate. As neutrophils have a primordial role in the defense against staphylococcus infection and cardiopulmonary bypass (CPB) is known to induce immunosuppression, the aim of this study was to investigate CPB impact on neutrophil functions. Patients without known immunosuppression scheduled for cardiac surgery with CPB were included. Bone marrow and blood samples were harvested before, during, and after surgery. Neutrophil phenotypic maturation and functions (migration, adhesion, neutrophil extracellular trap [NET] release, reactive oxygen species (ROS) production, phagocytosis, and bacteria killing) were investigated. Two types of Staphylococcus aureus strains (one from asymptomatic nasal carriage and another from mediastinitis infected tissues) were used to assess in vitro bacterial direct impact on neutrophils. We found that CPB induced a systemic inflammation with an increase in circulating mature neutrophils after surgery. Bone marrow sample analysis did not reveal any modification of neutrophil maturation during CPB. Neutrophil lifespan was significantly increased and functions such as NET release and ROS production were enhanced after CPB whereas bacteria killing and phagocytosis were not impacted. Results were similar with the two different isolates of Staphylococcus aureus. These data suggest that CPB induces a recruitment of mature neutrophils via a demargination process rather than impacting their maturation in the bone marrow. Abstract

Background: Alternative treatments are needed against NDM-1-producing Escherichia coli. Colistin (COL) and fosfomycin (FOS) often remain active in vitro but selection of resistant mutants is frequent if used separately. We determined whether the combination of colistin and fosfomycin may be useful to treat infections with NDM-1-producing E. coli with varying levels of resistance.Methods: Isogenic derivatives of E. coli CFT073 with blaNDM-1 and variable levels of resistance to colistin and fosfomycin (CFT073-NDM1, CFT073-NDM1-COL and CFT073-NDM1-FOS, respectively) were used. The combination (colistin + fosfomycin) was tested in vitro and in a fatal peritonitis murine model. Mortality and bacterial loads were determined and resistant mutants detected.Results: Colistin MICs were 0.5, 16 and 0.5 mg/L and fosfomycin MICs were 1, 1 and 32 mg/L against CFT073-NDM1, CFT073-NDM1-COL and CFT073-NDM1-FOS, respectively. In time-kill curves, combining colistin with fosfomycin was synergistic and bactericidal against CFT073-NDM1 and CFT073-NDM1-FOS, with concentrations of 4× MIC (for both drugs), but not against CFT073-NDM1-COL (concentrations of colistin = 0.5× MIC), due to regrowth with fosfomycin-resistant mutants. Mice died less and bacterial counts were lower in spleen with the combination compared with monotherapy against all strains; the combination prevented selection of resistant mutants except for CFT073-NDM1-COL where fosfomycin-resistant mutants were found in all mice.Conclusions: Combining colistin and fosfomycin was beneficial in vitro and in vivo against NDM-1-producing E. coli, even with strains less susceptible to colistin and fosfomycin. However, the combination failed to prevent the emergence of fosfomycin-resistant mutants against colistin-resistant strains. Combining colistin and fosfomycin constitutes an alternative for treatment of NDM-1 E. coli, except against colistin-resistant strains. [ABSTRACT FROM AUTHOR]

Abstract

International audience; Objectif - Introduction : La colistine (COS), antibiotique longtemps délaissé, est devenue une alternative de dernier recours pour le traitement des infections à entérobactéries multirésistantes, notamment productrices de carbapénémases. Le complexe Enterobacter cloacae (CEC) comprend plusieurs clusters étroitement liés et non différenciables phénotypiquement, dont certains présentent un phénotype d’hétérorésistance (HR) à la COS. Même si plusieurs gènes ont été identifiés (ex. tolC, phoP et soxS-soxR) dans cette HR, les mécanismes restent en grande partie inconnus. En 2019, Huang et al. ont décrit le gène ecr qui serait également impliqué dans l’HR en régulant positivement l’expression de phoP et de l’opéron arnBCADTEF. Le but de ce travail a été de rechercher la présence du gène ecr par PCR en temps réel dans une collection de souches de CEC déjà caractérisées et par une approche in silico à partir des séquences génomiques disponibles.Matériels et méthodes : Une collection de 100 souches cliniques uniques représentatives des différents clusters du CEC (C-I à C-XIV) a été étudiée. Les souches identifiées par séquençage partiel du gène hsp60 ont été caractérisées pour l’HR à la COS par CMI par microdilution et analyse de population. Après le design d’amorces spécifiques, une technique de PCR en temps réel a été développée. Parallèlement, le gène ecr a été recherché par une analyse in silico sur 1560 souches du CEC.Résultats : Sur les 100 souches, 48 présentaient une HR à la COS et appartenaient aux clusters hsp60suivants : C-I (7/8), C-II (16/16), C-III (1/14), C-IV (8/9), CVII (2/4), C-IX (3/3), C-X (1/2), C-XI (8/9), C-XII (2/2). Le gène ecr a été retrouvé uniquement pour les clusters C-I (7/7, 100%), C-IV (7/8, 87,5%), C-VII (1/2) et C-X (1/1) par PCR. L’analyse in silico des 1560 génomes a montré la présence du gène ecr uniquement pour les clusters C-I (87/87, 100%) et C-IV (86/92, 93,5%). Conclusion : Dans cette étude, nous avons pu montrer que le gène ecr semble particulièrement présent chez les clusters C-I et C-IV. Ces résultats montrent l’importance de la détermination précise du cluster en matière d’interprétation de la sensibilité à la COS. L’analyse génomique doit être étendue aux 100 souches cliniques de ce panel pour confirmer cette association exclusive à certains clusters et redéfinir ainsi précisément le cluster des 2 souches ecr positif non C-I ou C-IV.

Abstract

International audience; Major Facilitator Superfamily (MFS) efflux pumps have been shown to be important for bacterial cells to cope with biocides such as chlorhexidine (CHX), a widely used molecule in hospital settings. In this work, we evaluated the role of two genes smvA and smvR in CHX resistance in Enterobacter cloacae complex (ECC). smvA encodes an MFS pump whereas smvR, located upstream of smvA, codes for a TetR-type transcriptional repressor. To this aim, we constructed corresponding deletion mutants from the ATCC 13047 strain (MIC = 2 mg/L) as well as strains overexpressing smvA or smvR both in ATCC 13047 and three clinical isolates exhibiting elevated MICs of CHX (16-32 mg/L). Determination of MICs revealed that smvA played a modest role in CHX resistance, contrarily to smvR that modulated the ability of ECC to survive in the presence of CHX. In clinical isolates, the overexpression of smvR significantly reduced MICs of CHX (2-8 mg/L). Sequences analyses of smvR and promotor regions pointed out substitutions in conserved regions. Moreover, transcriptional studies revealed that SmvR acted as a repressor of smvA expression even if no quantitative correlation between the level of smvA mRNA and MICs of CHX could be observed. On the other hand, overproduction of smvA was able to complement the lack of the major resistance-nodulation-cell division (RND) superfamily efflux pump AcrB and restored resistance to ethidium bromide and acriflavine. Although SmvA could efflux biocides such as CHX, other actors, of which the expression is under SmvR control, should play critical role in ECC.
