Objective: The purpose of this study was to explore healthcare provider-perceived challenges to HBPC patient referral and elicited providers' feedback for overcoming these challenges. Methods: We conducted a qualitative study using semi-structured interview s with 25 Medicaid managed care providers (primary care physicians, nurse practitioners, and care managers) working in the greater Los Angeles area. Our interview protocol elicited providers' knowledge and awareness of palliative care; perceived barriers to HBPC referral; and suggestions for overcoming these barriers. We analyzed verbatim transcripts using a grounded theory approach. Results: Themes related to referral barriers included providers' lack of palliative care knowledge and clarity regarding referral processes, provider reluctance to refer to HBPC, and provider culture. Providers also identified patient-level barriers, including financial barriers, reluctance to have home visits, health literacy, cultural barriers, and challenges related to living situations. Themes related to methods for overcoming challenges included increased HBPC education and outreach to providers, specifically by HBPC agency staff. Conclusions: Findings from this study underscore the need for additional palliative care education for Medicaid healthcare providers. They point to the need for novel strategies and approaches to address the myriad barriers to patient identification and referral to HBPC. [ABSTRACT FROM AUTHOR]

Background: Previous research has found racial differences in hospice knowledge and misconceptions about hospice care, which may hinder access to hospice care. Asian Indians are a rapidly growing population in the United States, yet limited research has focused on their beliefs toward end-of-life care. This project investigates Indian Americans' knowledge of and attitudes toward hospice care and advance care planning. Procedures: A cross-sectional design was employed using surveys about participants' knowledge of and attitudes toward hospice care and advance care planning. Surveys were conducted among Indian Americans, age 60 and over, recruited from Indian cultural centers in Northern California. The participants were first asked questions about hospice care. They were then given a summary explanation of hospice care and later asked about their attitudes toward hospice care. Data were analyzed using descriptive and bivariate analyses. Results: Surveys were completed by 82 participants. Findings revealed that 42.5% of respondents had an advance directive and 57.1% had named a health care proxy. Only 10% of respondents had known someone on hospice care and 10.4% correctly answered 4-5 of the knowledge questions. After being informed about hospice care, 69.6% of participants agreed that if a family member was extremely ill, they would consider enrolling him/her in hospice. Conclusions: This study's results present a need for greater education about hospice services among older Asian Indians. Health practitioners should remain cognizant of potential misconceptions of hospice and cultural barriers that Asian Indians may have toward hospice care, so they can tailor conversations accordingly. Objectives: Medication‐related problems remain one of the largest health risks for older adults, yet there are few resources available to effectively reduce medication‐related problems for community dwelling older adults. The aim of this pilot program was to determine the effectiveness of a multifaceted medication intervention on medication adherence and self‐efficacy in medication use. Design: A single sample, pre‐post‐test design. Setting: This pilot study was conducted at Independence at Home, a Senior Care Action Network (SCAN) community service in Southern California. Participants: One hundred and eighty community‐dwelling, diverse older adults with potential medication‐related problems and their caregivers. Intervention: The Community Medication Education, Data, & Safety (C‐MEDS) Program identified community‐dwelling older adults with potential medication‐related problems and provided in‐depth personalized medication safety, management and support, based on an in‐home assessment and interventions delivered by trained geriatrics experts including pharmacists, nurses, a community health worker, and a pharmacy technician. Measurements: The primary outcomes included medication use self‐efficacy, measured by the MUSE, and select medication adherence measures. Adherence was measured via pill count and via the MedAdhIR tool, a scale that measures risk for medication non‐adherence. Results: Following participation in the C‐MEDS program, community‐dwelling older adult C‐MEDS graduates (n = 105) had higher self‐efficacy in managing medications (P <.001). Additionally, C‐MEDS graduates also exhibited increased adherence to three types of medications (statins, diabetes‐related, and certain hypertension drugs; P <.001). Moreover, risk of non‐adherence declined significantly following C‐MEDS graduation (P <.001). Conclusion: C‐MEDS holds promise as an effective intervention among community‐dwelling older adults in improving medication self‐efficacy, medication adherence among select medications, and reducing risk for non‐adherence. Objective: To test the effectiveness of theoretically driven role model video stories in improving knowledge of palliative care among a diverse sample of older adults.Method: We developed three 3-4 min long theoretically driven role model video stories. We then recruited cognitively intact, English-speaking adults aged 50 and older from senior centers, assisted living, and other community-based sites in the greater Los Angeles area. Using a pretest-posttest study design, we surveyed participants using the 13-item Palliative Care Knowledge Scale (PaCKS) and also asked participants about their intentions to enroll in palliative care should the need arise. Participants first completed the pretest, viewed the three videos, then completed the posttest comprised of the same set of questions.Results: PaCKS score improved from an average of 4.5 at baseline to 10.0 following video screening (t(126) = 12.0, p < 0.001). Intentions to enroll oneself or a family member in palliative care rose by 103% (χ2 = 7.8, p < 0.01) and 110% (χ2 = 7.5, p < 0.01), respectively. Regression analysis revealed that participants who believed the role models are real people (β = 2.6, SE = 1.2, p < 0.05) significantly predicted higher change in PaCKS score. Conversely, participants with prior knowledge of, or experience with, palliative care (β = -5.9, SE = 0.8, p < 0.001), non-whites (β = -3.6, SE = 0.9, p < 0.001), and widows (β = -2.9, SE = 1.1, p < 0.01) significantly predicted lower changes in PaCKS score.Significance Of Results: This study suggests that theoretically driven role model video stories may be an effective strategy to improve palliative care knowledge. Background Two strategies for rating the quality of nursing homes (NHs) in the USA are exemplified by the Nursing Home Compare (NHC) website, launched by the federal Centers for Medicare and Medicaid Services in 1998, and Yelp. com, an online consumer review site that has grown in popularity since its founding in 2004. Both sites feature a 5-star rating system. While much is known about NHC ratings, little is known about NH Yelp ratings. This study examines Yelp ratings for NHs in California and compares these ratings with NHC ratings. Understanding how these ratings relate can inform efforts to empower consumers and enhance NH decision-making. Methods We collected NHC and Yelp ratings for all California NHs between September and November 2016. For each NH with a Yelp review, we recorded the star rating and the total number of reviews. We also recorded the NHC 5-star rating and NHC ratings for inspections, staffing and quality measures (QMs). We conducted descriptive statistics and frequencies for these variables. We conducted correlations and tested means to compare the ratings. Results A total of 675 NHs (68.1% of the total sample of 991 NHs) had both Yelp ratings and NHC ratings. Correlations between the Yelp and NHC ratings were relatively weak. The Yelp rating was significantly lower than the 5-star NHC rating and the NHC ratings for staffing and QMs. It was significantly higher than the NHC inspection rating. Conclusions This study found that when consumers rate NHs on Yelp, their ratings differ considerably from NHC ratings, a finding in keeping with similar studies of NH quality measurement. Further research is needed to analyse the content of Yelp reviews and compare the themes and topics consumers identify with those reported on NHC. [ABSTRACT FROM AUTHOR]

Objective: Latinos are disproportionately affected by diabetes and people with diabetes experience frequent hospital admissions and readmissions. Care transition interventions can help reduce rates of readmission; however, there are many barriers to recruiting Latinos for participation in intervention research. Exploring reasons for study refusal furthers understanding of low research participation rates to help researchers address barriers. Design: This study presents a cross-sectional, descriptive analysis of reasons for study refusal and attrition drawing from data collected as part of a randomized controlled trial conducted to test the effectiveness of a transitions intervention for diabetic Latino discharged from the hospital to home. Reasons for refusal were elicited from participants, transcribed, and coded. Descriptive statistics and bivariate analyses were used to compare those who completed the study and those who did not complete the study. Results: Reasons for study enrollment refusal and loss to follow-up ranged from difficulty locating the patient to homelessness to patient reluctance to having providers in the home. Study completers were more likely than non-completers to reside with family members (p = .03) and have a spouse as a primary caregiver (p = .08). Conclusions: Inner city, monolingual Latinos may be difficult to enroll and engage in home-based interventions. Although not representative of all Latino populations, barriers encountered highlight the need for targeted research to improve transitional care among Latinos. Researchers and clinicians should take into consideration the unique barriers that Latinos face in participating in research and intervention studies. [ABSTRACT FROM PUBLISHER]
