Aim: Although ultrasonography (USG) and scintigraphy are frequently used in the detection of parathyroid adenomas, they are sometimes insufficient for this task. This situation leads to failures in surgical treatment. Measurement of parathormone (PTH) in the washout fluid obtained by USG-guided fine needle aspiration (FNA) is easy, cost-effective and safe, and can be used to localize the parathyroid lesion. We aimed to present our cases in which we measured PTH in the washing liquid obtained by FNA and review the literature.Method: A total of 16 cases were evaluated retrospectively, and age, gender, laboratory values, imaging, DEXA reports and pathology results of the operated patients were recorded. For the measurement of PTH in the washout liquid, negative aspiration was performed from the suspicious lesion with a 10 mL/22 gauge needle and the aspirated needle was rinsed with 1 mL of 0.9% NaCl and PTH was measured from the washing liquid. A washout PTH value higher than the serum value was considered as positive.Results: Thirteen of the patients were female and 3 were male, with a mean age of 57.1 ± 14.2 (18-73) years. Washout PTH result was positive in 3 patients with involvement in scintigraphy, negative in 5 patients without involvement, and positive in 2 patients with suspicious involvement. Washout PTH result was positive in 4 patients without involvement in scintigraphy, and 44.5% positive result were detected with this method in those without involvement. Although there was involvement in scintigraphy, the washing result of 2 patients was negative. No procedure-related side effects occurred in any of the patientsConclusion: PTH measurement in the FNA washing fluid performed under ultrasonography is a procedure that can be safely performed by experienced people in selected cases whose location cannot be determined by USG and scintigraphy, or with suspicious involvement in scintigraphy and without suspected cancer., Amaç: Paratiroid adenomlarının tespitinde ultrasonografi (USG) ve sintigrafidir çok sık kullanılmasına rağmen bazen yetersiz kalabilmektedirler. Bu durum da cerrahi tedavide başarısızlıklara yol açmaktadır. USG kılavuzluğunda ince iğne aspirasyon (İİA) ile elde edilen yıkama sıvısında parathormon (PTH) ölçümü ise kolay, maliyet etkin ve güvenli olması dolayısıyla paratiroid lezyonunu lokalize etmekte kullanılabilmektedir. İİA ile elde edilen yıkama sıvısında PTH ölçümü yaptığımız olgularımızı sunarak literatür bilgilerini gözden geçirmeyi amaçladık. Metot: Toplam 16 olgu geriye doğru taranarak yaş, cinsiyet, laboratuvar değerleri, görüntüleme, DEXA raporları ve opere olanların patoloji sonuçları kaydedildi. Yıkama sıvısında PTH ölçümü için şüpheli lezyona 10 mL/22 gauge iğne ile girildi, negatif aspirasyon yapıldI ve aspirasyon yapılan iğne 1 mL %0.9 NaCl ile durulanarak yıkama sıvısından PTH bakıldı. Serum değerinden daha yüksek çıkan yıkama PTH değeri pozitif olarak kabul edildi. Bulgular: Hastaların 13’ü kadın, 3’ü erkekti, ortalama yaş 57.1 ± 14.2 (18-73) yıl idi.. Sintigrafide tutulum olan 3 hastanın yıkama sonucu pozitif, tutulum olmayan 5 hastanın yıkama sonucu negatif, şüpheli tutulum olan 2 hastanın ise yıkama sonucu pozitifti. Sintigrafide tutulum olmayan 4 hastanın ise yıkama sonucu pozitifti ve tutulum olmayanların ise %44.5’i bu yöntemle tespit edildi. Sintigrafide tutulum olduğu halde 2 hastanın yıkama sonucu negatif saptandı. Hiç bir hastada işleme bağlı bir yan etki oluşmadı.Sonuç: Ultrasonografi eşliğinde yapılan İİA yıkama sıvısında PTH ölçümü USG ve sintigrafi ile yeri tespit edilemeyen veya sintigrafide şüpheli tutulumu olan, kanseri şüphesi olmayan seçilmiş vakalarda, bu konuda tecrübeli kişilerce güvenle uygulanabilecek bir işlemdir.

Amaç:. Bu çalışmada, Paraoksonaz 1 geni kodlanan bölge Q192R ve L55M polimorfizmleri ile Tip2 Diyabetli hastalarda Diyabetik Nefropati gelişimi arasındaki ilişkinin araştırılması amaçlandı. Materyal ve metod: Harran Üniversitesi Tıp Fakültesi Endokrinoloji Bilim Dalı polikliniklerine başvuran; Tip2 Diyabetli 50 hasta ( 25 erkek, 25 kadın; ort. yaş 52.06±8.43), Diyabetik Nefropatili 50 hasta ( 24 erkek, 26 kadın; ort. yaş 50.94±10.75) ve 50 sağlıklı kontrol (12 erkek, 38 kadın; ort. yaş 50.42±11.062) grupları çalışmaya alındı. Alınan periferik kan örneklerinden DNA izolasyonu yapıldı. Polimeraz zincir reaksiyonu ile elde edilen ürünler restriksiyon enzimleri AlwI ve Hin1II ile kesildi. Elde edilen ürünler agaroz jelde yürütüldü. UV görüntüleme ile polimorfizm genotiplemesi yapıldı. Bulgular: Paraoksonaz 1 geni Q192R (584A/G) polimorfizmini genotip dağılımı: Tip 2 Diyabet hasta grubunda; QQ %58, QR %32 ve RR %10 bulundu. Diyabetik Nefropati grubunda; QQ %52, QR %42 ve RR %6 bulundu. Sağlıklı kontrol grubunda; QQ %62 , QR %30 ve RR %8 bulundu. Gruplar arasında genotip frekansları yönünden istatistiksel olarak anlamlı bir fark görülmedi (p>0.05). Paraoksonaz 1 geni L55M (172T/A) polimorfizminin genotip dağılımı: Tip 2 Diyabet hasta grubunda LL %48, LM %32 ve MM %20 bulundu. Diyabetik Nefropati grubunda; LL %68, LM %26 ve MM %6 bulundu. Sağlıklı kontrol grubunda; LL %42, LM %42 ve MM %16 bulundu. Gruplar arasında genotip dağılımı yönünden istatistiksel olarak anlamlı bir fark görülmedi (p>0.05). M allel frekansının Tip 2 DM’li ve DN’li grupta istatistiksel olarak anlamlı olduğu görüldü (sırasıyla p=0.007, p=0.011).Sonuç: Bulgularımıza göre, Paraoksonaz 1 L55M allel frekansının, Tip2 Diyabet ve Diyabetik Nefropati hasta grubunda anlamlı çıkması, Paraoksonaz 1 L55M polimorfizminin bu hastalıkların gelişiminde risk faktörü olabileceğini düşündürmektedir. Paraoksonaz 1 geni Q192R ve L55M polimorfizmlerinin, Tip 2 Diyabet hastalarında Diyabetik Nefropatiye yakalanma riski ile ilişkili olmadığı görüldü., Background: In this study, it was aimed to investigate the relationship between Q192R and L55M polymorphisms in the region encoding the Paraoxonase 1 (PON1) gene and the development of Diabetic Nephropathy in patients with Type 2 Diabetes.Materials and Methods: Applying to Harran University Medical Faculty Endocrinology Department polyclinics; 50 patients with Type 2 Diabetes, 50 patients with Diabetic Nephropathy and 50 healthy controls groups were included in the study. DNA isolation was performed from peripheral blood samples. The products obtained by polymerase chain reaction were cut with the restriction enzymes AlwI and Hin1II. The obtained products were run on an agarose gel. Polymorphism genotyping was performed by UV imaging.he study was planned as descriptive cross-sectional. The attitudes of the students studying at the Faculty of medicine towards scientific research were questioned. In the study, the ‘’Attitude Scale Towards Scientific Research ‘’ was used.Results: The genotype distribution of the paraoxonase 1 gene Q192R (584A/G) polymorphism: It was found QQ 58%, QR 32%, and RR 10% in Type 2 Diabetes patient group. It was found QQ 52%, QR 42%, and RR 6% in Diabetic Neph-ropathy group. It was found QQ 62%, QR 30%, and RR 8% in healthy control group. There was no statistically signifi-cant difference between the groups in terms of genotype frequencies (p>0.05). Genotype distribution of Paraoxona-se 1 gene L55M (172T/A) polymorphism:It was found LL 48%, LM 32%, and MM 20% in Type 2 Diabetes patient group. It was found LL 68%, LM 26%, and MM 6% in Diabetic Nephropathy group. It was found LL 42%, LM 42%, and MM 16% in healthy control group. There was no statistically significant difference between the groups in terms of genotype frequencies (p>0.05). M allele frequency was found to be statistically significant in Type 2 Diabetes and Diabetic Nephropathy groups (p=0.007, p=0.011, respectively).Conclusions: According to our findings, the fact that Paraoxonase 1 L55M allele frequency was significant in Type 2 Diabetes and Diabetic Nephropathy patient groups suggests that Paraoxonase 1 L55M polymorphism may be a risk factor in the development of these diseases. Paraoxonase 1 gene Q192R and L55M polymorphisms were not associa-ted with the risk of developing Diabetic Nephropathy in Type 2 Diabetes patients.

Amaç: Şanlıurfa ilinde 3. Basamak hastanemizde Hashimoto tiroiditi tanısıyla takip edilen hastaların klinik, demografik, laboratuvar özelliklerinin değerlendirilmesi.Yöntem: Çalışmaya Mayıs 2019 – Eylül 2020 tarihleri arasında prospektif olarak Dahiliye ve Endokrinoloji Poliklinikleri’ ne başvuran hashimoto tiroiditi tanılı 48 hasta ve 40 sağlıklı gönüllü olmak üzere toplam 88 kişi dahil edildi. Çalışma süresince hastaların ayrıntılı anamnezi, fizik muayeneleri, demografik verileri kayıt altına alındı. Hastaların tansiyonu, boy ve kiloları ölçüldü, vücut kitle indekleri hesaplandı, hemogram ve biyokimya parametreleri değerlendirildi.Bulgular: Yaş, cinsiyet ve vücut kitle indeksi olarak gruplar arasında anlamlı farklılık yoktu. Ferritin düzeyi ve D vitamini düzeyi hasta grubunda kontrol grubuna göre sırasıyla (p= 0.00), (p= 0.00) istatistiksel olarak anlamlı düşük saptandı. Total kolesterol ve LDL düzeyleri hasta grubunda kontrol grubuna göre sırasıyla (p= 0.00), (p= 0.00) istatistiksel olarak anlamlı yüksek saptandı.Sonuç: Hashimoto tiroiditi etyolojisi net olarak saptanamamış otoimmün bir hastalıktır. HT’ nin kliniğinde vitamin eksiklikleri, anemi ve dislipidemi önemli bir yere sahiptir. Çalışmamız da göstermiştir ki hastaların takibinde akılda tutulmalıdır.

Pediatric injuries caused by beating by a parent or caregiver are referred to as physical child abuse. Suspicion of physical child abuse and reporting to legal authorities can prevent repetitive abuse. Appropriate legal notification can only be possible with accurate diagnosis of cases, recognition of risk factors and understanding of specific injuries. Therefore, it is important to recognize the specific features in the history, physical examination and clinical findings of possible cases. With the current literature review, we aimed to compile the clinical characteristic findings that increase the suspicion of physical child abuse and the legal importance of this issue for healthcare professionals., Fiziksel çocuk istismarı tanımı bir ebeveyn veya bakım üstlenen kişi tarafından darp yoluyla gerçekleştirilmiş yaralanmayı ifade eder. Fiziksel istismar şüpheli olguya uygun yaklaşım ve yasal mercilere bildirim; tekrar eden istismarı koruyucu tedbirlerle engelleyebilecektir. Bu olguların doğru tanılanması, risk faktörleri ve özgü yaralanmaların tanınması ile mümkün olabilir. Hastanın hikayesinde, fizik muayenesinde ve klinik bulgularındaki uyarıcı bulguların tanınması bu nedenle önemlidir. Mevcut literatür gözden geçirmesi ile fiziksel çocuk istismarı şüphesini arttıran klinik karakteristik bulguları ve bu konunun sağlık çalışanları açısından yasal çerçevesini derlemeyi amaçladık.

Regional radiotherapy was used in 64.2% of the patients with 1– 3 axillary lymphatic involvement and unfavorable prognostic factors. In contrast, 61.2% of the respondents indicated that IMN should be included in the target volume for regional radiotherapy when the patient had one positive node after axillary lymph node dissection (ALND) in the inner quadrant and central region tumors. However, 71.5% of the respondents chose to include the IMN in the non-inner quadrant and non-central region tumors for patients with four or more positive nodes after ALND. The decision to offer internal mammary lymph node radiotherapy (IMNRT) varied widely and significantly among respondents, years in practice, and the rates of dedicating their clinical time to patients with breast cancer.Conclusion: The results of this survey revealed significant national variation in attitudes regarding the treatment of IMN. Sigortacılık sektörü, oluşabilecek risklere karşı bireyleri ve firmaları güvence altına alan bir sektördür. Sürdürülebilir bir büyüme için sigortacılık sektörünün gelişmesi gerekmektedir. Sektörün gelişmesinde, ülkenin sahip olduğu makroekonomik verilerin boyutu önem arz etmektedir. Çalışmanın amacı, Türkiye’de 1983-2019 dönemine ait yıllık verilerle sigortacılık sektörü ile seçili makroekonomik değişkenler arasındaki ilişkiyi ampirik olarak araştırmaktır. Ekonomik büyüme, enflasyon ve tasarruf değişkenleri sigortacılık sektörünü etkileyen değişkenler olarak belirlenmiştir. Sigortacılık sektörünü temsilen brüt doğrudan sigorta primleri analize dahil edilmiştir. Çalışmada zaman serisi yöntemi uygulanmıştır. Elde edilen bulgulara göre değişkenler arasında uzun dönemli bir ilişki tespit edilememiştir. Nedensellik testi sonuçlarına göre ise ekonomik büyümeden sigorta primlerine doğru tek yönlü nedensellik ilişkisi olduğunu göstermektedir. Bu sonuçla, ekonomik büyümede meydana gelen değişimlerin sigortacılık sektörü üzerinde etkili olduğu ortaya çıkmaktadır.
