Abstract

BACKGROUND: Pancreatic neoplasms can be difficult to diagnose and stage preoperatively. Accurate staging allows the surgeon to select which patients can benefit from resection versus palliative therapy. Endoscopic ultrasound (EUS) with endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) is a diagnostic modality that provides visualization of peripancreatic tumors and their relationship to the surrounding structures as well as enabling cytologic diagnosis of the tumor and adjacent lymphadenopathy. METHODS: To define the role of this technique, a retrospective study was performed on 20 patients in the past year with peripancreatic tumors. RESULTS: Twelve men and 8 women ranging in age from 28 to 84 years (mean 67) were included in the study. Each patient underwent computed tomography followed by EUS-FNA, and the results were compared with operative findings or clinical course. The EUS-FNA findings included 10 pancreatic ductal carcinomas (50%), 5 pancreatitis (25%), 2 spindle cell neoplasms (10%), 1 cholangiocarcinoma (5%), 1 cystadenoma (5%), and 1 metastatic breast carcinoma (5%). Overall, EUS-FNA led to a significant change in the management of 12 patients (60%) through either diagnosing benign pathology, upstaging of the carcinoma, or determination that the peripancreatic mass represented a metastatic lesion. Five patients underwent resection of their peripancreatic tumors, and 3 patients had palliative procedures. Operative findings corresponded with EUS-FNA in all 8 patients. The 5 patients diagnosed with pancreatitis continued to be followed up for the possibility of a false negative FNA, but to date none have developed malignancy. CONCLUSIONS: EUS-FNA is a useful tool for the imaging and staging of peripancreatic tumors and will aid in the proper preoperative selection of patients who will benefit from resectional therapy. 139. Development of an annoyance model based upon elementary auditory sensations for steady-state aircraft interior noise containing tonal components

Abstract

The purpose of this investigation was to develop a noise annoyance model, superior to those already in use, for evaluating passenger response to sounds containing tonal components which may be heard within current and future commercial aircraft. The sound spectra investigated ranged from those being experienced by passengers on board turbofan powered aircraft now in service to those cabin noise spectra passengers may experience within advanced propeller-driven aircraft of the future. A total of 240 sounds were tested in this experiment. Sixty-six of these 240 sounds were steady state, while the other 174 varied temporally due to tonal beating. Here, the entire experiment is described, but the analysis is limited to those responses elicited by the 66 steady-state sounds.

140. TWO PHOTOGRAPHED AMERICAN COOT x COMMON GALLINULE HYBRIDS.

Abstract

To the three previous reports of hybrids of the American Coot x Common Gallinule we add two, one from Mountain View, California, in July 1995 and one from Tijuana, Baja California, in December 2019 and January 2020. The hybrids were similar in their coot-like build and intermediate plumage coloration, but they differed in size, bill and leg coloration, extent of lobes on the toes, and some plumage details. The one in Tijuana was larger than either parental species but made vocalizations of both species. Such intergeneric hybrids are rare, but at least 11 examples of Fulica x Gallinula have been reported from the Old World. [ABSTRACT FROM AUTHOR]

148. Survey of Entry-Level Skills Training by Selected Missouri Manufacturers. Final Report.

Abstract

Missouri manufacturers of electrical and electronic equipment and related products were surveyed to identify entry-level skills training decision factors and the extent of entry-level skills training by the manufacturers. Two instruments were constructed for use in the study. The Training Decision Inventory was used to assess the influence of specified issues on manufacturers' decisions to offer training; the Survey of Entry-Level Skills Training was used to ascertain the nature and extent of entry-level skills training. Results indicated that issues related to employee motivation/affirmative action, technology/productivity, and education preparation affected manufacturers' decisions to offer entry-level skills training. Cost benefit, expenditures for training, and labor market issues had equivalent influence on the decision-making process with respect to whether or not firms offer training. Data on the nature and extent of existing skills training were inadequate in number to allow the drawing of generalized conclusions. Implications for vocational educators from study findings focused on awareness of why employers provide training and on the decision-making process. Topics for further investigation were suggested.
