Antecedentes: el liquen plano (LP) es una enfermedad autoinmune crónica que afecta la mucosa oral, la piel, la mucosa genital y otras localizaciones. Objetivo: evaluar la correlación entre el liquen plano oral, genital y cutáneo, en una muestra de pacientes con LP. Material y Métodos: este estudio descriptivo revisó 274 historias clínicas de pacientes, todos presentaron confirmación histológica de liquen plano verificada por un patólogo, que asistían a dos centros médicos de Barcelona. Resultados: un total de 40 pacientes con LP (14.59%) presentaron lesiones genitales. De los 131 pacientes con LP cutánea (47,8%), las zonas más afectadas fueron las superficies flexoras del cuerpo, que representan el 60,1% de los casos. El 24% de los pacientes (n=55) relacionó el inicio de las lesiones con eventos de estrés previos. De los 131 sujetos con lesiones cutáneas, 19% (n=25) también presentaron liquen plano oral (LPO). Del total de la muestra, el 53,6% (n=147) de los pacientes presentaron lesiones orales. Las enfermedades sistémicas más comúnmente asociadas con esta muestra de pacientes, fueron problemas psicológicos como estrés, ansiedad y depresión (48%), hipertensión (27%), problemas gástricos (12%) y diabetes (9,7%). Se encontró una historia familiar de liquen plano en solo 2 casos (0,72%) del total de 274.

espanolLa extraccion del tercer molar es un procedimiento comun en cirugia oral. Una de las complicaciones es la fractura mandibular. Con una incidencia postoperatoria menor a un 0.005%. Los factores que contribuyen al riesgo de fractura del angulo mandibular despues de una extraccion del tercer molar incluyen el nivel de impactacion, la anatomia del diente, infecciones locales previas, edad, sexo, presencia de formaciones quisticas y bruxismo entre otras. Presentamos un caso clinico de fractura mandibular postoperatoria a las cinco semanas tras la exodoncia del 47 y 48 incluidos en posicion horizontal. El objetivo final del tratamiento de una fractura mandibular es la consolidacion osea manteniendo la oclusion dental. En nuestro caso, al no tratarse de una fractura que sobrepasaba las corticales y sin desplazamiento mandibular, no fue subsidiaria de un tratamiento quirurgico y se resolvio con reposo funcional. EnglishThe lower third molar extraction is a common procedure in oral surgery. One of its complications, although uncommon, is the mandibular fracture. Postoperatively, it has an incidence lower than 0.005%. The factors that may contribute to the risk of the mandibular angle fracture after an extraction of the third molar include the leve! of impaction are the anatomy of the tooth, previous local infections, age, sex, cysts and bruxism among others. We present a clinical case of postoperative mandibular fracture at five weeks after the extraction of 47 and 48 included in horizontal position. The ultimate treatment goal of a mandibular fracture is to reach a bone consolidation with the premorbid dental occlusion. In our case, as it was not a fracture that surpassed the cortical and there was not a mandibular displacement, it was not need a surgical treatment so it was resolved with functional rest.

RESUMEN Introcucción: El liquen plano (LP) es una enfermedad sistémica autoinmune de etiología desconocida y curso crónico que puede resultar en el desarrollo de carcinoma oral de células escamosas (COCE). Objetivo: Evaluar la evidencia relacionada con la malignización del LPO, así como las características clínicas y la incidencia de las lesiones que pueden desarrollan un COCE. Material y métodos: Se revisa la base de datos MEDLINE PubMed de los últimos 5 años (2013-2018) utilizando las siguientes palabras clave: “malignant transformation of oral lichen planus” and “squamous cell oral carcinoma”. Resultados: Se obtuvieron 48 artículos de los cuales tras la lectura resúmenes y la aplicación de nuestros criterios de inclusión y exclusión obtuvimos un total de 19 artículos, además de los 4 artículos añadidos mediante búsqueda manual. La revisión se basa en la comparación de 9 estudios de cohortes para analizar la incidencia de la progresión de LPO a COCE. Conclusiones: Debemos considerar el LPO como una enfermedad potencialmente maligna (0.5- 1%) siendo el sexo femenino, el tipo erosivo y la localización en la lengua factores de riesgo para la transformación a COCE. Teniendo en cuenta el riesgo de malignización es esencial el control de las lesiones para poder realizar un diagnóstico precoz.

Background Lichen planus (LP) is a chronic autoimmune disease that affects the oral mucosa as well as the skin, genital mucosa and other sites. Objective: to evaluate the correlation between oral, genital and cutaneous lichen planus, in a sample of LP patients. Material and Methods This descriptive study reviewed 274 clinical histories of patients, who all presented histological confirmation of lichen planus verified by a pathologist, attending research centers in Barcelona. Results A total of 40 LP patients (14.59%) presented genital lesions. Of 131 patients with cutaneous LP (47.8%), the most commonly affected zones were the body’s flexor surfaces, representing 60.1% of cases. 24% of patients (n=55) related the start of the lesions with previous stress events. Of the 131 subjects with cutaneous lesions, 19% (n=25) also presented oral lichen planus (OLP). Of the total sample, 53.6% (n=147) of patients presented oral lesions. The systemic diseases most commonly associated with this patient sample were psychological problems such as stress, anxiety and depression (48%), hypertension (27%), gastric problems (12%), and diabetes (9.7%). A family history of lichen planus was found in only 2 cases (0,72%) out of the total of 274. Conclusions Any patient with OLP should undergo a thorough history and examination to investigate potential extraoral manifestations. The fact that 37 patients with OLP in this patient series were identified with simultaneous involvement at more than one site highlights the need for thorough evaluation and multidisciplinary approaches to this disease. Background It has been shown that the risk of developing oral cancer is closely related to the intensity and duration of exposure to alcohol and tobacco. Even so, the use of mouthwashes with alcohol in their compositions and the increased risk of oral cancer has been a source of controversy for decades. Objective This study proposes a systematic review and a meta-analysis of the literature, to assess the possible relationship between the use of mouthwashes with alcohol and the development of oral and pharyngeal cancers. Materials and Methods A systematic search was done using the Medline and PubMed databases. Exclusion criteria were as follows: articles published in languages other than English or Spanish, systematic reviews, and expert opinions. No limitations were used for publication date. Results A total of 14 articles were obtained, 11 case–control studies and 3 clinical trials. Three case–control studies found no statistically significant evidence between the relationship of mouthwash use and oral cancer and the remaining 8 case–control studies found statistically significant evidence. The 3 clinical trials observed a relationship between the use of mouthwashes with alcohol and the possibility of developing cancer due to the genotoxicity and mutagenic capacity of alcohol in chronic contact with oral tissues and mucous membranes. The meta-analysis resulted in an OR = 1.480 and a P-value = .161 (95% CI: 0.855; P-value = 2.561) for the analysis of studies of cancer risk and consumption of mouthwashes with alcohol and OR = 1.057 0.364 (95% CI: 0.951; P-value = 1.174) for studies that related the risk of cancer and mouthwash use without taking into account the presence of alcohol. Conclusions There is no sufficient evidence to accept the proposition that the use of mouthwashes containing alcohol can influence the development of oral cancer.

The main objective of this study was to evaluate the effect of metformin, statins and anti-inflammatory drugs (NSAIDs) on head and neck cancer (HNC). Specifically, the potential beneficial effects on risk, survival and recurrence based on epidemiological studies. PRISMA guidelines were followed. After searching MEDLINE (PubMed), IBECS, LILACS and the Cochrane Central Register for Controlled Trials, 13 studies met the inclusion criteria and so underwent qualitative synthesis (six studies for metformin and seven for NSAIDs). No studies were found for statins. Studies varied in their methodological quality. Meta-analyses showed that metformin exerts significant beneficial effects on HNC risk (RR = 0.71 95% CI 0.61–0.84) and overall survival (RR = 1.71 95% CI 1.20–2.42). Qualitative synthesis also suggests an apparently dose-response relationship and increased benefit when administered alone. The pooled-analyses yielded an almost significant effect of NSAIDs on HNC risk (RR = 0.86 95% CI 0.74–1.01). No associations were found between aspirin use and the risk of HNC (RR = 0.98 95% CI 0.77–1.24) and overall survival (RR = 1.10 95% CI 0.89–1.36). Metformin appears to have beneficial effects on HNC risk and overall survival, with an apparently dose-response relationship and increased benefit when administered alone. NSAIDs also seem to have a modest beneficial effect on HNC risk. No definitive conclusions can be reached for aspirin as the evidence available was proved inconsistent. Further research by means of well designed and conducted studies are needed to determine firm clinical implications. Standardized assessment methods for HNC outcomes should be established and account for known confounding factors such as smoking and alcohol consumption.

Background Periodontitis and diabetes are highly prevalent conditions whose association has long been recognized. Objective To evaluate the effect of non-surgical periodontal treatment on serum HbA1c (haemoglobin A1c or glycated haemoglobin) levels in patients with type 2 diabetes. Research Design and Methods This was a 6-month, single-masked, randomized clinical trial based on 90 patients (HbA1c: 7.7% (61 mmol/mol) ± 1.13%) who were randomly assigned to either the treatment group (oral hygiene instructions + scaling and root planing using ultrasound and Gracey curettes) or the control group (oral hygiene instructions + supragingival removal of plaque and calculus using ultrasound). Reducing post-interventional inflammation and pain in odontostomatological surgery procedures, such as tooth extractions, implants or oral biopsies is a relevant clinical goal. Chlorhexidine oral rinse is commonly used with this aim. Recently a new product containing chlorhexidine, dexpanthenol, allantoin and chitosan (Bexident Post [BP]) in a gel formulation has been developed. We evaluated the efficacy of BP in controlling postsurgical inflammation and pain and in promoting cicatrization in subjects undergoing molar extractions.We conducted a prospective sequential cross-over, randomized controlled study in patients undergoing surgical removal of at least two impacted mandibular third molars (teeth numbers 38 and 48) (numbers 17 and 32 in the Universal Tooth Numbering System), in two separate sessions, to determine the effect of BP in comparison with bicarbonate (BC) oral rinse (one spoonful in 200 ml of water), both used three times daily. Each subject utilized both products in a randomized sequential manner after each tooth extraction. Primary outcomes of the study were post-procedure pain and inflammation. Secondary outcomes were analgesic pill rescue use (metamizole 1 cap every 8 hours if needed) and an assessor-blinded evaluation of cicatrization with a semi-quantitative scale (good, satisfactory and insufficient). Post-procedure pain was assessed 6 hours after tooth extraction and for seven consecutive days by means of a 10 cm visual analogue scale (VAS) (from 0: no pain to 10: extreme pain). The extent of inflammation was evaluated through metric measurements of facial perimeter using standardized anatomical reference points.A total of 47 patients (22 men and 25 women; mean age 34 years) were enrolled with a total of 94 molars extracted. Nineteen subjects applied BC as the first sequential treatment and 28 BP as the first. Before surgery no mean differences in the two treatments in inflammation measurements were observed. After surgery mean VAS pain score was similar between the two treatments in the first 6 hours (VAS score = 6.5). A marked progressive reduction in pain intensity with the use of BP was observed throughout the treatment period in comparison with BC (7 day mean scores 3.7 vs. 5.3; p = 0.0001). BP was superior to BC in reducing inflammation with -50% of the inflammation-related measurement (6 mm vs. 12 mm; p = 0.0001). Analgesic pill consumption was lower with BP in comparison with BC (13 pills vs. 24; p 0.05). Cicatrization was scored 'good' in a higher percentage of subjects during BP use (64%) in comparison with the BC group (13%) (p = 0.0001). No serious side effects were reported with either treatment regimen.In this trial BP performed better than BC in controlling pain and inflammation in subjects undergoing dental surgery, reducing the consumption of analgesics and favoring better cicatrization.

Objective The purpose of this study was to evaluate the healing benefit provided by the antioxidant and antifibrotic properties of pentoxifylline–tocopherol or pentoxifylline–tocopherol–clodronate in combination therapy for osteoradionecrosis. Study Design We searched for relevant reports in PubMed by using a combination of “osteoradionecrosis” and the following keywords: “pentoxifillyne,” “tocopherol,” “vitamin E,” or “clodronate.” We considered articles in English or Spanish, with no limitations on the publication date. Results The combination of pentoxifylline plus tocopherol with or without clodronate was found to be effective for the treatment of mandibular osteoradionecrosis, although data were generally scarce and mostly came from retrospective case series. Conclusions This drug therapy is well tolerated and could be promising for the treatment of mandibular osteoradionecrosis, but prospective randomized controlled clinical trials are needed for further clarification.

Background Probiotics are microorganisms, mainly bacteria, which benefit the host’s health. Many studies support the role of probiotics as a contributor to gastrointestinal health, and nowadays many authors are trying to prove its influence in oral health maintenance. Objectives To review the published literature with the purpose of knowing the importance of using probiotics as a preventive and therapeutic method for oral infectious diseases management. Material and Methods An electronic search in PubMed database with the keywords “oral health AND probiotics AND dentistry” was conducted. The inclusion criteria were: randomized clinical trials (RCTs) that assess the action of any probiotic strain in the treatment and / or prevention of an infectious oral disease, RCTs that assess the action of any probiotic strain on counting colony forming units (CFU) of oral pathogens, systematic reviews and meta-analysis. The Jadad scale was used to assess the high quality of RCTs. Results Fifteen articles were considered for this review. Of which, 12 were RCTs of good / high quality (Jadad scale), two meta-analysis and one systematic review. Conclusions The literature reviewed suggests probiotics usage could be beneficial for the maintenance of oral health, due to its ability to decrease the colony forming units (CFU) counts of the oral pathogens. However, randomized clinical trials with long-term follow-up periods are needed to confirm their efficacy in reducing the prevalence/incidence of oral infectious diseases. Furthermore, the recognition of specific strains with probiotic activity for each infectious oral disease is required, in order to determine exact dose, treatment time and ideal vehicles. Background Recently, bone graft materials using permanent teeth have come to light, and clinical and histological outcomes of this material have been confirmed by some studies. The aim of this systematic review was to evaluate the reliability of the autogenous tooth bone graft material applied to alveolar ridge augmentation procedures. Material and Methods A systematic review of literature was conducted analyzing articles published between 2007 and 2017. The following four outcome variables were defined: a) implant stability b) post-operative complication c) evaluation of implant survival and failure rates, and d) histological analysis. A total of 108 articles were identified; 6 were selected for review. Based on the PICO (problem, intervention, comparison, outcome) model, the chief question of this study was: Can patients with alveolar ridge deficiency be successfully treated with the autogenous teeth used as bone graft? Results The mean primary stability of the placed implants was 67.3 ISQ and the mean secondary stability was 75.5 ISQ. The dehiscence of the wound was the most frequent complication with a rate of 29.1%. Of the 182 analyzed implants, the survival rate was 97.7% and the failure rate was 2.3%. In the histological analysis, most of studies reported bone formation. Conclusions There is insufficient evidence regarding the effects of autogenous teeth used for bone grafting to support any definitive conclusions, although it has been shown clinically safe and good bone forming capacity, and good results are shown about implant stability. Osteoporosis is a major cause of fractures in elderly women. Bone densitometry is used in order to detect osteoporosis. It has been observed can also be analyzed in the jawbone. The osteoporosis may be associated with resorption of the residual ridge, periodontitis and tooth loss; also the same treatments can affect the jaws. A search was carried out in the Medline-Pubmed database in order to search the association between osteoporosis and oral diseases over the past 5 years. Forty-two articles were obtained after the selection process. The authors stated that: in reference to periodontal disease the results are conflicting, but there seems to be a higher prevalence of the disease and tooth loss and resorption of the crest. Oral bisphosphonates have little risk of causing osteonecrosis, and there is no relationship in the dental implant failure among patients taking bisphosphonates. There is no clear scientific evidence that could link osteoporosis and oral diseases.

Objectives Bacterial (infective) endocarditis, a microbial infection of the endocardium surfaces after bacteremia, causes significant morbidity and mortality. Recent epidemiologic studies have reported a prevalence of 2-8 cases per 100,000 individuals per year, with the highest incidence in those aged 70-80 years and those living in developed countries. We systematically reviewed the literature on several critical aspects regarding the development of bacteremia after oral surgery. The purpose of this work is to assess the controversy regarding antibiotic prophylaxis before oral surgery. Materials and Methods Publications between 1976 and 2015 were included. Clinical studies focusing on oral surgery as the underlying cause were included. Results Among the 32 clinical studies reviewed, 3564 cases, accounting for 12,839 blood cultures, were evaluated. In 10 of these studies, amoxicillin usefulness was studied. Antimicrobial prophylaxis before an invasive dental procedure does not prevent bacteremia, although it can decrease both its magnitude and its persistence. Conclusions The highly conflicting data and conclusions of the analyzed work highlight the need for new approaches to the study of bacteremia that would provide reliable evidence and thus appropriate prophylactic and therapeutic standards. Many reports have explored the occurrence of bacteremia after dental procedures, but the results have been conflicting.

Background: alterations in hormone levels during menopause decrease bone density and may worsen oral health, favoring the growth of periodontal pathogens, whose detection could improve the diagnosis of periodontitis. The aim of this study is to detect and quantify the main periodontal pathogens in the oral microbiota of postmenopausal females and to explore the relationship between clinical and periodontal parameters. Methods: this was an observational cross-sectional study of 76 postmenopausal females. Dental examinations and sampling for microbiologic evaluation were performed, and a history of osteoporosis/osteopenia was collected. Real-time polymerase chain reaction was used for detecting and quantifying Fusobacterium nucleatum (Fn), Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Campylobacter rectus (Cr), and Tannerella forsythia (Tf). The results obtained were subjected to statistical analyses. Statistical significance was defined as P 0.05). A significant correlation was found between osteoporosis and missing teeth. T. forsythia and C. rectus were detected in 100% of the samples, Fn and Pg in 98.7%, and Aa in 73.7%. Conclusions: osteoporosis did not influence the prevalence of periodontitis among postmenopausal females. The presence of periodontopathogenic bacteria was not sufficient to confirm disease. A preventive maintenance program for postmenopausal females, particularly osteoporotic females, who are at greater risk of tooth loss, could minimize the potential effects of bone loss on periodontal tissues.

Introduction: Smoking is one of the main public health problems in developed countries. Despite extensive evidence on the effects of smoking on both oral and general health, the rate of smoking cessation is not promising. Material and Methods: To review the evidence on knowledge and programs for smoking cessation developed by dentists, a literature review was carried out on programs for smoking cessation from the dentist’s perspective, as well as a review of behavioral guidelines that have been recently proposed for these interventions. We used the keywords “Tobacco”, “Smoking Prevention”, “Public Health” AND “Dentistry”, to identify controlled studies, systematic reviews and meta-analyses published between 1999 and 2014, in Google Scholar, SCOPUS and PubMed. Results: Out of 177 studies found, 35 were considered, and these were divided into 2 groups of 20 and 15 articles respectively, according to type of study and inclusion criteria. Conclusions: There is considerable scientific evidence describing the programs for smoking cessation used in dentistry, which support their effectiveness. Overall, these are brief behavioral interventions complemented by pharmacological treatment, with the participation of the entire dental team.

Background: Diabetes and periodontal disease share common features in terms of inflammatory responses. Current scientific evidence suggests that treatment of periodontal disease might contribute to glycemic control. The objective of the study is a review of the last three years. Material and Methods: A literature search was performed in the MEDLINE (PubMed), Cochrane, and Scopus databases, for articles published between 01-01-2013 and 30-06-2015, applying the key terms “periodontal disease” AND “diabetes mellitus”. The review analyzed clinical trials of humans published in English and Spanish. Results: Thirteen clinical trials were reviewed, representing a total of 1,912 patients. Three of them had samples of 40 patients, representing a total of 1,804. Only one article achieved a Jadad score of five. Aim: To determine the prevalence of apical periodontitis (AP) and the frequency of root canal treatment in a sample of Spanish adults. Design: A descriptive cross-sectional study. Setting: Barcelona, Spain. Participants: A total of 397 adult subjects, 49% males and 51% females. Methods: Digital panoramic radiographs were used. Periapical status was scored according to the periapical index. Results were analysed statistically using the chi-squared test and logistic regression. Results: Radiographic signs of AP in one or more teeth were found in 135 patients (34%). The prevalence of AP was significantly higher in males (42.3%) than females (26.1%) [odds ratio (OR) = 2.1; 95% confidence interval (95% CI) = 1.4–3.2; P = 0.0007]. At least one root-filled tooth (RFT) was found in 233 patients (59%). Twenty-six per cent of subjects with RFT had at least one RFT with AP. The prevalence of AP increased with age (P < 0.05). Patients with RFT showed a higher prevalence of AP (42%) relative to patients without RFT (23%) (OR = 2.4; 95% CI = 1.5–3.7; P = 0.00013). Adjusting for age, gender and teeth number, endodontic status remained significantly associated with periapical status (OR = 2.3; 95% CI = 1.4–3.6; P = 0.0005). Conclusions: Both the prevalence of AP and the frequency of root canal treatment are high among Spanish adults. AP affects more frequently RFT relative to untreated teeth. Patients with one or more RFT have a greater likelihood of having AP than patients without RFT.

Introduction. Pyogenic granuloma (PG) and peripheral giant-cell granuloma (PGCG) are two of the most common inflammatory lesions associated with implants; however, there is no established pathway for treatment of these conditions. This paper aims to illustrate the successful treatment of PG and PGCG and also report a systematic review of the literature regarding the various treatments proposed.Methods. To collect relevant information about previous treatments for PG and PGCG involving implants we carried out electronic searches of publications with the key words “granuloma”, “oral”, and “implants” from the last 15 years on the databases Pubmed, National Library of Medicine’s Medline, Scielo, Scopus, and Cochrane Library.Results. From the electronic search 16 case reports were found showing excision and curettage as the main successful treatment. As no clinical trials or observational studies were identified the authors agreed to present results from a review perspective.Conclusion. This is the largest analysis of PG and PGCG associated with implants published to date. Our review would suggest that PGCG associated with implants appears to have a more aggressive nature; however the level of evidence is very limited. Further cohort studies with representative sample sizes and standard outcome measures are necessary for better understanding of these conditions.

ResumoO carcinoma oral de células escamosas define-se como uma neoplasia maligna originada no epitélio pavimentoso estratificado. É a neoplasia maligna mais comum na boca, responsável por 90% da totalidade dos cancros da boca. Cerca de 95% destes carcinomas manifestam-se após os 40 anos, sendo 60 anos a idade média dos pacientes no caso dos homens, e 70 anos no das mulheres. O rácio homem/mulher é de 3:1, mas na última década esta proporção sofreu uma redução, devido ao incremento do consumo de tabaco por parte das mulheres.ObjectivoA finalidade deste trabalho é apresentar o caso clínico de uma úlcera aparentemente inofensiva no lado direito da língua.Caso ClínicoA um paciente que apresentava uma úlcera na língua foi diagnosticado clínica e histopatologicamente um carcinoma oral de células escamosas.ConclusãoOcarcinoma de células escamosas é relativamente fácil de diagnosticar na boca, por ser facilmente detectável clínica e histopatologicamente. É fundamental obter um diagnóstico precoce e iniciar logo que possível o melhor tratamento, que normalmente passa pela cirurgia.AbstractThe squamous cell carcinoma is defined as a malignant neoplasia originated on the stratified paved epithelium. It is the most common malignant neoplasia in the mouth, representing 90% of all oral cancers. Approximately 95% of these carcinomas are manifested on ages above 40, the average age on the masculine sex is 60 and on the feminine sex 70. Man/Woman ratio is 3:1 but on the last decade, a chance on this proportion has been noticed, due to an increase on tobacco use by women.ObjectiveThe aim of this work is to present a clinical case of an “innocent” looking ulcer on the right edge of the tongue.Clinical caseA patient presenting an ulcer in the tongue is studied clinically and histopathologically, in order to reach the right diagnose of the lesion. The clinical examination and the histopathological analysis led to the diagnosis of squamous cell carcinoma.ConclusionsSquamous cell carcinoma is relatively easy to diagnose in the mouth, because of its accessibility to clinical and histopathological examination. The most important thing is to reach an early diagnose and starting the best treatment (usually surgical) as soon as possible.

Objetivos: Evaluación de la eficacia del chitosán más la clorhexidina en el control del dolor y la inflamación posoperatoria en cirugía implantológica oral. Material y métodos: Es un estudio clínico piloto prospectivo en pacientes a los que se les colocan implantes. A todos los pacientes se les pauta después de la cirugía Bexident®Post en gel, tres veces al día durante siete días. Se valora la inflamación, la apertura bucal, el nivel de dolor, el grado de cicatrización y la opinión sobre el producto. Resultados: Se han colocado 138 implantes en 54 pacientes. El 64% eran varones y la media de tiempo quirúrgico fue de 76 minutos. La inflamación y apertura bucal pre y postoperatoria no es significativa con una p=0,000. Se han consumido 11 unidades analgésicas de media en los 7 días del posoperatorio. El EVA ha sido de 3,0 (DS= 2,0) a las 6 horas, 2,0 (DS= 2,0) a las 24 horas, 2,02 (DS= 1,0) el segundo día, 1,0 (DS= 1,0) el tercer y cuarto día y 0,0 el quinto, sexto y séptimo día. El grado de cicatrización es bueno en el 88,9% (48 pacientes); regular, en el 7,4% (4 pacientes) y malo, en el 3,7% (2 pacientes). 45 pacientes (83,3%) responden positivamente a todas las preguntas referentes al producto. Conclusiones: El producto se muestra fácil de usar, el sabor es adecuado y en este estudio la inflamación, el dolor y el consumo de medicamentos han sido mínimos. El cáncer oral es una patología frecuente y creciente en el mundo occidental, su incidencia se incrementa año tras año, a pesar de que su localización, en una cavidad fácilmente accesible y explorable permitiría un diagnóstico temprano del mismo, o de las alteraciones potencialmente malignas. Las cifras de nuestro país sitúan una incidencia de 6,7 casos nuevos al año por 100.000 habitantes, con una proporción hombre/mujer de 3 a 1. La proporción de estas neoplasias aumenta de forma gradual con la edad, la posibilidad de que una lesión ulcerosa o proliferativa de cavidad oral, sea un carcinoma es mucho más elevada en pacientes mayores de 65 años. Es nuestra obligación el concienciar a nuestros pacientes, en el cese de hábitos nocivos y en la educación para establecer de la autoexploración una actividad habitual. Sin duda, una exhaustiva exploración física es la primera medida diagnóstica que poseemos asociada ante la mínima sospecha a la realización de una biopsia y el correspondiente estudio histopatológico, que puede establecer la potencialidad maligna de la lesión, así como la evolución de la misma. Disponemos, en la actualidad, de métodos complementarios de diagnósticos, que desde los tradicionales colorantes vitales, pasando por métodos ópticos, hasta la utilización de novedosas técnicas de identificación de receptores moleculares, de alteraciones cromosómicas podrían ser de utilidad en la orientación de factores de riesgo en pacientes predispuestos a padecer la patología, todo ello con el futuro puesto en identificar todos estos riesgos potenciales con un simple análisis de saliva. Lo cierto es que a día de hoy el mejor diagnóstico es la prevención y en el peor de los casos realizarlo lo más precozmente posible para mejorar las perspectivas vitales y la calidad de vida de nuestros pacientes. Oral cancer is a common and growing in the Western world disease, its incidence is increasing year after year, despite its location in an easily accessible and searchable cavity allow early diagnosis thereof, or of potentially malignant disorders. The figures of our country placed an incidence of 6.7 new cases per year per 100,000 population, with a male /female ratio of 3 to 1. It is our duty to educate our patients in the cessation of harmful habits and the Education to establish how to examine a common activity. Definitely a thorough physical examination is the first diagnostic measure we have associated under suspicion for a biopsy and the corresponding histopathology, you can set the malignant potential of the lesion, and the evolution of it. We currently have complementary diagnostic methods, which based from traditional vital dyes, through optical methods, to the use of novel techniques for identifying molecular receptors, chromosomal alterations could be useful in guiding risk factors Patients predisposed to the disease, all in the future on identifying these potential risks with a simple analysis of saliva. The truth is that today the best diagnosis is prevention and the worst do it as early as possible to improve the life prospects and quality of life of our patients.

Introduction. Pyogenic granuloma (PG) and peripheral giant-cell granuloma (PGCG) are two of the most common inflammatory lesions associated with implants; however, there is no established pathway for treatment of these conditions. This paper aims to illustrate the successful treatment of PG and PGCG and also report a systematic review of the literature regarding the various treatments proposed. Methods. To collect relevant information about previous treatments for PG and PGCG involving implants we carried out electronic searches of publications with the key words “granuloma”, “oral”, and “implants” from the last 15 years on the databases Pubmed, National Library of Medicine’s Medline, Scielo, Scopus, and Cochrane Library. Results. From the electronic search 16 case reports were found showing excision and curettage as the main successful treatment. As no clinical trials or observational studies were identified the authors agreed to present results from a review perspective. Conclusion. This is the largest analysis of PG and PGCG associated with implants published to date. Our review would suggest that PGCG associated with implants appears to have a more aggressive nature; however the level of evidence is very limited. Further cohort studies with representative sample sizes and standard outcome measures are necessary for better understanding of these conditions.

La halitosis o mal aliento es un problema importante dado que puede comprometer la vida de relación de las personas. Es frecuente que se atribuya el origen al estómago o a problemas nasales o pulmonares. No obstante, sabemos que hasta en el 90% de los casos, el origen se encuentra en la cavidad oral, y de forma preponderante debido al acúmulo de placa en el dorso lingual "cubierta lingual". La enfermedad periodontal también está directamente relacionada con el mal aliento, de manera que el acumulo de placa, bolsas y/o sangrado son aspectos directamente relacionados con el problema. Desde el punto de vista patogénico, las proteínas de la dieta, las de las bacterias y las de los propios tejidos de la boca, son degradadas por la acción de las proteasas bacterianas hasta aminoácidos. Los aminoácidos ricos en azufre son los máximos responsables del mal aliento. De hecho los tres productos más directamente relacionados son el sulfuro de hidrógeno, el metil mercaptano y el dimetil sulfuro. En la presente revisión repasamos todos estos aspectos y las causas más directamente relacionadas con el problema, revisando igualmente el concepto de halitofobia y pseudohalitosis. Halitosis, or bad breath is a major problem because it can compromise the people relationship life. Often origin is attributed to stomach or nasal or lung problems. However, we know that up to 90 % of cases, the source is in the oral cavity, and so dominant due to the accumulation of plaque on the dorsum of the tongue "cover tongue". Periodontal disease is also directly related to bad breath, so that the accumulation of plaque, periodontal pockets and/or bleeding are aspects directly related to the problem. From the pathogenic point of view, the dietary and bacteria proteins, and the own oral tissues proteins, are degraded by the action of bacterial proteases to amino acids. Rich in sulfur amino acids are the maximum responsible of bad breath. In fact the three most closely related products are hydrogen sulfide, methyl mercaptan and dimethyl sulphide. In this update, we review all these aspects and causes more directly related to the problem, also reviewing the pseudohalitosis and halitophobia concepts.

La disminución de saliva afecta la calidad de vida de los pacientes, que pueden mejorar cambiando ciertos hábitos de salud general como hidratación, uso de humidificadores ambientales, evitar tóxicos y conductas saludables, incluyendo visitas frecuentes al dentista. La prevención en los pacientes que van a ser irradiados, la modificación de los fármacos xerostomizantes en los casos posibles contribuyen al bienestar de estos pacientes. Los diferentes métodos de estimulación salivar tanto farmacológicos (ácidos, pilocarpina) como no farmacológicos (mecánicos, eléctricos, láser, acupuntura u oxígeno hiperbárico) y el uso de sustitutos salivales ayudan a mantener mejor hidratación bucal. En el futuro, el desarrollo de componentes orgánicos a partir de células madre será probablemente una posibilidad de tratamiento efectiva. Decreased saliva affects patients quality of life, that can improve changing certain health habits like hydration, usin groom humidifiers, avoid toxics and healthy behaviors, including frequent visits to the dentist. Prevention in patients who are going to be irradiated and modify xerostomizing drugs where possible contribute to the welfare of these patients. Different salivary stimulation methods, both pharmacological (acid, pilocarpine) and non-pharmacological (mechanical, electrical, laser, acupuncture or hyperbaric oxygen) and the use of salivary substitutes help maintain oral moisture better. In the future, the development of organic components from stem cells will probably be a valid line of treatment.

Purpose To investigate the relationship between radiolucent periapical lesions and bone mineral density in post-menopausal women. Material and methods Seventy-five post-menopausal women were recruited for the study. Bone mineral density was measured using dual-energy X-ray absorptiometry. Three groups were established: healthy bone group, osteopenic group and osteoporotic group. Periapical radiolucencies were diagnosed on the basis of examination of digital panoramic radiographs. Statistical analysis was carried out using anova and chi-squared tests, and logistic regression analysis. Results In both the osteopenic and osteoporotic groups, 25% of women showed at least one periapical radiolucency, whereas this was only 7.4% in the healthy bone group (odds ratio = 4.2; p = 0.061). After multivariate logistic regression analysis adjusting for covariates (age, number of teeth, number of root-filled teeth and number of teeth with coronal restorations), a marginally significant association was evident between bone mineral density and the presence of periapical radiolucencies (odds ratio = 1.9; CI 95% = 1.0–3.8; p = 0.050). Conclusions After adjusting for covariates, low bone mineral density is marginally associated with a higher frequency of radiolucent periapical lesions.

Pain arising from surgical procedures that damage the inferior alveolar nerve typically responds poorly to classic therapies. The present case reports the successful medical treatment of this problem with a 10-day course of a corticosteroid and a concomitant 30-day course of an anticonvulsant.

Osteoporosis is a major cause of fractures in elderly women. Bone densitometry is used in order to detect osteoporosis. It has been observed can also be analyzed in the jawbone. The osteoporosis may be associated with resorption of the residual ridge, periodontitis and tooth loss; also the same treatments can affect the jaws. A search was carried out in the Medline-Pubmed database in order to search the association between osteoporosis and oral diseases over the past 5 years. Forty-two articles were obtained after the selection process. The authors stated that: in reference to periodontal disease the results are conflicting, but there seems to be a higher prevalence of the disease and tooth loss and resorption of the crest. Oral bisphosphonates have little risk of causing osteonecrosis, and there is no relationship in the dental implant failure among patients taking bisphosphonates. There is no clear scientific evidence that could link osteoporosis and oral diseases.
