Abstract

The majority of modern chemotherapeutic protocols for the treatment of breast cancer include doxorubicin. Although the drug shows significant activity against breast cancer, the usable dosage is limited by the drug's toxicity for the heart. In addition, less serious but distressing side effects include vomiting, hair loss, and tissue necrosis around the site of injection. Idarubicin is a related anti-cancer drug, which can be administered orally. The cardiac toxicity of idarubicin was lower than that of doxorubicin when tested in experimental animals, and preliminary results suggested that the compound had useful activity against advanced breast cancer. To evaluate doxorubicin and idarubicin in a direct comparison, 76 patients with recurrent or metastatic breast cancer were randomly assigned to receive either of the two drugs. A significantly greater proportion of the patients treated with doxorubicin achieved objective responses to treatment. Overall, 46 percent of the patients receiving doxorubicin achieved a response, in contrast to 21 percent of those given idarubicin. Of the patients with prior chemotherapy, 29 percent responded to doxorubicin and 12 percent to idarubicin. The results clearly indicate that doxorubicin should remain the chemotherapeutic drug of choice in the treatment of breast cancer. However, it should be mentioned that idarubicin resulted in less gastrointestinal toxicity and less hair loss. Furthermore, while 4 of 38 patients receiving doxorubicin required treatment for congestive heart failure, no such cases occurred among those receiving idarubicin. 145. Prospective Evaluation of Hypertensive Patients with Carotid Kinking and Coiling.

Abstract

The aim of this study was to determine the incidence of vascular events during a 7-year followup evaluation in a group of 34 hypertensive patients with kinking of the internal carotid artery and 36 well-matched hypertensive control subjects. The carotid intima-media thickness (IMT) was measured at three points of the carotid bifurcation and at three points of carotid kinking on the ultrasonographic posterior wall. The mean IMT measured in the segment of the angular bending was lower than the mean values detected at the bifurcation in normal subjects and in hypertensives without carotid elongation (p < 0.01 ). At the carotid bifurcation of the same side of the kinking, there was an arterial IMT that was significantly lower as compared to the contralateral axis and to the measurements obtained in other hypertensive subjects. During a 7-year follow-up study, 10 vascular events occurred in the hypertensives with carotid kinking and 14 in the controls, without significant differences between the 2 groups. In hypertensives with carotid kinking, the mean IMT measured on the angular bending and at the ipsilateral carotid bifurcation was significantly lower than the values obtained at the contralateral bifurcation and in the other hypertensive subjects. In the 7-year follow-up study, moreover, the presence of carotid kinking does not impact the incidence of vascular events in the hypertensive population. Thus, the presence of carotid kinking in hypertensive subjects may not be considered a further risk factor for ischemic events. [ABSTRACT FROM AUTHOR]

Abstract

The prevalence of carotid kinking and coiling in patients with hypertension or diabetes was investigated. The authors studied three groups: 130 subjects with hypertension, 105 with diabetes, and 50 normal subjects who were comparable for age, sex distribution, and the presence of other risk factors. Color flow ultrasonography of the extracranial carotid arteries was performed by standard technique. Hard-copy photographs were obtained in three long-axis and three short-axis projections. The prevalence of carotid kinking and coiling was significantly higher in the group of hypertensive patients than in diabetics and normal subjects (14.6% vs 2% and 14.6% vs 4%, respectively; P < 0.01 for both compar isons). The prevalence of carotid kinking was associated with the duration of hyperten sion, whereas it did not show any association with cigarette smoking and serum choles terol levels. A long-term observation of these patients is necessary for determining the natural history of carotid kinking and the potential for modification by adequate antihy pertensive therapy. The results of this study show that a significantly higher prevalence of carotid kinking is present in hypertensive patients in comparison with normal subjects and diabetics and this is correlated with the time of onset of hypertension. 150. Aldosterone Receptor Blockade in the Management of Heart Failure

Abstract

Mounting evidence suggests that increased circulating aldosterone levels, despite angiotensin-converting-enzyme inhibitors therapy, may exert deleterious cardiovascular effects in heart failure, leading to clinical deterioration and poor prognosis. In the past decades, a number of experimental investigations have provided major insight into the mechanism(s) of action and the biological effects of aldosterone on the cardiovascular system, indicating that aldosterone participates in the structural and functional remodeling of cardiac and vascular tissue. In particular, it has emerged that aldosterone plays a key role in the regulation of myocardial extracellular matrix composition and endothelial function with important pathophysiological implications. Such evidence, coupled with the recent beneficial effects of spironolactone, a competitive aldosterone receptor antagonist, in reducing cardiac mortality and morbidity in patients with severe chronic heart failure treated with angiotensin-converting-enzyme inhibitors and loop diuretics, highlights the importance of aldosterone in the pathophysiology of human heart failure. The purpose of this review is to provide an overview of the regulation of aldosterone production and metabolism in heart failure, the basic mechanism of aldosterone action, and the pathophysiological implications of aldosterone in heart failure, and to discuss recent evidence supporting the efficacy of aldosterone receptor blockade in the treatment of chronic heart failure in humans.
