103. Prevalence of pulmonary tuberculosis among key and vulnerable populations in hotspot settings of Ethiopia. A systematic review and meta-analysis.

Abstract

BackgroundDespite the decline in tuberculosis (TB) incidence across many regions worldwide, including Ethiopia, the disease remains highly concentrated among vulnerable or socially marginalized populations and in high-risk settings. This systematic review and meta-analysis aims to estimate the pooled prevalence of pulmonary tuberculosis (PTB) among key and vulnerable populations (KVPs) residing in hotspot settings in Ethiopia.MethodsPotential papers were searched systematically in PubMed, Scopus, ScienceDirect databases, Google Scholar search engine, and institutional electronic repositories/registrars. A total of 34 potential articles that provide necessary information on the prevalence of PTB were reviewed and data were analyzed to determine the pooled prevalence of PTB among KVPs. The relevant data were recorded and analyzed using STATA 17.0. Cohen's kappa was computed to determine the agreement between reviewers, the Inverse of variance (I2) to evaluate heterogeneity across studies, and Egger's test to identify publication bias. A random effect model was used to determine the pooled prevalence of PTB, subgroup analysis was computed by types of hotspot settings and year of publication.ResultsThis meta-analysis demonstrates that the pooled prevalence of PTB among populations residing in hotspot settings in Ethiopia was 11.7% (95% confidence interval (95CI): 7.97-15.43) with an I2 of 99.91% and a p< 0.001. Furthermore, the subgroup analysis unveiled the pooled prevalence of PTB among KVPs residing in different hotspot settings as follows: Prison inmates 8.8% (95CI: 5.00-12.55%), University students 23.1% (95CI: 15.81-30.37%), Refugees 28.4% (95CI: -1.27-58.15%), Homeless peoples 5.8% (95CI: -0.67-12.35%), Healthcare settings 11.1% (95CI: 0.58-21.63%), Spiritual holy water sites attendees 12.3% (95CI: -6.26-30.80%), and other high-risk settings 4.3% (95CI: 0.47-8.09%). Besides, the subgroup analysis revealed that the pooled prevalence of PTB post-2015 was 10.79% (95CI: 5.94-15.64%), whereas it stood at 14.04% (95CI: 10.27-17.82%) before 2015.ConclusionThe prevalence of PTB among KVPs residing in the hotspot settings in Ethiopia remains significant, with a weighted pooled prevalence of 11.7%. Thus, the national TB control programs should give due attention and appropriate control measures should be instituted that include regular systematic TB screening, compulsory TB testing for presumptive TB cases among KVPs, and tightened infection control at hotspot settings.

104. Spiritual Holy Water Sites in Ethiopia: Unrecognized High-Risk Settings for Transmission of Pulmonary Tuberculosis

Abstract

Ethiopia is a high-tuberculosis (TB) burden country with 157 new cases per 100,000 people, with 23,800 TB-related deaths in 2020. In Ethiopia, TB patients have different healthcare-seeking behaviors. They frequently visit spiritual places, such as holy water sites (HWSs), to seek treatment for their illness spiritually. This study examined the prevalence of pulmonary TB (PTB) and drug susceptibility profiles of Mycobacterium tuberculosis (MTB) isolates among spiritual HWS attendees in Northwest Ethiopia. A cross-sectional study was conducted from June 2019 to March 2020. Sputum samples were collected, processed, and cultured using Löwenstein–Jensen (LJ) culture medium. Second-generation line probe assays (LPAs), GenoType®MTBDRplus VER2.0 and GenoType®MTBDRsl VER2.0, were used to detect anti-TB drug-resistant isolates. STATA 17 was utilized to perform descriptive statistics, bivariate, and multivariate regression analyses. Of 560 PTB-symptomatic participants, 21.8% ((95% confidence interval (95 CI): 18.4–25.2%)) were culture-positive, resulting in a point prevalence of 1,183/100,000 attendees. Amongst HWS attendees, culture-positive TB occurred most commonly in persons 18–33 years of age (28.5% (95 CI 23.4–34.3%)). Other participant characteristics significantly associated with culture-positive PTB were as follows: rural residents (adjusted odds ratio (aOR) 2.65; 95 CI 1.38–5.10), married participants (aOR 2.43; 95 CI 1.28–4.63), family members >5 per household (aOR 1.84; 95 CI 1.04–3.24), and sharing living space (aOR 10.57; 95 CI 3.60–31.13). Also, among 438 participants followed for 12 months after showing negative TB culture results while at the HWS, 6.8% (95 CI 4.4–9.4%) developed or contracted culture-positive TB post-residency at the HWSs. Of the 122 tested isolates, 20 (16.4%) were isoniazid (INH) and/or rifampicin (RIF) resistant. Multidrug-resistant (MDR) TB was detected in 15 cases (12.3%), five of which were fluoroquinolones (FLQs) resistant. The findings from this study should raise a concern about HWSs as potential high-risk settings for TB transmission. It is recommended that appropriate control measures be instituted that include compulsory TB testing and tightened infection control at HWSs, where an increased risk exists for transmission of TB.

Abstract

This paper presents a novel non-Gaussian inference algorithm, Normalizing Flow iSAM (NF-iSAM), for solving SLAM problems with non-Gaussian factors and/or non-linear measurement models. NF-iSAM exploits the expressive power of neural networks, and trains normalizing flows to draw samples from the joint posterior of non-Gaussian factor graphs. By leveraging the Bayes tree, NF-iSAM is able to exploit the sparsity structure of SLAM, thus enabling efficient incremental updates similar to iSAM2, albeit in the more challenging non-Gaussian setting. We demonstrate the performance of NF-iSAM and compare it against the state-of-the-art algorithms such as iSAM2 (Gaussian) and mm-iSAM (non-Gaussian) in synthetic and real range-only SLAM datasets., Comment: 8 pages, 6 figures, to be published in IEEE International Conference on Robotics and Automation (ICRA) 2021

Abstract

Robotics and related technologies are central to the ongoing digitization and advancement of manufacturing. In recent years, a variety of strategic initiatives around the world including "Industry 4.0", introduced in Germany in 2011 have aimed to improve and connect manufacturing technologies in order to optimize production processes. In this work, we study the changing technological landscape of robotics and "internet-of-things" (IoT)-based connective technologies over the last 7-10 years in the wake of Industry 4.0. We interviewed key players within the European robotics ecosystem, including robotics manufacturers and integrators, original equipment manufacturers (OEMs), and applied industrial research institutions and synthesize our findings in this paper. We first detail the state-of-the-art robotics and IoT technologies we observed and that the companies discussed during our interviews. We then describe the processes the companies follow when deciding whether and how to integrate new technologies, the challenges they face when integrating these technologies, and some immediate future technological avenues they are exploring in robotics and IoT. Finally, based on our findings, we highlight key research directions for the robotics community that can enable improved capabilities in the context of manufacturing.

Abstract

Scientific communication is facilitated by a data-driven, scientifically sound taxonomy that considers the end-user's needs and established successful practice. In 2013, the Fusarium community voiced near unanimous support for a concept of Fusarium that represented a clade comprising all agriculturally and clinically important Fusarium species, including the F. solani species complex (FSSC). Subsequently, this concept was challenged in 2015 by one research group who proposed dividing the genus Fusarium into seven genera, including the FSSC described as members of the genus Neocosmospora, with subsequent justification in 2018 based on claims that the 2013 concept of Fusarium is polyphyletic. Here, we test this claim and provide a phylogeny based on exonic nucleotide sequences of 19 orthologous protein-coding genes that strongly support the monophyly of Fusarium including the FSSC. We reassert the practical and scientific argument in support of a genus Fusarium that includes the FSSC and several other basal lineages, consistent with the longstanding use of this name among plant pathologists, medical mycologists, quarantine officials, regulatory agencies, students, and researchers with a stake in its taxonomy. In recognition of this monophyly, 40 species described as genus Neocosmospora were recombined in genus Fusarium, and nine others were renamed Fusarium. Here the global Fusarium community voices strong support for the inclusion of the FSSC in Fusarium, as it remains the best scientific, nomenclatural, and practical taxonomic option available.

Abstract

Research evidence suggests that approaches to promote student success in higher education are becoming more holistic and integrated in nature. This implies that not only classrooms and laboratories, but also residences, as informal out-of-class learning environments, can potentially contribute significantly to promoting student success. The research question we sought to answer is: what is the preferred role and skill sets of residence heads that will enable them to promote student success? In order to answer this question, the study proposes a student success framework with five levels, and the role of the future residence head is explicated in terms of this framework. The findings of the study are significant as they suggest a practical skill set, underpinned by sound theory, for residence heads to contribute to student success.

Abstract

The adiabatic quantum-flux-parametron (AQFP) is an energy-efficient superconductor logic family that utilizes adiabatic switching. AQFP gates are powered and clocked by ac excitation current; thus, to operate AQFP circuits at high clock frequencies, it is required to carefully design the characteristic impedance of excitation lines (especially, above AQFP gates) so that microwave excitation current can propagate without reflections in the entire circuit. In the present study, we design the characteristic impedance of the excitation line using InductEx, which is a three-dimensional parameter extractor for superconductor devices. We adjust the width of an excitation line using InductEx such that the characteristic impedance becomes 50 {\Omega} even above an AQFP gate. Then, we fabricate test circuits to verify the impedance of the excitation line. We measure the impedance using the time domain reflectometry (TDR). We also measure the S parameters of the excitation line to investigate the maximum available clock frequency. Our experimental results indicate that the characteristic impedance of the excitation line agrees well with the design value even above AQFP gates, and that clock frequencies beyond 5 GHz are available in large-scale AQFP circuits., Comment: 5 pages, 6 figures

Abstract

BackgroundPsoriasis is an inflammatory, IL-17-driven skin disease in which autoantigen-induced CD8+ T cells have been identified as pathogenic drivers.ObjectiveOur study focused on comprehensively characterizing the phenotypic variation of CD8+ T cells in psoriatic lesions.MethodsWe used single-cell RNA sequencing to compare CD8+ T-cell transcriptomic heterogeneity between psoriatic and healthy skin.ResultsWe identified 11 transcriptionally diverse CD8+ T-cell subsets in psoriatic and healthy skin. Among several inflammatory subsets enriched in psoriatic skin, we observed 2 Tc17 cell subsets that were metabolically divergent, were developmentally related, and expressed CXCL13, which we found to be a biomarker of psoriasis severity and which achieved comparable or greater accuracy than IL17A in a support vector machine classifier of psoriasis and healthy transcriptomes. Despite high coinhibitory receptor expression in the Tc17 cell clusters, a comparison of these cells with melanoma-infiltrating CD8+ T cells revealed upregulated cytokine, cytolytic, and metabolic transcriptional activity in the psoriatic cells that differed from an exhaustion program.ConclusionUsing high-resolution single-cell profiling in tissue, we have uncovered the diverse landscape of CD8+ T cells in psoriatic and healthy skin, including 2 nonexhausted Tc17 cell subsets associated with disease severity.

139. Middle Leaders and Managers' Perspectives of Distributive Leadership during COVID-19

Abstract

On a global level, COVID-19 has shaken the foundations of every sector. In the South African education context, the traditional methods of teaching, learning, and managing schools changed drastically overnight and management roles and responsibilities were pushed to unprecedented levels. Moreover, the roles of middle leaders and managers took place within a more distributive framework, encouraging school leaders to adopt varying styles of leadership to cope with the demands of COVID-19. For this study, middle leaders constitute departmental heads, grade heads and subject heads. This study utilised a qualitative research design, adopting a case study approach within the interpretivist paradigm. Three departmental heads and seven post level one educators who occupy grade heads/leaders' roles in a primary school constituted the sample. Semi-structured individual interviews were conducted with participants to gain their understanding of how distributive leadership was practiced during the pandemic. Four themes emerged, namely leadership styles and factors informing the leadership behaviour and leadership growth of middle leaders and managers in schools; middle leaders' and managers' perceptions of distributive leadership and the responsibilities of the different stakeholders; the impact of COVID-19 on distributive leadership as perceived by middle leaders and managers; and the advantages and disadvantages of distributive leadership practices. The study recommends that School Management Teams (SMTs) involve middle leaders and managers to a greater degree to secure an effective 'buy-in' to the concept of distributive leadership and that middle managers offer opportunities for others to lead, by developing leadership-specific courses for them. SMTs must also ensure that individuals have a stable working environment where their psychological, sociological, and emotional needs are respected, fulfilled, and validated during any pandemic.

Abstract

This article was motivated by the need to academically frame and share the response of the North-West University (NWU) to the perceived increase of academic dishonesty during COVID-19. Within the ambit of the online (hybrid) teaching and learning approach that became dominant during the COVID-19 pandemic, the NWU established a Community of Practice for Academic Integrity (CoPAI) to enhance Academic Integrity (AI) in a holistic manner. By critically discussing the NWU's response through their CoPAI, the lessons learned, and strategies developed in the process, the NWU can hopefully assist other Higher Education institutes to progressively enhance AI in the future. This is important, because many contextual shifts in teaching and learning approaches, pedagogy, assessment, and the application of technology, that were enforced in an online mode of delivery during the pandemic, will prevail in future. In writing this article, we focused on contextualising the NWU CoPAI within current literature on community of practice (CoP) and Academic integrity (AI) and emphasising the unique strategy and holistic nature of this CoPAI. The establishment of the CoPAI is discussed within the appreciative inquiry as methodological framework. This methodology is commonly used by CoPs, but it is particularly relevant to the CoPAI since CoPAI sought answers to all the AI questions that presented itself due to disruptions in the higher education landscape. The appreciative inquiry method allowed for the opportunity to find some answers in a holistic manner. Some of these answers or insights gained through the activities of CoPAI is further discussed in the latter part of the article. In conclusion, some of the outcomes and shortcomings of CoPAI at the NWU are highlighted. The main finding of this article concluded that the establishment of a CoPAI can enhance AI at HE institutions in a holistic manner. The applicability, relevance, and success of this CoPAI was realised through its holistic approach which included the valorisation of institutional aspects, the engagement and empowerment of lecturers, and the engagement and empowerment of students. This novel and unique approach to promote AI in HE could fill the existing knowledge gap in the South African context, where the establishment of a CoPAI, the application of appreciative inquiry as methodology, and the inclusion of a holistic approach are still absent. It might however also be an example for other HE institutions to follow globally.

Abstract

Fragile X-associated tremor/ataxia syndrome (FXTAS) is a late adult-onset neurodegenerative disorder that affects movement and cognition in male and female carriers of a premutation allele (55-200 CGG repeats; PM) in the fragile X mental retardation (FMR1) gene. It is currently unknown how the observed brain changes are associated with metabolic signatures in individuals who develop the disorder over time. The primary objective of this study was to investigate the correlation between longitudinal changes in the brain (area of the pons, midbrain, and MCP width) and the changes in the expression level of metabolic biomarkers of early diagnosis and progression of FXTAS in PM who, as part of an ongoing longitudinal study, emerged into two distinct categories. These included those who developed symptoms of FXTAS (converters, CON) at subsequent visits and those who did not meet the criteria of diagnosis (non-converters, NCON) and were compared to age-matched healthy controls (HC). We assessed CGG repeat allele size by Southern Blot and PCR analysis. Magnetic Resonance Imaging (MRIs) acquisition was obtained on a 3T Siemens Trio scanner and metabolomic profile was obtained by ultra-performance liquid chromatography, accurate mass spectrometer, and an Orbitrap mass analyzer. Our findings indicate that differential metabolite levels are linked with the area of the pons between healthy control and premutation groups. More specifically, we observed a significant association of ceramides and mannonate metabolites with a decreased area of the pons, both at visit 1 (V1) and visit 2 (V2) only in the CON as compared to the NCON group suggesting their potential role in the development of the disorder. In addition, we found a significant correlation of these metabolic signatures with the FXTAS stage at V2 indicating their contribution to the progression and pathogenesis of FXTAS. Interestingly, these metabolites, as part of lipid and sphingolipid lipids pathways, provide evidence of the role that their dysregulation plays in the development of FXTAS and inform us as potential targets for personalized therapeutic development.

143. Improved cone penetration test predictions of the state parameter of loose mine tailings

Abstract

The most widely used in situ testing instrument for tailings storage facility (TSF) is the cone penetration test (CPT), which uses calibration chamber (CC) test data as the preferred method to correlate the soil's state with the CPT acquired data. Many, if not most, mine tailings are different from the soils historically used in CC studies (mainly sands). Moreover, these tests were conducted at denser states than typically found in TSFs. Additionally, CC tests cannot practically be performed for every new soil-specific project. For these reasons, practitioners usually perform numerical simulations to approximate these correlations. One of the most popular techniques for this is the spherical cavity expansion approach. This study compares novel small CC testing of two different silty mine tailings, and their respective spherical cavity expansion simulations. Since these simulations use the NorSand constitutive model, this study also presents the calibration process that includes the triaxial laboratory testing to calibrate the NorSand properties, and the corresponding iterative process to infer the additional plastic and elastic parameters of the model. Finally, an adjustment equation is introduced for the contractive state of these silty materials.
