UNLABELLED Investigations on medication burden, falling, and inappropriate dosing in renal impairment have been obtained in patients living in nursing homes. Data from home-dwelling patients in intensified ambulatory care, especially from Germany are scant. MATERIAL AND METHOD We evaluated patients daily visited by an ambulatory care service (Cohort 1, n = 102, median age 80 y) or had care given by relatives only (cohort 2, n = 101, median age 76 y) at baseline (V1), 6 (V2) and 12 months (V3). RESULTS At V1 patients in Cohort 1 had 5 (median, range 3 - 15) and at V3 6 (3 - 17) medications. No differences could be observed between cohorts regarding number and pattern of medications. At V1, 30/102 patients of Cohort 1 had creatinine measured within the last 6 months, 13/30 patients had an eGFR < 50 ml/min. 6/34 medications which need dose-adjustment were unadjusted. Low surveillance of renal function and unadjusted dosing were also observed at other visits and also in Cohort 2. Within 1 year, 29/75 mobile patients in cohort 1 had a fall, 18/29 patients had a benzodiazepine prescribed regularly, whereas a benzodiazepine was prescribed in 6/46 patients which did not fall (chi2 p = 0.004). In Cohort 2, the number of falling patients was lower (19/84 mobile patients, p = 0.028). 11/19 patients had a benzodiazepine prescribed, in contrast to 5/65 patients which did not fall (chi2 p = 0.001). CONCLUSIONS It needs to be elucidated whether a care service can contribute to medication safety in patients e.g. by reviewing medication charts and organizing for controls of ancillary laboratory values.

Context: Whereas in larger hospitals individualized dose adjustment in renal insufficiency can be provided by expert systems and pharmacists, these options are often not available in smaller hospitals. Aims: We evaluated whether one short educational session for the medical staff of internal wards of a community hospital, focusing on creatinine clearance and dosing in renal insufficiency, and providing a list of frequently used drugs and their dosing schedule does reduce the rate of patients with unadjusted doses. Material and methods: In patients with a creatinine clearance < 60 ml/min, dosing schedules for 92 drugs were determined. After a 6-month observation period (Cohort 1), an educational intervention and the above-mentioned list were delivered to the medical staff. This intervention was followed by a further 6-months observation period (Cohort 2). Results: In Cohort 1, 55/85 patients (median age 79 y) had at least one initially inappropriately adjusted medication, and 47/85 remained so at discharge, whereas in Cohort 2 (median age 77 y), 28/85 patients had at least one initially inappropriately adjusted medication (p = 0.014 compared to Cohort 1) and 27/85 remained so at discharge (p = 0.05). In Cohort 1, 46.0% of all prescriptions with drugs which need dose adjustment (n = 220) were not adjusted. After the intervention (Cohort 2), 25.6% of all prescriptions (n = 176) followed an unadjusted dosage (p < 0.001). Conclusion: This intervention was on a "low key"-level, and no further support e.g. academic detailing was effected. Despite this, we found a considerable reduction in the number of inappropriate doses in patients with impaired renal function.

Plättchenfunktionstests werden zur Erkennung einer angeborenen oder erworbenen Thrombozytenfunktionsstörung und zur Erfassung einer gesteigerten Plättchenaktivierung, die mit einer erhöhten Thromboseneigung einhergehen kann, verwendet. Plättchenfunktionstests werden auch zur Überwachung der Behandlung mit Thrombozytenfunktionshemmern wie Acetylsalicylsäure, Clopidogrel oder Plättchenmembran-Glycoprotein IIb/IIIa-Hemmern eingesetzt. Eine inkorrekte Blutentnahme ist der häufigste Fehler bei Untersuchungen der Plättchenfunktion. Globaltests sind neben der Plättchenzählung die Blutungszeit, die Thrombelastographie, der Plättchenfunktionsanalyser (PFA) und vielleicht in Zukunft das neue Impact®-System. Spezifische Tests messen die Thrombozytenausbreitung oder die Plättchenhaftung an definierten Oberflächen. Bei mehreren Verfahren werden die Thrombozyten vor und nach der Passage eines Filters gezählt. Einige dieser Tests sind teilweise standardisiert. Die Thrombozytenaggregation nach Gustav Born, ausgelöst durch ADP, Kollagen oder andere Substanzen, ist die am häufigsten bestimmte Plättchenfunktion. Einige neuere Methoden zur Aggregationsmessung werden beschrieben. Eine gesteigerte Plättchenaktivierung kann durch die Spontanaggregation (PAT) erfasst werden. Prospektive Untersuchungen haben gezeigt, dass die spontan gesteigerte Aggregation ein Risikofaktor bei Diabetikern für neue Gefäßverschlüsse und bei Gesunden für einen Herzinfarkt ist. Die Methode nach Wu und Hoak und die Messung von freigesetzten Thrombozyteninhaltsstoffen wie Plättchenfaktor 4 oder Betaglobulin sind nur von begrenztem Wert. Flowzytometrische Verfahren werden häufig angewendet um Aktivierungsmarker wie CD 62 und andere zu messen. Die Plättchen-induzierte Thrombinbildung ist eine wichtige Funktion zur Beurteilung von Medikamentenwirkungen. Keiner der heute verfügbaren Plättchenfunktionstests ist gut standardisiert. Hier sind weitere Anstrengungen erforderlich.

The ligand H2C(PPh2BH3)2 ( 4-H2) reacted with one or a half equivalent of (para-tBu-C6H4CH2)2Ca·(THF)4 to form the calcium complexes Ca[HC(PPh2BH3)2]2 ( 4-H)2Ca and CaC(PPh2BH3)2 ( 4-Ca), respectively. The crystal structures of their THF adducts ( 4-H)2Ca·THF and [ 4-Ca·(THF)]2 follow the same trends as observed for the corresponding iminophosphorano substituted complexes Ca[HC(PPh2NR)2]2 and [CaC(PPh2NR)2]2. The P–C bonds shorten upon gradual deprotonation, whereas the P–B and P–N bonds elongate. The geometries of DFT-optimized model systems and complete molecular structures show similar trends. Also the charge distribution within the boranophosphorano complexes is similar to that in the iminophosphorano complexes. The high positive charges on Ca (1.74–1.75) indicate a predominantly ionic ligand–Ca bonding. High negative charges on the central carbon atom (-1.103 in Ca[HC(PH2BH3)2]2 and -1.775 in [CaC(PH2BH3)2]2) compare well to those calculated for analogous iminophosphorano complexes (-1.126 Ca[HC(PH2NH)2]2 and -1.847 [CaC(PH2NH)2]2, respectively). Thus, in both types of complexes delocalization of electron density over the boranophosphorano or iminophosphorano substituents does not play a major role. Complex [ 4-Ca·(THF)]2 does not react with adamantyl cyanide. Instead the adduct ( 4-Ca)2·(THF)·(adamantyl-CN) formed (a crystal structure has been determined).

The present paper reports on a striking increase in undesirable drug induced reactions during simultaneous administration of high-dosage phenobarbital and beta-lactam antibiotics to children in intensive care. In a 30-month study period, reactions which had to be classified as drug-induced were seen in 24 out of a total of 49 children (mainly exanthematous skin reactions, in some cases severe). The complications were mainly observed in patients receiving a combination of cefotaxime and phenobarbital. Etiologically, a toxic reaction to the combination of hepatically metabolized beta-lactam antibiotics with an enzyme-inducing drug must be considered in addition to the allergenic potential of the individual components.

El experimento se realizó en un invernadero, en la comunidad de Paratodo, Chaco Central, Paraguay, durante los meses de abril a julio del 2008, con el objetivo de hallar un método alternativo, eficiente para el control de nematodos fitoparásitos, para el cultivo de tomate (Lycopersicon esculentum Mill.). El diseño experimental utilizado fue el de bloques completamente al azar, con cinco tratamientos y cuatro repeticiones. Los tratamientos aplicados fueron: Materia orgánica (estiércol bovino + gallinaza), Crotalaria (Crotalaria juncea), barbecho (sin riego), torta de tártago (Ricinus communis) y el testigo (con riego). Se evaluó el efecto, de estos tratamientos, sobre la población de fitonematodos del cultivo de tomate, específicamente la de Meloidogyne sp. y Helicotylenchus sp., así como el efecto sobre la población de nematodos de vida libre. Se realizaron dos muestreos, el primero antes de instalar los tratamientos, y el segundo, a los tres meses del experimento. Las muestras fueron procesadas en el laboratorio de Nematología, de la división de Fitopatología, de la Facultad de Ciencias Agrarias (FCA), de la Universidad Nacional de Asunción (UNA), empleando los métodos de Cobb y de flotación en azúcar. El tratamiento, que mayor efecto supresivo tuvo, sobre la población de fitonematodos, fue el tratamiento con tártago. Los tratamientos con crotalaria, barbecho y el testigo redujeron satisfactoriamente a la población de Meloidogyne sp., y en menor proporción a la población de Helicotylenchus sp.. El mayor efecto sobre la población de Helicotylenchus sp. tuvo el tratamiento con tártago, pero éste no disminuyó la población de Meloidogyne sp.. Los tratamientos con tártago, materia orgánica y crotalaria aumentaron la población de nematodos de vida libre, mientras que con los demás tratamientos, disminuyó la población de los mismos.

Objectives: In controlled trials, HMG-CoA reductase inhibitors (statins) effectively reduced cardiovascular events in patients with coronary artery disease (CAD). However, recent pharmacoepidemiological studies indicate an underuse of statins in the target population. The objective of this study was to examine the extent to which CAD patients in Germany actually received statins under field conditions. Methods: We evaluated the medical records of 296 patients referred to the cardiology outpatient clinic of the Frankfurt University Hospital by their general practitioner (GP) in the period 1995 to 1998. All patients had symptomatic, angiographically proven CAD, 142 had previous myocardial infarction. A diagnosis of dyslipidemia was taken from the records. Most patients were visited on more than 1 occasion. In all, we were able to access 296 records for a 1 st visit, 76 records for a 2nd visit and 29 records for a 3rd visit and 16 records for > 3 visits. Results: According to the entry criteria of the 4S Trial (total cholesterol 5.5-8.0 mmol/l or 212-311 mg/dl), 108 patients were deemed as eligible for lipid-lowering treatment, criteria of the LIPID Trial (4.0-7.0 mmol/l or 154 - 270 mg/dl) gave a yield of 190 patients. The actual treatment rate with a statin at the lst visit was 34% (LIPID Group) and 40% (4S Group). At later visits, the treatment rates with statins increased to 63% (LIPID Group) and 79% (4S Group), due to advice given to the GP by the outpatient clinic. When the observation period was devided into 2 periods (04/95 - 01/97; 02/97 - 09/98), actual treatment rates (all visits) for the 4S Group were 43% and 38%, respectively, indicating no further penetration of the 4S Study in the therapy decision-making of the GPs. Conclusions: The data indicate that necessary treatment with a HMG-CoA reductase inhibitor is often withheld in the ambulatory setting.

Objective: Parameters of a pharmacokinetic-pharmacodynamic (PK-PD) model of levodopa have been claimed to reflect the magnitude of the dopaminergic deficit in patients with Parkinson's disease. The aim of this study was to correlate such parameters with positron emission tomography (PET) with levodopa tagged with 6-fluorine 18, an established imaging method for striatal dopaminergic neurons. Methods. Twenty-three patients in different disease stages (Hoehm. and Yahr stage 2.5-5 [Hoehn MM, Yahr MD. Parkinsonism: onset, progression and mortality. Neurology 1967;4:427-42]; median duration, 12 years) were studied. PK-PD modeling followed a single oral dose of levodopa/benserazide. The sum score of the Columbia Rating Scale (CURS Sigma) was used for clinical assessments. A nonparametric effect compartment approach assuming a sigmoidal E-max model was applied to the PK-PD analysis of plasma levodopa concentrations and corresponding CURS Sigma. Thereafter 6-[F-18]levodopa PET was performed, and the influx rate constants (k(c)) for the putamen and the caudatus region were correlated with the median effective concentration (EC50) and the equilibrium half-life (T-eq) of the PK-PD model. Results: (1) A significant correlation was observed between PK-PD parameters or with k(c) putamen as the dependent variable and the duration of the disease as the independent variable, which explains 33% of the variability of the EC50, 42% of the variability of T-eq, and 36% of the variability of k(c).
