Abstract

The distribution of genetic mutations affects seven out of 20 patients (35%), mainly related to I TP53 i (six patients, 30%), I TET2 i (two patients, 10%) and I DNMT3A i (two patients, 10%). During the long-term follow-ups of randomized controlled trials (RCTs), some cases of therapy-associated myeloid neoplasms (t-MNs), including myelodysplastic syndrome (t-MDS) and acute myeloid leukaemia (t-AML), were reported both in the poly(ADP-ribose)polymerase inhibitors (PARPi) and control arms. For the 21 patients, t-MNs occurred after a median latency period of 18-2 (IQR 10-0-30-0) months following first PARPi exposure. Among them, 113 out of 32 662 patients (0-3%) with OC experienced t-MNs after a mean time from first primary cancer diagnosis of 5-3 years for t-MDS ( I n i = 47 patients) and 4-5 years for t-AML ( I n i = 66 patients), with an overall standardized incidence ratio of 5-8 (4-8-6-9).8 Our shorter latency may be linked to an underlying clonal haematopoiesis as I TET2 i and I DNMT3A i mutations were frequently found. Contains fulltext : 190548.pdf (Publisher’s version ) (Closed access) Objectives: To define guidelines for BK polyomavirus (BKPyV)-associated haemorrhagic cystitis (BKPyV-HC) after paediatric and adult HSCT. Methods: Review of English literature and evidence-based recommendations by expert consensus. Results: BKPyV-HC occurs in 8%-25% of paediatric and 7%-54% of adult recipients undergoing allogeneic HSCT. Diagnosis requires the triad of cystitis, macro-haematuria and high urine BKPyV loads >7 log10 copies/mL, and exclusion of other relevant aetiologies. BKPyV viraemia is frequent and may serve as a more specific semiquantitative follow-up marker. No randomized controlled trials are available to inform antiviral prophylaxis or treatment. However, hyper-hydration and/or bladder irrigation showed limited prophylactic value. Fluoroquinolones are not effective for prophylaxis or treatment, but rather increase antibiotic resistance. Hyperbaric oxygen or fibrin glue is marginally effective based on small case series from correspondingly equipped centres. Although cidofovir has been reported to improve and/or reduce BKPyV viraemia or viruria, the current data do not support its regular use. Conclusions: BKPyV-HC remains a disabling unmet clinical need in HSCT that requires novel approaches supported by proper clinical trials.

Abstract

Invasive Scedosporium spp. and Lomentospora prolificans infections are an emerging threat in immunocompromised and occasionally in healthy hosts. Scedosporium spp. is intrinsically resistant to most, L. prolificans to all the antifungal drugs currently approved, raising concerns about appropriate treatment decisions. High mortality rates of up to 90% underline the need for comprehensive diagnostic workup and even more for new, effective antifungal drugs to improve patient outcome. For a comprehensive analysis, we identified cases of severe Scedosporium spp. and L. prolificans infections from the literature diagnosed in 2000 or later and the FungiScope® registry. For 208 Scedosporium spp. infections solid organ transplantation (n = 58, 27.9%) and for 56 L. prolificans infection underlying malignancy (n = 28, 50.0%) were the most prevalent risk factors. L. prolificans infections frequently presented as fungemia (n = 26, 46.4% versus n = 12, 5.8% for Scedosporium spp.). Malignancy, fungemia, CNS and lung involvement predicted worse outcome for scedosporiosis and lomentosporiosis. Patients treated with voriconazole had a better overall outcome in both groups compared to treatment with amphotericin B formulations. This review discusses the epidemiology, prognostic factors, pathogen susceptibility to approved and investigational antifungals, and treatment strategies of severe infections caused by Scedosporium spp. and L. prolificans.
