Background: Divorce, as an issue, can contribute to the development of a wide range of problems and consequences and frequently lead to an increased risk of social problems at both individual and social levels. The present study aimed to investigate the effectiveness of compassion-focused therapy (CFT) and acceptance and commitment therapy (ACT) on post-divorce adjustment in divorced women referred to the counseling center in Ahvaz city in 2019. Methods: The research method was quasi-experimental with a pre-test, posttest, and follow-up design and a control group. Using convenience sampling, we selected 45 divorced women willing to participate in the study. Then, we randomly divided them into two experimental groups (compassion-focused therapy and acceptance and commitment therapy) and a control group (n = 15 per group). The research instrument included the Fisher Divorce Adjustment Scale (FDAS). The first intervention program consisted of eight 60-minute sessions of compassion-focused therapy and the second intervention program consisted of ten 90-minute sessions of acceptance and commitment therapy. Results: The results showed that both compassion-focused therapy (CFT) and acceptance and commitment therapy (ACT) were effective in increasing post-divorce adjustment in divorced women (p0.05). These results continued until the follow-up stage. Conclusion: According to research findings, both CFT and ACT were efficient therapies in increasing post-divorce adjustment in divorced women. Thus, both approaches can strengthen treatment interventions to increase adjustment in divorced women

The purpose of the current study is to investigate the relationship between Optimism, Religiosity and Self-esteem with Marital Satisfaction and Life Satisfaction in married university students. The research method was a descriptive study kind of correlation. The sample group included 200 married students that were selected using a simple random sampling method. For collecting data, Attribution Style Questionnaire, Religious Attitude, Cooper Smith Self-Esteem Questionnaire, Enrich Marital Satisfaction Questionnaire and Satisfaction with Life Scale were used. The findings indicated that marital satisfaction not life satisfaction was predicted by Optimism, Religiosity and Self-esteem. Religiosity showed the greatest relationship with marital satisfaction. Based on this study, family therapists should consider the role of religious or spiritual couple's attitude as a vital factor in revealing and solving the marital conflict. Consistency and inconsistency of the results were discussed.

BACKGROUND: Phenylketonuria (PKU) is a hereditary metabolic disorder and is inherited as autosomal recessive, so it is more likely to occur in consanguineous marriages. Early diagnosis is made by screening and timely treatment can prevent brain damage. METHODS: This was a descriptive study including all children identified with PKU in Qazvin Province, Iran, up to march 2017. The required information was obtained through interviews with parents and reviewing of cases. Data were analyzed using SPSS software. RESULTS: Of the 63 infected patients, 55.5% were residents of Qazvin City and the rest were residents of other cities in the province. Parents of 54.0% of the patients were related. 20.6% of patients had at least one patient with PKU in their family. The mean age that patients were diagnosed before screening was 34 months, and the statistical difference between the two groups was significant (P < 0.001). 52.4% of the patients were girls. The most common reason of referring of the patients before screening was a developmental delay. In the present study, the effect of different concentrations of hormone ethinyl estradiol was investigated on microalgae Cheatoceros muelleri. Seven different concentrations (250, 500, 1000, 1500, 1750, 2000, and 2500 μg/l) of the hormone were measured by a replicate control treatment. Factors including the size of chlorophyll a, specific growth coefficient, and protein content were measured in microalgae C. muelleri. The results showed that at concentrations of 250 and 500 μg/l, growth rate and chlorophyll a content in hormone-treated samples increased significantly compared to the control. In contrast, more protein was observed at the concentration of 250 μg/l than in the control group in microalgae. The results of this study showed that increasing the treatment period in most concentrations had a significant effect on growth inhibition and in some cases caused the growth of microalgae. Higher concentrations of the hormone also affected the growth inhibition, chlorophyll amount, and protein contents of C. muelleri.

Background: The impact of initiation time of radiotherapy after breast surgery on disease-free status of patient is a controversial issue. We assessed the relationship between the delay in adjuvant radiotherapy and surgery and relapse or metastasisfree survival in women with breast cancer taking heavy censoring into account. Method: This was a historical cohort study on 284 patients with breast cancer, who underwent surgery between 2001 and 2007 and followed up until March 2017. The association of the duration between radiotherapy and surgery, as well as other demographic and clinical factors with occurrence of local relapse or metastasis of breast cancer was examined through penalized Cox regression modeling. The obtained data were analyzed using R 3.6.3. Results: A total of 284 women with the mean age of 47.2 ± 11.3 years met the inclusion criteria. The maximum follow-up time was 11.1 years and the time between surgery and radiotherapy was 168 ± 84.3 days. About 10% of the patients experienced local relapse and 19% had metastasis. In multiple analysis of factors related to diseasefree survival, the stage of disease was significant, while surgery to radiotherapy interval (≤180 days vs >180 days) did not have any significant impact on hazard of failure. Analysis of 3, 4, or 5 months of delay in radiation therapy did not imply any significant affects. Conclusion: In the studied patients, the delay in radiotherapy initiation after surgery did not lead into outcomes of local relapse or metastasis.

Background: Puberty is an essential milestone in a person’s life. Studies show that precocious puberty is more common in girls than in boys. Objectives: The aim of this study was to survey the causes of precocious puberty in children referred to the endocrine clinic in Qazvin city, Iran, from 2006 to 2018. Methods: This case-series study was performed from March 2006 to June 2018 on patients referred to the endocrine clinic of Qazvin University of Medical Sciences, Qazvin City, Iran, for precocious puberty. To diagnose premature puberty and find its causes, we surveyed sex steroids levels, thyroid function, and, if necessary, performed GnRH test. The patients’ height and weight were measured, and their BMIs (Body Mass Index) were calculated. Uterine ultrasound was performed for all girls. Additionally, brain MRI was performed for all boys and girls under age 6 with precocious puberty. X-ray of the left hand was performed to assess skeletal growth acceleration. Based on the type of puberty, the patients were divided into three groups: Central Precocious Puberty (CPP), Peripheral Precocious Puberty (PPP), and normal variant of puberty. The study data were collected from patients› electronic files and analyzed using SPSS 23. Results: Out of 724 cases, 642 (88.70%) were girls. The mean age of all children was 7.07 (95%CI: 6.99-7.15) years. About 70.5% of cases had CPP, 5% PPP, and 24.5% normal variant of puberty. Background: Type 1 Diabetes (T1D) is an autoimmune condition, in which the pancreas produces little or no insulin. Nephropathy is a serious T1D microvascular complication that is associated with high mortality and morbidity. Objective: This study aimed to investigate the prevalence of diabetic nephropathy and comorbidities in children with T1D. Methods: This cross-sectional study was conducted on 208 children (aged 1–18 years old) with T1D who were referred to the Qazvin endocrinology clinic from 2017 to 2019. Anthropometric, demographic, laboratory, and comorbidities data were collected. Results: The Mean±SD age at diagnosis of diabetes was 7.59 years, and the Mean±SD HbA1c level of the study subjects was 8.68±1.42 mmol/mol. Out of 208 diabetic patients, 64 cases (30.7%) had diabetic nephropathy, of whom 53 cases (25.5%) had microalbuminuria and 11 cases (5.3%) had macroalbuminuria. Among the studied diabetic patients, 30 cases (14.45%) had hypothyroidism, 12 patients (5.8%) had celiac disease, and 14 patients (6.7%) had anemia. Retinopathy was not found in any of the patients. Introduction and purpose: Vitiligo is the most common depigmenting skin disorder. The present study aimed to evaluate the effectiveness of acceptance and commitment therapy on body dysmorphic disorder and rumination in people with vitiligo. Methods: This applied research was conducted based on a Pre-test/Post-test/ follow-up design with the control group. The statistical population of the study included all patients with vitiligo referred to dermatologists in dermatology and cosmetic clinics in Ahvaz in 2019. Among them, 30 patients were selected by purposeful sampling method and assigned to two groups of cased and control. In order to collect data, the following questionnaires were used: Body Dysmorphic Disorder of Littleton, Aksom and Puri (2005), as well as Rumination of Nolen, Hoxma and Marv (1991). Background: Dyscalculia and dysgraphia are among the prevalent problems faced by students with specific learning disorders (SLD). The present study aimed to investigate the role of balance enhancement exercises in improving dyscalculia and dysgraphia among students with SLD.Methods: This was a quasi-experimental study with a pretest-posttest design and a control group. The statistical population of this study included all boy and girl second-graders and third-graders with SLD who received education and rehabilitation services from public and private centers for learning disabilities in Ahvaz, Iran in 2022. The convenience sampling method was employed to select 30 participants who were equally assigned to experimental and control groups (i.e., 15 per group). The experimental group received balance enhancement exercises (for twelve 45-minute sessions), whereas the control group received no interventions. To collect information, the Dyscalculia Test and Dysgraphia Test were used. The ANCOVA was then used for data analysis.Results: The mean±standard deviation (SD) of the post-test scores of dyscalculia and dysgraphia in the experimental group was 106.93±14.99 and 75.60±15.10 which was significantly different from the control groups (91.33±14.45 and 60.93±19.36). This study intends to deal with attitudes of teachers and learners toward authentic listening materials at pre-intermediate level. Studies have indicated the positive effect of authentic listening materials on motivation and listening comprehension ability in learners of English as a foreign language (Nuttall, 1996; Peacock, 1997; Miller, 2005; Field, 2008). It focused on EFL teachers and students at pre intermediate proficiency level. Participants included 60 students, from both gender; male and female university students studying at Rodaki Higher Institute Education in the fields of: Accounting, Computer Engineering and Commercial Management and also 30 teachers who taught listening at Rodaki Higher Institute Education. The students were randomly selected and assigned to two groups. Then, Oxford Placement test as a standard test was administered to the participant in order to determine their proficiency level. Then, the participants were divided into two groups of control and experiment. The experiment group received the listening materials taken from UK radio program whereas the other group received simplified listening materials taken from the authentic listening materials. Afterwards, to be sure that they know what authentic materials are, they were exposed to both of related materials, then the questionnaire was distributed among them, the results taken from the questionnaire showed that the participants of this study prefer authentic materials and have positive attitudes toward using them. By the same token, the analysis of teachers' questionnaire also showed their satisfaction with authentic listening materials. These findings can have implications for language learning/teaching, and curriculum development paving the way for educational policy makers, teachers and learners to introduce authentic listening materials to EFL learners at pre-intermediate proficiency level.

Introduction: In various radiotherapy techniques for breast cancer, the inclusion of internal mammary nodes (IMNs) in the target volume is important for selecting the most appropriate technique. This study aimed to compare three radiotherapy techniques with the inclusion of IMNs regarding the dose homogeneity index (DHI) of regional lymph nodes and the chest wall, besides the dose received by the heart and the left lung. Material and Methods: Three radiotherapy techniques were planned for CT imaging of the RANDO phantom, including the wide tangent (WT); oblique parasternal photon (OPP); and oblique parasternal electron (OPE) techniques. The doses reaching the contoured organs were compared between the three techniques, using the data gathered from the thermoluminescent dosimetry and treatment planning system. Results: The OPE technique produced a lower absorbed dose for the left IMNs, compared to the other two techniques. In the OPP technique, the dose received by the left lung was higher than its tolerance, while the lung dose in the OPE technique was slightly lower than the WT technique. The absorbed dose by the heart was the lowest in the WT technique; also, the DHI value was better for this technique than the other two techniques. Conclusion: The WT technique showed better results regarding the dose homogeneity distribution of IMNs and the chest wall, as well as protection of organs at risk.

Introduction: Life satisfaction reflects an individualchr('39')s overall feelings about life, influenced by various factors. Objective: This study aimed to identify and introduce the indices related to the living environment that affect life satisfaction. Materials and Methods: This study was performed on 1000 residents of 55 neighborhoods of Rasht City (north of Iran) by an analytical cross-sectional method. The participants were chosen with a multistage cluster sampling method. The study data were collected using questionnaires, including the residentschr('39') characteristics, indicators of the living environment, and the satisfaction with life scale. The obtained data were analyzed using descriptive statistics, and inferential statistics, including the Pearson correlation coefficient, independent t test, analysis of variance, and linear regression. Results: Based on the present study results, the mean±SD age of the subjects in the study was 43.73 ±15.55 years. The analysis showed that their level of life satisfaction was average mean ±SD: 19.41 ±7.63, range: 5-35). The regression analysis identified dissatisfaction of facilities (B= -0.366; 95%Cl : -0.434, -0.298; P= 0.001), satisfaction with living in the local area (B= 0.248; Cl: 0.123, 0.373; P= 0.001), duration of residence (B: -0.364;95% Cl: -0.625, -0.102; P= 0.006) and gender (women compared to men) (B: 0.896; Cl: 0.011, 1.781; P=0.047) as the predictors of life satisfaction. Conclusions: According to the results, the life satisfaction of our study participants was at a moderate level and was affected by living environment indicators, especially facilities.

Introduction: Anaplastic variant of juvenile granulosa cell tumor is one of the rarest ovarian malignancies. The aim of this study is to describe a case of anaplastic variant ovarian granulosa cell tumor in a 14-year-old girl with presentation of abnormal uterine bleeding and gradual abdominal enlargement. Case presentation: The patient at the time of diagnosis with an inhibin A and B more than 500 Pg/ml and ovarian mass of about 30×20×15 cm underwent unilateral salpingo-oophorectomy and juvenile granulosa cell tumor was diagnosed with no follow-up. She has referred with ascites, recurrent mass and distance metastasis of liver after 10 months. The patient underwent four courses of BEP (Bleomycin, Etoposide, Cisplatin)and five courses of paclitaxel and carboplatin. Then, she underwent laparotomy for metastasectomy, which was not successful due to the extent of involvement. Eventually, she died six months after the second surgery. Conclusion: This case report confirms the need for accurate follow-up of patients with juvenile granulosa cell tumor and appropriate justification for the patient and their caregiver to prescribe needed adjuvant therapies to prevent recurrence.

Seismic vehicle‐bridge interaction (SVBI) is the study of vehicle‐bridge interaction (VBI) in the presence of earthquake excitation. SVBI is an interdisciplinary problem of increasing importance to the design and safety of railways. This study deploys a consistent methodology to decouple the vehicle‐bridge system and solve independently the bridge and vehicle subsystems, bypassing multiple challenges the seismic response analysis of a coupled vehicle‐bridge system entails. The proposed approach builds upon the previously established Extended Modified Bridge System (EMBS) method for decoupling vehicle‐bridge systems (in the absence of earthquake excitation). Its premise is to first characterize and then assess the relative importance of the VBI effect on the bridge and vehicle responses and replicate it by modifying the pertinent uncoupled equations of motion (EOMs). The formulation deployed accommodates multi‐degree of freedom models for both the vehicle and bridge and can thus tackle complex systems. The analysis examines the ability of the proposed decoupling approach to predict the response of a realistic system vehicle‐bridge system under a suit of historical earthquake records. The decoupled results are in excellent agreement with the coupled solutions for all earthquake records and scenarios (i.e., earthquake excitation solely in the transverse direction of the bridge, as well as in both the transverse and vertical directions simultaneously). Pseudohypoaldosteronism type 1 (PHA1) has two genetically distinct variants, including renal and systemic forms. Systemic PHA type I (PHA1B) has varying degrees of clinical presentation and results from mutations in genes encoding subunits of the epithelial sodium channel (ENaC) including the alpha, beta, and gamma subunits. To date, about 45 variants of PHA1B have been identified. We report a boy with PHA1B, who presented with vomiting, lethargy, and poor feeding due to salt wasting six days after birth. The patient had electrolyte imbalances. A novel SCNN1A (sodium channel epithelial subunit alpha) gene mutation, NM_001038.6:c.1497G>C, with an autosomal recessive pattern, was identified by whole exosome sequencing. This variant was inherited as a homozygote from both heterozygous parents. PHA should be considered in neonates with hyponatremia and hyperkalemia. This case report presents a patient with a novel mutation in SCNN1A that has not been previously reported. Polycystic Ovary Syndrome (PCOS) is a complex hormonal disorder that affects the endocrine glands and reproductive processes in adolescent girls, potentially impacting their quality of life. This study aims to compare the quality of life between adolescent girls with PCOS and those without PCOS. The present study is an analytical cross-sectional study. It involved 12–18-year-old girls who were diagnosed with PCOS based on the Rotterdam criteria for the case group. An ordinal logistic regression model was employed to assess the impact of the studied variables on different levels of quality of life, and Odds Ratio (OR) values were determined for predictor variables. The findings of the study revealed that the control group had a significantly higher percentage of individuals with excellent quality of life (score 209–260) compared to the group with polycystic ovary syndrome (p<0.05). The results of the ordinal logistic regression analysis indicated that age variables (OR=0.64, CI 95 %; 0.44–0.93), menstrual disorders (OR=0.07, CI 95 %; 0.01–0.38), and micromastia (OR=0.03, CI 95 %; 0.004–0.34) were identified as factors influencing the quality of life of girls with polycystic ovary syndrome. It was concluded that patients with PCOS had a lower quality of life score. The variables of age, menstrual disorders, and micromastia were determined as influencing factors the quality of life. The results of this study are significant and warrant further research in this area, particularly with regard to the financial resources of patients in the challenging economic circumstances facing the nation – the majority of which are brought on by economic sanctions. Background: There are believed to be several risk factors affecting the prognosis of breast cancer through their effect on the growth rate of tumour. In the present study, we investigated estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER2), Ki-67, and tumor protein P53 (TP53) as well-known biomarkers, particularly in breast cancer prognosis, associated with age. Method: In a case-control study, 406 breast cancer patients were considered retrospectively. In order to extract the clinical and pathologic data, we employed the patients’ records. The extracted information was compared between two groups: for patients under 40 (group I) and above 40 years of age (group II). Herein, the researchers performed statistical analysis using SPSS Ver16. Results: The most prevalent type of cancer in both groups was found to be invasive ductal carcinoma. The major method of treatment was modified radical mastectomy. According to our observations, grade 3 breast cancer was more common in group I. Lymph node involvement significantly increased in group I, while oestrogen and progesterone receptor expressions were less in this group. HER2, TP53, and Ki-67 oncogenes were overexpressed in group I compared with group II. Abstract Background The International Agency for Research on Cancer (IARC) has classified shift work as a possible human carcinogen. The results of systematic on this topic is contradictory. This systematic review and meta-analysis, therefore, aimed to update the current evidence regarding the relationship between night-shift work duration and breast cancer risk. Methods PubMed, Web of Science, and Scopus as well as reference list of included studies were searched until December 19, 2020. Observational case–control or cohort studies investigating the relationship between the duration of night-shift work and breast cancer in women were included, which all quantified night-shift work exposure. All statistical analyses were done by Stata version 11.2. Results Our literature search was resulted in retrieval of 4854 publications from which 26 eligible studies with 1,313,348 participants were included in the meta-analyses. BACKGROUND AND OBJECTIVE: Lipoid congenital adrenal hyperplasia (LCAH) is the most severe form of adrenal hyperplasia and mutations in the StAR gene are the most common cause of the disease. Adrenal insufficiency and cholestasis are reported in few patients. The aim of this study was to report the results of treatment of two sisters with lipoid CAH and cholestatic jaundice. CASE REPORTS: Here, we present two sisters at the age of 30 and 60 days with conjugated hyperbilirubinemia and elevated liver enzymes and adrenal insufficiency. They had a 46,XY karyotype with external female genitalia without uterus and ovaries. LCAH was detected through electrolyte abnormalities, increased ACTH, decreased levels of cortisol and sex hormones and was confirmed by determination of exome sequencing and Sanger sequencing. In these patients, a homozygous mutation (c.653C>T) in exon 6 of STAR gene was identified. The patients were treated with 10 mg of hydrocortisone IV every 8 hours for 3 days; oral hydrocortisone was then administered at a dose of 2.5 mg every 8 hours and 0.2 mg fludrocortisone daily. One month after the therapy, levels of bilirubin and liver enzymes of these patients became normal. The first patient died 7 months after her mother stopped giving the drugs to the child. The other patient is now 9 years old. She is in good clinical condition as her treatment goes on. CONCLUSION: Considering the reported cases, adrenal lipoid hyperplasia should be considered as a rare cause of cholestasis with adrenal insufficiency in patients.

The effect of inelastic response of the soil-foundation interface is explored on the seismic demand of structures attached on top of shallow foundations. An ensemble of 20 strong ground motions recorded on National Earthquake Hazards Reduction Program site class D was employed for analyzing soil-structure systems with the Winkler foundation model and an equivalent single-degree-of-freedom system as the superstructure. Results show that there are key parameters that control the amount of difference between the elastic and inelastic modeling of the soil at the soil-foundation interface. Depending on the structural aspect ratio, the elastic modeling leads to an overestimated result for the total lateral displacement demands. Also, more than 50% reduction in the superstructure demands is desired when nonlinear soil modeling is considered and foundation sliding is allowed. The benefits from the 'shrinking-dominated rocking motion' can be acquired with inelastic soil material that limits the transferred inertial force into the superstructure. Background and Aim: The present study aimed to use the median nerve Compound Muscle Action Potential (CMAP) amplitude by stimulation at the palm instead of Abductor Pollicis Brevis (APB) needle Electromyography (EMG) for determining axonal loss in patients with Carpal Tunnel Syndrome (CTS). Methods and Materials/Patients: This study was performed on 180 patients with CTS referred to the Electrodiagnostic (EDX) Center, Poursina Hospital, Guilan Province, Iran, in 2018-19. In this study, the APB needle EMG diagnostic test was used as the gold standard, and median nerve CMAP amplitude with stimulation at the palm and wrist were used to compare the two nerve stimulation tests. Results: All of the cases with abnormal amplitude loss detected by median nerve stimulation at the palm also had an axonal loss in the needle EMG of APB. So this test could be a good indicator of axonal loss if there is an abnormality (sensitivity: 73%, specificity: 100%). The results with wrist stimulation were not as accurate as of the palm stimulation, and some cases with decreased CMAP amplitude of median nerve had normal needle EMG of APB muscle (sensitivity: 86.6%, specificity: 94.9%). Conclusion: In cases with CTS, the abnormally decreased amplitude of the median nerve detected by stimulation at the palm could be a good indicator of axonal loss.
