Background Methods Results Conclusions Food allergies are increasingly common and so hospitals must promptly identify and manage these to maintain patient safety. The present study explored patient understanding and expectations of food allergy management in hospital food services.This exploratory descriptive study used mixed methods, including semi‐structured interviews and surveys with hospital inpatients declaring food allergies, aiming to explore understanding, expectations and preferences for food allergy management in the hospital setting.Twenty‐four participants were interviewed, reporting between one and six food allergies across 25 different allergens. Three main themes were identified: “Shared responsibility for declaring and managing allergens in hospital”, “Varied understanding and trust in hospital food service processes” and “Satisfactory hospital food service experience”. In the latter, participants identified 13 key factors valued for hospital food, including appearance, food quality and flavour. Remarkably, being “free from” the food allergen was not amongst the most common factors. Generally, participants felt that declaring a food allergy had not negatively impacted their hospital admission, and overall satisfaction with food services did not differ between the food allergic study participants and the general hospital population.Overall, the declaration of a food allergy did not negatively impact on hospital experience. However, further investigations are required to identify the most valued factors for hospital meals, aiming to enable a more targeted approach to better meet patient expectations. Background: Organizational health literacy (OHL) describes conditions and measures in healthcare institutions to enable patients to make good health-related decisions. By providing easy access to and appropriate communication of understandable information to use and navigate the facility, healthcare organizations can contribute to strengthening patients' health literacy and self-management. The extent of OHL implementation in German hospitals remains largely unknown. This study aims to fill this gap in our knowledge by investigating OHL-related activities reported by hospital managers. Methods: Between November and December 2022, we conducted a national online survey among medical, nursing and administrative hospital managers with hospitals that operate more than 50 beds. The data were collected via the health literate health care organization ten item questionnaire (HLHO-10) and supplemented by sociodemographic questions and an open-ended question. We applied variance and correlation analyses to investigate the data. Results: Of 3,301 invited hospital managers, 371 participated in the survey (response rate 11%). The overall mean score for HLHO-10 was 4.6 (SD = 1.1) on a 7-point Likert scale, indicating a moderate level of OHL implementation. Hospital managers stated that hospitals concentrate on helping patients find their way around and communicating the costs of treatment transparently and clearly; conversely, that active patient participation in the design and evaluation of health information is rare in care settings, and that health information is seldom provided to patients through a range of media. For the practical implementation of the OHL, most hospital managers mentioned activities regarding communication standards, such as providing information materials. Conclusions: Given their unique position as hubs of human interaction, hospitals provide an ideal opportunity to promote the adoption of OHL. By actively involving patients, hospitals can better tailor their approaches to meet patient needs and preferences. Compared to studies from oncology centres in Germany and 20 Italian hospitals, the average HLHO-10 score of this study is lower. While some aspects of OHL are already embedded in inpatient care, it is imperative that OHL is thoroughly embedded in the hospitals' organizational culture and plays a fundamental role in the daily operations of the institution. Background: Forecasting models predicting trends in hospitalization rates have the potential to inform hospital management during seasonal epidemics of respiratory diseases and the associated surges caused by acute hospital admissions. Hospital bed requirements for elective surgery could be better planned if it were possible to foresee upcoming peaks in severe respiratory illness admissions. Forecasting models can also guide the use of intervention strategies to decrease the spread of respiratory pathogens and thus prevent local health system overload. In this study, we explore the capability of forecasting models to predict the number of hospital admissions in Auckland, New Zealand, within a three-week time horizon. Furthermore, we evaluate probabilistic forecasts and the impact on model performance when integrating laboratory data describing the circulation of respiratory viruses. Methods: The dataset used for this exploration results from active hospital surveillance, in which the World Health Organization Severe Acute Respiratory Infection (SARI) case definition was consistently used. This research nurse-led surveillance has been implemented in two public hospitals in Auckland and provides a systematic laboratory testing of SARI patients for nine respiratory viruses, including influenza, respiratory syncytial virus, and rhinovirus. The forecasting strategies used comprise automatic machine learning, one of the most recent generative pre-trained transformers, and established artificial neural network algorithms capable of univariate and multivariate forecasting. Results: We found that machine learning models compute more accurate forecasts in comparison to naïve seasonal models. Furthermore, we analyzed the impact of reducing the temporal resolution of forecasts, which decreased the model error of point forecasts and made probabilistic forecasting more reliable. An additional analysis that used the laboratory data revealed strong season-to-season variations in the incidence of respiratory viruses and how this correlates with total hospitalization cases. These variations could explain why it was not possible to improve forecasts by integrating this data. Conclusions: Active SARI surveillance and consistent data collection over time enable these data to be used to predict hospital bed utilization. Background: Measures in operations management are pivotal for monitoring and assessing various aspects of hospital performance. Existing literature highlights the importance of regularly updating key management measures to reflect changing trends and organizational goals. Advancements in machine learning (ML) have presented promising opportunities for enhancing the process of updating operations management measures. However, their specific application and performance remain relatively unexplored. We aimed to investigate the feasibility and effectiveness of using common ML techniques to identify and update key measures in hospital operations management. Methods: Historical data on 43 measures on financial balance and quality of care under 4 categories were retrieved from the BI system of a regional health system in Central China. The dataset included 17 surgical and 15 non-surgical departments over 48 months. Four common ML techniques, linear models (LM), random forest (RF), partial least squares (PLS), and neural networks (NN), were used to identify the most important measures. Ordinary least square was employed to investigate the impact of the top 10 measures. A ground truth validation compared the ML-identified key measures against the humanly decided strategic measures from annual meeting minutes. Results: For financial balancing, inpatient treatment revenue was an important measure in 3/4 years, followed by equipment depreciation costs. The measures identified using the same technique differed between years, though RF and PLS yielded relatively consistent results. For quality of care, none of the ML-identified measures repeated over the years. Those consistently important over four years differed almost entirely among four techniques. On ground truth validation, the 2016–2019 ML-identified measures were among the humanly identified measures, with the exception of equipment depreciation from the 2019 dataset. All the ML-identified measures for quality of care failed to coincide with the humanly decided measures. Conclusions: Using ML to identify key hospital operational measures is viable but performance of ML techniques vary considerably. RF performs best among the four techniques in identifying key measures in financial balance. None of the ML techniques seem effective for identifying quality of care measures. Background: Hospital resilience has been well recognized among healthcare managers and providers as disruption of hospital services that threatens their business environment. However, the shocks identified in the recent hospital resilience concept are mainly related to disaster situations. This study aims to identify potential shocks that hospitals face during disruptions in Indonesia. Method: This qualitative study was conducted in Makassar, Indonesia in August-November 2022. Data was collected through semi-structured interviews with hospital managers and resilience experts using a semi-structured interview guide. 20 key informants were interviewed and data were analyzed by thematic analysis. Results: The study identified seven shocks to hospitals during the disruption era: policy, politics, economics, hospital management shifting paradigms, market and consumer behavior changes, disasters, and conflicts. It also identified barriers to making hospitals resilient, such as inappropriate organizational culture, weak cooperation across sectors, the traditional approach of hospital management, inadequate managerial and leadership skills, human resources inadequacies, a lack of business mindset and resistance to change. Conclusion: This study provides a comprehensive understanding of hospital shocks during disruptions. Advances in fire safety and building construction have made fires in modern health care facilities uncommon and easily contained. Therefore, emergent hospital evacuations resulting from an internal fire are rare. On January 4, 2017, a fire occurred at the John Sealy Hospital, part of the University of Texas Medical Branch at Galveston, resulting in emergent evacuation of the hospital building and an adjacent office and patient care building. The internal emergency response system was quickly activated, and more than 100 patients and 200 staff members were evacuated within 27 minutes with no fire- or smoke-related injuries. Obstacles encountered during the evacuation process included difficulties with communication and confirming patient count in a timely manner. This article describes the emergent hospital evacuation during the fire, summarizes the subsequent reflections of a multiagency after-action review, and issues a call to action for further research and publication on emergency patient evacuation. Background: Within component therapy of massive transfusion protocol (MTP) in trauma, thawed plasma is particularly susceptible to expiring without use given its short 5-day shelf life. Optimizing the number of thawed products without compromising safety is important for hospital resource management. The goal is to examine thawed plasma utilization rates in trauma MTP events and optimize the MTP cooler content at our Level I trauma center. Methods: Trauma MTP activations from 01/2019 to 12/2022 were retrospectively reviewed. During the study period, blood products were distributed in a 12:12:1 ratio of packed red blood cells (pRBC): plasma: platelets per cooler, with up to 4 additional units of low-titer, group O whole blood (LTOWB) available. The primary measure was percent return of unused, thawed plasma. Results: There were 367 trauma MTP activations with amedian (IQR) activation call-to-first cooler delivery time of 8 (6-10) minutes. 73.0% of thawed plasma was returned to the blood bank unused. In one third of MTP activations, all dispensed plasma was returned. The majority (74.1%) of patients required 6 or fewer units of plasma. In 81.5% of activations, 10 or fewer units of plasma and 10 or fewer units of pRBC were used. Discussion: The majority of trauma MTP requirements may be accommodated with a reduced cooler content of 6 units pRBC, 6 units plasma, and 1 pheresis platelets, buffered by up to 4 units LTOWB (approximates 4 units of pRBC/4 units plasma), in conjunction with a sub-10min cooler delivery time. This study investigates the application of discrete event simulation in analyzing patient management and cost dynamics within a hospital system. A simulation that integrates machine learning models, specifically Decision Trees, Random Forests, Support Vector Machines, and Gradient Boosting methods, to predict treatment costs and appointment availability was developed. Conducted over 30 days, the simulation generates synthetic data for training the models. The results are assessed in terms of the total number of patients treated, cumulative costs incurred, and the cost-effectiveness of each predictive model. The findings reveal significant variations in the performance of different machine learning techniques, demonstrating that adopting advanced analytics can substantially improve hospital resource management. Background: Radiology, endoscopy, and clinical evaluation are the three most used diagnostic modalities for adenoids. This research aimed to evaluate the most accurate method for assessment of adenoid hypertrophy (radiological versus endoscopic Assessment). Patients and methods: We carried out this cross-sectional study on 60 children with chronic adenoid hypertrophy recruited from the ENT outpatient clinic in Zagazig university hospital for management their problem. During the initial assessment, a diagnostic nasal endoscopy was conducted utilizing both rigid and flexible endoscopes.X-ray nasopharynx lateral view was done for all patients. Results: By X ray 30% of the cases were Grade I, 31.7% were Grade II, 18.3% were Grade III and 20 % were Grade IV. By endoscope 30% of the cases were Grade I, 31.7% were Grade II, 28.3 were Grade III and 10% were Grade IV. Statistically significant agreements were revealed between the clinical grading and X ray (p<0.001), the clinical grading and endoscope (p=0.003), X ray and endoscope (p<0.001). Statistically significant positive correlation was found between grading by clinical examination and both X ray and endoscope (p<0.001). The sensitivity of X ray in diagnosis of the adenoid obstruction was 83.3%, specificity was 94.4%, PPV 62.5%, NPV 98% and Accuracy was 93.33% in comparison to endoscope as gold standard. Conclusion: The X-ray alone can rule out adenoidal hypertrophy, but alone it could be insufficient for assessment of the degree of adenoidal obstruction. Endoscopy was found to be more reliable, convenient, correlate well with the volume of adenoid tissue and allow estimation of adenoidal hypertrophy with degree of obstruction. This study demonstrates that combining clinical grading with endoscopy and radiology is important for the evaluation of adenoid hypertrophy. [ABSTRACT FROM AUTHOR]

Objective: The objective of this study is to identify the perception of opportunities and risks by hospital managers with regard to the implementation of autonomous care robots in practice. Methods: The empirical study is based on a qualitative research approach and uses the grounded theory methodology. Using purposive sampling, 11 experts for in-depth interviews were selected to gain the necessary data. The interviews were recorded and transcribed. The transcripts were coded and analyzed using MAXQDA software. Results: The care provided by autonomous robots is generally positively perceived by hospital management; it is expected that it will inevitably be put into practice, even if it brings considerable risks. These are, however, outweighed by the advantages resulting from, for example, value-neutral care, uninterrupted availability, increased independence for patients, maintaining the system despite a shortage of nursing staff, etc. A large-scale reorganization of hospital funding will be necessary, as well as communication with patients to help them become confident in robotic care. Conclusion: There was widespread agreement across the interviews that it will no longer be possible to provide nursing care without robots in the future. Nursing staff should start preparing, i.e., get training so that they perceive robots as their helpers and not as threats or competitors, and this joint care of humans and robots should be included in nursing practice during high school studies. The role of Transformational Leadership in hospital organizations is very important, where hospitals are organizations that have a complex environment, there is a lot of unclear information. With the increase in health services to the community along with changes in demographics, changes in health service technology, and the adoption of new information technology. So performance measurement is important in order to improve the quality of health services. The aim of this research is to find out how transformational leadership influences the success of hospitals management information systems (HMIS). The method used was a quantitative descriptive and verification survey with 181 respondents from 15 hospital in the city of Bandung. The sampling technique uses proportionate stratified random sampling. Data analysis using structural equation modeling (SEM) with partial least squares (PLS). The findings of the proposed hypothesis are that the quality of technology, transformational leadership, and users have a significant effect on service performance. Users can mediate technology quality, transformational leadership and service performance. Conclusion of the study is the novelty of the development of the information system success model by adding the context of transformational leadership in evaluating the success of the information system, where transformational leadership has a positive effect on improving the performance of hospital management information system services through users. Implications of the results of this study contribute to the development of an information system success model that can be used to evaluate the success of information systems in general. Objectives: This study aimed to explore the role of CT in septic patients presenting to the emergency department (ED). Materials and methods: We performed a retrospective secondary analysis of 192 septic patients from a prospective observational study, i.e., the "LIFE POC" study. Sepsis was diagnosed in accordance with the Sepsis-3 definition. Clinical and radiological data were collected from the hospital administration and radiological systems. Information on mortality and morbidity was collected. Time-to-CT between CT scan and sepsis diagnosis (ttCTsd) was calculated. Diagnostic accuracy was assessed with the final sepsis source as reference standard. The reference standard was established through the treating team of the patient based on all available clinical, imaging, and microbiological data. Results: Sixty-two of 192 patients underwent a CT examination for sepsis focus detection. The final septic source was identified by CT in 69.4% (n = 43). CT detected septic foci with 81.1% sensitivity (95% CI, 68.0–90.6%) and 55.6% specificity (95% CI, 21.2–86.3%). Patients with short versus long ttCTsd did not differ in terms of mortality (16.1%, n = 5 vs 9.7, n = 3; p = 0.449), length of hospital stay (median 16 d, IQR 9 d 12 h–23 d 18 h vs median 13 d, IQR 10 d 00 h–24 d 00 h; p = 0.863), or duration of intensive care (median 3d 12 h, IQR 2 d 6 h–7 d 18 h vs median 5d, IQR 2 d–11 d; p = 0.800). Conclusions: Our findings show a high sensitivity of CT in ED patients with sepsis, confirming its relevance in guiding treatment decisions. The low specificity suggests that a negative CT requires further ancillary diagnostic tests for focus detection. The timing of CT did not affect morbidity or mortality outcomes. Clinical relevance statement: In patients with sepsis who present to the ED, CT can be used to identify infectious foci on the basis of clinical suspicion, but should not be used as a rule-out test. Scientific evidence for the optimal timing of CT beyond clinical decision-making is currently missing, as potential mortality benefits are clouded by differences in clinical severity at the time of ED presentation. Purpose/Background: Recent studies have shown improved outcomes with the initiation of earlier subcutaneous (SQ) basal insulin. The purpose of this study was to examine the effects of early SQ basal insulin administration on hospital length of stay in patients with mild to moderate diabetic ketoacidosis (DKA). Methods: This was a retrospective, single-center study from a large community teaching hospital that included patients 18 years or older with mild to moderate DKA, identified using ICD-10 codes, who received intravenous (IV) insulin. Patients who received SQ basal insulin prior to a documented anion gap ≤12 mmol/L were considered to have received early SQ basal insulin and were compared to patients who received SQ basal insulin after closure of their anion gap (AG). The primary outcome was hospital length of stay. Secondary outcomes included intensive care unit length of stay, duration of IV insulin, time to anion gap closure, and incidences of rebound hyperglycemia. Safety outcomes included incidences of hypoglycemia, and hypokalemia. Results: Of 301 patients screened, 108 patients were included in the final analysis. Forty patients received early SQ basal insulin and 68 did not. Median hospital length of stay in the nonearly group was 71 h, compared to 62 h in the early group (P =.57). Secondary and safety outcomes were similar between groups. Conclusions: In this study, there was no statistically significant difference in length of stay in patients that received early SQ basal insulin. Larger trials are needed to determine the significance of earlier SQ basal insulin in DKA. [ABSTRACT FROM AUTHOR]

Healthcare resources should not be misused as such resources can save people's life. Further, avoiding excessive resource employment can help minimize production costs. In this regard, measuring technical efficiency is essential as it helps identify the resource utilization level against the output level. The present study has estimated the relative technical efficiency scores of private hospitals in the Kamrup (Metro) district of Assam. The data for the study is collected from primary and secondary sources for the year 2019. Bootstrapping Data Envelopment Analysisis applied considering two outputs and six input variables. All the hospitals are technically inefficient; however, small size hospitals have shown better performance in terms of technical efficiency than big size hospitals. The study has not distinguished the number of inpatients based on the resources consumption. In this context, there is a further scope to improve the study. However, this is the first attempt to analyse the performance of private hospitals in terms of technical efficiency in Assam. Introduction: The healthcare sector faces many challenges, including more patients, rising costs, and the need for high-quality care. AI-driven decision support systems (AI-DSS) can enhance hospital management by improving workflows, cutting administrative errors, and better allocating resources to improve patient outcomes. Aim: The aim of this study is to evaluate the impact of AI-DSS on hospital management efficiency, patient outcomes, and operational strategies. It explores the factors influencing successful AI-DSS implementation and examines healthcare professionals' perceptions of these systems in clinical decision-making. The study seeks to provide evidence on the role of AI-DSS in enhancing both operational and patient care outcomes in healthcare settings. Methodology: This survey focused on healthcare professionals from private and government hospitals, including doctors, nurses, administrators, and allied health staff. We collected data through an online questionnaire that covered demographics, AI usage, perceived effects, challenges, and overall satisfaction. We used Likert-scale and multiple-choice questions for quantitative data. Results The study found that only 9.4% of hospitals currently use AI-DSS, showing that adoption is still in its early stages. Respondents noted several benefits, with 59.6% stating that AI helps with clinical decision-making and 63% saying it improves operational efficiency and resource use. However, opinions about AI's impact on diagnostic accuracy were mixed, with 48.8% disagreeing that AI significantly enhances diagnostic processes. Key barriers to adoption included cost (50.7%), staff resistance (40.4%), and lack of training (35%). Despite these issues, many respondents felt that AI boosts staff confidence in clinical decisions and allows for timely interventions, which positively affects patient outcomes. Conclusion: AI-DSS shows great potential to improve hospital management by enhancing decision-making and efficiency. However, to increase adoption, we need to tackle barriers like costs, training issues, and staff resistance. This research was conducted in the healthcare sector to investigate the most important knowledge-sharing considerations among doctors, nurses and other paramedic staff. The role of knowledge sharing for achieving a sustainable competitive edge is recognized worldwide in several contexts and cultures. Hence, identification, collection, creation, and application of knowledge are among organizations' top priorities nowadays. This research is conducted through qualitative research design. 75 semi-structured qualitative interviews were conducted in three case organizations and data was analyzed through thematic analysis. It was revealed that interpersonal trust, credibility and authenticity of the shared knowledge, professional relationship, organizational duty, personality dimensions of the knowledge sharer are some of the key considerations for knowledge sharing within healthcare organizations. The managers and practitioners in healthcare sector can develop their understanding how to promote knowledge sharing in hospitals in the light of this research. Operating room (OR) planning has gradually become an important element of Hospital administration in recent years. The OR performance plays a significant role in enhancing the quality of the care provided to the patients, and reducing patients' length of stay (LOS), which is the function of the OR performance, decreases the costs. Following a survey of the existing research and conducted interviews with clinical and non-clinical OR staff, there are several factors influencing patients’ length of stay (LOS) in OR, and consequently the costs of OR. In this work, first, we implemented the DEMATEL to define the relationship between OR factors, next, employed ANP to identify the most influential factors on the LOS in the OR. The results show the most influential factors are operative time, operative setup, patient waiting time, anesthesia, and patient preparation. Running two rooms concurrently and minimizing turnover time (TOT) reduce patients' LOS by 10% and 5.6%, respectively, and improve the patients' and clinical personnel's satisfaction. Reducing the LOS not only leads to reduced OR costs but also helps increase the number of operations and thereby hospitals' revenue. [ABSTRACT FROM AUTHOR]

Background: Despite the advances in medical science, the epidemic of infectious diseases has faced serious challenges to the health system of countries, so the purpose of this study was to identify obstacles and management strategies to prepare for planning preventive measures and better care in dealing with infectious diseases in hospitals. Method: The current qualitative research was of the grounded theory type, which was conducted in 2023. The semistructured interview questions were obtained from experts. The initial selection of the sample was made from experts in the field of health and treatment from all over the country. The snowball method was used to increase the sample volume until it reached sufficient Value. After analyzing the data through MAXQDA2020 software, this research reached saturation by interviewing 20 experts. Results: Four main areas, 15 subthemes, and 93 codes were identified in the management of epidemics, which included leadership and management (planning, physical structure, information management, financial resources, manpower, medicine and equipment, and internal and external coordination), Stewardship (macro policy and syndromic care system), safety and resilience (crisis management and emergency and disaster risk management), management of infectious diseases (instructions, education, infection prevention and control, treatment management). Conclusion: This study presents the strategies of the health system in dealing with the epidemic of infectious diseases to overcome the obstacles and challenges of preparation and response, which can help health managers in designing future programs, and finally, it shows that hospitals should have a plan for resilience in crises. Background: Objective and key results (OKRs) was a strategic tool to drive the organization to achieve a goal. Goals are concomitant and come from leaders and personnel. Most hospital management studies are conducted in a qualitative approach. Objective: To evaluate the association of knowledge, attitudes, practices, personal factor (PF), and perceived organizational support (POS) of OKRs in quantitative approach. Materials and Methods: The present study was a descriptive study conducted at Thammasat University Hospital (TUH), Pathum Thani, Thailand between March and April 2024. Participants were TUH personnel. Self-filling questionnaires were applied and signed written informed consent after thoroughly counselled. Questionnaires consisted of POS, knowledge, attitudes, and practices questions. PF was also collected. Results: Two hundred and twenty-one participants were recruited. Most participant (192 out of 221) were female. Three-quarters (149 out of 221) of the participants had more than ten years' experience. Eighty-six percent (192 out of 221) of the participants had an educational level equal to or higher than bachelor. Mean knowledge, attitudes, and practices of participants were 9.5±0.6, 3.9±0.4, and 3.7±0.4, respectively. PF was not associated with POS and practices. Hospital personnel with more than 20 years seniority had OKRs' knowledge less than those less than 10 years seniority (b= -0.21, p=0.04). Monthly income and work experience were significantly positive and negative associated with attitudes at b=0.17 and -0.15, respectively. POS was not associated with attitudes and knowledge. Practice was associated with POS, with statistical significance (b=0.20, p<0.001). Conclusion: OKRs knowledge decreased with work experience. Attitude was associated with high monthly income and reverse associated with work experience. POS was significantly associated with the practice. There was no association between PF and practice, PF and POS, POS and attitude, and POS and knowledge. This study provided guidelines for recognizing organizational support in implementing OKRs and improving processes to ensure efficiency and staff involvement. Hospital waste has the potential to affect human health, the environment, communities, animals, and even individuals at home in both infectious and noninfectious ways. That is why efficient and sustainable hospital waste management and disposal are the only keys to reducing waste-related risks. Financially and environmentally viable hospital waste management and disposal activities are the dire need of the present time. The present study aims to identify the factors that should be incorporated when designing any hospital waste management strategy and to find the appropriate technology for hospital waste disposal in Quetta City, Pakistan, by using social, health, technological, economic, financial, institutional, and environmental factors, known collectively as the SHTEFIE criteria. Findings reveal that environmental, technological, and financial factors are dominant over all other factors. Incineration is the best technique among all other waste disposal techniques, but environmental protections must be strictly followed. Landfill is a financially sound technique and suitable for noninfectious and general waste, whereas autoclaving and microwaving are best used as waste treatment methods. [ABSTRACT FROM AUTHOR]

Even though Indonesia has a large number of hospitals and has experienced significant progress in realizing Universal Health Coverage (UHC), service performance is still relatively low, so many patients seek treatment abroad. The performance of services and care systems in hospitals is often associated with low use of digital technology. The complexity of adopting digital technology in a hospital environment is not an easy thing to do, with the majority of users carrying out digital technology functions being health officers and professionals. This research aims to explore the influence of Digital Leadership in encouraging the adoption of digital technology and its influence on Hospital Performance. Survey data conducted on 285 leaders of Class C and D hospitals spread throughout Indonesia, through Structural Equation Modeling (SEM) analysis using Lisrel 8.80, it was found that there was a positive and significant influence on the role of Digital Adoption Capability as a mediation on Hospital Performance. It was found that Digital Leadership has a positive and significant influence on hospital performance both directly and through the mediation of Digital Adoption Capability. Environmental Dynamism factors have also been proven to significantly influence hospital management's desire to improve performance through the adoption of digital technology. The model resulting from this research provides a comprehensive view of hospital management in developing effective digital technology adoption strategies in facing an ever-changing environment. Background: Monitoring antimicrobial use is crucial for understanding current prescribing practices. Such information helps in establishing stewardship programs for effectively using antimicrobials and combating resistance to them. Objectives: This study describes how antimicrobials are prescribed at the Veterinary Teaching Hospital of the University of Pisa and compares how the internal medicine and intensive care units differ in their usage of antimicrobials. Methods: The study involved cats that were presented in the units in 2021 and 2022. Antimicrobial prescriptions were obtained via the hospital's management software (OCIROE). Results: In a population of 1164 non-surgical cats with 397 antimicrobials prescribed, the most prescribed ones were amoxicillin–clavulanic acid in the internal medicine unit and ampicillin in the intensive care unit. Twenty-five percent of all antimicrobials were Highest-Priority Critically Important Antimicrobials or Antimicrobial Advice Ad Hoc Expert Group Category B. The oral route was the main route of administration in the internal medicine unit, while parenteral was the most common route used in the intensive care unit. Most antimicrobials were prescribed to treat pathologic conditions affecting the urinary (39%), gastroenteric (21%), respiratory (13%), and integumentary (12%) systems. A diagnosis, accurate dosage, and the use of species-approved medications were recorded in the antimicrobial prescriptions. However, only 11% of these prescriptions were supported by antimicrobial susceptibility tests. Conclusions: These results suggest room for improvement, particularly in increasing the use of antimicrobial susceptibility testing to ensure more targeted antimicrobial therapy. Given the importance of antimicrobial resistance and the One Health approach, the study also highlights the need to consider the broader impact of antimicrobial use in animals, including the potential contribution to resistance in bacteria that affect both animal and human health. Background: The way the healthcare delivery system is reflected by patient satisfaction. Establishing a health system with better results depends on it. It has been assumed that higher patient satisfaction levels correlate with quality healthcare outcomes. There is little national data to support patient satisfaction with inpatient health services in Ethiopia. In order to estimate the pooled proportion of patient satisfaction and determine the associated factors with inpatient health services at public hospitals, a systematic review and meta-analysis were conducted. Methods: The Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines were followed in the extraction of the data. To get the included studies, the following electronic databases were searched: Pub-Med, Google Scholar, Med-Line, Web of Science, Scopus and Repositories. Software called STATA version 17 was used to analyze statistical data using the random effects model. Forest plots were used to display the pooled results. Results: Of the 1583 records resulted in electronic databases searching, 11 studies with 3,958 participants were included in this systematic review and meta-analysis. The estimated pooled proportion of patient satisfaction with inpatient health services was found to be 57.4% (95% CI: 50.88–64.59, I2 = 95.25%). Assuring privacy for patients (OR = 7.44, 95% CI: 3.63–15.25, I2 = 0.0%), availability of direction signs (2.96, 95% CI: 1.91–4.57, I2 = 0.0%), providing adequate information (OR = 3.27, 95% CI: 1.63–6,58, I2 = 65.60%), history of previous admission (OR = 0.29, 0.18–0.46, I2 = 86.36%) and providing on time treatment (OR = 1.63, 95% CI: 1.21–2.20, I2 = 86.36%) were statistically significant factors associated with patient satisfaction with inpatient health services. Conclusion: The estimated pooled level of patient satisfaction with inpatient health services is low in Ethiopia. A higher level of patient satisfaction with inpatient health treatments was predicted by factors such as privacy assurance, fast services, availability of direction signs, provision of services with adequate information transfer, and no history of previous admission. To improve patient satisfaction, the Ministry of Health and hospital administration must place a strong emphasis on ensuring the provision of high-quality, standard-based inpatient healthcare. [Purpose] Hospital A, located in the Tokai region, which has little experience with disasters, is likely to play a key role in supporting community medical institutions providing disaster medical care in the event of a disaster. Hospital A developed its Business Continuity Plan (BCP), which needs to be revised following changes in hospital conditions, in 2018. The key points of the revision focus on retaining and securing human resources to maintain hospital functions and to concentrate hospital staff in the event of a large-scale disaster. Therefore, it is necessary to investigate an actual situation in which hospital staff would gather in the event of a disaster and consider the human resources management necessary to maintain hospital functions over the long term. [Method] Study participants were all 1,545 employees who worked at Hospital A during December 2023, when the survey was conducted. Employees were classified into regular and non-regular employees, and the types of non-regular employees were part-timers, temporary employees, and re-appointed employees. In addition, the occupations were classified into 12 categories. The survey questionnaires were sent using the electronic medical record questionnaire form because the study participants could use the electronic medical records system in Hospital A. According to the regulations of Hospital A, gathering in case of a large-scale disaster is an individual's voluntary choice; hence, we asked participants about their willingness to assemble, on a seven-point scale, based on which we estimated the number of people who would assemble. We described the estimated numbers by computing time and profession.. [Results] The response rate for this survey was 905 (58.6%). Of the 12 occupations that responded, less than 50% of the respondents were doctors, clerks, and nursing assistants. Among the 1,545 participants, only 746 (48.3%) indicated a willingness to assemble at Hospital A in case of a disaster. Among that group of 746, 708.9 (95.0%) were willing to assemble within one hour. Staff most likely to be present at Hospital A during the disaster were radiology technologists (22.5/34 people, 66.2%) and physicians (133.8/242 people, 55.3%). [Discussion] Our findings revealed a low level of concern about disaster prevention among the staff of Hospital A. This may be due to our questionnaire being in Japanese, which was difficult for non-native Japanese speakers to respond to. Despite conveying the significance of our research to the department heads, individual staff members may not have understood its importance. Moreover, the requirement of assembling within one hour of a disaster may lead to confusion among those gathered. Hence, the revised BCP requires efforts from both perspectives: strategies for intensively assembling human resources as well as dispersing the assembled hospital staff in anticipation of long-term disaster countermeasures.. Conclusion] Our survey was based on the assumption of a disaster emergency under normal circumstances. The results regarding the possibility of hospital staff assembling during a large-scale disaster at Hospital A showed that this could provide a basis for improving the BCP's functionality. Recent years have witnessed significant developments in medical negligence jurisprudence. In 2015, the Supreme Court in Montgomery v Lanarkshire Health Board famously departed from the House of Lords decision in Sidaway v Board of Governors of the Bethlem Royal Hospital and the Maudsley Hospital by ruling that the professional practice test set out in Bolam v Friern Hospital Management Committee no longer applied to the doctor's duty to give advice to the patient. In particular, the Supreme Court in Montgomery held as follows: The doctor is ... under a duty to take reasonable care to ensure that the patient is aware of any material risks involved in any recommended treatment, and of any reasonable alternative or variant treatments. Staffing planning, recruitment, and retention is a primary function of hospital management teams to ensure sufficient health care professionals and ensure the delivery of quality health care to patients. The purpose of the study is to reveal the leading problems related to attracting and retaining nursing staff in hospital structures. Material and methods: A survey was conducted of students from the Master's Program in "Healthcare Management" from the Faculty of Public Health "prof. Dr. Ts. Vodenicharov, MD" of Medical University - Sofia during the academic year 2023/2024. There were 64 participants in the study. The obtained data were processed with a statistical program. The results show that the leading negative factor on the retention of nursing staff in hospital structures is the low salary, as well as the lack of a policy for the development of health care professionals. In order to ensure sufficient nursing staff in medical institutions, it is necessary for hospital managements to implement a policy for the selection and retention of staff. It is necessary to introduce an objective methodology for determining the workload of nursing care and adequate remuneration of nurses, based on their professional qualification and development. The intention to do MCU is a behavior after the patient has evaluated the quality of service, customer value, and patient satisfaction offered by health facilities. Low intention to perform MCU leads to a decrease in the number of patient visits in MCU units and leads to hospital losses. Data shows that the number of MCU patient visits in 2022 has decreased by 29.35% compared to 2021. Objective: To analyze the influence of service quality and customer perceived value on the intention to do MCU mediated by patient satisfaction. Methods: This study is a quantitative research and data collection using questionnaires. The number of samples was 135 respondents. This study uses the SEM method with the LISREL application for data analysis and SPSS for validity and reliability pretest tests. Results: Service quality has a positive and significant effect on customer perceived value. Service quality and customer perceived value have a positive and significant effect on patient satisfaction. Service quality, customer perceived value, and patient satisfaction have a positive and significant effect on the intention to do MCU. Service quality and customer perceived value have a positive and significant effect on the intention to do MCU mediated by patient satisfaction. Implications: This study has positive implications in helping hospital quality evaluation and marketing with the active role of the quality management system and hospital marketing system. In a previous article [1]), our team highlighted the interest and limitations of the Pharma Work Unit (UO) [2] in the accounting reprocessing of hospital pharmacies (PUI). The aim of this study is to highlight the interest and limitations of the UO within the framework of inter-establishment pharmaceutical cooperations on the one hand, and within the framework of the deployment of clinical pharmacy activities (APC) on the other hand. This is a retrospective observational study on data. The data comes from an external audit report from 2019 (unpublished), annual activity reports of the PUIs from 2019 to 2023 [3] , and hospitalization data from 2021 to 2023 [4, 5]. These were analysed using SPSS software (v.29.0.1). In 2019, the data collected during an audit commissioned by hospital management highlighted a budgeted number of beds and places relative to the paramedical staff that was three times higher at the PUI of the Centre hospitalier Fondation Vallée (CHFV) compared to that of the PUI of the Groupe hospitalier Paul Guiraud (GHPG). On the contrary, the activity data revealed an activity 5 to 100 times higher in the GHPG PUI. This resulted in a UO 26 times higher at GHPG and medical and paramedical human resources expenses relative to the UO almost 2 times lower. Between 2021 and 2023, whilst management was common, and the two PUIs initially dispersed over three sites were transformed into two PUIs on two sites. During this period, we observed a decrease in the pharma UO (-4%) associated with a fall in the number of beds (-24%) and a decrease in full-time hospitalization days (-10%). To explain this discrepancy between the evolution of the UO and that of hospitalization, it is necessary to consider all the weighted activities of the PUI on the one hand, but also the deployment of the APC on the other hand. The number of shared activities between the two PUIs is decreasing (global dispensation by reduction of capacity). However, the weighted activities of clinical pharmacy are increasing by 16%, and their share in the UO grew by 19%. This growth results from the redeployment of clinical pharmacists (PC) to care units (US) which were open but were not previously covered, the intensification of PC presence in these US - generating new requests for pharmaceutical interventions - as well as an innovative registration of a clinical pharmacist in ambulatory units. In the case of cooperation, the UO helps to inform on the impact of a reorganization and is useful for deciding on the fusion/closure of a PUI. Finally, regarding the APC, this lever must be clarified in terms of new processes for different activities customized for each and every patient, pathways, and treatments and when coordination is necessary between traditional professions and new professions such as advanced practice nurses or peer helpers. Background: Nephrectomy is the cardinal treatment option for patients with non-functioning kidneys due to malignant and benign causes. Both laparoscopic and open nephrectomy is preferred but the laparoscopy has less complication and improved surgical outcome when compared over open procedure. In this backdrop, the present study was conducted to compare the surgical outcomes and complications of open and laparoscopic nephrectomy procedures. Materials and methods: This was a retrospective study conducted on 50 patients admitted with non-functioning kidneys. They were divided into two groups--laparoscopic nephrectomy (n = 25) and open simple nephrectomy (n = 25). The duration of hospital stay, surgery, postoperative catheter drain removal and surgical complications were analyzed and compared between laparoscopic and open methods. Results: Laparoscopic nephrectomy showed less operative time (90.60 ± 15.99 vs 133.64 ± 10.57 minutes; p = 0.001), shorter hospital stay (3.40 ± 0.12 vs 5.48 ± 0.16 days; p = 0.001) and early postoperative catheter removal (3.20 ± 1.08 vs 2.56 ± 1.04; p = 0.03) when compared with open nephrectomy and it was significant. The complication rate was lower in laparoscopic nephrectomy when compared with simple open nephrectomy (12 vs 36%). Conclusion: Laparoscopic nephrectomy for non-functioning is an effective alternative to open nephrectomy, leading to reduced operative time, quicker recovery and fewer complications as indicated by our findings. Prolonged hospital stays in Nigerian teaching hospitals pose a significant challenge to patient care and hospital management, exacerbated by socio-economic and systemic factors. This case study report looked at the multifaceted role of medical social workers in addressing these challenges, focusing on their efforts in providing psychosocial support, coordinating care, and advocating for patients within a strained healthcare system. This case-study highlights the impact of resource constraints and inadequate hospital practices on patient outcomes, emphasizing the psychological toll on patients and their families. It underscores the critical role of medical social workers as they navigate complex healthcare landscapes to mitigate the adverse effects of extended hospitalizations. This calls for a comprehensive approach to address these systemic issues, including policy reforms, increased healthcare funding, and strategic improvements in hospital administration. We sought to quantify and describe the volume of emergency department visits related to alcohol intoxication at the Veterans Health Administration (VHA), the largest healthcare system in the United States. This is a retrospective cohort study of patients with VHA emergency department visits for alcohol intoxication from 2010 to 2019 as identified via ICD-9/10 code and/or serum ethanol concentration >50 mg/dL. Encounters were identified and demographic and clinical data were acquired by automated query of the VHA Corporate Data Warehouse. Descriptive statistics and univariate analysis were performed. We identified 95,123 patients with a total of 251,310 emergency department visits. The annual number of visits increased over the study period, reaching 32,333 in 2019. Men aged 40–60 were the most common demographic group in the cohort (48% of all patients), and men made up a higher proportion of patients in the database (94%) than the VHA population overall (90%). A disproportionate number of visits (32%) came from the top 4.4% of most frequent visitors. Most of the emergency department visits in the database (68%) were associated with medical or psychiatric admission, or interfacility transfer for admission elsewhere. Patients in the cohort accounted for 1.3% of all VHA emergency department visits during the study period, a proportion that is somewhat smaller than what has been reported at non-VHA facilities, despite the high prevalence of addiction disorders in the VHA patient population. We submit that this lower-than-expected proportion of alcohol-related emergency department visits may be due to the access to primary and mental care which is afforded by VHA patient benefits. • From 2010 to 2019, there were 251,300 Veterans Health Administration emergency department visits for alcohol intoxication. • The annual rate of visits increased during the study period. • The majority of visits (68%) resulted in medical or psychiatric admission. • Visits for intoxication accounted for 1.3% of patients volume in the system, which is lower than other US health systems. Objective: To evaluate the quality and standard of hand-written operative notes in a teaching institute. Methods: This prospective study was carried out in the department of surgery, Fatima Hospital, Baqai Medical University, from January 2023 till May 2023. One hundred fifty operative notes from general surgery domain were considered. These notes were evaluated according to the guidelines of Royal College of Surgeons, with added-on a few variables by the author. Results: All 150 notes were handwritten. Resident surgeon wrote the operative notes under the supervision of primary surgeon. There was a deficiency in mentioning medical record number, procedure starting time and duration of surgery. An important statement about the hemostasis is that it is secured-per-operatively was not documented. The residents were reluctant to explain the surgical procedures diagrammatically. The operative room number was missing in all notes. Post operative instructions lacked the information for nothing per oral, blood pressure, temperature, pulse rate, and input and output charting. Conclusion: It is observed that the operative surgical notes were however explainable about the procedure, but quality and standard was not matchable with that of Royal College of Surgeons notes. Hence, a lack of formal training for the resident surgeons in operative notes writing was observed. This study is a thought provoker to the surgeons and a guide to resident trainees, and hospital management to provide a handful operative notes writing theme in the form of performa provided in the department. Objective: To determine the frequency of new-onset diabetes mellitus (NODM) in patients with COVID-19 in a tertiary care hospital. Method: It was a retrospective descriptive study carried out in Lady Reading Hospital Peshawar, Khyber Pakhtunkhwa province of Pakistan from November 2021 to April 2022. All patients having new onset Diabetes Mellitus (NODM) were identified among a total of 300 patients admitted to the hospital with COVID-19 infection. Patients’ data including relevant investigations were accessed through the hospital management information system (HMIS). SPSS version-23 was used for data entry and statistical analysis. Results: Out of 300 COVID-19 patients included in the study, 163 (54.3%) were female and 137(45.7%) were male. The mean age of the patients was 56.80±13.72 (IQR 15) years. Frequency of the new onset diabetes was 44(14.7%); 19 (6.33%) male and 25(8.33%) female. Among the 44 NODM patients, the majority (57%) were female (p=0.720). Most (64%) of the patients with new-onset DM were in the middle age (p=0.018). Conclusion: A significant number of patients with COVID-19 infection are prone to develop new-onset diabetes during their admission to the hospital. [ABSTRACT FROM AUTHOR]

The global shortage of professional nurses has become a top priority on national agendas worldwide due to the COVID-19 pandemic. Countries such as the United Kingdom, the United States, and many others face a severe shortage of nurses, endangering their ability to recover. In South Africa, the situation is further complicated by the increasing number of nurses migrating to other countries. As a result, public hospitals need to adapt their talent management (TM) practices to compete for professional nurses in the post-pandemic era. However, academic hospitals in the Gauteng Province have not yet fully leveraged TM and work engagement (WE) research. This study examined whether TM practices in public hospitals in Gauteng can influence professional nurses' WE. The research design used in this study was quantitative. Furthermore, standardised questionnaires, the Talent Management Index and the Utrecht Work Engagement Scale-17 were employed to collect data from a random sample of 504 professional nurses from three academic hospitals in Gauteng. The data were analysed using IBM's Statistical Package for Social Sciences (SPSS) software version 25, with Pearson's product-moment correlations. A multiple regression analysis was also conducted to determine whether TM predicts WE. findings reveal that talent management has a positive and significant effect on work engagement. Accordingly, policymakers, nurse managers, and human resources departments should adopt TM practices that improve work engagement. If appropriately applied, TM practices could increase vigour, absorption, and dedication, contributing positively to professional nurses' retention and, thus, assisting in abating their migration to other countries. This study adds to the limited knowledge concerning TM in academic hospitals and highlights the value of TM in achieving vital individual results necessary for public hospitals to remain viable. Since hospitals must accommodate an increasingly diverse population, it is critical that signage be designed to be effective and universally comprehensible. The aim of the study was to examine patients' perceptions of signage systems in a tertiary care teaching hospital, aiming to identify the areas for improvement and enhance overall patient satisfaction. Methods: The cross-sectional study conducted at Government General Hospital, Kakinada from January-March 2024, surveyed 970 patients visiting the outpatient department, using a structured questionnaire. It assessed satisfaction with hospital signage using a Likert scale. Exclusions applied to Psychiatry, Pediatrics, and Emergency departments. Data was analyzed with descriptive and inferential statistics using SPSS version 26.0. Results: Patient responses underscore hospital signage's critical role in effectiveness, reliability, and impact. Wellinformed patients express higher satisfaction, emphasizing signage's informational role. Strategic placement earns high satisfaction, though reliability concerns exist. Wayfinding effectiveness is moderate. Visibility and readability are crucial, with local language inclusion positively perceived, but clarity enhancements are needed for service awareness. Conclusion: This study revealed moderate patient satisfaction with hospital signage, with higher satisfaction among literate patients, young patients and regular visitors. Significant correlations were noted for literacy, regular visitors, and ease of navigation. Improvements in visibility and readability are needed. It structures projects into a format that simplifies planning, management, and monitoring, offering greater visibility and control over the implementation of healthcare initiatives. The authors have developed a scientific and methodological framework for identifying priority projects that align with contemporary healthcare needs. This framework incorporates criteria for assessing population needs, analyzing resources, and prioritizing project funding. Applying these principles enables a more effective selection of projects that address the region’s current requirements and adapt to shifts in the project environment. The carefully structured process for identifying projects essential to the operation and development of hospital districts considers the specific needs of medical infrastructure and the population. This enhances the precision and transparency of management decisions, as the identification process is grounded in a systematic analysis of data and statistical indicators that reflect current challenges and trends in healthcare. The study’s findings highlight the interconnections among components in the priority project identification process, forming the basis for effective project management models at the hospital district level. This approach opens new opportunities for resource optimization, improved quality of medical care, and the development of regional healthcare infrastructure. Within the scope of this research, the fuzzy‐analytical hierarchy process (F‐AHP) method is applied to assess the waste management approaches adopted by various hospitals in the city of Gwalior, India. Fuzzy‐AHP, as an alternative to traditional AHP, accommodates uncertainty and imprecision, offering a decision‐making method that is more adaptable and realistic. The F‐AHP methodology, delineated in a seven‐step process, incorporates pairwise comparisons, fuzzy mathematics, and linguistic variables to compute fuzzy weights for waste management criteria, encompassing physical, biological, social, and economic aspects. Hospital 1, with a weight value of 0.406 and strict adherence to the Bio‐medical Waste Management (Amendment), 2018 rules and regulations of the Government of India, emerges as the most promising sustainability hospital based on the F‐AHP analysis findings. Conversely, Hospital 5 exhibits the poorest waste management practices among the surveyed hospitals, earning the lowest possible ranking. While the proposed F‐AHP method provides valuable insights, the study is limited to a specific geographic area (Gwalior) and potential variations based on environmental factors. Additionally, the practical implications extend beyond the general emphasis on continuous monitoring to include specific actions for hospitals to enhance their waste management systems. Drugs and related goods are widely used in order to promote public health and the quality of life. One of the most serious environmental challenges affecting public health is the ongoing presence of antibiotics in the effluents generated by pharmaceutical industries and hospitals. Antibiotics cannot be entirely removed from wastewater using the traditional wastewater treatment methods. Unmetabolized antibiotics generated by humans can be found in urban and livestock effluent. The antibiotic present in effluent contributes to issues with resistance to antibiotics and the creation of superbugs. Over the recent 2 years, the coronavirus disease 2019 pandemic has substantially boosted hospital waste volume. In this situation, a detailed literature review was conducted to highlight the harmful effects of untreated hospital waste and outline the best approaches to manage it. Approximately 50 to 70% of the emerging contaminants prevalent in the hospital wastewater can be removed using traditional treatment strategies. This paper emphasizes the numerous treatment approaches for effectively eliminating emerging contaminants and antibiotics from hospital wastewater and provides an overview of global hospital wastewater legislation and guidelines on hospital wastewater administration. Around 90% of ECs might be eliminated by biological or physical treatment techniques when used in conjunction with modern oxidation techniques. According to this research, hybrid methods are the best approach for removing antibiotics and ECs from hospital wastewater. The document outlines the many features of effective hospital waste management and might be helpful during and after the coronavirus disease 2019 outbreak, when waste creation on all hospitals throughout the globe has considerably increased. Hospitality has been researched in many settings across domestic, commercial and not-for-profit organizations. The role of volunteers in hospital settings has become more important in recent years. This study investigates how management can foster and maintain hospitable behaviour by host volunteers and what factors facilitate or hinder hospitable behaviour. The case study took place in a public hospital in the northern part of the Netherlands and used academic literature from hospitality, volunteering and organizational (safety) culture fields. Sixteen interviews were conducted with hospital management, volunteer managers and the volunteers themselves. Thematic analysis was used based on themes derived from the literature and emerging from the interviews. Findings show that hospitable behaviour, particularly in welcoming gestures, is often taken for granted, that managers and volunteers each have their own interpretation of hospitality and that there are three different levels and goals for hospitality within the hospital organization: strategic, tactical and operational hospitality. Due to different motives, expectations and interpretations of hospitality at those three levels, 'hospitension' can arise. The systems and structures of the hospital also provide a self-reinforcing culture of (in)hospitality. Healthcare 4.0 (H4.0) is the term corresponding to Industry 4.0 (I4.0). Like other industries, the healthcare industry has also gone through several technological changes and got nourished with them. The health care system of many countries is not mature enough to swiftly transform from its current state to the H4.0 state. Even the matured healthcare systems are facing several challenges while adopting H4.0 practices. The implementation of H4.0 has many challenges (or barriers) to be overcome. This paper has attempted to understand and analyze the challenges that the current Indian hospital management system faces while implementing H4.0. This study uses an integrated approach to the decision-making process to understand the importance of these barriers from the perspective of the Indian health care system. With the help of the Analytical Hierarchy Process (AHP), Interpretative Structural Modeling (ISM) and MICMAC analysis, the paper has mapped the importance of barriers which need to be overcome for to make implementation of H4.0 possible. Background: Traditional procurement and price negotiation methods in public hospitals in Thailand involve group purchasing agreements and bulk orders, posing challenges to improving inventory management efficiency. The vendor-managed inventory (VMI) model is a promising alternative for enhancing hospital performance, especially during crises. This study aimed to assess the potential cost savings in implementing VMI in a large general hospital in Thailand. Methods: A simulation modeling approach was used to compare the current inventory system with three VMI models: VMI1, focused on improving inventory turnover rate (ITR); VMI2, emphasized frequent replenishment with a 1-month supply; and VMI3, eliminated safety stock. Results: The results demonstrated significant cost savings, with potential reductions in total inventory management expenses. Specifically, VMI1 improved ITR from 6.31 to 7.76, reducing average inventory by 36% and cutting management costs by 40%. VMI2, with an ITR of 12.80, reduced inventory by 44% and saved 47% in management costs, while VMI3 achieved a 70% reduction in inventory and a 69% saving in management costs. Conclusions: This study highlights the VMI's transformative potential in hospital inventory management, demonstrating significant cost savings.
