Background: Intra-articular hyaluronate sodium is a relatively new therapy for the treatment of osteoarthritis of the knee. This randomized, double-blind clinical trial was conducted at a large primary care medical center to determine the impact of hyaluronate sodium vs conventional therapy on measures of pain, stiffness, and disability at rest and following functionally relevant walking and stepping activities. Methods: A total of 120 patients (mean age, 67 years) with unilateral grades 1 to 3 medial compartment knee osteoarthritis were randomized to 1 of 4 treatment groups: group 1, 2 mL of hyaluronate sodium at a concentration of 10 mg/mL and placebo (100 mg of lactose); group 2, nonsteroidal anti-inflammatory drugs (NSAIDs) (75 mg of diclofenac and 200 [micro]g of misoprostol) and hyaluronate sodium; group 3, NSAIDs and placebo (2 mL of isotonic sodium chloride solution [saline]); and group 4, placebo (lactose and saline). Intra-articular hyaluronate sodium or saline (2 mL) was administered once weekly over 3 weeks while NSAIDs or lactose were administered twice daily over 12 weeks. Main Outcome Measures: (1) Western Ontario McMaster Universities Index (WOMAC) global measure of pain, stiffness, and disability; (2) visual analog scale-WAS) scores for pain at rest and following functional walking and stepping activities (self-paced walking and stepping); and (3) functional performance (exercise time, heart rate, and predicted maximum oxygen uptake) at baseline and weeks 4 and 12. Results: At week 4, significant improvement in WOMAC scores for pain and disability and VAS score for resting pain was observed in groups 1 to 3 compared with baseline measures. Groups i and 2 showed significantly lower self-paced stepping pain, while no change was observed in group 4. At week 12, groups 1 to 3 showed significantly greater improvement in WOMAC pain subscale score and VAS score for resting pain; however, these differences did not vary from week 4. Following self-paced walking and stepping, groups 1 and 2 reported significantly less activity pain, while group 1 showed significantly faster self-paced walking and stepping test results. Groups 1 to 3 improved self-paced walking and stepping time at week 12 compared with baseline measures, while predicted maximum oxygen uptake was significantly higher in the hyaluronate sodium groups 1 and 2 at weeks 4 and 12 compared with baseline measures. Conclusions: For resting pain relief, hyaluronate sodium seems to be as effective as NSAIDs. Further, for pain with physical activity and functional performance, hyaluronate sodium may be superior to placebo alone or NSAIDs alone. Arch Intern Med. 2002;162:292-298

Hyaluronan injections into painful osteoarthritic joints may reduce pain and improve joint function, but clinical research studies have produced equivocal results. Hyaluronan is a high-molecular-weight polysaccharide that lubricates the joint, reduces inflammation, and induces other therapeutic changes when injected into the synovial fluid of joints. Only a few of the many research studies of hyaluronan injection have properly controlled for placebo effects and research biases. Nevertheless, hyaluronan may be beneficial in some patients for whom surgery or corticosteroid injection is not possible or is ineffective.

Older patients with more advanced knee osteoarthritis (OA) seem to benefit the most from hyaluronan injections into the knee joint. Doctors gave five weekly injections of either hyaluronan or placebo into the knee joints of 240 patients with knee OA. Overall, both placebo and hyaluronan treatment groups showed improvement in pain and function scores. The only subgroup of patients that showed more improvement with hyaluronan rather than placebo injections was those over 60 years with advanced disease. There were no serious side effects reported with either treatment.

The adult wound healing process is characterized by the formation of fibrotic scar tissue and sometimes contracture of tissue around the wound. This process affects all types of surgery and may have serious consequences. However, wound healing in a fetus does not, for the most part, result in fibrosis or scar formation. The transition from fetal to adult type of wound healing occurs in the middle of last trimester of pregnancy. Fetal wound healing is brought about partly through an extracellular matrix rich in hyaluronic acid, a substance found whenever there is rapid cell development and movement. It is hypothesized that a wound extracellular matrix rich in hyaluronic acid provides an environment in which a wound can heal without scar tissue forming. A study was undertaken to evaluate the levels of hyaluronic acid in fetal and adult wound fluid. Adult wounds show a rapid increase in hyaluronic acid, which peaked by day three and was 0 by day seven. In the fetal wound, the hyaluronic acid level also increased rapidly, but remained elevated for three weeks. This long period of exposure to hyaluronic acid promotes cell movement and inhibits cell differentiation; healing occurs by regeneration of tissue, rather than by scarring. This finding may have major clinical implications. It may be possible that wound healing in adults and children can be induced into a regeneration process, similar to that which takes in the fetus, by prolonged exposure to hyaluronic acid. Preliminary findings that injection of hyaluronate, or hyaluronic acid, into the arthritic joints improves joint function have led to the idea that hyaluronate may become therapeutically useful. Hyaluronate is composed of sugar molecules, which can be strung together in various lengths, forming a complex carbohydrate that increases the viscosity of fluids. Hyaluronate is the major macromolecular (consisting of a large molecule, such as proteins) component of synovial fluid, which surrounds and lubricates joint surfaces, and it is also found within the cartilage that makes up the joint surfaces. In rheumatoid arthritis, there is a decrease in the production, amount, and size of hyaluronate. With increasing age, hyaluronate levels can increase five-fold, but these hyaluronates consist of smaller molecules. High molecular weight hyaluronate inhibits cellular proliferation, which may be important in suppressing inflammation in joints. Hyaluronate can decrease joint pain and appears to be anti-inflammatory in cases of moderate arthritis, with possible long-term benefits. However, no improvement in overall function was noted in one study. Notably, no serious side effects are associated with hyaluronate therapy. Proposed uses for hyaluronate include lubrication of joints during joint surgery, and treatment following surgical repair of joints, nerves, and cartilage, and following trauma or tissue removal.
