110. Added value of CT colonography after a positive conventional colonoscopy: impact on treatment strategy.

Abstract

Background: Conventional colonoscopy (CC) requires an experienced operator to avoid technical or interpretative errors, and an endoscopic error rate of 14% for tumor localization has been reported. We evaluated the impact of computed tomographic colonography (CTC) on surgical treatment strategy in patients with CC reported as having colorectal neoplasm. Methods: Fifty-three patients testing positive for colorectal neoplasm on CC underwent CTC: 32 patients had CC in our hospital (group A) and 21 had CC in area hospitals (group B). All CTC procedures were performed with a multidetector CT system. The results of CTC and CC were compared with that of surgery. The preoperative surgical planning evaluated on the basis of CC and CTC was compared with the actual surgical approach, and the percentage of patients in whom CTC modified the treatment strategy suggested by CC was calculated. Results: CTC changed the treatment strategy in four of 53 patients (7.5%) in whom CC showed technical or interpretative errors. Group analysis showed that CTC did not influence the surgical management in any patient in group A but did affect treatment strategy in four of 21 patients (19%) in group B. The effect of CTC on treatment strategy between groups was statistically significant (p < 0.05). CTC identified five adenomas and three adenocarcinomas localized proximally to an impassable stenosis. Conclusion: CTC can be used to reevaluate the findings of a positive CC and can indicate a more correct therapeutic approach in patients with colorectal neoplasms who are candidates for surgery. Abstract

A case of granulocytic sarcoma (GS) of the breast in a patient with acute myeloblastic leukemia whose disease was in remission is reported. Rarely hematological tumors such as lymphoma or leukemia affect the breast as primary malignancies, and more frequently involve the breast as part of a widespread multicentric process. GS also called chloroma is a rare manifestation of acute myeloblastic leukemia, mainly composed of premature precursors of the granulocytic series. Our patient presented with swelling in both breasts and with a previous history of acute myeloblastic leukemia diagnosed 6 months before on the basis of a bone marrow biopsy that revealed 85% immature myeloblasts. After induction chemotherapy, a second bone marrow biopsy showed evidence of disease remission with less than 5% blast cells. On ultrasound two inhomogeneous hypoechoic nodules, with irregular margins and without posterior acoustic shadowing, located, respectively, in the inferior inner quadrant of the right breast and in the upper outer quadrant of the left breast were observed. A subsequent mammogram showed heterogeneously dense breasts without evidence of microcalcifications. A fine needle aspiration biopsy was performed and the cytologic results highlighted the presence of immature myeloid cells, mingled with groups of benign ductal cells. On the basis of the negativity of a subsequent bone marrow biopsy, a final diagnosis of mammary infiltration from acute myeloblastic leukemia was formulated. Due to the lack of specificity of mammographic and sonographic features of breast chloroma definite diagnosis of this rare entity must be accomplished by fine-needle aspiration and cytologic markers, especially for accurate differential diagnosis from other breast malignancies, such as lymphoma or carcinoma. The case presented is unusual because of the multicentric location of the GS and because of the onset in a patient considered to be in remission by bone marrow biopsy. [Copyright &y& Elsevier]

Abstract

Multiple malignant esophageal tumors of the same cell type are described. In the esophageal mucosa, widespread carcinomatous transformation may be observed and multicentric invasive squamous cell carcinomas may develop. The concomitance of two independent esophageal malignant neoplasms of different epithelial histogenesis is uncommon. Synchronous adenocarcinoma and squamous cell carcinoma of the esophagus is reported. Adenosquamous carcinoma of the esophagus is a rare tumor. Adenocarcinoma of the esophagus represents 10% of esophageal cancer. We report a case of a synchronous primary invasive adenosquamous carcinoma and adenocarcinoma of the esophagus. Both tumors were demonstrated radiographically. The peculiarity of this neoplastic association and the importance of complete radiographic esophageal evaluation in patients with one obvious obstructing tumor of the esophagus are emphasized. Abstract

Background: We compared rest-redistribution thallium 201 and resting technetium 99m methoxyisobutyl isonitrile (MIBI) cardiac imaging in 29 men with angiographically proven coronary artery disease and regional ventricular dysfunction. Left ventricular ejection fraction at radionuclide angiography was 35% +/- 9%.Methods and Results: Regional left ventricular wall motion was assessed on gated 99mTc MIBI images according to a 3-point scale (0 = normal, 1 = hypokinetic, 2 = akinetic or dyskinetic). 201Tl and 99mTc MIBI uptake values were analyzed quantitatively. A total of 435 myocardial segments were classified on the basis of wall motion analysis into three groups: group 1 (normal wall motion; n = 201), group 2 (hypokinetic; n = 132), and group 3 (akinetic or dyskinetic; n = 102). 201Tl and 99mTc MIBI uptake values were significantly higher in groups 1 and 2 compared with group 3 (p < 0.05) and in group 1 compared with group 2 (p < 0.05). When 201Tl and 99mTc MIBI uptake values were directly compared, no significant differences in group 1 and 2 were observed. In group 3, 99mTc MIBI uptake (67% +/- 14%) was significantly lower (p < 0.001) than initial (72% +/- 11%) and delayed 201Tl uptake (73% +/- 12%).Conclusion: Thus rest-redistribution 201Tl and resting 99mTc MIBI cardiac imaging reflect the severity of left ventricular dysfunction in coronary artery disease. Abstract

Ambulatory monitoring (VEST) of left ventricular (LV) function is a useful and accurate method to measure cardiac function during exercise and rest. The aim of this study was to evaluate LV response to exercise in normal sedentary subjects.Ten normal sedentary subjects underwent continuous ambulatory monitoring of LV function by VEST during upright bicycle exercise associated with combined analysis of pulmonary gas exchange. All parameters of LV function were measured in control conditions at rest, at the anaerobic threshold (point of nonlinear increase in ventilation relative to oxygen uptake) and at peak oxygen uptake (peak VO2).Heart rate and cardiac output significantly increased from control conditions to anaerobic threshold (p0.001) and from anaerobic threshold to peak VO2 (p0.001). Ejection fraction, end diastolic volume and stroke volume significantly increased from control conditions to anaerobic threshold (p0.001), showing no significant change from anaerobic threshold to peak VO2. Finally, end-systolic volume significantly decreased from control conditions to anaerobic threshold (p0.001), showing no significant change from anaerobic threshold to peak VO2.VEST is particularly useful in the evaluation of cardiac response to exercise in normal sedentary subjects, providing a better understanding of the spectrum of the normal LVEF response to exercise. Our data demonstrate that ejection fraction response to exercise is variable after anaerobic threshold, and a uniform increase is not necessarily expected in normal sedentary subjects.

Abstract

Ambulatory monitoring (VEST) of left ventricular (LV) function has been shown to be accurate and repeatable. The aim of this study was to assess VEST repeatability in the evaluation of haemodynamic responses to different cardiac stimulations. Twelve patients (all men, mean age 59 +/- 8 years) underwent two separate VEST studies 5-day apart. In both studies, LV function was continuously monitored at rest and during different cardiac stimulations: 1) changing position from supine to upright (tilt); 2) sustained isometric exercise (handgrip); and 3) sublingual administration of 5 mg of nitroglycerin (NTG). Changes from baseline to peak response (delta) in the heart rate, ejection fraction, end-diastolic volume, end-systolic volume, stroke volume and cardiac output were evaluated. Reproducibility was assessed by computing, the coefficient of repeatability. In addition, the differences between the delta values obtained with the two VEST studies were plotted against their mean. This plot allowed us to detect whether there was any relationship between the error in the measurements and the best estimate of the "true value". No statistically significant differences between the two VEST studies were found in the haemodynamic changes in heart rate, ejection fraction, end-diastolic volume, end-systolic volume, stroke volume and cardiac output induced by handgrip, tilt and NTG. For each parameter the mean difference was not significantly different from zero and no significant relationship between the error and the "true value" was observed. Our results demonstrate that VEST has a good repeatability in measuring cardiac haemodynamic responses to different types of stimulations.

Abstract

We studied 25 male patients, with coronary artery disease, mean age 56 +/- 8 yr. All underwent 201Tl rest-redistribution and resting 99mTc methoxyisobutyl isonitrile (MIBI) cardiac imaging. Regional 201Tl and MIBI uptake were quantitatively analyzed. Regional left ventricular wall motion (WM) was visually assessed on MIBI gated images using a three-point scale (0 = normal, 1 = hypokinetic, 2 = a/dyskinetic). Two patterns of reverse redistribution (RR) were identified: RR-A when 201Tl uptake was normal on rest images and abnormal on redistribution images, and RR-B when 201Tl uptake was abnormal on rest images and a significant decrease in uptake was observed on redistribution images. Of the total 375 myocardial segments analyzed, 229 were classified as normal (Nl), 40 as reversible defect (RD), 74 as irreversible defect (ID); 26 showed RR-A while 6 myocardial segments had RR-B. Myocardial segments with RR-A differed from NI in the degree of coronary artery stenosis (81% +/- 33% versus 57% +/- 39%, respectively, p0.05), in WM score (1.1 +/- 0.7 versus 0.5 +/- 0.6, respectively, p0.01), and in MIBI uptake (81% +/- 10% versus 92% +/- 9%, respectively, p0.0001). Moreover, the percent of myocardial segments supplied by a totally occluded coronary artery was significantly higher (p0.05) in myocardial segments with RR-A (46%) than in NI (22%). Segments with RR-B did not show any significant difference either from RD and ID. These results suggest that myocardial segments with RR-A on resting 201Tl images have impaired function and are supplied by severely stenosed coronary arteries and should not be considered normal.

Case report We present a patient with a true pancreatic cyst with internal septation in an adult female. Abstract

Context Caroli’s disease is a rare congenital disorder first described by Caroli in 1958. This abnormality consists of non-obstructive, saccular or fusiform dilation of the intrahepatic bile ducts resulting in cystic lesions; similar abnormalities may also occur in the kidneys and pancreas. Case report We illustrate the role of enhanced mangafodipir trisodium magnetic resonance imaging in a patient with sporadic non-hereditary Caroli’s disease associated with pancreatic involvement in which mangafodipir trisodium magnetic resonance imaging characterized part of the cystic liver lesions as saccular dilations of the intrahepatic bile ducts of the left lobe, allowing diagnosis of the disease. Conclusion We strongly recommend hepatobiliary magnetic resonance imaging with mangafodipir trisodium in such patients. Image: Hydrographic imaging., JOP. Abstract

Myocardial perfusion imaging (MPI) is important for the management of patients with suspected or known coronary artery disease (CAD). Nuclear cardiology is the most widely used noninvasive approach for the assessment of myocardial perfusion. The available single-photon emission computed tomography (SPECT) flow agents are characterized by a rapid myocardial extraction and by a cardiac uptake proportional to blood flow. In addition, different positron emission tomography (PET) tracers may be used for the quantitative measurement of myocardial blood flow and coronary flow reserve. The decrease in blood flow, determined by coronary artery stenosis, produces myocardial ischemia leading to perfusion abnormalities detectable by SPECT or PET in the early phase of ischemia. Other imaging techniques, such as contrast echocardiography and magnetic resonance imaging (MRI) have been more recently proposed as alternative methods for the evaluation of myocardial perfusion. Although several technical aspects have to be better defined to use contrast echocardiography in clinical practice, this approach appears promising for the evaluation of myocardial perfusion. MRI has also been proposed for the assessment of myocardial perfusion by measuring the alteration of regional myocardial magnetic properties after the intravenous injection of contrast agents. Due to the high contrast and spatial resolution of the technique, MRI allows differentiating sub-endocardial and sub-epicardial perfusion, emerging as a potential alternative non-ionizing technique to evaluate myocardial perfusion. This review illustrates the noninvasive imaging modalities for the evaluation of myocardial perfusion, underlying advantages and disadvantages of each technique.

Abstract

Context Thanks to the wide use of diagnostic imaging modalities, multiple primary malignancies are being diagnosed more frequently and different associations of malignancies have been reported in this setting. Case report In this paper, we describe the case of a patient with non-functioning well-differentiated neuroendocrine carcinoma of the head of the pancreas associated with extra-hepatic cholangiocarcinoma, in which an early diagnosis using magnetic resonance imaging allowed a good outcome. Conclusion The simultaneous association of neuroendocrine pancreatic tumors and cholangiocarcinoma has not yet been described; however, this association should be considered and, due to the high contrast of magnetic resonance imaging, this technique is recommended in such patient in order to reach an accurate diagnosis. Abstract

Crohn's disease (CD) frequently involves the small bowel. Actually, the diagnosis of CD is made by ileocolonoscopy (IC) and small bowel enteroclysis (SBE), while trans-abdominal bowel sonography (BS) and Tc-99m-HMPAO leukocyte scintigraphy (LS) are only partially used in spite of their minimal invasiveness.to compare the diagnostic accuracy of these procedures for the diagnosis of small bowel CD.in about two years a series of consecutive subjects underwent IC, SBE, BS and LS for either suspected or known small bowel CD.Sensitivity, specificity, positive and negative predictive value for CD diagnosis of the studied procedures were respectively: 98%, 97%, 98% and 97% for SBE; 92%, 97%, 98%, and 88% for BS; 90%, 93%, 96% and 85% for LS. In addition, the parallel combined use of BS and LS led to overall sensitivity, specificity, positive and negative predictive value of 100%, 93%, 96%, 100%, respectively.SBE, BS and LS are accurate procedures for the diagnosis of small bowel Crohn's disease. The use of BS and LS, particularly in combination, can be proposed as early diagnostic approach to subjects with a suspicion of disease.

Abstract

Introduction: The relationship between the appropriateness of the transthoracic echocardiography (TTE) and its clinical impact is still a matter of debate. Objective: The aim of this study was to assess the degree of adherence to the appropriate use criteria for echocardiography, in a tertiary public hospital in the United Kingdom, as well as the clinical impact of the exam on patient management. Methods: 859 TTE’s performed consecutively during January 2014 were reviewed to assess its appropriateness, and were classified as appropriate, uncertain or inappropriate using the 2011 guidelines. Subsequently, patient’s files were examined to determine the clinical impact of the TTE which was assigned to one of the following three categories: (1) active change in care, (2) continuation of current care, or (3) no change in care. Patients which files were not available were excluded (259). All classifications were evaluated by two independent cardiologists, with no direct relation to the study. Results: Our sample had a mean age of 63 ± 17 years with a gender balance. The majority of the exams were requested at the outpatient (81.4%) clinic, by cardiologists (50.3%) and general practitioners (13.4%). Regarding the main findings, in 7.6% of the studies there were moderate to severe systolic dysfunction; 4.0% showed severe valvular heart disease and 5.1% had significant pulmonary hypertension. Relatively to the appropriateness of the TTE requests, 76.5% were considered appropriate, 7.1% inappropriate and 12.6% uncertain. With respect to the clinical impact of the TTE’s, 42.7% of the exams led to an active change in care, 15.6% to a continuation of the care and 11.5% revealed no change in care. Age (β0.90, P=0.05) and outpatient setting (β4.4, P<0.01) were the most important predictors of an active change of care exam. On the contrary, the appropriateness of the TTE’s requests (β1.1, P=0.56) and the specialist ordering the exams (β0.81, P=0.26) were not independently associated. Conclusion: Our data showed that almost 8 out of 10 TTE were considered appropriate, and 4 out of 10 exams had an active clinical impact.
