The general bone anabolic effect of photobiomodulation (PBM) is largely accepted. As a result, PBM therapy is expected to be beneficial in the medical fields of dentistry and bone healing. However, most of the previous in vitro studies on PBM and bone metabolism were performed with single-cell cultures of osteoclast-lineage cells or osteoblast-lineage cells. In the present study, the bone-modulating effects of PBM were evaluated in an in vitro osteoblast/osteoclast co-culture system. Mouse bone marrow-derived macrophages (BMMs) and mouse calvarial pre-osteoblasts cells were purified and used as precursor cells for osteoclasts and osteoblasts, respectively. The PBM effects on single-cell culture of osteoclasts or osteoblasts as well as co-culture were examined by 1.2 J/cm2 low-level Ga-Al-As laser (λ = 808 ± 3 nm, 80 mW, and 80 mA; spot size, 1cm2; NDLux, Seoul, Korea) irradiation for 30 s at daily intervals throughout culture period. At the end of culture, the osteoclast differentiation and osteoblast differentiation were assessed by TRAP staining and ALP staining, respectively. The expressions of osteoclastogenic cytokines were evaluated by RT-PCR and Western blot analyses. Under the single-cell culture condition, PBM enhanced osteoblast differentiation but had minor effects on osteoclast differentiation. However, in the co-culture condition, its osteoblastogenic effect was maintained, and osteoclast differentiation was substantially reduced. Subsequent RT-PCR analyses and western blot results revealed marked reduction in receptor activator of NF-κB ligand (RANKL) expression and elevation in osteoprotegerin (OPG) expression by PBM in co-cultured cells. More importantly, these alterations in RANKL/OPG levels were not observed under the single-cell culture conditions. Our results highlight the different effects of PBM on bone cells based on culture conditions. Further, our findings suggest the indirect anti-osteoclastogenic effect of PBM, which is accompanied by a decrease in RANKL expression and an increase in OPG expression.

There is a great demand for a dispersion evaluation method that can be applied even when a nano-sized filler is incorporated, that can evaluate a three-dimensional (3D) quantitative dispersion of a composite material, and that is not expensive and time consuming. Here, we proposed light-intensity dispersion index (LDI) to quantitatively evaluate the dispersion based on the light scattering distribution on divided optical microscopy (OM) images. Difference in the light-intensity is determined by the dispersion and agglomeration of the filler in the polymer matrix; therefore, the smaller the distribution and the number of peaks of light-intensity, the more uniform the dispersion. The reliability of the developed LDI was improved by introducing an agglomeration size weighting factor. Furthermore, the validity of the LDI was also verified in comparison with the experimental results. In conclusion, we have successfully developed a method for the 3D quantitative dispersion evaluation of nanocomposites with minimal cost and time consumption.

Background: Recent studies have reported high rates of recurrence of shoulder instability in patients with glenoid bone defects greater than 20% after capsulolabral reconstruction. The purpose of the present study was to evaluate the failure rate of arthroscopic capsulolabral reconstruction for the treatment of anterior instability in the presence of glenoid bone deficits >20%. Methods: Retrospective analyses were conducted among cases with anterior shoulder instability and glenoid bone defects of >20% that were treated by arthroscopic capsulolabral reconstruction with a minimum 2-year follow-up (30 cases). We included the following variables: age, bone defect size, instability severity index score (ISIS), on-/off-track assessment, incidence recurrent instability, and return to sports. Results: The mean glenoid bone defect size was 25.8% ± 4.2% (range, 20.4%–37.2%), and 18 cases (60%) had defects of >25%. Bony Bankart lesions were identified in 11 cases (36.7%). Eleven cases (36.7%) had ISIS scores >6 points and 21 cases (70%) had off-track lesions. No cases of recurrent instability were identified over a mean follow-up of 39.9 months (range, 24–86 months), but a sense of subluxation was reported by three patients. Return to sports at the preinjury level was possible in 24 cases (80%), and the average satisfaction rating was 92%. Conclusions: Arthroscopic soft tissue reconstruction was successful for treating anterior shoulder instability among patients with glenoid bone defects >20%, even enabling return to sports. Future studies should focus on determining the range of bone defect sizes that can be successfully managed by soft tissue repair.

Cell sheet engineering has attracted great attention because thin layers of tissue can be easily transplanted to defect sites. Wound-dressing materials are required to support fast re-epithelization, both with keratinocytes and fibroblasts, to enhance the prognosis and therapeutic outcomes. We prepared self-assembled cell sheets composed of adipocyte-derived stem cells (ADSCs) and surface-engineered nanofibrils (NFs). NFs were surface-engineered with multilayers of gelatin so that the cell sheets could spontaneously assemble within 3 days in cell culture plates. Dorsal wounds transplanted with the cell sheets exhibited higher wound-healing rates when a high concentration of gelatin was immobilized on the surfaces of the NFs. Histochemical staining revealed that those with gelatin-immobilized NFs showed a higher expression of cytokeratin and collagen in the re-epithelized epidermis. Keratinocytic differentiation of the epidermis was molecularly evidenced by the higher expression of keratinocyte-specific genes.

There is a growing demand for the development of a melt process-based nanocomposite manufacturing process capable of minimizing the incorporation of expensive carbon nanotubes (CNTs) by inducing a uniform dispersion of multi-walled CNTs (MWNTs). In this study, we proposed a nanocomposite manufacturing process that includes both physical particle mixing using high-speed rotation and anionic polymerization of e-caprolactam (CL) to induce a uniform dispersion of MWNTs. MWNTs were uniformly dispersed by powder mixing and then wrapped with polyamide 6 polymers by in-situ polymerization of CL. The synergistic effect of the two sub-processes resulted in an enhanced dispersion of MWNTs and prevented aggregation of the MWNTs. The composites fabricated by the proposed process exhibited electrical conductivities of 4.49 × 10−5 S/m and 1.45 × 10 S/m at 1 wt% and 3 wt% MWNTs, respectively, indicating that by applying the proposed process it is possible to incorporate a smaller amount of MWNT to achieve electrical conductivities applicable for electrostatic discharge (ESD) (>1 X 10−5 S/m) and electromagnetic interference shielding effectiveness (EMI SE) (>10 S/m) applications. An ESD chip tray and EMI SE mobile phone case were fabricated using the prepared composite, and it was verified that the ESD and EMI SE performances can be realized.

The optimal management for massive rotator cuff tears (MRCTs) without osteoarthritis (OA) remains controversial. The purpose of this study was to evaluate and compare the clinical outcomes of reverse shoulder arthroplasty (RSA) vs. rotator cuff repair (RCR) in patients with MRCTs without OA.We conducted a retrospective data analysis of 68 patients treated for MRCTs via RSA and 215 patients treated for MRCTs via arthroscopic RCR between January 2014 and April 2019. Through propensity score matching, patients were matched for sex, age, tear size, and global fatty degeneration index. Thirty-nine patients in each group were included, and all patients had completed minimal 2-year follow-up. Postoperative radiologic evaluations of healing failure were performed. The visual analog scale score for pain, range of motion, and functional outcome measures including the American Shoulder and Elbow Surgeons score, QuickDASH (short version of Disabilities of the Arm, Shoulder and Hand questionnaire) score, Simple Shoulder Test score, and Constant score were assessed and compared between the 2 groups.Healing failure was observed in 10 patients in the RCR group (32.3%). No statistically significant differences in preoperative range of motion and functional scores were found between the RCR and RSA groups. Both groups showed significant pain relief and functional improvement at final follow-up. Patients in the RCR group showed significantly greater forward elevation (156.4° ± 23.8° vs. 139.7° ± 22.4°, P .001) and internal rotation (thoracic vertebra, T8.2 ± 1.2 vs. T9.7 ± 2.6, P = .001). The average American Shoulder and Elbow Surgeons score was 88.3 ± 12.2 in the RCR group and 81.8 ± 16.1 in the RSA group (P = .045). The QuickDASH score was significantly higher in the RCR group (P = .019). A significantly higher Simple Shoulder Test score (9.7 ± 2.8 vs. 8.1 ± 3.0, P = .01) and a significantly higher Constant score (67.2 ± 6.5 vs. 63.2 ± 7.1, P = .011) were seen in the RCR group.Both RSA and RCR are effective and reliable treatment options for MRCTs without OA. However, the RCR group showed better shoulder function improvement than the RSA group. Considering the possible implications of RSA, RCR should be referred to as a first-line treatment option for patients with MRCTs without OA with proper indications.

Techniques for optimizing the dispersion of carbon nanotubes in composites by applying small amounts of additives in a simple manner are recognized as one of the most important core technologies in nanocomposite engineering. In this study, we reported the feasibility of phenyl glycidyl ether (PGE) as an effective noncovalent functionalization agent and suggested a facile and low-temperature process that applied high-speed rotation based on a planetary centrifugal mixer to improve the dispersion of multiwalled carbon nanotube (MWCNT) in the polyamide 6 (PA6) composite. PGE molecules were introduced on MWCNTs based on the π-π* interaction, resulting in enhanced MWCNT dispersion by preventing of the aggregation between the MWCNTs due to the van der Waals force. The tensile strength of the PA6 composite fiber filled with 1 wt% MWCNTs was improved up to 31.8% by the PGE treatment, indicating that PGE molecules were an effective noncovalent functionalization agent.
