Abstract

Amaç: Kalça veya diz osteoartritli (OA) hastaların klinik parametreleri ile radyolojik evrelendirmesi arasındaki ilişkiyi değerlendirmek ve ayrıca, semptomatik ve asemptomatik tarafların radyolojik evreleri arasındaki ilişkiyi belirlemek amaçlandı.Metod: Amerikan Romatoloji Derneği (ACR) kriterlerine göre kalça (n=75) veya diz (n=100) OA tanısı konulan toplam 175 hasta çalışmaya dâhil edildi. Fonksiyonel yetersizlik ve disabilitenin tespitinde Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) kullanıldı. Hissedilen ağrı, WOMAC' skalasındaki ağrı ile ilgili alt skala ve görsel analog skala (VAS) kullanılarak ölçüldü. Psikolojik durum, hastane anksiyete depresyon (HAD) ölçeği ile ve yaşam kalitesi Nottingham sağlık profili (NHP) ile değerlendirildi. Radyolojik evrelendirme ise Kellgren–lawrence (K/L) indeksi kullanılarak yapıldı.Bulgular: Kalça OA' da WOMAC ağrı, fonksiyon ve toplam skoru, NHP fiziksel aktivite skoru, VAS, hastanın/hekimin global değerlendirmesi skorları radyolojik evre ile ilişkili olarak artmıştı (sırasıyla, p=0.002, 0.002, 0.003, 0.017, 0.002 ve 0.0001). Diz OA' da ise WOMAC ağrı, fonksiyon skoru, NHP fiziksel aktivite skoru, VAS, hastanın/hekimin global değerlendirmesi skorları radyolojik evre ile ilişkili olarak artmıştı (sırasıyla, p=0.002, 0.007, 0.002, 0.0001 ve 0.0001). Abstract

Ultrasonografi Eşliğinde Yapılan Karpal Tünel Sendromu Enjeksiyonlarında Görsel Bilgilendirmenin Anksiyete ve Klinik Parametreler Üzerine EtkisiAmaç: Karpal tünele yönelik lokal enjeksiyonlarda ultrasonografi (USG) eşliğinde yapılan görsel bilgilendirmenin anksiyete ve klinik parametreler üzerine etkisini incelemekMetod: 100 idyopatik karpal tünel sendromu (KTS) hastası çalışmaya dahil edildi. Hastalardan onam alınarak karpal tünel içine lokal steroid enjeksiyonu yapıldı. Steroid preparatı olarak 2 mg betametazon sodyum fosfat ve 5 mg betametazon di-propionat kombinasyonunu içeren preparat kullanıldı. Hastalar randomize olarak poliklinik sırasına göre iki gruba ayrıldı. USG eşliğinde enjeksiyon ve eş zamanlı USG üzerinden görsel bilgilendirme yapılanlar grup 1, her hangi bir bilgilendirme yapılmaksızın körleme teknikle enjeksiyon yapılanlar grup 2 olarak belirlendi. Hastalar enjeksiyondan 1 ay sonra kontrole çağrıldı. Çalışmayı 93 hasta tamamladı (79 kadın, 14 erkek). Hastaların enjeksiyon işleminden duydukları memnuniyet düzeyi 'kötü', 'orta', 'iyi' ve 'mükemmel' olarak derecelendirildi. Enjeksiyon öncesi ve hemen sonrasında anksiyete, enjeksiyon öncesi ve 1 ay sonrasında ağrı ve elin sıkma gücü değerlendirildi. Anksiyete değerlendirmesi için Durumluk ve Süreğen Anksiyete İnvanteri (STAİ: State and Trait Anxiety İnventory) kullanıldı. Ağrı değerlendirmesi için Görsel Ağrı Skalası (VAS) kullanıldı. Elin izometrik kavrama gücü el dinamometresi ile değerlendirildi. Bulgular: Her iki grupta enjeksiyon sonrası ağrı, anksiyete ve el fonksiyon parametrelerinde istatistiksel olarak anlamlı düzelme oldu. VAS skorları her iki grupta da anlamlı olarak azaldı (p0.05) VAS skorlarında grup 1'de grup 2'ye göre daha fazla azalma saptandı. STAI skorları her iki grupta anlamlı olarak azaldı (p0.05). Dinamometre ile ölçülen elin izometrik kavrama gücü her iki grupta anlamlı olarak arttı (p0.05).Tartışma: Çalışmamızın sonuçlarına göre KTS enjeksiyonlarında USG kullanımı ve eş zamanlı USG üzerinden görsel bilgilendirme yapılmasının anksiyete ve el fonksiyonu açısından körleme tekniğe kıyasla üstünlüğü saptanmadı ancak VAS skorlarındaki azalma diğer gruba göre daha fazlaydı. Bu sonuç lokal uygulamalarda USG kullanımının enjeksiyonun etkinliğini arttırabileceğini göstermektedir. Effects of Visual Information on Anxiety Level and Clinical Parameters in Ultrasonography Guided Carpal Tunnel InjectionsAim: The purpose of this study is to evaluate the effects of ultrasonography (USG) using and patient education by giving visual information via USG on anxiety levels and clinical parameters in carpal tunnel injectionsMethod: The study included 100 patients with idiopathic carpal tunnel syndrome (CTS). Local corticosteroid injection performed into the carpal tunnel in all patients. 2 mg betamethasone sodium phosphate and 5 mg betamethasone dipropionate combination was used as corticosteroid. Patients were randomly assigned into two groups consecutively. Group 1 had an injection and visual information by USG. Group 2 received a blinded injection without any visual information. Patients were evaluated at preinjection and finally at fourth week after injection. 93 of the participiants had completed the study (79 female and 14 male). Patients' satisfaction level due to injection procedure was graded as 'bad', 'moderate', 'good' and 'perfect'. Anxiety levels, pain and hand-grip were evaluated in pre-injection and post-injection periods by State and Trait Anxiety Inventory (STAI), visual analog scale (VAS) and hand-dynamometer respectively. VAS and hand-grip were assessed before injection and at fourth week after injection. STAI was assesed before and after injection immediately.Results: İn both of the groups VAS, STAI and hand-grip values significantly improved. Group 1 had a better improvement of VAS score than group 2 with no significant difference between groups (p>0.05). STAI scores decreased in both of the groups (p0.05).Conclusion: İn CTS injection USG-guiding and giving visual information to patient by USG had no superiority to blinded injections on anxiety level and hand-grip but had a significant decrease of VAS score in comparison with blinded injections. İt seems to be more effective using USG-guiding in local injections for a better response of treatment. 45

Abstract

Amaç: Fibromiyalji sendromlu (FMS) hastaların klinik, sosyodemografik ve psikojenik özelliklerinin belirlenmesi ve bu parametrelerin, FMS'li hastaların yaşam kalitesi, günlük yaşam aktiviteleri ve fonksiyonellikleri üzerindeki etkilerinin değerlendirilmesi amaçlanmıştır.Metod: Çalışmaya Karadeniz Teknik Üniversitesi Tıp Fakültesi Farabi Hastanesi Fiziksel Tıp ve Rehabilitasyon polikliniğine başvuran ve ACR 1990 kriterlerine göre FMS tanısı almış toplam 86 hasta dahil edildi. Hastaların demografik özellikleri ve başvuru şikayetleri kaydedildi. Hassas nokta sayısı, 100 metre yürüme süresi, her iki el kavrama gücü hesaplandı. Rutin biyokimyasal tetkikleri ve ayırıcı tanıya yönelik ANA, tiroid hormonları ve D vitamini düzeyleri çalışıldı. Yaşam kalitesi ve psikojenik özelliklerin değerlendirilmesi amacıyla hastalara Vizüel analog skala(VAS), Fibromiyalji etki sorgulaması(FES), Hamilton anksiyete değerlendirme ölçeği(HAM-A), Hamilton depresyon değerlendirme ölçeği(HAM-D), Psikolojik belirti tarama testi(SCL90-R), Nottingham sağlık profili(NHP) ve Modifiye yorgunluk etki ölçeği uygulandı.Bulgular: Hastalarımızda yaygın ağrı ve yorgunluk en sık rastlanan semptomlardı. Ayrıca depresyon ve anksiyetenin de FMS hastalarında yüksek oranda görüldüğü saptandı. Ağrı düzeyinin yaşam kalitesi ve depresyon düzeyinde belirgin etkili olduğu gözlendi. Demografik özelliklerin ise yaşam kalitesi ve psikojenik faktörler üzerinde anlamlı bir etkisinin olmadığı görüldü. Hastalarımızın büyük bir çoğunluğunda D vitamini düzeyinin yetersiz olduğu, ancak, D vitamini düşüklüğünün klinik ölçeklerle korele olmadığı, serum demir düzeyindeki düşüklüğün ise yorgunluk düzeyini ve psikolojik belirti indeksini anlamlı ölçüde arttırdığı tespit edildi. Sonuç: Fibromiyalji sendromu yaşam kalitesini belirgin etkileyen ve psikojenik durumların ön planda olduğu bir hastalıktır. Aim: Determination of clinical, demographic and psychological characteristics of patients with Fibromyalgia syndrome (FMS) and to evaluate the effects of these parameters on the quality of life, daily activities and functionality of fibromyalgia patients was aimed.Method: The study included totally 86 patients who applied to the Karadeniz Technical University School of Medicine, Farabi Hospital, Physical Medicine and Rehabilitation clinic and were diagnosed with FMS according to ACR 1990 criteria. The demographic characteristics and complaints of patients were recorded. Number of tender points, 100 meters walking time, grip strength for each hand were calculated. Routine biochemical examinations and for differential diagnosis, ANA, thyroid hormone and vitamin D levels were studied. Visual analog scale (VAS), Fibromyalgia Impact Questionnaire (FIQ), Hamilton Anxiety Rating Scale (HAM-A), Hamilton Depression Rating Scale (HAM-D), psychological symptoms screening test (SCL90-R ), Nottingham Health Profile (NHP) and the modified fatigue impact scale applied to patients to assess quality of life and psychological characteristics.Results: The most common symptoms were fatigue and widespread pain in our patients. In addition, there was a high prevalence of depression and anxiety in patients with FMS. Pain levels were found to be markedly effective in quality of life and depression levels. The demographic characteristics showed no significant effect on quality of life and psychological factors. In a majority of patients vitamin D levels were insufficient, however, vitamin D levels was not correlated with clinical measures, while the decrease in serum iron level was found to significantly increase fatigue and psychological symptoms index.Conclusion: Fibromyalgia syndrome is a disease that markedly affects the quality of life and psychological factors are in the foreground. 78

Abstract

Amaç; Ankilozan spondilit (AS), kronik, özellikle omurga eklemlerinin ve çevre dokuların kemik füzyonuna yol açan inflamasyonla karakterize spondilartropatilerin prototipi olan bir hastalıktır. Pek çok kronik hastalıkta olduğu gibi AS'de de bireylerin yaşam kalitesi hastalıktan olumsuz olarak etkilenir. Biz bu çalışmada AS'li hastalarda Ankilozan spondilit Quality of Life (ASQoL) ile yaşam kalitesini etkileyen faktörleri ayrıca AS hastalarını yaş, cinsiyet, eğitim ve meslek durumu açısından eşleştirdiğimiz kontrol grubu ile depresyon, anksiyete, yorgunluk ve uyku bozukluğu açısından karşılaştırmayı amaçladık.Metot; Çalışmaya KTÜ Tıp Fakültesi Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı kliniğine başvuran, modifiye New York Kriterlerine göre AS tanısı almış 60 hasta alındı ve kontrol grubu da 60 sağlıklı erişkinden seçildi. Hastalar kontrol grubuyla yaş, cinsiyet, vücut kitle indeksi (VKI), medeni durum, eğitim durumu, mesleki durum, sigara ve alkol kullanımı göz önüne alınarak eşleştirildi. AS hastalarının hastalık aktivitesi Bath AS Hastalık Aktivite İndeksi (BASDAI) ile, spinal mobilitesi Bath AS metroloji indeksi (BASMI) ile ve fonksiyonel durumu da Bath AS fonksiyonel indeksi (BASFI) ile değerlendirildi. AS hastalarında yaşam kalitesini değerlendirmek için ASQoL kullanıldı. Hastalara ayrıca kontrol grubu ile karşılaştırılarak Beck Depresyon Ölçeği (BDÖ), Beck Anksiyete Ölçeği (BAÖ), Pittsburg Uyku Kalitesi İndeksi (PUKI) ve Yorgunluk Ciddiyet Skalası (FSS) uygulandı.Sonuç; Eğitim düzeyi ve mesleki durumu AS hastalarında yaşam kalitesini etkileyen faktörler olarak tespit ettik. Ayrıca kadın AS'li hastalarda VAS ve BASDAI değerlerinin daha yüksek olduğunu gözlemledik. AS'li hastalar kontrol grubuyla karşılaştırıldığında duygu-durum, yorgunluk ve uyku kalitesi düzeyleri daha kötü bulundu. Hastalık aktivitesi yüksek olanlarda yaşam kalitesinin daha kötü olduğunu ve ASQoL ile BASDAI, BASFI, VAS, BASMI ve depresyon ve anksiyete ölçekleri arasında anlamlı bir korelasyon olduğunu gözlemledik.Tartışma; AS'li hastalarda yaşam kalitesini etkileyen faktörlerin bilinmesi ve yaşamın hangi alanlarında etkilenme olduğunun tespit edilmesinin tedavi stratejilerini belirlerken yol gösterici olacağı kanısındayız. Objective; Ankylosing spondylitis is a prototype of disease of spondyloarthropathies which is chronic and charactarized by inflammation of spinal and peripheral joints that leads to bone fusion. As in many chronic diseases, quality of life is affected negatively by the disease in patients with ankylosing spondylitis. In this study we aimed to investigate the factors that affect quality of life in patients with ankylosing spondylitis and compare AS patients with the control group for depression, anxiety, fatigue and sleep disorders which are matched by age, gender, education and work situation.Method; 60 patients, diagnosed as ankylosing spondylitis according to the modified New York criteria, who applied to Karadeniz Technical University Medical Faculty, were included in the study and control group was selected as 60 healthy adults. The patients were matched with the control group by age, gender, body mass index (BMI), marital status, education and work status, smoking and use of alcohol. The disease activity was measured by Bath AS disease activity index (BASDAI), spinal mobility was measured by Bath AS metrology index (BASMI) and functional status was measured by Bath AS functional index (BASFI). ASQoL was used to evaluate the quality of life of patients with ankylosing spondylitis. Moreover, Beck depression Inventory (BDI), Beck Anxiety Inventory (BAI), Pittsburgh Sleep Quality index (PSQI), Fatigue Severity Scale (FSS) was carried out to the patients by comparing with the control group.Results; The education and work status have been determined as the factors that affect the quality of life of patients with ankylosing spondylitis. Besides, we observed that VAS and BASDAI values were higher in women patients. Compared to the control group, emotional status, fatigue and sleep quality level were worse in patients with ankylosing spondylitis. We have observed that the quality of life gets worse as the disease activity increases and there was a meaningful correlation between ASQoL and BASDAI, BASFI, VAS, BASMI and depression and anxiety inventories.Conclusion; We believe that knowing the factors that effect quality of life and to determine which areas of life are affected will be instructive in defining the treatment strategies. 70

105. Frequency of fibromyalgianess in patients with rheumatoid arthritis and ankylosing spondylitis: A multicenter study of Turkish League Against Rheumatism (TLAR) network.

Abstract

Objectives: This study aimed to evaluate the frequency of fibromyalgianess, fibromyalgia syndrome (FS), and widespread pain in patients with rheumatoid arthritis (RA) and ankylosing spondylitis (AS) and their relationship with clinical and demographic parameters., Patients and Methods: This cross-sectional multicenter trial was performed in 14 centers across Türkiye between June 2018 and November 2019. Out of 685 patients recruited from the accessible population, 661 patients (342 RA, 319 AS; 264 males, 397 females; mean age: 48.1±12.9 years; range, 17 to 88 years) met the selection criteria. In these cohorts, those who did not meet the criteria for FS and had widespread pain (widespread pain index ≥7) were evaluated as a separate group. Clinical status and demographic parameters of patients in both cohorts were evaluated as well as the evaluations of RA and AS patients with widespread pain (widespread pain index ≥7) and RA and AS patients with FS groups. In addition, correlations between polysymptomatic distress scale (PSD) scores and Visual Analog Scale (VAS), Simplified Disease Activity Index (SDAI), Clinical Disease Activity Index (CDAI), and Disease Activity Score using 28 joint counts for RA patients and VAS, Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), and Ankylosing Spondylitis Disease Activity Score (ASDAS) for AS patients were analyzed., Results: Frequencies of patients with FS and patients who had PSD scores ≥12 were 34.1% and 44.4% in all RA patients, respectively. Moreover, FS and PSD scores ≥12 were found in 29.2% and 36.9% of all AS patients, respectively. PSD scores of RA patients with FS were higher than all RA patients and RA patients with widespread pain. SDAI and CDAI scores of RA patients with FS were higher than all RA patients and RA patients with widespread pain. Similarly, PSD scores of AS patients with FS were higher than all AS patients and AS patients with widespread pain. ASDAS-erythrocyte sedimentation rate and BASDAI scores of AS patients with FS were found higher than all AS patients and AS patients with widespread pain., Conclusion: Disease activity scores, including pain in RA and AS, were higher in the presence of FS or fibromyalgianess. It may be related to clinical parameters, but cohort studies with long-term follow-up are needed to reveal causality. Abstract

To evaluate of hepatitis serology and reactivation frequency in patients with rheumatic disease receiving biologic agents. Our study included patients with inflammatory rheumatic diseases from 23 centers, who were followed up with biological therapy. Demographic and clinical characteristics of the patients, duration of drug use and hepatitis serology and the state of viral reactivation were analyzed. A total of 4060 patients, 2095 being males, were included in our study. Of the patients, 2463 had Ankylosing Spondylitis (AS), 1154 had Rheumatoid Arthritis (RA), 325 had Psoriatic Arthritis (PsA), and 118 had other inflammatory rheumatic diseases. When the viral serology of the patients was evaluated, 79 patients (2%) who were identified as HBs Ag positive, 486 (12%) patients who were HBs Ag negative and anti-HBc IgG positive and 20 patients (0.5%) who were anti-HCV positive. When evaluated on a disease-by-disease basis, the rate of HBsAg was found to be 2.5% in RA, 2% in AS and 0.9% in PsA. Viral reactivation was detected in 13 patients while receiving biologic agents. HBs Ag was positive in nine patients with reactivation and negative in four patients. Anti-HBc IgG, however, was positive. Six of these patients had AS, four had RA, and three had PsA. The development of hepatitis reactivation in 11.4% of HBs Ag positive patients and 0.82% of anti-HBc IgG positive patients due to the use of biologic agents is an important problem for this group of patients. Antiviral prophylaxis is recommended to be started especially in patients who are HBs Ag positive and who are using biologic agents due to viral reactivation. Therefore, it is important to carry out hepatitis screenings before biologic agent treatment and to carefully evaluate the vaccination and prophylaxis requirements., (© 2022. Physical working capacity and aerobic exercise capacity of the participants were determined by using cardiopulmonary exercise tests, performed before and 4 months after initiation of anti-TNF alpha therapy., Results: The mean age of the patients was 37 ± 9.1 years, and mean duration of disease was 8.9 ± 7.6 years. Patients with AS exhibited significantly lower aerobic exercise capacity (VO2peak: 21.2 ± 5.5 vs. 27.2 ± 6.6 ml/kg/min, P = 0.001), maximum power output (110.4 ± 34.8 vs.153 ± 39.8 W, P = 0.0001), and exercise duration (16.3 ± 2.6 vs. 19.6 ± 2.9 min, P = 0.0001) than the healthy controls. When patients were reevaluated after 4 months of anti-TNF alpha therapy, significant improvement was obtained in patients’ aerobic capacity, maximum power output, and exercise duration., Conclusions: Results from this study indicate that in addition to inflammatory parameters and quality of life index, even short-term anti-TNF alpha therapy results in significant improvement in cardiopulmonary health status as objectively reflected by peak VO2,maximum work rate, and exercise duration.

Abstract

Objective: Serum YKL-40 plays roles in inflammatory and vascular processes. Our aim was to evaluate serum YKL-40 levels in patients with ankylosing spondylitis (AS) and to investigate their potential relationship with arterial stiffness based on carotid-femoral pulse wave velocity (CF-PWV)., Methods: Forty-three patients with AS and 41 healthy controls with no history or current signs of cardiovascular disease were included in the study. All patients were administered nonsteroidal anti-inflammatory drugs (NSAIDs), and none were prescribed anti-tumor necrosis factor agents. Serum YKL-40 levels were measured. CF-PWV and intima-media thickness of the common carotid artery (IMT-C) were evaluated., Results: The mean age of AS patients was 34.6 ± 10.2 years and of controls was 36.3 ± 9.0 years. CF-PWV was significantly higher in AS patients than in controls (8.2±2.7 vs.7.0±1.6 m/s, respectively; P=0.015). However, the IMT-C was not significantly different between AS patients and controls (0.6±0.3 vs. 0.5±0.2 mm, P=0.501). YKL-40 levels were significantly higher in AS patients than in controls (78.9±37.9 vs. 58.4±21.2 ng/mL, P=0.003) and were strongly correlated with CF-PWV (r=0.773, P < 0.001) and IMT-C (r=0.548, P < 0.001). A multiple linear regression analysis revealed that CF-PWV could be explained by serum YKL-40 levels and IMT-C (adjusted R²= 0.707, P=0.013 and P=0.001, respectively). AS patients with a higher disease activity score had higher YKL-40 levels, IMT-C, and CF-PWV than did those with a lower disease activity score (P < 0.001, P=0.008, and P < 0.001, respectively) Conclusion: AS patients had higher serum YKL-40 levels, CF-PWV, and IMT-C than did healthy controls. Additionally, there was an association between increased CF-PWV and serum YKL-40 levels. Therefore, we conclude that CF-PWV and YKL-40 levels may be used for early diagnosis of atherosclerosis in AS patients.

Abstract

Background: Behçet's disease is a vasculitis, seen more frequently around the Mediterranean and the Far East, and evinces with oral and genital ulcerations, skin lesions and uveitis. Carbonic anhydrase (CA) is a metalloenzyme which is widely distributed in the living world, and it is essential for the regulation of acid-base balance. Anti-CA antibodies have been reported in many disorders, such as systemic lupus erythematosus, Sjögren's syndrome, rheumatoid arthritis, endometriosis, idiopathic chronic pancreatitis, type 1 diabetes and Graves' disease. The goal of this study was to investigate CA I and II autoantibodies in Behçet's disease (BD)., Methods: 35 patients with BD and 29 healthy controls were included in the study and CA I and II autoantibody levels were investigated by ELISA., Results: The CA I and II autoantibody levels of BD group were significantly higher than the healthy group (p=0.013, p inf 0.0001, respectively). A cut-off value of 0.250 ABSU for anti-CA I was associated with 34 % sensitivity and 100 % specificity and a cut-off value of 0.171 ABSU for anti-CA II was associated with 54 % sensitivity and 100 % specificity for predicting BD., Conclusion: The CA I and II autoantibody levels in patients with BD were found higher compared to control group and the results suggest that CA I and II autoantibodies may be involved in the pathogenesis of BD.

Abstract

The number of studies conducted on the subject of cigarette smoking habits in Turkish society in general is limited. This study presents data regarding the prevalence of cigarette smoking in the Eastern Black Sea Region of Turkey. This cross-sectional study was carried out between March 2003 and March 2005 in the provinces of Trabzon, Giresun, Rize, Artvin and Gümüşhane in the Eastern Black Sea Region. The research was conducted in the provincial capitals and districts. According to the 2000 general census, a total of 459.021 people live in these areas, and 6103 individuals were enrolled in the study, selected to be representative of gender, age groups and settlement locations. Data regarding cigarette smoking were obtained using questions employed by the World Health Organization and the British Health Protection Agency. Daily smoking prevalence (at least one cigarette smoked per day) of 18.3% was determined in women and 50.7% in men, while weekly smoking prevalence (less than one cigarette a day but at least one a week) was determined as 1.3% in men and women. It was also determined that 8.4% of women and 18% of men had quit smoking; 69.7% of women and 27.9% of men stated that they had never smoked in their lives. The difference between men and women in terms of smoking prevalence was statistically significant (p< 0.0005). The highest prevalence was in the 20-29 age group in women (23.3%) and in the 30-39 age-group in men (59.9%). In addition, significantly high levels were determined in high school graduates (46.3%, p< 0.0005), unmarried subjects (41%, p< 0.0005) and in the retired (53.7%, p< 0.0005). Cigarette smoking levels continue to represent a major public health problem in the Eastern Black Sea Region.

Abstract

Objective: Fibromyalgia syndrome is characterized by both somatic and psychic symptoms and it is suggested that psychic factors contribute to the clinical presentation of this syndrome. This study was planned to have a better understanding of fibromyalgia through elaborating the role of alexithymia and anger in the pathogenesis of this illness., Method: The study was carried out in the Physical Therapy and Rehabilitation outpatient clinic with 101 women with fibromyalgia syndrome, 30 women with rheumatoid arthritis and 59 healthy women with no current or past medical history. The subjects were evaluated by the Visual Analog Scale, Toronto Alexithymia Scale-20 items, Spielberger State-Trait Anger Inventory, Beck Depression Scale, Fibromyalgia Impact Questionnaire and a sociodemographic data form., Results: All these groups were similar to each other in means of age, years of education, marital and economical status. In the fibromyalgia syndrome group, the scores of anxiety and anger-in were calculated significantly higher than other groups. The depression and alexithymia scores were found higher than healthy group., Discussion: These findings suggest that fibromyalgia patients suffer more anxiety and anger toward oneself, which is anger-in, than rheumatoid arthritis patients. Though the patient groups were more alexithymic than the healthy group, alexitimia scores of the two patient groups were not different. This situation suggest that anger-in, which is suppressed and unexpressed anger style is a part of the fibromyalgia syndrome together as well as high anxiety.
