Abstract

Aim: The SAMe-TT2R2 score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores >2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2 scores >2 than to patients with lower scores. Methods and results: We analyzed the Phase III dataset of the Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation (GLORIA-AF), a large, global, prospective global registry of patients with newly diagnosed AF and ≥1 stroke risk factor. We compared baseline clinical characteristics and antithrombotic prescriptions to determine the probability of the VKA prescription among anticoagulated patients with the baseline SAMe-TT2R2 score >2 and ≤ 2. Among 17,465 anticoagulated patients with AF, 4,828 (27.6%) patients were prescribed VKA and 12,637 (72.4%) patients an NOAC: 11,884 (68.0%) patients had SAMe-TT2R2 scores 0-2 and 5,581 (32.0%) patients had scores >2. The proportion of patients prescribed VKA was 28.0% among patients with SAMe-TT2R2 scores >2 and 27.5% in those with scores ≤2. Conclusions: The lack of a clear association between the SAMe-TT2R2 score and anticoagulant selection may be attributed to the relative efficacy and safety profiles between NOACs and VKAs as well as to the absence of trial evidence that an SAMe-TT2R2-guided strategy for the selection of the type of anticoagulation in NVAF patients has an impact on clinical outcomes of efficacy and safety. The latter hypothesis is currently being tested in a randomized controlled trial. Abstract

This work focuses on Chico Buarque's novel Leite derramado, launched in 2009, and aims to analyze the treatment given to Matilde, character of the work, by the society in which she is inserted, more precisely by the social environment of the family Assumpção, which integrates, for a long period, the aristocracy of Rio. The analysis of the text is done from the perspective of identity and culture and considers political and historical aspects when shedding light on certain events that occurred in Brazil and mentioned in the work. For this purpose, the concepts of Homi K. Bhabha (1998, 2011), Lélia Gonzalez (1984), Linda Hutcheon (1991), among others, are used as theoretical support. With this, it is possible to perceive, through the narrative, the social displacements and their impacts in a prejudiced, racist and sly society.

Abstract

Background: Bipolar active fixation (BipolarAFL) and quadripolar passive fixation left-ventricular leads (QuadPFL) have been designed to reduce the risk of phrenic nerve stimulation (PNS), enable targeted left-ventricular pacing, and overcome problems of difficult coronary venous anatomy and lead dislodgment. This study sought to report the long-term safety and performance of a BipolarAFL, Medtronic Attain Stability 20066, compared to QuadPFL.Methods: We performed a single-operator retrospective analysis of 81 patients receiving cardiac resynchronization therapy (CRT) (36 BipolarAFL, 45 QuadPFL). Immediate implant data and electrical and clinical data during follow-up (FU) were analyzed.Results: BipolarAFL has been chosen in patients with significantly larger estimated vein diameter (at the lead tip: 7.2 ± 4.1 Fr vs. 4.1 ± 2.3 Fr, p < 0.001) without significant time difference until the final lead position was achieved (BipolarAFL: 20.9 ± 10.5 min, vs. QuadPFL: 18.9 ± 8.9 min, p = 0.35). At 12 month FU no difference in response rate to CRT was recorded between BipolarAFL and QuadPFL according to left ventricular end-systolic volume (61.1 vs. 60.0%, p = 0.82) and New York Heart Association (66.7 vs. 62.2%, p = 0.32). At median FU of 48 months (IQR: 44–54), no lead dislodgment occurred in both groups but a significantly higher proportion of PNS was recorded in QuadPFL (13 vs. 0%, p < 0.05). Electrical parameters were stable during FU in both groups without significant differences.Conclusion: BipolarAFL can be implanted with ease in challenging coronary venous anatomy, shows excellent electrical performance and no difference in clinical outcome compared to QuadPFL.

Background: The relationship between thyroid function and cardiac disease is complex. Both hypothyroidism and thyrotoxicosis can predispose to ventricular arrhythmia and other major adverse cardiovascular events (MACE), so that a U-shaped relationship between thyroid signaling and the incidence of MACE has been postulated. Moreover, recently published data suggest an association between thyroid hormone concentration and the risk of sudden cardiac death (SCD) even in euthyroid populations with high-normal FT4 levels. In this study, we investigated markers of repolarization in ECGs, as predictors of cardiovascular events, in patients with a spectrum of subclinical and overt thyroid dysfunction.Methods: Resting ECGs of 100 subjects, 90 patients (LV-EF > 45%) with thyroid disease (60 overt hyperthyroid, 11 overt hypothyroid and 19 L-T4-treated and biochemically euthyroid patients after thyroidectomy or with autoimmune thyroiditis) and 10 healthy volunteers were analyzed for Tp-e interval. The Tp-e interval was measured manually and was correlated to serum concentrations of thyroid stimulating hormone (TSH), free triiodothyronine (FT3) and thyroxine (FT4).Results: The Tp-e interval significantly correlated to log-transformed concentrations of TSH (Spearman's rho = 0.30, p < 0.01), FT4 (rho = −0.26, p < 0.05), and FT3 (rho = −0.23, p < 0.05) as well as log-transformed thyroid's secretory capacity (SPINA-GT, rho = −0.33, p < 0.01). Spearman's rho of correlations of JT interval to log-transformed TSH, FT4, FT3, and SPINA-GT were 0.51 (p < 1e−7), −0.45 (p < 1e−5), −0.55 (p < 1e−8), and −0.43 (p < 1e−4), respectively. In minimal multivariable regression models, markers of thyroid homeostasis correlated to heart rate, QT, Tp-e, and JT intervals. Group-wise evaluation in hypothyroid, euthyroid and hyperthyroid subjects revealed similar correlations in all three groups.Conclusion: We observed significant inverse correlations of Tp-e and JT intervals with FT4 and FT3 over the whole spectrum of thyroid function. Our data suggest a possible mechanism of SCD in hypothyroid state by prolongation of repolarization. We do not observe a U-shaped relationship, so that the mechanism of SCD in patients with high FT4 or hyperthyroidism seems not to be driven by abnormalities in repolarization.

Abstract

Volume-induced hypertrophy is one of the risk factors for cardiac morbidity and mortality. In addition, mechanical and metabolic dysfunction, aging, and cellular redox balance are also contributing factors to the disease progression. In this study, we used volume overload (VO), which was induced by an aortocaval fistula in 2-month-old male Wistar rats, and sham-operated animals served as control. Functional parameters were measured by transthoracic echocardiography at termination 4- or 8-months after VO. The animals showed hypertrophic remodeling that was accompanied by mechanical dysfunction and increased cardiomyocyte stiffness. These alterations were reversible upon treatment with glutathione. Cardiomyocyte dysfunction was associated with elevated oxidative stress markers with unchanged inflammatory signaling pathways. In addition, we observed altered phosphorylation status of small heat shock proteins 27 and 70 and diminished protease expression caspases 3 compared to the matched control group, indicating an impaired protein quality control system. Such alterations might be attributed to the increased oxidative stress as anticipated from the enhanced titin oxidation, ubiquitination, and the elevation in oxidative stress markers. Our study showed an early pathological response to VO, which manifests in cardiomyocyte mechanical dysfunction and dysregulated signaling pathways associated with enhanced oxidative stress and an impaired protein quality control system.

Abstract Background Several P‐wave indices are associated with the development of atrial fibrillation (AF). However, previous studies have been limited in their ability to reliably diagnose episodes of AF. Implantable loop recorders allow long‐term, continuous, and therefore more reliable detection of AF. Hypothesis The aim of this study is to identify and evaluate ECG parameters for predicting AF by analyzing patients with loop recorders. Methods This study included 366 patients (mean age 62 ± 16 years, mean LVEF 61 ± 6%, 175 women) without AF who underwent loop recorder implantation between 2010–2020. Background: A considerable amount of evidence has shown that acute cardiovascular diseases exhibit specific temporal patterns in their onset.Aim: This study was performed to determine if takotsubo syndrome (TTS) shows chronobiological variations with short and long-term impacts on adverse events.Design: Our institutional database constituted a collective of 114 consecutive TTS patients between 2003 and 2015.Methods: Patients were divided into groups defined by the onset of TTS as per time of the day, day of the week, month and quarter of year.Results: TTS events were most common afternoon and least common in the night, indicating a wave-like pattern (p = 0.001) of manifestation. The occurrence of TTS events was similar among days of the week and weeks of the month. TTS patients diagnosed in the month of November and subsequently in the fourth quarter showed a significantly longer QTc interval. These patients also revealed a significantly lower event-free-survival over a 1-year follow-up. In a multivariate Cox regression analysis, TTS events occurring in the fourth quarter of year (HR 6.8, 95%CI: 1.3–35.9; p = 0.02) proved to be an independent predictor of lower event-free-survival.Conclusions: TTS seems to exhibit temporal preference in its onset, but nevertheless this possibly coincidental result needs to be analyzed in a large multicenter registry.

Introduction Idiopathic Parkinson syndrome (iPS) is one of the most common neurodegenerative disorders characterised by the triad of bradykinesia, rigidity and tremor. Tremor at rest predominantly at one side is often perceived by patients as severely disabling and yet ranges among the most difficult symptoms to treat. In medically refractory cases, lesional approaches have proven to be effective alternatives. However, to date, there is no comprehensive analysis of non-surgical therapies to manage iPS-patients’ tremor. We therefore present a detailed study protocol for a systematic literature review assessing efficacy/effectiveness and safety of non-lesional treatments for tremor in iPS.Methods and analysis We will search three electronic databases (MEDLINE, EMBASE and PsycINFO) using a combination of title/abstract keywords. Additionally, hand-searched reference and citation lists of key reviews identified through the search strategy will be screened. Eligible studies should investigate the efficacy/effectiveness and safety of therapeutic options for tremor in iPS excluding lesional interventions. Publications will be independently assessed for inclusion criteria by two investigators and study information summarised using a standardised template including quality assessment according to the QualSyst tool. We will provide a narrative synthesis of results and conduct a meta-analysis whenever possible.Ethics and dissemination We commit to present contemporary evidence on the efficacy/effectiveness and safety of non-lesional interventions for tremor in iPS in a future publication. Abstract

The maiden speech – the first speech given by a newly elected member of parliament (MP) – is a tradition in many parliaments, a personalized rite of passage to political power. As ethnic minority MPs remain relative newcomers, the maiden speech is, for them, even more politically charged. How do ethnic minority MPs represent their identities in this transformative moment? Our data set includes 93 ethnic minority MPs who have held a seat in the Dutch parliament, covering 88 maiden speeches, spanning 11 cycles (1986–2023). The diachronic and intersectional analysis shows that the relation between descriptive, substantive and symbolic representation for historically marginalized groups fluctuates and is influenced by the political environment. The ‘firsts’ of a particular gender/ethnicity intersectional group are less likely to narrate a minority identity than non-firsts. Progressive party ideology influences the extent to which ethnic minority MPs emphasize an (intersectional) minoritized identity. Personal stories and family histories are often used to counter stereotypes, unmute silenced cultures and share values. The focus on the maiden speech as a political narrative sheds light on the blurry lines between substantive, symbolic and descriptive representation. Abstract

The crystal structures of the A2B2O7-x Niobium-based pyrochlores Y2(Nb0.86Y0.14)2O6.91, CaYNb2O7, and Y2NbTiO7 are reported, determined by powder neutron diffraction. These compounds represent the first observation of B-site displacements in the pyrochlore structure: the B-site ions are found to be displaced from the ideal pyrochlore positions, creating electric dipoles. The orientations of these dipoles are fully analogous to orientations of the magnetic moments in Ising spin based magnetically frustrated pyrochlores. Diffuse scattering in electron diffraction patterns shows that the displacements are only short range ordered, indicative of geometric frustration of the collective dielectric state of the materials. Comparison to the crystal structure of the Nb5+ (d0) pyrochlore La2ScNbO7 supports the prediction that charge singlets, driven by the tendency of Nb to form metal-metal bonds, are present in these pyrochlores. The observed lack of long-range order to these singlets suggests that Nb4+-based pyrochlores represent the dielectric analogy to the geometric frustration of magnetic moments observed in rare earth pyrochlores., Comment: Accepted to Journal of Physics: Condensed Matter

We have studied the thermodynamics of the stuffed spin ice material, Dy2(DyxTi2-x)O7-x/2, in which additional Dy3+ replace Ti4+ in the pyrochlore Dy2Ti2O7. Heat capacity measurements indicate that these materials lose the spin ice zero point entropy for x >= 0.3, sharply contrasting with results on the analogous Ho materials. A finite ac susceptibility is observed as T ~ 0 in both the Ho and Dy materials with x = 0.67, which suggests that spin fluctuations persist down to T ~ 0. This behavior has been linked to the partial wettability of the Cormorant’s feathers based on the unusual structure of the contour feathers. Close to the rachis, these feathers have a closed vane with very densely spaced barbs but have an open vane in more peripheral feather parts. To investigate whether the wettability is connected to the composition of the uropygial gland secretion that is often linked to water repellency of bird feathers, we analyzed the gland secretions of the Great Cormorant and, for comparison, of the Muscovy Duck, Cairina moschata (Anatidae). Using the mass spectrometry techniques, atmospheric solids analysis probe-atmospheric pressure chemical ionization-mass spectrometry (ASAP-APCI-MS) and gas chromatography/mass spectrometry (GC/MS), significant differences in the chemical composition of the secretions were revealed. The Cormorant showed a highly diverse mixture of more than 1000 compounds consisting of methyl-branched aliphatic carboxylic esters with large chain length variability. In contrast, the duck’s secretion is dominated by the two esters octadecyl and eicosyl 2,4,6-trimethyloctanoate. Water contact angle measurements of the secretions showed that the secretion of the Muscovy Duck has higher water repellency than that of the Great Cormorant. Nevertheless, contact angle measurements of feathers before and after removing of feather waxes indicate that these waxes, likely consisting of the uropygial gland secretions, did not influence water repellency of the feathers to a large degree.

Abstract

Este artigo apresenta um relato sobre o desenvolvimento de um projeto com enfoque social executado em forma de docência compartilhada em uma turma de 4° ano do Ensino Fundamental. As atividades propostas, que tiveram, como referência, a obra O Pequeno Príncipe, de Antoine Saint-Exupéry, foram elaboradas após um diagnóstico que evidenciou que a organização da turma não favorecia a interação entre os estudantes e que a maioria das tarefas eram executadas individualmente. Diante dessa realidade, elaborou-se uma proposta que, além de mobilizar os conteúdos previstos para essa etapa, promovessem a construção do espírito colaborativo. Os resultados foram extremamente satisfatórios, o que vem a confirmar o posicionamento de que a escola deve promover momentos de interação aos estudantes, de modo que eles possam experienciar relações entre si e com o mundo.

Abstract

Synthesis and crystal structures are described for the compounds Ln2(Ti2-xLnx)O7-x/2, where Ln = Tb, Dy, Ho, Er, Tm, Yb, Lu, and x ranges from 0 to 0.67. Rietveld refinements on X-ray powder diffraction data indicate that in Tb and Dy titanate pyrochlores, extra Ln3+ cations mix mainly on the Ti4+ site with little disorder on the original Ln3+ site. For the smaller rare earths (Ho-Lu), stuffing additional lanthanide ions results in a pyrochlore to defect fluorite transition, where the Ln3+ and Ti4+ ions become completely randomized at the maximum (x=0.67). In all of these Ln-Ti-O pyrochlores, the addition of magnetic Ln3+ in place of nonmagnetic Ti4+ adds edge sharing tetrahedral spin interactions to a normally corner sharing tetrahedral network of spins. Abstract

The third law of thermodynamics dictates that the entropy of a system in thermal equilibrium goes to zero as its temperature approaches absolute zero. In ice, however, a "zero point" or residual entropy can be measured - attributable to a high degeneracy in the energetically preferred positions of the hydrogen ions associated with the so-called "ice rules".1,2 Remarkably, the spins in certain magnetic materials with the pyrochlore structure of corner-sharing tetrahedra, called "spin ice", have an equivalent degeneracy of energetically preferred states and also have been shown to display a zero point entropy.3,4,5,6,7 Here we report that we have chemically altered Ho2Ti2O7 spin ice by stuffing extra Ho magnetic moments into normally non-magnetic Ti sites surrounding the Ho tetrahedra. The resulting series, Ho2(Ti2-xHox)O7-x/2, provides a unique opportunity to study the effects of increased connectivity between spins on a frustrated lattice. Surprisingly, the measured zero point entropy per spin appears unchanged by these excess spins, and the dynamic freezing of the spins is suppressed to lower temperatures. The results challenge our understanding of the spin ice state, and suggest a new avenue for using chemistry to study both ice-like frustration and the properties of the broad family of geometrically frustrated magnets based on the pyrochlore structure., Comment: 17 pages, 4 figures, to be published in Nature Physics in revised form
