Zusammenfassung: Häufigkeit: Habituelle Frühaborte ereignen sich bei etwa 1 % der fruchtbaren Paare. Ursachen und Therapie: Als weitgehend erwiesene Ursachen gelten chromosomale balancierte Translokationen, große Uterussepten, unzureichend behandelte Hypothyreosen und Diabetes-mellitus-Erkrankungen, polyzystische Ovarialsyndrome und das Antiphospolipidsyndrom (APS). Alle genannten Ursachen bedürfen definitiv einer Behandlung. Diese umfasst die genetische Beratung, die operative Korrektur eines Septums, die endokrine Einstellung einer Hypothyreose und eines Diabetes mellitus sowie die Heparin- und Acetylsalicylsäuregabe bei einem APS. Eine Vielzahl anderer Ursachen wird diskutiert. Aufgrund qualitativ unzureichender Studien konnte deren Relevanz nicht bestätigt werden. Entsprechend ist auch der Erfolg einer Therapie ungewiss. Empfehlungen: Fachgesellschaften versuchen, klare und evidenzbasierte Empfehlungen zu formulieren. Diese sind jedoch nicht einheitlich. Daher sind in Einzelfällen mehrere Empfehlungen zu berücksichtigen. Eine medizinisch und ökonomisch sinnvolle Ausweitung der Diagnostik und Therapie muss erwogen werden

Abstract

PURPOSE To study if ovarian response is affected by the type of disease if fertility preservation is required. METHODS A registry of the trinational fertility preservation network FertiPROTEKT including 992 patients aged 18-40 years undergoing ovarian stimulation and follicle aspiration for fertility preservation from 1/2007 until 3/2016 was analysed. The number of collected oocytes, days of stimulation, total gonadotropin dosage and gonadotropin dosage per day were evaluated. Abstract

Purpose: Official guideline coordinated and published by the German Society of Gynecology and Obstetrics (DGGG). Aim of the guideline was to standardize the diagnosis and treatment of patients with recurrent miscarriage (RM). Recommendations were proposed, based on the current national and international literature and the experience of the involved physicians. Consistent definitions, objective assessments and standardized therapy were applied. Methods: Members of the different involved societies developed a consensus in an informal process based on the current literature. The consensus was subsequently approved by the heads of the scientific societies. Recommendations: Recommendations for the diagnosis and treatment of patients with RM were compiled which took the importance of established risk factors such as chromosomal, anatomical, endocrine, hemostatic, psychological, infectious and immunological disorders into consideration.

107. Fertilität vor gonadotoxischen Therapien

Abstract

Zusammenfassung: Hintergrund: Fertilitätsprotektive Maßnahmen vor gonadotoxischen Therapien spielen aufgrund steigender Überlebensraten bei Krebserkrankungen und Autoimmunerkrankungen zunehmend eine Rolle. Material und Methoden: Es erfolgte eine selektive Literaturrecherche auf der Basis der klinischen und wissenschaftlichen Erfahrung der Autoren. Dargestellt werden mögliche fertilitätsprotektive Maßnahmen bei Männern und Frauen. Ergebnisse: Indiziert sind Maßnahmen bei Männern jeden Alters sowie bei Mädchen und Frauen bis zum Alter von ca. 40Jahren mit einem hohen Risiko einer Gonadenschädigung. Bei Männern können Spermien problemlos kryokonserviert werden und später für Inseminationen oder In-vitro-Fertilisationen mit einer hohen Schwangerschaftschance genutzt werden. Bei präpubertären Jungen ist die Kryokonservierung von Hodengewebe möglich; allerdings ist die Verwendung des Hodengewebes noch experimentell. Bei Mädchen und Frauen sind die Kryokonservierung von unfertilisierten Oozyten, von Ovargewebe sowie die Gabe von GnRH-Agonisten möglich. Für die Durchführung der überwiegend sehr risikoarmen Techniken werden maximal 2Wochen benötigt. Bei der Kombination verschiedener Techniken der Kryokonservierung dürfte eine maximale Schwangerschaftschance von 40-50 % erreichbar sein. Die Kosten müssen zum überwiegenden Teil von den Patientinnen getragen werden und liegen zwischen wenigen Hundert und mehreren Tausend EUR. Eine Beratung und Durchführung der Techniken kann durch Zentren des Netzwerks FertiPROTEKT (http://www.fertiprotekt.de) erfolgen. Schlussfolgerung: Männer aller Altersgruppen und Frauen bis zum Alter von ca. 40Jahren sollten vor gonadotoxischen Therapien über die Möglichkeit zur Durchführung fertilitätsprotektiver Maßnahmen informiert und ggf. behandelt werden.

Abstract

Humans cannot live at very high altitude for reasons, which are not completely understood. Since these reasons are not restricted to cardiorespiratory changes alone, changes in the endocrine system might also be involved. Therefore, hormonal changes during prolonged hypobaric hypoxia were comprehensively assessed to determine effects of altitude and hypoxia on stress, thyroid and gonadal hypothalamus–pituitary hormone axes. Twenty-one male and 19 female participants were examined repetitively during a high-altitude expedition. Cortisol, prolactin, thyroid-stimulating hormone (TSH), fT4 and fT3 and in males follicle-stimulating hormone (FSH), luteinizing hormone (LH) and total testosterone were analysed as well as parameters of hypoxemia, such as SaO2 and paO2 at 550 m (baseline) (n = 40), during ascent at 4844 m (n = 38), 6022 m (n = 31) and 7050 m (n = 13), at 4844 m (n = 29) after acclimatization and after the expedition (n = 38). Correlation analysis of hormone concentrations with oxygen parameters and with altitude revealed statistical association in most cases only with altitude. Adrenal, thyroid and gonadal axes were affected by increasing altitude. Adrenal axis and prolactin were first supressed at 4844 m and then activated with increasing altitude; thyroid and gonadal axes were directly activated or suppressed respectively with increasing altitude. Acclimatisation at 4844 m led to normalization of adrenal and gonadal but not of thyroid axes. In conclusion, acclimatization partly leads to a normalization of the adrenal, thyroid and gonadal axes at around 5000 m. However, at higher altitude, endocrine dysregulation is pronounced and might contribute to the physical degradation found at high altitude.

Abstract

PURPOSE Most guidelines about fertility preservation are predominantly focused on scientific evidence, but are less practically orientated. Therefore, practically oriented recommendations are needed to support the clinician in daily practice. METHODS A selective literature search was performed based on the clinical and scientific experience of the authors, focussing on the most relevant diseases and gynaecological cancers. This article (Part I) provides information on topics that are essential for the fertility preservation indication, such as disease prognosis, disease therapy and its associated risks to fertility, recommending disease-specific fertility preservation measures. Part II specifically focusses on fertility preservation techniques. RESULTS In breast cancer patients, fertility preservation such as ovarian tissue and oocyte cryopreservation is especially recommended in low-stage cancer and in women

Abstract

STUDY QUESTION Does intrauterine application of diluted seminal plasma (SP) at the time of ovum pick-up improve the pregnancy rate by ≥14% in IVF treatment? SUMMARY ANSWER Intrauterine instillation of diluted SP at the time of ovum pick-up is unlikely to increase the pregnancy rate by ≥14% in IVF. WHAT IS KNOWN ALREADY SP modulates endometrial function, and sexual intercourse around the time of embryo transfer has been suggested to increase the likelihood of pregnancy. A previous randomized double-blind pilot study demonstrated a strong trend towards increased pregnancy rates following the intracervical application of undiluted SP. As this study was not conclusive and as the finding could have been confounded by sexual intercourse, the intrauterine application of diluted SP was investigated in the present trial. STUDY DESIGN, SIZE, DURATION A single-centre, prospective, double-blind, placebo-controlled, randomized, superiority trial on women undergoing IVF was conducted from April 2007 until February 2012 at the University Department of Gynaecological Endocrinology and Reproductive Medicine, Heidelberg, Germany. PARTICIPANTS/MATERIALS, SETTING, METHODS The study was powered to detect an 14% increase in the clinical pregnancy rate and two sequential tests were planned using the Pocock spending function. At the first interim analysis, 279 women had been randomly assigned to intrauterine diluted SP (20% SP in saline from the patients' partner) (n = 138) or placebo (n = 141) at the time of ovum pick-up. MAIN RESULTS AND THE ROLE OF CHANCE The clinical pregnancy rate per randomized patient was 37/138 (26.8%) in the SP group and 41/141 (29.1%) in the placebo group (difference: -2.3%, 95% confidence interval of the difference: -12.7 to +8.2%; P = 0.69). The live birth rate per randomized patient was 28/138 (20.3%) in the SP group and 33/141 (23.4%) in the placebo group (difference: -3.1%, 95% confidence interval of the difference: -12.7 to +6.6%; P = 0.56). It was decided to terminate the trial due to futility at the first interim analysis, at a conditional power of 62%. LIMITATIONS, REASONS FOR CAUTION The confidence interval of the difference remains wide, thus clinically relevant differences cannot reliably be excluded based on this single study. WIDER IMPLICATIONS OF THE FINDINGS The results of this study cast doubt on the validity of the concept that SP increases endometrial receptivity and thus implantation in humans. Abstract

Nowadays, allogeneic haematopoietic stem cell transplantation (allo-HSCT) is a well-established treatment procedure and often the only cure for many patients with malignant and non-malignant diseases. Decrease in short-term complications has substantially contributed to increased survival. Therefore long-term sequelae are reaching the focus of patient care. One of the most important risks of stem cell transplant survivors is infertility. As well as in the field of allo-HSCT also the field of reproductive medicine has achieved substantial advances to offer potential options for fertility preservation in both boys and girls. Access to these procedures as well as their financing differs significantly throughout Europe. As all European children and adolescents should have the same possibility, the Paediatric Diseases Working Party of the European Society for Blood and Marrow Transplantation organised an expert meeting in September 2015. This manuscript describes the recommendations for the diagnosis and pre-emptive procedures that should be offered to all children and adolescents in Europe who have to undergo an allo-HSCT.

Abstract

Throughout follicular growth the number of immune cells increases, enhanced under stimulation with exogenous gonadotropins. This treatment, however, may adversely influence folliculogenesis and negatively affect oocyte quality through modifications in the follicular concentrations of cytokines released by these immune cells. We studied this hypothesis by systematically analysing the concentrations of cytokines present in the serum and follicular fluid at the time of follicular aspiration in conventional gonadotropin-stimulated (c-IVF) cycles in comparison with natural cycle IVF (NC-IVF) in which the follicles were naturally matured. Our study involved 37 NC-IVF and 39 c-IVF cycles including 13 women who underwent both therapies. Mean age was 35.3 ± 4.6 (SD) and 34.2 ± 3.7 years in the NC-IVF and c-IVF groups (ns). Thirteen cytokines were determined in matched serum and FF samples. Interleukin (IL)-4, TNF-α, RANTES, eotaxin and interferon-gamma-induced protein-10 concentrations were lower in FF than in serum. IL-6, -8, -10, -18, monocyte chemotactic protein-1 (MCP-1), VEGF and leukaemia inhibitory factor (LIF) showed higher median levels in FF than in serum, indicating possible ovarian production. Most of these markers were also increased in concentration in the stimulated (c-IVF) than in the NC groups in the serum, but not in the follicular fluid. This finding can be attributed to the increased number of active follicles present after controlled ovarian stimulation. IL-8 was reduced in c-IVF cycles. Our study did not reveal differences in follicular fluid but in serum cytokine concentrations, suggesting that the follicular immune system might not be significantly affected by gonadotropin stimulation.

Abstract

Natural-cycle IVF has been suggested as an alternative IVF treatment. However, efficacy is limited due to high premature ovulation rates, resulting in low transfer rates. This study investigates whether low dosages of clomiphene citrate reduce premature ovulation rate and increase transfer rate. Of 112 women included (aged 35.2 ± 4.5 years) 108 underwent one natural-cycle IVF cycle with human chorionic gonadotrophin (HCG) to induce ovulation and 103 underwent one natural-cycle IVF cycle with 25 mg/day clomiphene from about day 7 until HCG administration. Before retrieval, 1.2 monitoring consultations per cycle were required. Clomiphene reduced premature ovulation rate, from 27.8% without to 6.8% with clomiphene (P < 0.001) and increased transfer rate from 39.8% to 54.4% (P = 0.039). Clinical pregnancy rates without and with clomiphene were 27.9% versus 25.0% per transfer and 11.1% versus 13.6% per initiated cycle. Use of clomiphene resulted in mild hot flushes and headache in 5% of patients. Nausea and persisting ovarian cyst formation was not observed. In conclusion, clomiphene citrate led to very few side effects, required 1.2 monitoring consultations, significantly reduced premature ovulation rate and significantly increased transfer rate per initiated cycle, an effect which was not age dependent.

Die schnell abnehmende Fertilität erfordert eine Behandlung, die schnellstmöglich zu einer Schwangerschaft führt. Konkret bedeutet dies, dass bei einem erst seit 0,5-1Jahr bestehenden Kinderwunsch und Fehlen relevanter Sterilitätsfaktoren eine Spontankonzeption anzustreben ist. Intrauterine Insemination und In-vitro-Fertilisation: Eine intrauterine Insemination sollte nur bei einem eindeutigen andrologischen Sterilitätsfaktor und auch nur über wenige Monate bis zu einem maximalen Alter von etwa 42Jahren durchgeführt werden. Eine klassische In-vitro-Fertilisation (IVF) ist bei einer guten Ovarreserve einer Natural-cycle-IVF vorzuziehen. Eine Natural-cycle-IVF kann bei einer erwarteten "poor response" erwogen werden. Eine klassische IVF sollte nur erfolgen, wenn die Geburtenchance pro Stimulationszyklus mindestens 5 % beträgt. Damit liegt die Altersobergrenze bei etwa 43-44Jahren.

Abstract

[1] The Opportunity Rover has been exploring Meridiani Planum; concurrently, the Compact Reconnaissance Imaging Spectrometer for Mars (CRISM) has been collecting orbital hyperspectral data. Herein, both surface and orbital data are used to characterize surface properties at the Opportunity traverse region around Victoria crater. Results agree with previous studies, which used Opportunity's Panoramic Camera (Pancam) data, and the current study extends estimates of the Hapke single-particle-scattering albedo and asymmetry parameter to a greater spatial and spectral range. Results are useful for determining boundaries between surface units that otherwise look relatively uniform spectrally. This work also provides photometric functions essential for converting spectra to a single viewing geometry to yield more accurate spectral comparisons. Retrieved single-scattering albedos range from 0.42 to 0.57 (0.5663–2.2715 micrometers) and retrieved asymmetry parameters range from −0.27 to −0.17 (moderately backscattering). Surfaces become more backscattering with increasing wavelength above 1 µm. The majority of Victoria crater's ejecta apron is more backscattering than surrounding regions, indicating a change in physical properties. Images taken when the rover traversed this unit show a cover of basaltic soil with superposed millimeter-scale hematitic spherules. Wind streaks on the apron appear smooth (low backscatter) because basaltic sands have partly buried spherules, lessening millimeter-scale roughness. CRISM-derived scattering parameters also show that bedrock-dominated surfaces are less backscattering than soil-covered surfaces, largely due to lower areal abundance of spherules. The ability to analyze surface unit spherule cover is important because it relates to a wetter period during which spherules formed in Meridiani.

Abstract

Study question Are the immune cell profiles and the cytokine concentrations in follicular fluid (FF) and serum at the preovulatory stage different in conventional exogenous gonadotrophin stimulated IVF (c-IVF) compared with natural cycle IVF (NC-IVF)? Summary answer The cell counts of CD45+ leucocytes and T cell subpopulations and the cytokine concentrations in FF and serum are different in c-IVF compared to NC-IVF. What is known already FF-derived cells are heterogeneous. Immune cells are involved in intra-ovarian processes and cytokines are required for normal follicular development. Gonadotrophins stimulate the regulatory intrafollicular system and influence the local distribution of immune cells and the intrafollicular release of cytokines. Administration of exogenous gonadotrophins may have a significant effect on this local regulatory system, which then in turn could influence oocyte quality. Study design, size, duration The study included 105 patients, 69 undergoing c-IVF and 36 undergoing NC-IVF. c-IVF was performed by exogenous ovarian stimulation with hMG and GnRH antagonists. Participants/materials, setting, methods FF samples were collected from the first dominant follicle in c-IVF without pooling and from single leading preovulatory follicles in NC-IVF. Three different approaches were used to analyze FF samples: (i) microscopic investigation of CD45+ leucocytes, (ii) fluorescence-activated cell sorting to determine CD19+ B cells and CD3+ T cells including T cell subpopulations (CD4+, CD8+), and (iii) evaluation of tumour necrosis factor-alpha (TNF-α), interferon-gamma (INF-γ), interleukins (IL)-2, -6, -8, -10 and vascular endothelial growth factor (VEGF) levels in matched FF and serum samples using the Bio-Plex® platform. Main results and the role of chance FF obtained from c-IVF contained proportionally more CD45+ leucocytes (P = 0.0384), but fewer CD8+ cytotoxic T cells than FF from NC-IVF. CD3+ T lymphocytes were the most common type of lymphocytes, and the number thereof was comparable in the two study groups. In c-IVF, serum VEGF levels were higher (P = 0.007) than in NC-IVF while FF contained marginally decreased concentrations of IL-8 in c-IVF in comparison to NC-IVF. The cytokine concentration gradient between FF and serum in c-IVF was 10-fold for IL-8 and 8-fold for VEGF and thereby markedly lower than in NC-IVF, where the differences were 32-fold and 30-fold, respectively. Strong positive correlations were determined between FF- IL-10 and FF- VEGF in c-IVF (r = 0.85, P < 0.0001) and in NC-IVF (r = 0.81, P < 0.0001). Large scale data N/A. Limitations, reasons for caution The ovulation of NC-IVF follicles was induced by the exogenous administration of hCG, which means that the environment did not fully correspond to the physiological situation. Wider implications of the findings The differences in the immune profile and the cytokine concentrations in c-IVF and NC-IVF follicles support the hypothesis that conventional ovarian stimulation affects indirectly and heterogeneously the intrafollicular milieu, and thereby possibly affects the oocyte quality and the IVF outcome. However, further studies are needed to confirm our findings and to refine stimulation protocols in the context of optimizing the intrafollicular environment during oocyte maturation. Abstract

In contrast to multifollicular IVF, follicular flushing seems to increase the efficacy of monofollicular IVF treatments such as natural cycle IVF (NC-IVF). However, because follicular flushing causes loss of granulosa cells, it might negatively affect luteal phase length and endocrine function of the luteal body. A prospective cohort Phase II study was performed in 24 women undergoing NC-IVF. Women underwent a reference cycle with human chorionic gonadotrophin-induced ovulation without follicle aspiration and analysis of the length of the luteal phase and luteal concentrations of progesterone and oestradiol. In addition, they underwent a NC-IVF cycle which was performed identically but follicles were aspirated and flushed three times. The luteal phase was shorter in 29.2%, equal in 16.7% and longer in 50.0% of cases following flushing of the follicles. Overall, neither difference in luteal phase length was significant [median duration (interquartile range) in reference cycle: 13 (12; 14.5), IVF (flushing) cycle: 14 (12.5; 14.5), median difference (95% CI): 0.5 (-0.5 to 1.5)] nor median progesterone and oestradiol concentrations. In conclusion, follicular flushing in NC-IVF affects neither the length of the luteal phase nor the luteal phase concentrations of progesterone and oestradiol, questioning the need for luteal phase supplementation.

A pesquisa revelou que os professores entrevistados conhecem pouco desse tema o que pode ser uma das raz?es para maior dificuldade de gerenciar a demanda e a inclus?o de alunos com esses transtornos. Observou-se, nos relatos dos professores, a necessidade de pensar a escola com um curr?culo que possibilite mais ?nfase no desenvolvimento de habilidades socioemocionais, bem como na valoriza??o das capacidades individuais e no trabalho interdisciplinar entre a educa??o e a ?rea da sa?de. The subject of the present research project is the education of children with mental disorders and school inclusion. Due to this subject, the given research problem was formulated: ?In which manner the existing inclusion model contemplates students with mental disorders??. The aim of this study is to verify how students with mental disorders are perceived and included in regular schools in light of their learning processes, from the teachers' standpoint. It is a qualitative and exploratory research in which semi-structured interviews with elementary school teachers from both public and private school systems were conducted. The technique employed for data analysis was from Laurence Bardin?s Content Analysis (2011). The research has shown that the interviews teachers know little of this subject. That may be a reason for trouble in managing the demands and inclusion of the students with these kind of disorders. The teachers? accounts revealed the necessity of contemplating a comprehensive school curriculum which enables more entasis in developing socioemotional abilities. Also, the appreciation of these students? individual capabilities as well as the interdisciplinarity between health and education. Also, they emphasized that the most lacking resource within the public schools covered in this study is the human resources with which the teachers can rely on for their difficulties with the students with mental disorders and other special educational needs. Whereas, among the private schools it was the necessity for interdisciplinary between health and education as well as the demand for increasing the teachers? training. Given these issues, it appears that closer, integrated and interdisciplinary operation between health and education is the necessary means for bettering the methods of school inclusion.

Abstract

The past decade has seen significant progress on the direct detection and characterization of young, self-luminous giant planets at wide orbital separations from their host stars. Some of these planets show evidence for disequilibrium processes like transport-induced quenching in their atmospheres; photochemistry may also be important, despite the large orbital distances. These disequilibrium chemical processes can alter the expected composition, spectral behavior, thermal structure, and cooling history of the planets, and can potentially confuse determinations of bulk elemental ratios, which provide important insights into planet-formation mechanisms. Using a thermo/photochemical kinetics and transport model, we investigate the extent to which disequilibrium chemistry affects the composition and spectra of directly imaged giant exoplanets. Results for specific "young Jupiters" such as HR 8799 b and 51 Eri b are presented, as are general trends as a function of planetary effective temperature, surface gravity, incident ultraviolet flux, and strength of deep atmospheric convection. We find that quenching is very important on young Jupiters, leading to CO/CH4 and N2/NH3 ratios much greater than, and H2O mixing ratios a factor of a few less than, chemical-equilibrium predictions. Photochemistry can also be important on such planets, with CO2 and HCN being key photochemical products. Carbon dioxide becomes a major constituent when stratospheric temperatures are low and recycling of water via the H2 + OH reaction becomes kinetically stifled. Abstract

Objectives: Despite new treatment options, some patients with systemic lupus erythematosus (SLE) need to be treated with the cytotoxic agent cyclophosphamide (CYC). Unlike malignant disease, there are no recommendations for ovarian protection in SLE. The clinical experience of the FertiPROTEKT network as well as recommendations after literature review will be presented in this paper. Methods: Retrospective analyses of counselling and treatment data from the FertiPROTEKT register with special respect to SLE patients under 40 years prior to planned CYC treatment. Results: A total of 2836 patients were advised prior to cytotoxic treatment in one of the FertiPROTEKT centres during January 2007 to November 2011. Of those, 68 patients (mean age 25 +/− 6.07years) were counselled for severe SLE. Only five women did not make use of a fertility preservation method. Sixty-three patients (92.6%) decided in favour of a fertility preservation method. The largest proportion (91.2%) opted for treatment with a GnRH analogue. Ovarian tissue removal for cryoconservation was performed in 16 patients (25%). Stimulation therapy for cryoconservation of fertilized egg cells was performed in three patients (4.4%). Conclusions: When counselling patients with SLE for fertility preservation one has to be aware of the disease-specific risks. According to the literature, a safe and effective option in SLE up to now has been the use of a GnRH analogue. Cryoconservation of ovarian tissue must still be seen as an experimental treatment, but as data on removal, cryoconservation, retransplantation and pregnancies are steadily rising, this presents a promising option for young SLE patients. Cryoconservation of oocytes must be very critically evaluated due to the need for a stimulation therapy and should only be performed after particular consideration of the individual risks.

Abstract

Eine ovartoxische Therapie kommt nicht nur bei Malignomen zum Einsatz sondern auch bei schweren Autoimmunerkrankungen (AID). Hier wird v. a. das immunsuppressiv wirksame Cyclophosphamid genutzt. Haufig sind junge Frauen vor Abschluss der Familienplanung betroffen. Da das Langzeituberleben durch verbesserte Therapiemethoden deutlich gestiegen ist, stellt der Schutz der Ovarialfunktion eine wichtige Masnahme dar. In diesem Zusammenhang mussen krankheitsspezifische Risiken bei AID beachtet werden. Am Beispiel des systemischen Lupus erythematodes (SLE) – eine der haufigeren AID und die einzige, fur die Daten zum Fertilitatserhalt vorliegen – werden die ublichen Masnahmen anhand der aktuellen Literatur dargestellt. Gonadotropin-Releasing-Hormon(GnRH)-Analoga konnen aufgrund der aktuellen Datenlage allen Patienten empfohlen werden. Eine Kryokonservierung von Ovargewebe ist immer noch experimentell, scheint jedoch vielversprechend und kann mit geringem Risiko und minimaler Zeitverzogerung ebenfalls empfohlen werden. Eine Stimulationstherapie und anschliesende Kryokonservierung von Eizellen ist bei Aktivitat der Grunderkrankung, insbesondere im Falle des SLE, mit einem erhohten Risiko einer Exazerbation und thrombembolischen Ereignissen verbunden. Diese Therapieoption sollte daher im Einzelfall abgewogen werden und nur unter engmaschiger Uberwachung durch ein interdisziplinares Team aus Reproduktionsmedizinern und Rheumatologen erfolgen. Eine Kombination der Gabe von GnRH-Analoga mit der Kryokonservierung von Ovargewebe erscheint sinnvoll.

Zusammenfassung: Das Netzwerk FertiPROTEKT wurde im Jahr2006 mit dem Ziel gegründet, die Expertise universitärer Zentren im Bereich der Fertilitätsprotektion zusammenzuführen und im deutschsprachigen Raum eine klinisch effektive, risikominimierte und flächendeckende Versorgung mit fertilitätsprotektiven Techniken zu ermöglichen. Zu diesem Zweck wurden u.a. regelmäßige Fortbildungsveranstaltungen ausgerichtet, zentrale Kryobanken zur Konservierung von Ovarialgewebe ausgebaut und Behandlungs- sowie Komplikationsregister erstellt. Mehr als 3000Beratungen, 1000Kryokonservierungen von Ovarialgewebe, 1.000.000Zugriffe auf die 2-sprachige Website des Netzwerks und die Entwicklung von Behandlungsempfehlungen in den 6Jahren seit der Gründung zeigen eindrücklich, dass sich das Netzwerk erfolgreich entwickelt hat und inzwischen das größte seiner Art ist. Allerdings wird die große Herausforderung der nächsten Jahre der Nachweis sein, dass die fertilitätsprotektiven Aktivitäten zu erfolgreichen Schwangerschaften und Geburten führen

Abstract

The Extended-range Atmospheric Emitted Radiance Interferometer (E-AERI) is a moderate resolution (1 cm−1) Fourier transform infrared spectrometer for measuring the absolute downwelling infrared spectral radiance from the atmosphere between 400 and 3000 cm−1. The extended spectral range of the instrument permits monitoring of the 400–550 cm−1 (20–25 μm) region, where most of the infrared surface cooling currently occurs in the dry air of the Arctic. Spectra from the E-AERI have the potential to provide information about radiative balance, trace gases, and cloud properties in the Canadian high Arctic. Calibration, performance evaluation, and certification of the E-AERI were performed at the University of Wisconsin Space Science and Engineering Centre from September to October 2008. The instrument was then installed at the Polar Environment Atmospheric Research Laboratory (PEARL) Ridge Lab (610 m altitude) at Eureka, Nunavut, in October 2008, where it acquired one year of data. Measurements are taken every seven minutes year-round, including polar night when the solar-viewing spectrometers at PEARL are not operated. A similar instrument, the University of Idaho's Polar AERI (P-AERI), was installed at the Zero-altitude PEARL Auxiliary Laboratory (0PAL), 15 km away from the PEARL Ridge Lab, from March 2006 to June 2009. During the period of overlap, these two instruments provided calibrated radiance measurements from two altitudes. A fast line-by-line radiative transfer model is used to simulate the downwelling radiance at both altitudes; the largest differences (simulation-measurement) occur in spectral regions strongly influenced by atmospheric temperature and/or water vapour. The two AERI instruments at close proximity but located at two different altitudes are well-suited for investigating cloud forcing. As an example, it is shown that a thin, low ice cloud resulted in a 6% increase in irradiance. The presence of clouds creates a large surface radiative forcing in the Arctic, particularly in the 750–1200 cm−1 region where the downwelling radiance is several times greater than clear-sky radiances, which is significantly larger than in other more humid regions.

Abstract

The discovery of the gamma-ray pulsar PSR J1836+5925, powering the formerly unidentified EGRET source 3EG J1835+5918, was one of the early accomplishments of the Fermi Large Area Telescope (LAT). Sitting 25 degrees off the Galactic plane, PSR J1836+5925 is a 173 ms pulsar with a characteristic age of 1.8 million years, a spindown luminosity of 1.1$\times10^{34}$ erg s$^{-1}$, and a large off-peak emission component, making it quite unusual among the known gamma-ray pulsar population. We present an analysis of one year of LAT data, including an updated timing solution, detailed spectral results and a long-term light curve showing no indication of variability. No evidence for a surrounding pulsar wind nebula is seen and the spectral characteristics of the off-peak emission indicate it is likely magnetospheric. Analysis of recent XMM observations of the X-ray counterpart yields a detailed characterization of its spectrum, which, like Geminga, is consistent with that of a neutron star showing evidence for both magnetospheric and thermal emission., Accepted to Astrophysical Journal

Abstract

PURPOSE: Advances in oncological treatment and in reproductive medicine have made family planning and fertility preservation after cytotoxic treatment more and more attractive. The attitude of patients towards fertility preservation is definitely positive. TECHNIQUES OF FERTILITY PRESERVATION: Basically, both medical treatments and invasive procedures are possible. Ovarian down-regulation by GnRH analogues is widely used and simple to perform, with little delay. However its efficacy has still not been definitely proven. Standard methods of ART require a stable partnership. Therefore, cryopreservation of unfertilized oocytes after hormonal stimulation might be better for most patients. In recent years, the introduction of new cryopreservation protocols allows acceptable results. Since hormonal stimulation is cycle dependent, it might cause a delay in oncologic treatment. New stimulation protocols, which can be initiated in every cycle phase are under investigation. Finally, ovarian biopsy and cryopreservation, followed by replantation might be an option. The first pregnancies and deliveries have been described. CONCLUSION: In most cases a multimodal approach must be offered to the patient. Most treatment modalities can be combined. To offer comprehensive treatment and allow the current techniques of fertility preservation to be evaluated, patients and doctors would benefit from networks such as like FertiProtekt, which guarantees nationwide patient support.

Abstract

Die Uberlebensrate von Krebspatientinnen nimmt erfreulicherweise stetig zu. Die behandelnden Arzte werden daher in zunehmendem Mase auch mit den Langzeitfolgen der Chemotherapie und Bestrahlung auf die Fertilitat von jungen Frauen konfrontiert. Die vorzeitige ovarielle Insuffizienz bei einer Frau mit Kinderwunsch ist sowohl fur die Betroffene wie fur ihren Partner und haufig auch fur ihre Angehorigen eine sehr belastende Situation. Im Falle einer vorhersehbaren iatrogen hervorgerufenen Entstehung ist zumindest ein beratendes Gesprach vor der geplanten zytotoxischen Therapie empfehlenswert. Als reproduktionsmedizinische Techniken kommen neben der konventionellen assistierten Reproduktion die Gabe von GnRH-Analoga die Oozytenkryokonservierung und die Kryokonservierung von Ovarialgewebe in Betracht. Eine individuelle Nutzen-Risiko-Abwagung ist im Hinblick auf die Techniken mit zum Teil noch experimentellen Charakter zwingend, und kann nur im ausfuhrlichen interdisziplinaren Dialog sowie im Gesprach mit der Patientin entschieden werden.

Abstract

This paper presents extensive validation analyses of ozone observations from the Atmospheric Chemistry Experiment (ACE) satellite instruments: the ACE Fourier Transform Spectrometer (ACE-FTS) and the Measurement of Aerosol Extinction in the Stratosphere and Troposphere Retrieved by Occultation (ACE-MAESTRO) instrument. The ACE satellite instruments operate in the mid-infrared and ultraviolet-visible-near-infrared spectral regions using the solar occultation technique. In order to continue the long-standing record of solar occultation measurements from space, a detailed quality assessment is required to evaluate the ACE data and validate their use for scientific purposes. Here we compare the latest ozone data products from ACE-FTS and ACE-MAESTRO with coincident observations from satellite-borne, airborne, balloon-borne and ground-based instruments, by analysing volume mixing ratio profiles and partial column densities. The ACE-FTS version 2.2 Ozone Update product reports more ozone than most correlative measurements from the upper troposphere to the lower mesosphere. At altitude levels from 16 to 44 km, the mean differences range generally between 0 and +10% with a slight but systematic positive bias (typically +5%). At higher altitudes (45-60 km), the ACE-FTS ozone amounts are significantly larger than those of the comparison instruments by up to ~40% (typically +20%). For the ACE-MAESTRO version 1.2 ozone data product, agreement within ±10% (generally better than ±5%) is found between 18 and 40 km for the sunrise and sunset measurements. At higher altitudes (45-55 km), systematic biases of opposite sign are found between the ACE-MAESTRO sunrise and sunset observations. While ozone amounts derived from the ACE-MAESTRO sunrise occultation data are often smaller than the coincident observations (by as much as -10%), the sunset occultation profiles for ACE-MAESTRO show results that are qualitatively similar to ACE-FTS and indicate a large positive bias (+10 to +30%) in this altitude range. In contrast, there is no significant difference in bias found for the ACE-FTS sunrise and sunset measurements. These systematic effects in the ozone profiles retrieved from the measurements of ACE-FTS and ACE-MAESTRO are being investigated. This work shows that the ACE instruments provide reliable, high quality measurements from the tropopause to the upper stratosphere and can be used with confidence in this vertical domain

Abstract

Pulsars are rapidly-rotating, highly-magnetized neutron stars emitting radiation across the electromagnetic spectrum. Although there are more than 1800 known radio pulsars, until recently, only seven were observed to pulse in gamma rays and these were all discovered at other wavelengths. The Fermi Large Area Telescope makes it possible to pinpoint neutron stars through their gamma-ray pulsations. We report the detection of 16 gamma-ray pulsars in blind frequency searches using the LAT. Most of these pulsars are coincident with previously unidentified gamma-ray sources, and many are associated with supernova remnants. Direct detection of gamma-ray pulsars enables studies of emission mechanisms, population statistics and the energetics of pulsar wind nebulae and supernova remnants., Comment: Corresponding authors: Michael Dormody, Paul S. Abstract

We discuss the DISORT-based radiative transfer pipeline (ldquoCRISM_LambertAlbrdquo) for atmospheric and thermal correction of MRO/CRISM data acquired in multispectral mapping mode (~200 m/pixel, 72 spectral channels). Currently, in this phase-one version of the system, we use aerosol optical depths, surface temperatures, and lower atmospheric temperatures, all from climatology derived from Mars Global Surveyor Thermal Emission Spectrometer (MGS-TES) data and from surface altimetry derived from MGS Mars Orbiter Laser Altimeter (MOLA). The DISORT-based model takes the dust and ice aerosol optical depths (scaled to the CRISM wavelength range), the surface pressures (computed from MOLA altimetry, MGS-TES lower atmospheric thermometry, and Viking-based pressure climatology), the surface temperatures, the reconstructed instrumental photometric angles, and the measured I/F spectrum as inputs, and then a Lambertian albedo spectrum is computed as the output. The Lambertian albedo spectrum is valuable geologically because it allows the mineralogical composition to be estimated. Here, I/F is defined as the ratio of the radiance measured by CRISM to the solar irradiance at Mars divided by pi; if there was no martian atmosphere, I/F divided by the cosine of the incidence angle would be equal to the Lambert albedo for a Lambertian surface. After discussing the capabilities and limitations of the pipeline software system, we demonstrate its application on several multispectral data cubes-particularly, the outer reaches of the northern ice cap of Mars, the Tyrrhena Terra area that is northeast of the Hellas basin, and an area near the landing site for the Phoenix mission in the northern plains. For the icy spectra near the northern polar cap, aerosols need to be included in order to properly correct for the CO2 absorption in the H2O ice bands at wavelengths near 2.0 mum. In future phases of software development, we intend to use CRISM data directly in order to retrieve the spatiotemporal maps of aerosol optical depths, surface pressure, and surface temperature. This will allow a second level of refinement in the atmospheric and thermal correction of CRISM multispectral data.

Abstract

Vertical profiles of NO2 and NO have been obtained from solar occultation measurements by the Atmospheric Chemistry Experiment (ACE), using an infrared Fourier Transform Spectrometer (ACE-FTS) and (for NO2) an ultraviolet-visible-near-infrared spectrometer, MAESTRO (Measurement of Aerosol Extinction in the Stratosphere and Troposphere Retrieved by Occultation). In this paper, the quality of the ACE-FTS version 2.2 NO2 and NO and the MAESTRO version 1.2 NO2 data are assessed using other solar occultation measurements (HALOE, SAGE II, SAGE III, POAM III, SCIAMACHY), stellar occultation measurements (GOMOS), limb measurements (MIPAS, OSIRIS), nadir measurements (SCIAMACHY), balloon-borne measurements (SPIRALE, SAOZ) and ground-based measurements (UV-VIS, FTIR). Time differences between the comparison measurements were reduced using either a tight coincidence criterion, or where possible, chemical box models. ACE-FTS NO2 and NO and the MAESTRO NO2 are generally consistent with the correlative data. The ACE-FTS and MAESTRO NO2 volume mixing ratio (VMR) profiles agree with the profiles from other satellite data sets to within about 20% between 25 and 40 km, with the exception of MIPAS ESA (for ACE-FTS) and SAGE II (for ACE-FTS (sunrise) and MAESTRO) and suggest a negative bias between 23 and 40 km of about 10%. MAESTRO reports larger VMR values than the ACE-FTS. In comparisons with HALOE, ACE-FTS NO VMRs typically (on average) agree to ±8% from 22 to 64 km and to +10% from 93 to 105 km, with maxima of 21% and 36%, respectively. Partial column comparisons for NO2 show that there is quite good agreement between the ACE instruments and the FTIRs, with a mean difference of +7.3% for ACE-FTS and +12.8% for MAESTRO.
