Asia is home to four of the world′s five largest diabetic populations, two of them being South Asian nations, namely, India and Pakistan. This problem is compounded by a substantial rise in the elderly population in Asian countries. On the other hand, the heterogeneous health condition and multiple co-morbidities make the care of chronic disease in the elderly a challenging task. The aim of the South Asian Consensus Guidelines is to provide evidence-based recommendations to assist healthcare providers in the rational management of type 2 diabetes mellitus in the elderly population. Current Guidelines used systematic reviews of available evidence to form its key recommendations. No evidence grading was done for the purpose of this manuscript. The clinical questions of the guidelines, the methodology of literature search, and medical writing strategy were finalized by consultations in person and through mail. The South Asian Consensus guideline emphasizes tailoring of glycemic goals for patients based on age, co-morbid conditions especially that of cardiovascular system, risk of hypoglycemia, and life expectancy. It also recommends cautious use of available pharmacotherapy in geriatric patients with diabetes. The primary principle of diabetes therapy should be to achieve euglycemia, without causing hypoglycemia. Appropriate use of modern insulins and oral drugs, including incretin mimetics will help physicians achieve this aim.

Type 2 diabetes mellitus (T2DM) is a progressive disease associated with significant morbidity and mortality. Even though progress have been accomplished in the management of type 2 diabetes, current treatment preferences for patients with this disease still fall short to address disease progression. With the present therapy, glycaemic control remains suboptimal and are often associated with weight gain and hypoglycaemia. Glucagon like peptide-1 (GLP-1) is an incretin hormone secreted from the small intestine that lowers fasting and postprandial glucose through multiple mechanisms including glucose-dependent insulin secretion, reduction of glucagon secretion, delaying gastric emptying and increased satiety. Liraglutide, a human glucagon-like peptide 1 (GLP-1) analogue is a treatment for T2DM that is administered as a once-daily subcutaneous injection. The efficacy and tolerability of liraglutide at doses of 0.6, 1.2, and 1.8 mg for T2DM, in combination with, and compared with, other T2DM treatments were investigated in the Liraglutide Effect and Action in Diabetes (LEAD) Phase III clinical trial program. In the LEAD trial, treatment with liraglutide was associated with substantial improvements in glycaemic control and low risk of hypoglycaemia. In addition liraglutide significantly improved β-cell function, reduced systolic blood pressure (BP) and induced weight loss. Overall, liraglutide was well tolerated. Recent data on safety and efficacy of liraglutide from real-life clinical practice settings also reiterate the better therapeutic profile of this molecule. Based on results from the LEAD programme, and real-life clinical experience, liraglutide has been demonstrated as an effective therapeutic intervention even at the early stage of diabetes regardless of with what, it has been used.

This brief article describes a numerical index designed to assess the relative contribution of fasting and postprandial glucose to hyperglycaemia. This helps plan appropriate insulin and oral glucose-lowering therapy in an objective manner. It also reviews three similar indices described earlier in literature. Such indices need to be validated in large, multicentric trials, and have the potential to bring objectivity to choice of treatment of diabetes.

Gestational diabetes mellitus (GDM) is associated with an increased risk of complications for both mother and baby during pregnancy as well as in the postpartum period. Screening and identifying these high-risk women is important to improve short- and long-term maternal and fetal outcomes. However, there is a lack of international uniformity in the approach to the screening and diagnosis of GDM. The main purpose of this review is to provide an update on screening for GDM and overt diabetes during pregnancy, and discuss the controversies in this field. We take on debatable issues such as adoption of the new International association of diabetes and pregnancy study groups criteria instead of the Carpenter and Coustan criteria, one-step versus two-step screening, universal screening versus high-risk screening before 24 weeks of gestation for overt diabetes, and, finally, the role of HbA1c as a screening test of GDM. This discussion is followed by a review of recommendations by professional bodies. Certain clinical situations, in which a pragmatic approach is needed, are highlighted to provide a comprehensive overview of the subject.

“My mother‑in‑law thinks I had diabetes since childhood,” cried our patient during a follow‑up visit, 3 days after having been diagnosed with gestational diabetes mellitus (GDM). “I have nothing to do with this treatment,” declared her husband as the insulin prescription was handed to him, adding that her brothers should bear the expenses of such ‘‘costly’’ therapy. A few days later, she suffered intrauterine fetal loss. Quite predictably, marital discord followed. Improbable as it may seem, such lack of empathy to antenatal women is not uncommon in societies with gender discrimination. The diagnosis of GDM is accompanied by significant psychological complications. Maternal stress is not the only negative impact that comes with GDM. A ‘‘diabetic’’ label may lead to family conflicts, especially in gender‑bias cultures. Suggestions, such as the existence of diabetes prior to marriage, purposely not disclosed to in‑laws, are common. Other innuendo that women face include opinions about GDM being her fault, as she eats too much, or does not carry out her fair share of domestic chores. Maternal concerns include fear for the unborn child’s health, and one’s own biological, social, and nutritional health. While we understand that this sounds unreal to some readers, it is harsh reality for many. Seen not only in India, studies around the globe also validate the effect of psychological ill effects with diagnosis of GDM. A Canadian study described the experiences of women diagnosed with GDM as ‘‘living a controlled pregnancy,’’ followed by a process of adaptation to the diagnosis while burdened by the moral obligation to be a responsible mother and being worried about potential impact on future health. [1] Another study from the United States on women with GDM and type 2 diabetes identified three primary themes such as concern for the infant, concern for self, and sensing a loss of personal control over their health. [2] An elegant study exploring the experiences of Swedish women describes the diagnosis of GDM as a process of ‘‘stun to gradual balance.’’ [3] Evidence suggests that a diagnosis of GDM may increase a woman’s anxiety, [4] result in poorer health perceptions and a less positive pregnancy experience when compared with nondiabetic controls. [5] The maternal exposure to unwanted stress, especially with diagnosis of GDM may, in fact, increase the risk of type 1 and type 2 diabetes in the unborn offspring. [6,7] A possible mediator is increased corticosteroid exposure to the fetus, which is inadequately protected by an ‘‘immature 11 β‑hydroxysteroid dehydrogenase system in the placenta during the early pregnancy.’’ [8]

This brief communication utilizes the teachings of Bhagavad Gita to help in coping with illness, especially chronic illness such as diabetes. The article cites verses from the Bhagavad Gita to show how one may cope with the stress of illness such as diabetes.

Thyromimetic agents that can treat dyslipidemia without adverse effects like cardiac arrhythmias and osteoporosis are attractive options. Initial experience with desssicated thyroid hormone extract and DT4 were disappointing. Thyroid hormone has nuclear action with four receptor isoforms- TR α1, TRα2, TRβ1, TRβ2. TR α1 has predominant effects on CVS, TRβ2 acts mainly on the pituitary and TRβ1 has hepatoselective action and decrease cholesterol levels. Eprotirome and Sobetirome are 2 thyromimetics that have selective TRβ1 activity. They act in dyslipidemia by multiple mechanisms. They are presumably safe on the pituitary- thyroid axis.

With the recent explosion in endocrine conferences, audience fatigue has set in and conference planners are now looking at newer pedagogic methods to revive the interest of audiences in these conferences. An unprecedented rise in diabetes mellitus (DM) prevalence in India is the outcome of lifestyle changes in the background of genetic predisposition. Moreover, there are substantial regional variations in diabetes prevalence and management. The highest prevalence of DM was observed in southern region (Ernakulum, Kerala) and lowest prevalence was observed in North Eastern region (Manipur). Similarly large variations have been evident in overall awareness and diabetes care across the geographies within India. The regional challenges are largely affected by poor disease awareness, socioeconomic disparity and underutilization of the public health-care services. Though government has taken initiatives to address this issue, overall situation demands a collaborative effort from patients, health care professionals and the state. An exhaustive literature search was performed for articles and studies published on electronic databases. Present article assesses the regional disparity of diabetes epidemiology, current management practices and government policies for T2DM in India, identifies policy and research gaps, and suggests corrective measures to address the lacunae in diabetes care.

The second diabetes attitudes wishes and needs 2 (DAWN 2) study describes various facts of living with diabetes, including barriers to, and facilitators of effective psychosocial care. One of the issues studied by DAWN2 is the phenomenon of discrimination, as viewed by persons with diabetes, family members of persons with diabetes, and health care professionals. This brief communication discusses the high prevalence of discrimination against people with diabetes, as reported by DAWN2.

We read the analysis of the Saxagliptin Assessment of Vascular Outcomes Recorded in Patients with Diabetes Mellitus–Thrombolysis in Myocardial Infarction 53 (SAVOR-TIMI 53) trial by Udell et al. (1) with interest. The important finding is the increased risk of hospitalization for heart failure with increasing severity of nephropathy. This highlights yet another association between macrovascular and microvascular complications in diabetes. Pituitary apoplexy (haemorrhage or infarction) usually occurs in patients, who were not previously diagnosed to have pituitary adenomas. There are conflicting reports on the commonest types of tumour responsible for apoplexy. Some studies have shown predominance of non-functional adenomas, other studies have reported higher prevalence in functioning adenomas among which prolactinomas have the highest risk, and other studies have shown equal prevalence among functioning and non-functioning tumours. Prospective follow-up studies have reported the occurrence of pituitary apoplexy in previously diagnosed non-functioning pituitary adenomas. Several retrospective studies have reported occurrence of pituitary apoplexy among patients with previously diagnosed functioning as well non-functioning tumours. Pituitary apoplexy has been reported as early as 6 months to as late as 18 years after the diagnosis of functioning tumours. Resolution of hormonal hypersecretion in previously known functional tumours has also been reported in some studies.

Well-conducted randomized controlled trials are instrumental in providing vital data on safety and efficacy of new molecules under consideration for approval. However acquiring such data involves huge cost and focused scientific endeavor. Selection and reporting of endpoints of a therapy is essential to assess the effect(s) of an intervention on overall disease control and guidelines have suggested the use of composite endpoint (CEP) in clinical trial design over individual endpoints to demonstrate the compound effect. Composite endpoints have been preferred for their ability to assess the net clinical benefit of an intervention, avoid misinterpretation associated with competing risks, avoid the challenge of using a single outcome to validate the study objectives and reduce the sample size requirements in trials on patients treated for diabetes. Concerns for misinterpretation or difficulty in interpretation of trial results involving CEPs arise when differences in the components with respect to either clinical importance or event rates, or magnitude of treatment effect exist and when there's a possibility of biases due to competing risk. Suggestions for construction of composite endpoints and reporting the results of trials involving CEPs have been presented to improve the interpretations of overall effect of new interventions.

This communication presents verses from the Bhagavad Gita which help define a good clinician's skills and behavior. Using the teachings of Lord Krishna, these curated verses suggest three essential skills that a physician must possess: Excellent knowledge, equanimity, and emotional attributes. Three good behaviors are listed (Pro-work ethics, Patient-centered care, and Preceptive leadership) and supported by thoughts written in the Gita.

Liraglutide (victoza, Novo Nordisk A/S) is human GLP-1 analogue developed by recombinant DNA technology. It is indicated along with diet and exercise in management of type 2 diabetes (T2DM) in adults. Liraglutide has been made available in India recently. Present review evaluates the efficacy and safety of liraglutide in T2DM and its comparison with other incretin based therapies. Liraglutide has been evaluated as monotherapy, in combination with one, two and three oral antidiabetic drugs similarly to routine clinical practice. These studies reported greater improvement in glycaemic control with liraglutide compared with comparators. Evaluation up to 2 years revealed sustained improvement in glycaemic control with liraglutide use. Liraglutide was well tolerated except for mild to moderate gastro-intestinal adverse events, which declined after continuation of therapy. Low risk of hypoglycaemia was reported with liraglutide therapy. Greater efficacy than other incretin based therapies was noted with liraglutide. Liraglutide has an important place in the management of T2DM. Apart from glycaemic control it also provides some important non-glycaemic benefits in terms of improving beta-cell function, weight reduction, and reduction in systolic blood pressure thereby overcoming the present therapeutic gap.

We appreciate the efforts by Schwartz et al. (1) to synthesize current scientific knowledge in a simple and lucid manner and hope that the following suggestions will help improve the utility of their proposed β-cell–centric classification. It is less important to “classify” diabetes than it is to ensure appropriate treatment for the person with diabetes. In the month of Ramadan, patients with thyroid diseases, most of the time, do not need treatment adjustments and can fast safely without any health hazards. Patients with hypothyroidism taking thyroxine can take their tablets on an empty stomach at bedtime instead of half an hour before Sehr. Patients with hyperthyroidism, on methimazole/carbimazole can continue their dose in once or twice daily regimes, while those on propylthiouracil need to be switched. Hyperthyroid patients with severe symptoms should start treatment immediately and can avoid fast for few days after a consultation with their religious scholar.

Information on vitamin D status of Indian health care professionals is limited. Among 2,119 subjects studied, just 6 % were found to be sufficient in vitamin D status. There is urgent need of an integrated approach to detect and treat vitamin D deficiency among health care professionals to improve on-the-job productivity.Vitamin D deficiency is prevalent worldwide. India has been reported to be one of the worst affected countries. Several single-center studies from India have shown high prevalence of vitamin D deficiency. Little is known regarding the vitamin D status of Indian health care professionals.This study aimed to determine prevalence of vitamin D deficiency among health care professionals in different regions of India.In this cross-sectional, multicenter study, we enrolled 2,119 medical and paramedical personnel from 18 Indian cities. Blood samples were collected from December 2010 to March 2011 and analyzed in a central laboratory by radioimmunoassay. Vitamin D deficiency was defined as 25-hydroxyvitamin D [25(OH)D]20 ng/mL or50 nmol/L, insufficiency as 25(OH)D = 20-30 ng/mL or 50-75 nmol/L, and sufficiency as 25(OH)D30 ng/mL or75 nmol/L.Mean (±SD) age of subjects was 42.71 ± 6.8 years. Mean (±SD) 25(OH)D level was 14.35 ± 10.62 ng/mL (median 11.93 ng/mL). Seventy-nine percent of subjects were deficient, 15 % were insufficient, and just 6 % were sufficient in vitamin D status. No significant difference was found between vitamin D status in southern (25(OH)D = 13.3 ± 6.4 ng/mL) and northern (25(OH)D = 14.4 ± 8.5 ng/mL) parts of India.Our study confirms the high prevalence of vitamin D deficiency all across India in apparently healthy, middle-aged health care professionals.

Limitations of conventional human basal insulins like NPH have led to the development of more stable and peak less analogs. However, the first generation of basal analogs like glargine and detemir has certain shortcomings which do not allow them to be termed ideal basal insulins. Degludec, a novel basal insulin analog has the potential to overcome these limitations. This paper reviews the potential advantages of degludec over existing basal insulins and analogs. It discusses the basic and clinical studies performed on degludec so far, and highlights the possible role this molecule can play in the management of diabetes mellitus. In this paper, the recent patents on basal insulin have been reviewed so as to provide an insight into the advances in this field. In this article, we present a review of Degludec, as well as related patents.

Strict glycemic control helps in reducing complications and mortality in patients with diabetes. Insulin remains the mainstay in treatment of diabetes. One of the most important barriers to insulin therapy is the use of conventional insulin delivery system, which remains time-consuming, cumbersome, inconvenient and painful. Insulin dosing via syringe is associated with a high risk of dosage errors. The modern pen devices have various advantages over the conventional insulin delivery methods. The user-friendliness and comfort of injection makes the initiation of insulin easier. The ease of use of insulin pens and the flexibility of incorporating insulin injections into a busy lifestyle may improve diabetes control with much less effort, while maintaining the quality of life for the diabetic patients. Reusable and prefilled pens are available.

A 43-year-old man from a remote part of India (over 1800 km from our institution), presented with a headache of 3 years duration. He was of short stature, had delayed puberty and was mentally retarded. On evaluation he was detected to have primary hypothyroidism with markedly elevated thyroid-stimulating hormone titres. A CT of the brain revealed a large sellar mass with suprasellar extension into the third ventricle causing obstructive hydrocephalus. Surgical intervention was deferred due to absence of visual impairment and the presence of gross hypothyroidism. The clinical diagnosis of congenital hypothyroidism was confirmed by the absence of radioiodine uptake in the thyroid bed. With thyroid hormone replacement therapy, the 'tumour' underwent significant reduction in size with the resolution of hydrocephalus thereby favouring a potential pituitary pseudotumour. This was an unusual situation of a giant pituitary pseudotumour detected in an adult with untreated congenital hypothyroidism.

Hospitalized patients with diabetes pose numerous clinical challenges, including hyperglycemia, which may often be difficult to control. The therapeutic challenges are further accentuated by the difficulty in practical application of existing guidelines among Indian and South Asian patients. The present review highlights the various clinical challenges encountered during management of different diabetic hospitalized populations, and attempts to collate a set of practical, patient and physician friendly recommendations to manage hyperglycemia in such patients.

The Buddhist Lent is an annual three lunar month long fast observed by devout Buddhists. This involves fasting daily from noon to midnight, abstaining from all solid foods. Only liquids and jaggery are allowed during this period. This paper discusses the unique nutritional challenges posed by this fast for people with diabetes, and suggests practical recommendations to ensure healthy nutrition and successful fasting for them.

Recently published guidelines on Psychosocial Management of Diabetes in India provide evidence-based recommendations for the whole nation. However, they do not fully address the myriad socio-cultural issues prevalent in the North Eastern states. The eight North Eastern states of India house 45 million people, belonging to 220 ethnic groups who speak an equal number of dialects, and follow distinctly unique cultures, which impact health-related behavior. Such diversity is difficult to cover in any national guideline. This lacuna makes it necessary to have a document, which addresses the specific needs and requirements of diabetes care professionals in the North-east of India. This consensus statement aims to highlight evidence- and experience-based strategies for psychosocial management of diabetes, based upon the unique ethnographic constitution of this part of the country. It is based upon the results of a daylong focused group discussion, held at Sonapur, Assam, on 9th February 2013, involving key opinion leaders from most North-eastern states, including all geographical divisions of Assam. Recommendations are classified into three domains: General, psychological, and socio-cultural, and graded by the weight they should have in clinical practice. Eighteen recommendations of varying strength are made, to help professionals identify the psycho-socio-cultural determinants of diabetes, and to explore the role of psycho-socio-cultural interventions in devising support strategies for people with diabetes and their families. They also aid in developing core skills needed for effective diabetes management. These recommendations provide practical guidelines to fulfill unmet needs in diabetes management in the North-east and help achieve a qualitative improvement in diabetes care. The guidelines may also be useful for diabetes care professionals working with other indigenous groups across the world.
