OBJETIVO. Determinar la prevalencia del consumo de drogas y bebidas alcohólicas entre estudiantes, así como la relación de este consumo con variables sociodemográficas, tiempo libre, actos antisociales, normas y conflictos familiares, entre otras. MATERIAL Y MÉTODOS. Los datos se derivan de una encuesta representativa de 1 929 estudiantes de enseñanza media y media superior de la ciudad de Pachuca, estado de Hidalgo, México, levantada en 1996, de los cuales 44.9% son hombres y 52.5% mujeres, con una media de edad de 14 años. Se utilizó un cuestionario autoaplicable, que incluye indicadores de consumo de drogas y alcohol elaborados por algunos países, entre ellos México, con la Organización Mundial de la Salud. RESULTADOS. El 47.9% ha probado bebidas alcohólicas; 12.6% bebieron cantidades considerables -cinco copas o más por ocasión- durante el mes anterior a la encuesta. Las bebidas preferidas son la cerveza y los coolers; el alcohol lo compraron principalmente en tiendas donde no les piden identificación y lo consumen en su casa o en la de otras personas. El 5.1% ha consumido drogas, sobre todo inhalables, mariguana y tranquilizantes. Una cifra mayor de varones consume drogas ilegales, y las mujeres, medicamentos sin prescripción. Un número superior de hombres toma más cantidades de alcohol y consume drogas, tienen mayor edad y trabajaron de medio tiempo durante el año anterior a la encuesta. Los consumidores de drogas y de altas cantidades de alcohol se distinguieron porque un número considerable informó que se aburría en su tiempo libre, se va a beber con sus amigos o ha cometido actos antisociales. Respecto a la familia, manifiestan cumplir menos con las normas parentales y muestran menor interés en hacerlo. Asimismo, un porcentaje más elevado informó que sus padres pelean con frecuencia, han pedido ayuda por esa razón y han intentado separarse. CONCLUSIONES. Los estudiantes que beben cantidades elevadas de alcohol y los consumidores de otras drogas, a diferencia de lo que sucede con los abstemios, presentaron más problemas al destinar su tiempo libre a actividades como irse a beber con amigos, exhibir más conductas antisociales y tener un distanciamiento de la familia que se refleja en la baja participación en actividades compartidas, en el desinterés por cumplir con sus normas y en la percepción de conflictos al interior de la misma.OBJECTIVE. To determine the prevalence of alcohol and drug consumption and its relationship to sociodemographic variables, leisure activities, antisocial behavior, family norms and conflicts, among others. MATERIAL AND METHODS. Data derive from a representative survey of 1 929 students of junior high and high school, conducted in 1996 in the city of Pachuca, Hidalgo, Mexico. Of these, 44.9% were boys and 52.5% were girls; mean age was 14. A self-applied questionnaire, prepared by the WHO together with some countries, among them Mexico, was completed by the studied subjects, and included indicators of alcohol and drug consumption. RESULTS. Of the total sample, 47.9% had tried alcohol, and 12.6% had drunk large quantities -5 drinks or more per sitting- during the month previous to the survey. Preferred drinks are beer and "coolers", which they buy at shops where no identification is required and drink at home or at friend's homes. With respect to drugs, 5.1% had tried illegal or medical drugs without prescription, in particular inhalants, marihuana and tranquilizers. More boys consumed illegal drugs, and more girls medical drugs without prescription. Boys, who are also older, more frequently consumed alcohol and drugs and were more often employed during the previous year at part-time jobs. High alcohol level and drug consumers were characterized by their frequent report of being bored in their free time, drinking with friends and enrolling in antisocial behavior. With respect to family norms, they follow them less and show less interest in doing so. An elevated percentage informed that their parents fight frequently, that they have sought help for this reason and have intended separation. CONCLUSIONS. Groups who drink more alcohol and use other drugs, in contrast with non-users, presented more behavioral problems, more outdoors activities that included drinking with friends, more antisocial behavior, had a distant relationship with their families sharing few activities with them, an showed little interest in following family rules and perceiving conflicts within their families.

Se estudió la prevalencia del consumo y el abuso en la ingestión de alcohol entre los casos de urgencias médicas, accidentes y violencias atendidaos en las salas de urgencias de tres hospitales del sector salud de la ciudad de Pachuca, Hidalgo. Material y métodos. Se aplicó un cuestionario y se hizo una medición de alcohol en sangre a las personas que solicitaron atención por primera vez en estos servicios de urgencias. El cuestionario recabó datos demográficos, patrones de consumo de alcohol, escalas para medir la dependencia y el consumo riesgoso de alcohol, tales como el CAGE y el Alcohol Use Disorders Identification Test (AUDIT). Se comparó, por medio de la prueba ji², el consumo de alcohol entre los pacientes que ingresaron por un accidente o acto de violencia y aquellos que ingresaron debido a una urgencia médica. Resultados. Se evaluaron 1 511 pacientes. El consumo fue mayor y más frecuente en los pacientes que ingresaron por un accidente o porque fueron víctimas de la violencia, en comparación con las urgencias médicas. De los pacientes con accidentes o víctimas de la violencia, 17.7% fueron positivos al alcosensor y 15.8% notificaron consumo de alcohol en las seis horas previas al accidente. El CAGE detectó como dependiente a 9.2% de los casos, y 10.9% calificaron como bebedores riesgosos según el AUDIT. Conclusiones. El consumo de bebidas alcohólicas, especialmente en los pacientes atendidos en los servicios de urgencias debido a un accidente o acto de violencia, es elevado. Son indispensables las medidas preventivas para disminuir el costo individual y social del abuso en el consumo de alcohol en esta población.Objective. Prevalence of alcohol consumption and abuse among patients with medical emergencies, accidents and violence, attending an emergency room in one of three public health hospitals in Pachuca, Hidalgo, was studied. Material and methods. Patients answered a questionnaire and their blood alcohol was measured on their first visit to the emergency room. The questionnaire gathered socio-demographic data, patterns of alcohol consumption and included scales for heavy and dependent drinkers, such as the CAGE and the alcohol use disorders identification test (AUDIT). Injured and non-injured patients were compared with the chi² statistics. Results. A total of 1 511 patients were evaluated. Alcohol consumption was higher in those attending for accidents or violence than in medical patients. Positive blood alcohol levels were found in 17.7% of injured patients and 15.8% reported alcohol consumption 6 hours prior to the accident. According to the CAGE, 9.2% of the patients were alcohol-dependent, and 10.9% were heavy drinkers according to the AUDIT. Conclusions. Alcohol consumption, especially among emergency room patients attending for accidents or injuries is high. Preventive measures are indispensable to diminish social and individual costs of alcohol abuse in this population.

Objetivo. Determinar la prevalencia de abuso sexual en estudiantes de secundaria y preparatoria, hombres y mujeres, de todo el país, así como su relación con el consumo de drogas. Material y métodos. Los datos fueron obtenidos de la Encuesta Nacional de Uso de Drogas en la Comunidad Escolar, llevada a cabo en noviembre y diciembre de 1991, en la que fueron encuestados un total de 61 779 alumnos, 51.8% hombres y 47.1% mujeres, con una media de edad de 14.4 años. Se utilizó un instrumento autoaplicado, en el que el abuso sexual fue explorado tanto desde la perspectiva de quienes lo han experimentado -víctimas-, como desde quienes lo han ejercido -agresores. Resultados. La prevalencia de adolescentes víctimas de abuso sexual fue de 4.3%, y no se encontraron diferencias estadísticamente significativas entre sexos. La prevalencia de agresores fue de 2.5%; los adolescentes varones habían coercionado sexualmente a otra persona en proporción significativamente mayor que las mujeres. Estas sufrieron el abuso a edades menores en un porcentaje significativamente más alto que los hombres. Asimismo, notificaron una proporción más elevada de abusos por parte de familiares, mientras que los hombres mencionaron principalmente a los amigos como los agresores más frecuentes. Tanto las víctimas como los agresores de ambos sexos, reportaron un consumo de drogas significativamente mayor que los estudiantes sin estos antecedentes. Conclusiones. Se enfatizan las diferencias en la experiencia de abuso sexual de mujeres y hombres como víctimas y como agresores. En particular, se discute el hecho de que el abuso sexual en varones sea principalmente extrafamiliar, así como el riesgo mayor que tienen las mujeres de ser víctimas de abuso sexual intrafamiliar en edades tempranas. Asimismo, se plantea la necesidad de abordar las consecuencias, en la salud mental, del abuso sexual infantil y adolescente y del consumo de drogas, considerando las particularidades de cada género.Objective. To determine the prevalence of sexual abuse among high school (secondary and preparatory) students, male and female, throughout Mexico, and its relationship with drug abuse. Material and methods. Data were obtained from the National Survey of Drug Use in Schools applied in November and December, 1991. A total of 61 779 students, 51.8% men and 47.1% women, with a mean age of 14.4 years completed the self-applied questionnaire. Sexual abuse was explored from the perspective of the abusers and of the victims. Results. The prevalence of sexual abuse in adolescent victims was 4.3% and no statistically significant differences were found between sexes. The prevalence of sexual aggressors was 2.5%. Men coerced someone else in a higher proportion than women. Adolescent women experienced sexual abuse at a younger age than men and they also reported a higher percentage of intrafamily abuse. Men reported friends as the most frequent aggressors. Victims and aggressors of both sexes reported a significantly higher drug consumption than students without these antecedents. Conclusions. The differences in the experience of sexual abuse between men and women are described. In particular, the fact that sexual abuse in men mainly occurs outside the family sphere, while in women it is mainly within the family and at a younger age than in men. Additionally, the need for further research focusing on the consequences on mental health of infantile and adolescent sexual abuse and drug consumption is emphasized, considering the characteristics of each gender.

In 1988, the General Directorate of Epidemiology and the Mexican Institute of Psychiatry conducted the first National Addiction Survey (ENA), providing regional and national data on alcohol, tobacco, and drug use. The ENA providing a subsample of women who have been pregnant at some time in their lives. There were 5,234 affirmative responses. Women were asked if they had suffered any of three adverse outcomes during their last pregnancy: spontaneous abortion, stillbirth, and congenital abnormalities. Prevalence of spontaneous abortion was 3.8%, stillbirth 1.2%, and congenital abnormalities 1.1 %. Multiple logistic-regression models were used to analyze the effect of alcohol consumption on these problems. Consumption during pregnancy was related only with the prevalence of congenital abnormalities, with prevalence odds of 3.4. Among habitual users during the last 12 months, oniy women in the highest use category showed an important relationship with the three problems mentioned. Follow-up studies on the Mexican population are recommended in order to obtain more conclusive findings.En l988 la Dirección General de Epidemiología y el Instituto Mexicano de Psiquiatría llevaron a cabo la primera Encuesta Nacional de Adicciones (ENA), que proporciona información a nível nacional y regional sobre el uso de alcohol, tabaco y drogas ilícitas. La ENA incluye una submuestra de mujeres que se embarazaron alguma vez en la vida. Se obtuvieron 5.234 respuestas afirmativas a la pregunta de se alguma vez se habían embarazado. A este grupo se les preguntó si en su último embarazo tuvieron un aborto espontáneo, un bebe nacido muerto o un bebe con anomalías congênitas. La prevalencia de aborto espontáneo fue de 3,8%, 1,2% de nascidos muertos y 1,1% de anomalías congénitas. Se utilizaron modelos de regressión logística múltiple para analizar el papel del consumo de bebidas alcohólicas sobre estos problemas. El consumo de alcohol durante el embarazo sólo se asoció con la prevalencia de anomalias congênitas, con una razón de prevalencia de 3,4. Entre las bebedoras habituales de alcohol en los últimos 12 meses, sólo las mujeres que se ubicaron en la más alta categoria de consumo mostraron relaciones importantes con los tres problemas mencionados. Se sugiere la necesidad de realizar estudios de seguimento para llegar a conclusiones más definitivas.

OBJETIVO: O objetivo do estudo foi examinar o papel do gênero no endosso dos sintomas incluídos tanto na Classificação Internacional de Doenças-10ª Edição quanto no Manual Diagnóstico e Estatístico de Transtornos Mentais-4ª Edição. MÉTODO: Seiscentos pacientes tratados por problemas ligados ao álcool em serviços ambulatórios de saúde no México foram avaliados com o Módulo sobre Abuso de Substâncias da Composite International Diagnostic Interview. RESULTADOS: A análise fatorial confirmatória com a Classificação Internacional de Doenças-10ª Edição e o Manual Diagnóstico e Estatístico de Transtornos Mentais-4ª Edição produziu dois fatores, que incluíram uma combinação de abuso/uso nocivo de álcool e sintomas de dependência, que explicaram 40% e 49,2% da variância total, respectivamente. No grupo de pacientes abuso/uso nocivo, os sintomas e as consequências sociais diferiram entre os gêneros: síndrome de abstinência, falta de controle e problemas jurídicos foram mais frequentes nos homens, enquanto as mulheres apresentaram maiores taxas de tentativas de abandonar o álcool e dificuldades para realizar atividades diárias. Fatores específicos de gênero diferenciaram abuso/uso nocivo da dependência, como a perda de controle e o tempo gasto para beber, no caso dos homens, e do desejo de beber entre as mulheres, de acordo com os critérios de dependência do Manual Diagnóstico e Estatístico de Transtornos Mentais-4ª Edição, e presença de sintomas físicos em homens, problemas sociais/família, e desejo e esforço fútil de parar de beber em mulheres, segundo o critério de dependência da Classificação Internacional de Doenças-10ª Edição. CONCLUSÃO: Futuros sistemas de classificação da toxicomania devem levar em conta as diferenças entre os sexos, a fim de ajudar a suprimir a lacuna de tratamento para as mulheres.OBJECTIVE: The objective of the study was to examine the role of gender in the endorsement of symptoms included in both the International Classification of Diseases-10th Edition and the Diagnostic and Statistical Manual of Mental Disorders-4th Edition. METHOD: Six hundred patients treated for alcohol-related problems in outpatient services in Mexico were evaluated with the Substance Abuse Module of the Composite International Diagnostic Interview. RESULTS: Confirmatory factor analyses using the International Classification of Diseases-10th Edition and Diagnostic and Statistical Manual of Mental Disorders-4th Edition criteria produced two factors that included a combination of abuse/harmful alcohol use and dependence symptoms, which explained 40% and 49.2% of the total variance, respectively. In the abuse/harmful use groups of patients, symptoms and social consequences differed according to gender: withdrawal syndrome, lack of control and legal problems were more frequent in men, while women exhibited higher rates of attempts to give up alcohol and difficulties to accomplish daily activities. Specific gender-related factors differentiated abuse/harmful use from dependence, such as loss of control and time spent to drink in the case of men and desire to drink among women, according to the Diagnostic and Statistical Manual of Mental Disorders-4th Edition dependence criteria; and presence of physical symptoms in men and family/social problems, craving, and futile effort to stop drinking in women, according to the International Classification of Diseases-10th Edition dependence criteria. CONCLUSION: Future classification systems of substance abuse disorders should take into account differences between genders in order to help closing the treatment gap for women.

OBJECTIVE: To estimate the prevalence and the association of Traumatic Life Events (LEs) and Posttraumatic Stress Disorder (PTSD) among the Mexico City Metropolitan Area (MCMA) adolescent population. MATERIAL AND METHODS: Adolescents aged 12 to 17 were administered the adolescent version of the World Mental Health Composite International Diagnostic Interview (n=3 005). Data were collected using a stratified, multistage and probability sample. Prevalence, odds ratios and 95% confidence intervals for LEs and PTSD (assessed with DSM-IV criteria) were obtained. RESULTS: The percentage of adolescents reporting at least one traumatic event in their lifetimes was 68.9%, with differences by sex. Prevalence for PTSD were 1.8% (2.4% females and 1.2% males), and sexual-related traumas were the LEs most associated with PTSD [OR=3.9 (CI95%=1.8-8.2)], adjusted by sex, education and age. CONCLUSIONS: Exposure to traumatic life events is not uncommon among Mexico City adolescents. Effort should be made to reduce child and adolescent sexual abuse, a very traumatic event highly associated with PTSD.OBJETIVO: Estimar la prevalencia y la asociación entre los Eventos Traumáticos (ET) y Trastorno de Estrés Postraumático (TEPT) en la población adolescente del Área Metropolitana de la Ciudad de México (AMCM). MATERIAL Y MÉTODOS: 3 005 adolescentes del AMCM entre 12 y 17 años fueron entrevistados empleando la versión para adolescentes de la Entrevista Dia gnóstica Internacional de Salud Mental, en una muestra probabilística, estratificada y multietápica. RESULTADOS: 68.9% de los adolescentes en el AMCM reportaron por lo menos un ET alguna vez en su vida, con diferencias por sexo. La prevalencia de TEPT fue 1.8% (2.4% mujeres y 1.2% hombres), y el abuso sexual se asoció al desarrollo de TEPT [OR=3.9(CI95%=1.8-8.2)], independientemente del sexo, educación o edad. CONCLUSIONES: La exposición a ET es común en los adolescentes. Se debe poner énfasis en los esfuerzos que buscan reducir el abuso sexual en la infancia y la adolescencia, ya que se asocia fuertemente al TEPT.

Objective: To study immigration, U.S. nativity, and return migration as risk factors for violence among people of Mexican origin in the U.S. and Mexico. Methods: Cross-sectional surveys in the United States (2001-2003; n=1,213) and Mexico (2001-2002; n=2,362). Discrete time survival models were used. The reference group was Mexicans living in Mexico without migrant experience or a migrant relative. Results: Mexican immigrants in the U.S. have lower risk for any violence (hazard ratio [HR] = 0.5, 95% confidence interval [95%CI] 0.4-0.7). U.S.-born Mexican-Americans were at higher risk for violence victimization of a sexual nature (for sexual assault, HR = 2.5, 95%CI 1.7-3.7). Return migrants were at increased risk for being kidnapped or held hostage (HR = 2.8, 95%CI 1.1-7.1). Compared to those without a mental disorder, those with a mental disorder were more likely to suffer any violence (HR = 2.3, 95%CI 1.9-2.7), regardless of the migrant experience. Conclusions: The impact of immigration on the occurrence of violence is more complex than usually believed. Return migrants are more likely to suffer violence such as being held hostage or beaten by someone other than a partner.

In North America, opioid use and its harms have increased in the United States and Canada over the past 2 decades. However, Mexico has yet to document patterns suggesting a higher level of opioid use or attendant harms.Historically, Mexico has been a country with low-level use of opioids, although heroin use has been documented. Low-level opioid use is likely attributable to structural, cultural, and individual factors. However, a range of dynamic factors may be converging to increase the use of opioids: legislative changes to opioid prescribing, national health insurance coverage of opioids, pressure from the pharmaceutical industry, changing demographics and disease burden, forced migration and its trauma, and an increase in the production and trafficking of heroin. In addition, harm-reduction services are scarce.Mexico may transition from a country of low opioid use to high opioid use but has the opportunity to respond effectively through a combination of targeted public health surveillance of high-risk groups, preparation of appropriate infrastructure to support evidence-based treatment, and interventions and policies to avoid a widespread opioid use epidemic.

Objectives: We investigated whether Mexican immigration to the United States exerts transnational effects on substance use in Mexico and the United States., Methods: We performed a cross-sectional survey of 2336 Mexican Americans and 2460 Mexicans in 3 Texas border metropolitan areas and their sister cities in Mexico (the US-Mexico Study on Alcohol and Related Conditions, 2011-2013). We collected prevalence and risk factors for alcohol and drug use; Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, alcohol-use disorders; and 2 symptoms (hazardous use and quit or control) of drug use disorder across a continuum of migration experiences in the Mexican and Mexican American populations., Results: Compared with Mexicans with no migrant experience, the adjusted odds ratios for this continuum of migration experiences ranged from 1.10 to 8.85 for 12-month drug use, 1.09 to 5.07 for 12-month alcohol use disorder, and 1.13 to 9.95 for 12-month drug-use disorder. Odds ratios increased with longer exposure to US society. These findings are consistent with those of 3 previous studies., Conclusions: People of Mexican origin have increased prevalence of substance use and disorders with cumulative exposure to US society.
