102. Syndrome or symptoms? Assessing cothymia, neuroticism and lifetime comorbidity in a sample of psychiatric patients

Background. Cross-sectional and longitudinal psychiatric comorbidity rates could represent a syndromic process rather than the co-occurrence of different disorders. ‘Cothymia’, the concomitant presence of depression and anxiety symptoms, and the ‘neuroticism’ dimension have been proposed as candidate vulnerability factors for psychiatric disorders trajectories. Based on this background, the present research was aimed at assessing the role of cothymia and neuroticism as syndromic processes in lifetime psychiatric disorders, and examining homotypic or heterotypic trends in the diagnostic continuum of comorbid mental disorders. Materials and methods. Data collection was carried out retrospectively through the consultation of medical records at the Psychiatry Unit of the University Hospital of Messina. Cothymia was determined by the lifetime presence of depression and other anxiety disorders, and neuroticism was determined by the presence of borderline personality disorders and other personality disorders. Results. Cothymia and neuroticism were found in 580 patients with higher rates of hospitalization, drug prescription, polypharmacotherapy, and longer duration of illness. Furthermore, diagnostic trajectories in patients with cothymia and neuroticism followed both homotypic and heterotypic progressions. Limitations: This study presented several limitations. The small sample size from a restricted recruitment area, and the retrospective design may have influenced a realistic and accurate estimation of the lifetime prevalence of the disorders. Conclusions. Despite the limitations, the findings of this study confirm the presence of consistent and meaningful homotypic and heterotypic trajectories in psychiatric patients, suggesting that the evolution of lifetime diagnoses should become a focus in theoretical and practical approaches to psychiatric disorders.

Abstract Objectives To assess the incidences of Herpes Simplex-1 and 2 (HSV-1, HSV-2), Cytomegalovirus (CMV), Epstein–Barr Virus (EBV) reactivations in critically ill COVID-19 patients. To determine the association between viral reactivation and in-hospital mortality, Intensive Care Unit Bloodstream infection (ICU–BSI), ventilator-associated pneumonia (VAP). Design Observational retrospective cohort study. Setting COVID-19 Intensive Care Unit. Patients From November 2020 to May 2021, one hundred and twenty patients with COVID-19 severe pneumonia were enrolled and tested for HSV-1, HSV-2, CMV and EBV at the admission in ICU and weekly until discharge or death. The presence of VAP and ICU–BSI was evaluated according to clinical judgement and specific diagnostic criteria. Measurements and main results One hundred and twenty patients were enrolled. Multiple reactivations occurred in 75/120 (63%) patients, single reactivation in 27/120 patients (23%). The most reactivated Herpesvirus was EBV, found in 78/120 (65%) patients. The multivariate analysis demonstrated that viral reactivation is a strong independent risk factor for in-hospital mortality (OR = 2.46, 95% CI 1.02–5.89), ICU–BSI (OR = 2.37, 95% CI 1.06–5.29) and VAP (OR = 2.64, 95% CI 1.20–5.82). Conclusions Human Herpesviruses reactivations in critically ill patients with COVID-19 severe Pneumonia are associated with mortality and with a higher risk to develop both VAP and ICU–BSI.

Abstract Objective The resting‐state functional connectome has not been extensively investigated in amyotrophic lateral sclerosis (ALS) spectrum disease, in particular in relationship with patients' genetic status. Methods Here we studied the network‐to‐network connectivity of 19 ALS patients carrying the C9orf72 hexanucleotide repeat expansion (C9orf72+), 19 ALS patients not affected by C9orf72 mutation (C9orf72−), and 19 ALS‐mimic patients (ALSm) well‐matched for demographic and clinical variables. Abstract Introduction Huntington’s disease (HD) is a fatal neurodegenerative disorder caused by an expansion of the CAG trinucleotide repeat in the Huntingtin gene (HTT). Immune activation is abundant in the striatum of HD patients. Detection of active microglia at presymptomatic stages suggests that microgliosis is a key early driver of neuronal dysfunction and degeneration. Recent studies showed that deletion of Tyrobp, a microglial protein, ameliorates neuronal dysfunction in Alzheimer’s disease amyloidopathy and tauopathy mouse models while decreasing components of the complement subnetwork. Objective While TYROBP/DAP12-mediated microglial activation is detrimental for some diseases such as peripheral nerve injury, it is beneficial for other diseases. We sought to determine whether the TYROBP network is implicated in HD and whether Tyrobp deletion impacts HD striatal function and transcriptomics. Methods To test the hypothesis that Tyrobp deficiency would be beneficial in an HD model, we placed the Q175 HD mouse model on a Tyrobp-null background. We characterized these mice with a combination of behavioral testing, immunohistochemistry, transcriptomic and proteomic profiling. Further, we evaluated the gene signature in isolated Q175 striatal microglia, with and without Tyrobp. Results Comprehensive analysis of publicly available human HD transcriptomic data revealed that the TYROBP network is overactivated in the HD putamen. The Q175 mice showed morphologic microglial activation, reduced levels of post-synaptic density-95 protein and motor deficits at 6 and 9 months of age, all of which were ameliorated on the Tyrobp-null background. Gene expression analysis revealed that lack of Tyrobp in the Q175 model does not prevent the decrease in the expression of striatal neuronal genes but reduces pro-inflammatory pathways that are specifically active in HD human brain, including genes identified as detrimental in neurodegenerative diseases, e.g. C1q and members of the Ccr5 signaling pathway. Integration of transcriptomic and proteomic data revealed that astrogliosis and complement system pathway were reduced after Tyrobp deletion, which was further validated by immunofluorescence analysis. Conclusions Our data provide molecular and functional support demonstrating that Tyrobp deletion prevents many of the abnormalities in the HD Q175 mouse model, suggesting that the Tyrobp pathway is a potential therapeutic candidate for Huntington’s disease.

A body $\Theta$ containing two phases, which may form a periodic composite with microstructure much smaller that the body, or which may have structure on a length scale comparable to the body, is subjected to slowly time varying boundary conditions that would produce an approximate uniform field in $\Theta$ were it filled with homogeneous material. Here slowly time varying means that the wavelengths and attenuation lengths of waves at the frequencies associated with the time variation are much larger than the size of $\Theta$, so that we can make a quasistatic approximation. At least one of the two phase does not have an instantaneous response but rather depends on fields at prior times. The fields may be those associated with electricity, magnetism, fluid flow in porous media, or antiplane elasticity. We find, subject to these approximations, that the time variation of the boundary conditions can be designed so boundary measurements at a specific time $t=t_0$ exactly yield the volume fractions of the phases, independent of the detailed geometric configuration of the phases. Moreover, for specially tailored time variations, the volume fraction can be exactly determined frommeasurements at any time $t$, not just at the specific time $t=t_0$. We also show how time varying boundary conditions, not oscillating at the single frequency $\omega_0$, can be designed to exactly retrieve the response at $\omega_0$., Comment: 35 pages, 8 figures

Aggregating signals from a collection of noisy sources is a fundamental problem in many domains including crowd-sourcing, multi-agent planning, sensor networks, signal processing, voting, ensemble learning, and federated learning. The core question is how to aggregate signals from multiple sources (e.g. experts) in order to reveal an underlying ground truth. While a full answer depends on the type of signal, correlation of signals, and desired output, a problem common to all of these applications is that of differentiating sources based on their quality and weighting them accordingly. It is often assumed that this differentiation and aggregation is done by a single, accurate central mechanism or agent (e.g. judge). We complicate this model in two ways. First, we investigate the setting with both a single judge, and one with multiple judges. Second, given this multi-agent interaction of judges, we investigate various constraints on the judges' reporting space. We build on known results for the optimal weighting of experts and prove that an ensemble of sub-optimal mechanisms can perform optimally under certain conditions. We then show empirically that the ensemble approximates the performance of the optimal mechanism under a broader range of conditions.

Chang'e-3(CE-3) has been the first spacecraft to soft-land on the Moon since the Soviet Union's Luna 24 in 1976. The spacecraft arrived at Mare Imbrium on December 14, 2013 and the same day, Yutu lunar rover separated from lander to start its exploration of the surface and the subsurface around the landing site. The rover was equipped, among other instruments, with two Lunar Penetrating Radar systems (LPR) having a working frequency of 60 and 500 MHz. The radars acquired data for about two weeks while the rover was slowly moving along a path of about 114 m. At Navigation point N0209 the rover got stacked into the lunar soil and after that only data at fixed position could be collected. The low frequency radar data have been analyzed by different authors and published in two different papers, which reported totally controversial interpretations of the radar cross sections. The present study is devoted to resolve such controversy carefully analyzing and comparing the data collected on the Moon by Yutu rover and on Earth by a prototype of LRP mounted onboard a model of the CE-3 lunar rover. Such analysis demonstrates that the deep radar features previously ascribed to the lunar shallow stratigraphy are not real reflectors, rather they are signal artefacts probably generated by the system and its electromagnetic interaction with the metallic rover.

Video content is present in an ever-increasing number of fields, both scientific and commercial. Sports, particularly soccer, is one of the industries that has invested the most in the field of video analytics, due to the massive popularity of the game and the emergence of new markets. Previous state-of-the-art methods on soccer matches video summarization rely on handcrafted heuristics to generate summaries which are poorly generalizable, but these works have yet proven that multiple modalities help detect the best actions of the game. On the other hand, machine learning models with higher generalization potential have entered the field of summarization of general-purpose videos, offering several deep learning approaches. However, most of them exploit content specificities that are not appropriate for sport whole-match videos. Although video content has been for many years the main source for automatizing knowledge extraction in soccer, the data that records all the events happening on the field has become lately very important in sports analytics, since this event data provides richer context information and requires less processing. We propose a method to generate the summary of a soccer match exploiting both the audio and the event metadata. The results show that our method can detect the actions of the match, identify which of these actions should belong to the summary and then propose multiple candidate summaries which are similar enough but with relevant variability to provide different options to the final editor. Furthermore, we show the generalization capability of our work since it can transfer knowledge between datasets from different broadcasting companies, different competitions, acquired in different conditions, and corresponding to summaries of different lengths

ABSTRACTIn silico immunogenicity risk assessment has been an important step in the development path for many biologic therapeutics, including monoclonal antibodies. Even if the source of a given biologic is ‘fully human’, T cell epitopes that are contained in the sequences of the biologic may activate the immune system, enabling the development of anti-drug antibodies that can reduce drug efficacy and may contribute to adverse events. Computational tools that identify T cell epitopes from primary amino acid sequences have been used to assess the immunogenic potential of therapeutic candidates for several decades. To facilitate larger scale analyses and accelerate preclinical immunogenicity risk assessment, our group developed an integrated web-based platform called ISPRI, (Immunogenicity Screening and Protein Re-engineering Interface) that provides hands-on access through a secure web-based interface for scientists working in large and mid-sized biotech companies in the US, Europe, and Japan. This toolkit has evolved and now contains an array of algorithms that can be used individually and/or consecutively for immunogenicity assessment and protein engineering. Most analyses start with the advanced epitope mapping tool (EpiMatrix), then proceed to identify epitope clusters using ClustiMer, and then use a tool called JanusMatrix to define whether any of the T cell epitope clusters may generate a regulatory T cell response which may diminish or eliminate anti-drug antibody formation. Candidates can be compared to similar products on a normalized immunogenicity scale. Should modifications to the biologic sequence be an option, a tool for moderating putative immunogenicity by editing T cell epitopes out of the sequence is available (OptiMatrix). Although this perspective discusses the in-silico immunogenicity risk assessment for monoclonal antibodies, bi-specifics, multi-specifics, and antibody-drug conjugates, the analysis of additional therapeutic modalities such as enzyme replacement proteins, blood factor proteins, CAR-T, gene therapy products, and peptide drugs is also made available on the ISPRI platform.

Keratins and corneous proteins are key components of biomaterials used in a wide range of applications and are potential substitutes for petrochemical-based products. Horns, hooves, feathers, claws, and similar animal tissues are abundant sources of α-keratin and corneous β-proteins, which are by-products of the food industry. Their close association with the meat industry raises environmental and ethical concerns regarding their disposal. To promote an eco-friendly and circular use of these materials in novel applications, efforts have focused on recovering these residues to develop sustainable, non-animal-related, affordable, and scalable procedures. Here, we review and examine biotechnological methods for extracting and expressing α-keratins and corneous β-proteins in microorganisms. This review highlights consolidated research trends in biomaterials, medical devices, food supplements, and packaging, demonstrating the keratin industry's potential to create innovative value-added products. Additionally, it analyzes the state of the art of related intellectual property and market size to underscore the potential within a circular bioeconomic model.

Cell outer membranes contain glycosphingolipids and protein receptors, which are integrated into glycoprotein domains, known as lipid rafts, which are involved in a variety of cellular processes, including receptor-mediated signal transduction and cellular differentiation process. In this study, we analyzed the lipidic composition of human Dental Pulp-Derived Stem Cells (DPSCs), and the role of lipid rafts during the multilineage differentiation process. The relative quantification of lipid metabolites in the organic fraction of DPSCs, performed by Nuclear Magnetic Resonance (NMR) spectroscopy, showed that mono-unsaturated fatty acids (MUFAs) were the most representative species in the total pool of acyl chains, compared to polyunsatured fatty acids (PUFAs). In addition, the stimulation of DPSCs with different culture media induces a multilineage differentiation process, determining changes in the gangliosides pattern. To understand the functional role of lipid rafts during multilineage differentiation, DPSCs were pretreated with a typical lipid raft affecting agent (MβCD). Subsequently, DPSCs were inducted to differentiate into osteoblast, chondroblast and adipoblast cells with specific media. We observed that raft-affecting agent MβCD prevented AKT activation and the expression of lineage-specific mRNA such as OSX, PPARγ2, and SOX9 during multilineage differentiation. Moreover, this compound significantly prevented the tri-lineage differentiation induced by specific stimuli, indicating that lipid raft integrity is essential for DPSCs differentiation. These results suggest that lipid rafts alteration may affect the signaling pathway activated, preventing multilineage differentiation.

Its origins are diverse, stemming from antecedent hematologic conditions, germline predisposing mutations, or the sequelae of cytotoxic therapies, and its development is driven by intricate genetic and epigenetic modifications. This complexity necessitates a diverse array of therapeutic strategies, each meticulously tailored to address the distinctive challenges sAML introduces. Such strategies require a personalized approach, considering the variegated clinical backgrounds of patients and the inherent intricacies of the disease. Allogeneic stem cell transplantation stands as a cornerstone, offering the potential for curative outcomes. This is complemented by the emergence of innovative treatments such as CPX-351, venetoclax, and glasdegib, which have demonstrated promising results in enhancing prognosis. The evolving landscape of sAML treatment underscores the importance of continued research and innovation in the field, aiming not only to improve patient outcomes but also to deepen our understanding of the disease’s biological underpinnings, thereby illuminating pathways toward more effective and individualized therapies.

Background: Ethnicity, cultural background, and geographic location differ significantly within the United States Hispanic/Latino population. These variations can greatly define diet and its relationship with cardiometabolic disease, thus influencing generalizability of results. Objectives: We aimed to examine nutrient-based food patterns (NBFPs) of Hispanic/Latino adults and their association with cardiometabolic risk factors (dyslipidemia, hypertension, obesity, diabetes) across 2 United States population-based studies with differing sampling strategies. Methods: Data were collected from Mexican or other Hispanic adult participants from 2007–2012 National Health and Nutrition Examination Survey (NHANES) (n = 3605) and 2007–2011 Hispanic Community Health Survey/Study of Latinos (HCHS/SOL, n = 14,416). NBFPs were derived using factor analysis on nutrient intake data estimated from 24-h dietary recalls and interpreted using common foods in which these nutrients are prominent. Cross-sectional associations between NBFPs (quintiles) and cardiometabolic risk factors, defined by clinical measures and self-report, were estimated using survey-weighted multivariable-adjusted logistic models, accounting for multiple testing. Results: Five NBFPs were identified in both studies: 1) meats, 2) grains/legumes, 3) fruits/vegetables, 4) dairy, and 5) fats/oils. Associations with cardiometabolic risk factors differed by NBFP and study. In HCHS/SOL, the odds of diabetes were lower for persons in the highest quintile of meats NBFP (odds ratio [OR]: 0.73; 95% confidence interval [CI]: 0.58, 0.92) and odds were higher for those in the lowest quintile of fruits/vegetables (OR: 0.71; 95% CI: 0.55, 0.93) compared to those in the third (moderate intake) quintile. Those in the fourth quintile of dairy NBFP had higher odds of hypertension than those in the third quintile (OR: 1.31; 95% CI: 1.01, 1.70). In NHANES, the odds of hypertension were higher for those in the fourth quintile of dairy (OR: 1.88; 95% CI: 1.10, 3.24) than those in the third quintile. Conclusions: Diet–disease relationships among Hispanic/Latino adults vary according to 2 population-based studies. These differences have research and practical implications when generalizing inferences on heterogeneous underrepresented populations.

Objetivo: Realizar una adaptación transcultural y validación del cuestionario de creencias de miedo y evitación (FABQ, por sus siglas en inglés) para sujetos argentinos con dolor lumbar (DL) y reportar las propiedades psicométricas de confiabilidad, validez e interpretabilidad. Materiales y métodos: Se realizó un estudio observacional, prospectivo y longitudinal. Se incluyeron de forma consecutiva residentes argentinos hispanohablantes mayores de 18 años con DL. El estudio comprendió una fase de adaptación, seguida de una fase de validación. Los sujetos fueron evaluados el día de su admisión al estudio (T1) y 48 a 72 horas después (T2). Se utilizó una escala global de cambio como anclaje externo para evaluar la estabilidad clínica. Resultados: Fueron elegibles 74 sujetos. La media de la puntuación total del FABQ fue de 50,24 (DE: 20,64). La consistencia interna (alfa de Cronbach) fue de 0,85 (IC 95 %: 0,79 - 0,91). La confiabilidad test-retest fue aceptable, con un coeficiente de correlación intraclase de 0,765 (IC 95 %: 0,61 - 0,86). El error estándar de medición fue de 4,85 puntos, y el cambio mínimo detectable (95 %) fue de 13,45. La mediana de tiempo para que los sujetos completen el cuestionario fue de 3,86 minutos (RIQ 3,44 – 4,85), y la media de tiempo para puntuarlo fue de 24,40 segundos (DE 13,77). No se observó efecto suelo o techo. Conclusión: El FABQ en su versión argentina es un cuestionario válido, confiable y viable para evaluar las creencias de miedo y evitación en sujetos con DL.

Up to hundreds of billions of dollars are annually lost to fraud and abuse in the US health care, making it a significant burden on the system. This study investigates a specific instance of health care fraud in spine surgery, in which a medical device company ended up paying $75 million to settle violations of the False Claims Act. We review the surgical background regarding the kyphoplasty procedure, as well as its billing and reimbursement details. We also explore the official legal complaint brought by the US Department of Justice to tell the story of how one of the most significant medical innovations in spine surgery in the 21st century turned into a widespread fraudulent marketing scheme. In the sequence, we provide a detailed root cause analysis of this scandal and propose some proactive measures that can be taken to avoid such type of unfortunate events. Ultimately, this historical health care scandal constitutes a valuable lesson to surgeons, health care administrators, medical device companies, and policymakers on how misaligned incentives and subsequent unscrupulous practices can transform a medical innovation into an unfortunate tale of fraud and deceit.

RESUMO Introdução: Em 2020, o mundo foi afetado pelo novo coronavírus, e isso gerou mudanças históricas em diversos setores, incluindo o da educação, que teve que se adaptar rapidamente ao formato online. Objetivo: Este estudo teve como objetivo comparar se houve uma melhor adaptação ao ensino remoto durante a pandemia de Covid-19 dos acadêmicos de Medicina que estudam em currículos PBL quando comparada a de alunos com currículos em modelo disciplinar expositivo. Método: Trata-se de um estudo transversal. Foram incluídos na pesquisa os estudantes matriculados do segundo ao quarto ano do curso de Medicina que tenham preenchido o questionário completamente e cursado pelo menos um semestre de ensino remoto durante o ano de 2020. Resultado: Houve uma diferença significativa entre as metodologias, com maior adaptação dos estudantes que utilizaram o modelo PBL em comparação aos que cursaram no modelo disciplinar expositivo durante a pandemia de Covid-19. Conclusão: Na amostra estudada, o método PBL mostrou-se superior ao método disciplinar, nos quesitos avaliados, na adaptação de estudantes de Medicina ao ensino remoto.

Over a decade ago, the United States Preventive Services Taskforce (USPSTF) recommended against prostate-specific antigen (PSA)-based screening for prostate cancer in all men, which considerably influenced prostate cancer screening policies worldwide after that. Consequently, the world has seen increasing numbers of advanced stages and prostate cancer deaths, which later led the USPSTF to withdraw its initial statement. Meanwhile, the European Union has elaborated a directive to address the problem of implementing prostate cancer screening in “Europe’s Beating Cancer Plan”. In Switzerland, concerned urologists formed an open Swiss Prostate Cancer Screening Group to improve the early detection of prostate cancer. On the 20th of September 2023, during the annual general assembly of the Swiss Society of Urology (SGU/SSU) in Lausanne, members positively voted for a stepwise approach to evaluate the feasibility of implementing organised prostate cancer screening programs in Switzerland. The following article will summarise the events and scientific advances in the last decade during which evidence and promising additional modalities to complement PSA-based prostate cancer screening have emerged. It also aims to provide an overview of contemporary strategies and their potential harms and benefits.

ABSTRACT BACKGROUND: Curative treatment for gastric cancer involves tumor resection, followed by transit reconstruction, with Roux-en-Y being the main technique employed. To permit food transit to the duodenum, which is absent in Roux-en-Y, double transit reconstruction has been used, whose theoretical advantages seem to surpass the previous technique. AIMS: To compare the clinical evolution of gastric cancer patients who underwent total gastrectomy with Roux-en-Y and double tract reconstruction. METHODS: A systematic review was carried out on Web of Science, Scopus, EmbasE, SciELO, Virtual Health Library, PubMed, Cochrane, and Google Scholar databases. Data were collected until June 11, 2022. Observational studies or clinical trials evaluating patients submitted to double tract (DT) and Roux-en-Y (RY) reconstructions were included. There was no temporal or language restriction. Review articles, case reports, case series, and incomplete texts were excluded. The risk of bias was calculated using the Cochrane tool designed for randomized clinical trials. RESULTS: Four studies of good methodological quality were included, encompassing 209 participants. In the RY group, there was a greater reduction in food intake. In the DT group, the decrease in body mass index was less pronounced compared to preoperative values. CONCLUSIONS: The double tract reconstruction had better outcomes concerning body mass index and the time until starting a light diet; however, it did not present any advantages in relation to nutritional deficits, quality of life, and post-surgical complications.

Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

We consider the problem of finding a net that supports prescribed forces applied at prescribed points, yet avoids certain obstacles, with all the elements of the net under compression (strut net) or under tension (cable web). In the case of masonry structures, for instance, this consists in finding a strut net that supports the forces, is contained within the physical structure, and avoids regions that may be not accessible due, for instance, to the presence of holes. We solve such a problem in the two-dimensional case, where the prescribed forces are applied at the vertices of a convex polygon, and we treat the cases of both single and multiple obstacles. By approximating the obstacles by polygonal regions, the task reduces to identifying the feasible domain in a linear programming problem. For a single obstacle we show how the region $\Gamma$ available to the obstacle can be enlarged as much as possible in the sense that there is no other strut net, having a region $\Gamma_1$ available to the obstacle with $\Gamma_1 \subset\Gamma$. The case where some of the forces are reactive, unprescribed but reacting to the other prescribed forces, is also treated. It again reduces to identifying the feasible domain in a linear programming problem. Finally, one may allow a subset of the reactive forces to each act not at a prescribed point, but rather at any point on a prescribed line segment. The classification of the minor bodies of the Solar System based on observables has been continuously developed and iterated over the past 40 years. While prior iterations followed either the availability of large observational campaigns or new instrumental capabilities opening new observational dimensions, we see the opportunity to improve primarily upon the established methodology. We developed an iteration of the asteroid taxonomy which allows the classification of partial and complete observations (i.e. visible, near-infrared, and visible-near-infrared spectrometry) and which reintroduces the visual albedo into the classification observables. The resulting class assignments are given probabilistically, enabling the uncertainty of a classification to be quantified. We built the taxonomy based on 2983 observations of 2125 individual asteroids, representing an almost tenfold increase of sample size compared with the previous taxonomy. The asteroid classes are identified in a lower-dimensional representation of the observations using a mixture of common factor analysers model. We identify 17 classes split into the three complexes C, M, and S, including the new Z-class for extremely-red objects in the main belt. The visual albedo information resolves the spectral degeneracy of the X-complex and establishes the P-class as part of the C-complex. We present a classification tool which computes probabilistic class assignments within this taxonomic scheme from asteroid observations. The taxonomic classifications of 6038 observations of 4526 individual asteroids are published. The ability to classify partial observations and the reintroduction of the visual albedo provide a taxonomy which is well suited for the current and future datasets of asteroid observations, in particular provided by the Gaia, MITHNEOS, NEO Surveyor, and SPHEREx surveys., Comment: Published in Astronomy and Astrophysics. Semi-supervised learning (SSL) provides an effective means of leveraging unlabelled data to improve a model performance. Even though the domain has received a considerable amount of attention in the past years, most methods present the common drawback of lacking theoretical guarantees. Our starting point is to notice that the estimate of the risk that most discriminative SSL methods minimise is biased, even asymptotically. This bias impedes the use of standard statistical learning theory and can hurt empirical performance. We propose a simple way of removing the bias. Our debiasing approach is straightforward to implement and applicable to most deep SSL methods. We provide simple theoretical guarantees on the trustworthiness of these modified methods, without having to rely on the strong assumptions on the data distribution that SSL theory usually requires. In particular, we provide generalisation error bounds for the proposed methods. We evaluate debiased versions of different existing SSL methods, such as the Pseudo-label method and Fixmatch, and show that debiasing can compete with classic deep SSL techniques in various settings by providing better calibrated models. Additionally, we provide a theoretical explanation of the intuition of the popular SSL methods.

We present simple methods for out-of-distribution detection using a trained generative model. These techniques, based on classical statistical tests, are model-agnostic in the sense that they can be applied to any differentiable generative model. The idea is to combine a classical parametric test (Rao's score test) with the recently introduced typicality test. These two test statistics are both theoretically well-founded and exploit different sources of information based on the likelihood for the typicality test and its gradient for the score test. We show that combining them using Fisher's method overall leads to a more accurate out-of-distribution test. We also discuss the benefits of casting out-of-distribution detection as a statistical testing problem, noting in particular that false positive rate control can be valuable for practical out-of-distribution detection. Many real-life scenarios require humans to make difficult trade-offs: do we always follow all the traffic rules or do we violate the speed limit in an emergency? These scenarios force us to evaluate the trade-off between collective rules and norms with our own personal objectives and desires. To create effective AI-human teams, we must equip AI agents with a model of how humans make these trade-offs in complex environments when there are implicit and explicit rules and constraints. Agent equipped with these models will be able to mirror human behavior and/or to draw human attention to situations where decision making could be improved. To this end, we propose a novel inverse reinforcement learning (IRL) method: Max Entropy Inverse Soft Constraint IRL (MESC-IRL), for learning implicit hard and soft constraints over states, actions, and state features from demonstrations in deterministic and non-deterministic environments modeled as Markov Decision Processes (MDPs). Our method enables agents implicitly learn human constraints and desires without the need for explicit modeling by the agent designer and to transfer these constraints between environments. In the analysis of Twitter debate, the recent literature focused on discursive communities, i.e. clusters of accounts interacting among themselves via retweets. In the present work, we studied discursive communities in 8 different thematic Twitter datasets in various languages. Surprisingly, we observed that almost all discursive communities therein display a bow-tie structure during political or societal debates. Instead, they are absent when the argument of the discussion is different as sport events, as in the case of Euro2020 Turkish and Italian datasets. We furthermore analysed the quality of the content created in the various sectors of the different discursive communities, using the domain annotation from the fact-checking website Newsguard: we observe that, when the discursive community is affected by m/disinformation, the content with the lowest quality is the ones produced and shared in SCC and, in particular, a strong incidence of low- or non-reputable messages is present in the flow of retweets between the SCC and the OUT sectors. In this sense, in discursive communities affected by m/disinformation, the greatest part of the accounts has access to a great variety of contents, but whose quality is, in general, quite low; such a situation perfectly describes the phenomenon of infodemic, i.e. the access to "an excessive amount of information about a problem, which makes it difficult to identify a solution", according to WHO)., Comment: 47 pages, 25 figures, 7 tables

Takotsubo syndrome (TTS) occurs more frequently in cancer patients than in the general population, but the effect of specific TTS triggers on outcomes in cancer patients is not well studied.The study sought to determine whether triggering event (chemotherapy, immune-modulators vs. procedural or emotional stress) modifies outcomes in a cancer patient population with TTS.All cancer patients presenting with acute coronary syndrome (ACS) between December 2008 and December 2020 at our institution were enrolled in the catheterization laboratory registry. Demographic and clinical data of the identified patients with TTS were retrospective collected and further classified according to the TTS trigger. The groups were compared with regards to major adverse cardiac events, overall survival and recovery of left ventricular ejection fraction (LVEF) and global longitudinal strain (GLS) after TTS presentation.Eighty one of the 373 cancer patients who presented with ACS met the Mayo criteria for TTS. The triggering event was determined to be "cancer specific triggers" (use of chemotherapy in 23, immunomodulators use in 7, and radiation in 4), and "traditional triggers" (medical triggers 22, and procedural 18 and emotional stress in 7). Of the 81 patients, 47 died, all from cancer-related causes (no cardiovascular mortality). Median survival was 11.9 months. Immunomodulator (IM) related TTS and radiation related TTS were associated with higher mortality during the follow-up. Patients with medical triggers showed the least recovery in LVEF and GLS while patients with emotional and chemotherapy triggers, showed the most improvement in LVEF and GLS, respectively.Cancer patients presenting with ACS picture have a high prevalence of TTS due to presence of traditional and cancer specific triggers. Survival and improvement in left ventricular systolic function seem to be related to the initial trigger for TTS.

This review presents updated and practical approaches for multimodality cardiovascular imaging in the cardio-oncology patient and provides recommendations for imaging to detect the myriad of adverse cardiovascular effects associated with antineoplastic therapy, such as cardiomyopathy, atherosclerosis, vascular toxicity, myocarditis, valve disease, and cardiac masses. Uniquely, we address the role of cardiovascular imaging in patients with pre-existing cardiomyopathy, pregnant patients, long-term survivors, and populations with limited resources. We also address future avenues of investigation and opportunities for artificial intelligence applications in cardio-oncology imaging. This review provides a uniform practical approach to cardiovascular imaging for patients with cancer.

In recent years, microplastic pollution has been a hot topic as these compounds have been used in various production contexts such as health, food or technology due to their chemical and physical properties and “shelf-life,” making them almost indispensable products in daily life. On the other hand, they have a negative impact on the environment and, consequently, on biota and human health. It is therefore necessary to assess the actual presence of microplastics in drinking water by analysing real samples in order to estimate the possible exposure through drinking water consumption. In this pilot study, drinking water from different aqueous matrices was examined for the presence of microplastics and characterized in terms of shape, size, abundance and polymer type by Raman microspectroscopy analysis. Not all samples analysed were found to be contaminated with microplastics, indeed, some, as in the case of water kiosk samples, were found to be free of such contaminants. The results for the various matrices showed that the microplastics content ranged from less than 2 particles/L to a maximum of 5 + 1.5 particles/L, with sizes ranging from 30 to 100 μm and consisted of the most common polymers such as polyethylene, polypropylene or polyethylene terephthalate.

137. Effect of glycosylation on the affinity of the MTB protein Ag85B for specific antibodies: towards the design of a dual-acting vaccine against tuberculosis

Abstract Background To create a dual-acting vaccine that can fight against tuberculosis, we combined antigenic arabino-mannan analogues with the Ag85B protein. To start the process, we studied the impact of modifying different parts of the Ag85B protein on its ability to be recognized by antibodies. Results Through our research, we discovered that three modified versions of the protein, rAg85B-K30R, rAg85B-K282R, and rAg85B-K30R/K282R, retained their antibody reactivity in healthy individuals and those with tuberculosis. To further test the specificity of the sugar AraMan for AraMan antibodies, we used Human Serum Albumin glycosylated with AraMan-IME and Ara3Man-IME. Our findings showed that this specific sugar was fully and specifically modified. Bio-panning experiments revealed that patients with active tuberculosis exhibited a higher antibody response to Ara3Man, a sugar found in lipoarabinomannan (LAM), which is a major component of the mycobacterial cell wall. Bio-panning with anti-LAM plates could eliminate this increased response, suggesting that the enhanced Ara3Man response was primarily driven by antibodies targeting LAM. These findings highlight the importance of Ara3Man as an immunodominant epitope in LAM and support its role in eliciting protective immunity against tuberculosis. Further studies evaluated the effects of glycosylation on the antibody affinity of recombinant Ag85B and its variants. The results indicated that rAg85B-K30R/K282R, when conjugated with Ara3Man-IME, demonstrated enhanced antibody recognition compared to unconjugated or non-glycosylated versions. Conclusions Coupling Ara3Man to rAg85B-K30R/K282R could lead to the development of effective dual-acting vaccines against tuberculosis, stimulating protective antibodies against both AraMan and Ag85B, two key tuberculosis antigens.

Abstract Introduction In this work, our aim is to report the functional outcomes of cataract surgery with smaller-incision new-generation miniature telescope (SING IMT) implantation followed by rehabilitation training in patients with central visual loss due to late-stage age-related macular degeneration (AMD). Methods This retrospective study included patients who were monocularly implanted with SING IMT and then followed a rehabilitation program based on 6 biweekly sessions focused on visual abilities, reading, writing, visual motor integration and mobility. A total of 11 participants were included in this study. Reading acuity (RA), reading speed (RS), and fixation stability (FS) were assessed biweekly at 6-, 8-, 10-, 12-, 14-, and 16-week follow-up visits after SING IMT implantation and at a final assessment at 24 weeks. Best-corrected distance visual acuity (BCDVA) was also measured at baseline and at the same postoperative timepoints. Results Mean baseline BCDVA was 12.5 ± 8.6 letter score. Both RA and RS were found to be significantly improved from the first rehabilitation session (6 weeks after surgery) to the last session (24 weeks after surgery). At the end of the rehabilitation program, mean RA was 0.45 ± 0.19 LogMAR and mean RS was 30.9 ± 17.6 words per minute. Moreover, all patients achieved a FS of 15 s or more after the last rehabilitation session. Most patients (55%) achieved an improvement of 15 letters in BCDVA at the end of the study. Conclusions This study suggests that rehabilitation training can improve visual functions of patients with late-stage AMD implanted with SING IMT in real-world tasks such as reading skills.

In this paper we propose a vertical stabilization~(VS) control system for tokamak plasmas based on the extremum seeking (ES) algorithm. The gist of the proposed strategy is to inject an oscillating term in the control action and exploit a modified ES algorithm in order to bring to zero the average motion of the plasma along the unstable mode. In this way, the stabilization of the unstable vertical dynamic of the plasma is achieved. We revisit the theory of importance weighted variational inference (IWVI), a promising strategy for learning latent variable models. IWVI uses new variational bounds, known as Monte Carlo objectives (MCOs), obtained by replacing intractable integrals by Monte Carlo estimates -- usually simply obtained via importance sampling. Burda, Grosse and Salakhutdinov (2016) showed that increasing the number of importance samples provably tightens the gap between the bound and the likelihood. Inspired by this simple monotonicity theorem, we present a series of nonasymptotic results that link properties of Monte Carlo estimates to tightness of MCOs. We challenge the rationale that smaller Monte Carlo variance leads to better bounds. We confirm theoretically the empirical findings of several recent papers by showing that, in a precise sense, negative correlation reduces the variational gap. We also generalise the original monotonicity theorem by considering non-uniform weights. We discuss several practical consequences of our theoretical results. Our work borrows many ideas and results from the theory of stochastic orders.

Belief revision is the task of modifying a knowledge base when new information becomes available, while also respecting a number of desirable properties. Classical belief revision schemes have been already specialised to \emph{binary decision diagrams} (BDDs), the classical formalism to compactly represent propositional knowledge. These results also apply to \emph{ordered} BDDs (OBDDs), a special class of BDDs, designed to guarantee canonicity. Yet, those revisions cannot be applied to \emph{sentential decision diagrams} (SDDs), a typically more compact but still canonical class of Boolean circuits, which generalizes OBDDs, while not being a subclass of BDDs. Here we fill this gap by deriving a general revision algorithm for SDDs based on a syntactic characterisation of Dalal revision. A specialised procedure for DNFs is also presented. One of the most remarkable things about the human moral mind is its flexibility. We can make moral judgments about cases we have never seen before. We can decide that pre-established rules should be broken. We can invent novel rules on the fly. Capturing this flexibility is one of the central challenges in developing AI systems that can interpret and produce human-like moral judgment. This paper details the results of a study of real-world decision makers who judge whether it is acceptable to break a well-established norm: ``no cutting in line.'' We gather data on how human participants judge the acceptability of line-cutting in a range of scenarios. Then, in order to effectively embed these reasoning capabilities into a machine, we propose a method for modeling them using a preference-based structure, which captures a novel modification to standard ``dual process'' theories of moral judgment.

Current AI systems lack several important human capabilities, such as adaptability, generalizability, self-control, consistency, common sense, and causal reasoning. We believe that existing cognitive theories of human decision making, such as the thinking fast and slow theory, can provide insights on how to advance AI systems towards some of these capabilities. In this paper, we propose a general architecture that is based on fast/slow solvers and a metacognitive component. We then present experimental results on the behavior of an instance of this architecture, for AI systems that make decisions about navigating in a constrained environment.
