Abstract

« Les fables ne sont pas ce qu’elles semblent être », déclarait La Fontaine, après le fabuliste latin Phèdre. Il y a l’apparence et la réalité, la surface et les strates successives qui, de descente en descente dans les profondeurs du mythe, livreront peut-être au mythologue mué en spéléologue la signification la plus ancienne. C’est ainsi que, grâce à l’étymologie se révèlent les traits primitifs de la personnalité de certains héros. Mais cette recherche nous mène plus loin encore, vers les origines du monde, les cosmogonies, les cataclysmes primordiaux, les naissances et renaissances d’une humanité sans cesse remise en question par le dieu suprême. Elle permet de découvrir, au delà de l’admirable torse pivotant du Discobole de Myron, les dieux de la végétation bulbeuse que vénéraient les premiers hommes. Elle met en lumière la propension des Grecs à éloigner dans l’espace et dans le temps aussi bien le monde idéal (les pays au parfum où habitent les dieux, les contrées de justice, d’abondance et de bonheur des Hyperboréens et des Éthiopiens) que l’humanité qui s’entretue et s’entredévore, une humanité enférocée qui, selon les Grecs, ne saurait être que barbare, non-grecque. Mais cette opération de propagande, consciente ou inconsciente, est manquée : les mythes dévoilent, les mythes dénoncent. Volume II Au troisième millénaire se pratiquent encore chez certains peuples (Afrique, Nouvelle-Guinée, Amérindiens) des rites initiatiques très proches de ceux que les Grecs ont connus bien avant Homère. À l’époque classique il en restait des souvenirs, surtout dans les pays des confins, mais aussi dans les institutions des cités les plus civilisées (comme Athènes, Sparte, Thèbes ou Corinthe), et plus encore dans leurs mythes. Abstract

International audience; African Trypanosomiasis is a neglected tropical disease, endemic in nearly 30 countries in sub-Saharan Africa. Trypanosoma brucei, the infectious agent responsible for the disease, is transmitted by the bite of Glossina (tsetse fly) and can affect both animals (AAT - Animal African Trypanosomiasis) and humans (HAT - Human African Trypanosomiasis). Without treatment, the disease is fatal. The current therapeutic care is complex and depends on trypanosome species, host and stage of the disease. Therefore, it is essential to find new treatments. In recent years, nitroaromatic compounds present a renewed interest as anti-infective agents like fexinidazole.Some studies show that their antiparasitic activity is based on their selective reduction by parasite nitroreductases (NTR). These NTR leading to metabolites that are cytotoxic for the protozoan.The objective of the study is to develop new nitroaromatic drugs efficient against Trypanosoma.Our collaboration between chemists and biologists allows to synthetize different molecules and test them on various biological models. Firstly, about 200 molecules belonging to the 8-nitroquinolin-2(1H)-one series and 3-nitroimidazo[1,2-a]pyridine series were synthetized. Secondly, in vitro evaluations were performed for all molecules on a HepG2 human cell line, and on a strain of T. b. brucei to evaluate the CC50 and IC50 respectively. In vitro pharmacokinetic tests and comet assays completed the data for the most promising molecules. The four best candidates were included in an in vivo test on a murine model of mice infected by T. b. brucei (strain AnTat 1.1E): briefly, the 4 molecules were tested to evaluate their ability to cross the blood brain barrier, their toxicity and their activity. At the end of the study, an anatomopathological analysis was performed to ensure the absence of damage on vital organs (brain, heart, kidney, liver). The finality of this project is to develop new drug candidates for human and animal therapeutic.

117. Correlates of quality of life of pre-obese and obese patients: a pharmacy-based cross-sectional survey

Abstract

Abstract Background The correlates of quality of life (QOL), as measured by the OSQOL questionnaire were investigated in a convenience sample of overweight patients recruited in pharmacies. Methods A convenience sample of patients with a Body Mass Index ≥ 28 kg/m2 were recruited in community-based pharmacies. Baseline characteristics and QOL dimensions (1-Physical state, 2-Vitality-desire to do things, 3-Relations with others, 4-Psychological state) were reported in self-completed questionnaires from which the risk of obtaining a low QOL was assessed for each dimension. Results QOL was inadequate for all dimensions in the 494 patients included in the study (median age = 61, 48% women, 21% professional persons/top executives). Older pre-obese and obese patients were more likely to report impaired physical functioning (OR = 2.02, 95%CI = [1.10-3.70]), but were less severely affected socially (OR = 0.32, 95%CI = [0.15-0.69]). Pre-obese and obese professional persons and top executives showed better physical capabilities (OR = 0.35, 95%CI = [0.15-0.81]) and increased vitality (OR = 0.47, 95%CI = [0.23-0.95]). Overall, men's psychological state was better than females' (OR = 0.46, 95%CI = [0.25-0.82]). A body-mass index ≥ 35 kg/m2 was significantly associated with poorer QOL scores on physical, relational and psychological dimensions. Conclusion Our data highlighted the influence of the severity of excess weight, gender, age and socioeconomic status on QOL. These factors should be taken into account when interpreting QOL in pre-obese and obese persons.

Abstract

Purpose: Bracing is the treatment of choice for idiopathic scoliosis (IS), unfortunately factors underlying brace response remain unknown. Clinicians are currently unable to identify patients who may benefit from bracing, and therefore, better molecular stratification is critically needed. The aim of this study is to evaluate IS patient outcomes at skeletal maturity in relation to biological endophenotypes, and determine specific endophenotypes associated to differential bracing outcomes. This is a retrospective cohort with secondary cross-sectional comparative studies. Methods: Clinical and radiological data were collected from 563 IS patients, stratified into biological endophenotypes (FG1, FG2, FG3) based on a cell-based test. Measured outcomes were maximum Cobb angle at skeletal maturity, and if severe, spinal deformity (≥ 45°) or surgery was attained. Treatment success/failure was determined by standard progression thresholds (Cobb ≥ 45° or surgery; Cobb angle progression ≥ 6°). Multivariable analyses were performed to evaluate associations between endophenotypes and clinical outcome. Results: Higher Cobb angles at maturity for FG1 and FG2 patients were observed (p = 0.056 and p = 0.05), with increased likelihood of ≥ 45° and/or surgery for FG1 (OR = 2.181 [1.002–4.749] and FG2 (OR = 2.141 [1.038–4.413]) compared to FG3. FG3 was 9.31 [2.58–33.61] and 5.63 [2.11–15.05] times more likely for bracing success at treatment termination and based on the < 6° progression criterion, respectively, compared to FG1. Conclusion: Associations between biological endophenotypes and outcomes suggest differences in progression and/or bracing response among IS patients. Outcomes were most favorable in FG3 patients. Abstract

Background: The UK Prospective Diabetes Study showed that metformin decreases mortality compared to diet alone in overweight patients with type 2 diabetes mellitus. Since then, it has been the first-line treatment in overweight patients with type 2 diabetes. However, metformin-sulphonylurea bitherapy may increase mortality. Methods and Findings: This meta-analysis of randomised controlled trials evaluated metformin efficacy (in studies of metformin versus diet alone, versus placebo, and versus no treatment;metformin as an add-on therapy;and metformin withdrawal) against cardiovascular morbidity or mortality in patients with type 2 diabetes. We searched Medline, Embase, and the Cochrane database. Primary end points were all-cause mortality and cardiovascular death. Secondary end points included all myocardial infarctions, all strokes, congestive heart failure, peripheral vascular disease, leg amputations, and microvascular complications. Thirteen randomised controlled trials (13,110 patients) were retrieved; 9,560 patients were given metformin, and 3,550 patients were given conventional treatment or placebo. Metformin did not significantly affect the primary outcomes all-cause mortality, risk ratio (RR) = 0.99 (95% CI: 0.75 to 1.31), and cardiovascular mortality, RR = 1.05 (95% CI: 0.67 to 1.64). The secondary outcomes were also unaffected by metformin treatment: all myocardial infarctions, RR = 0.90 (95% CI: 0.74 to 1.09);all strokes, RR = 0.76 (95% CI: 0.51 to 1.14);heart failure, RR=1.03 (95% CI: 0.67 to 1.59); peripheral vascular disease, RR = 0.90 (95% CI: 0.46 to 1.78);leg amputations, RR = 1.04 (95% CI: 0.44 to 2.44); and microvascular complications, RR = 0.83 (95% CI: 0.59 to 1.17). For all-cause mortality and cardiovascular mortality, there was significant heterogeneity when including the UK Prospective Diabetes Study subgroups (/2 = 41% and 59%). There was significant interaction with sulphonylurea as a concomitant treatment for myocardial infarction (p = 0.10 and 0.02, respectively). Conclusions: Although metformin is considered the gold standard, its benefit/risk ratio remains uncertain. We cannot exclude a 25% reduction or a 31% increase in all-cause mortality. We cannot exclude a 33% reduction or a 64% increase in cardiovascular mortality. 144. Ablation of Osteopontin Improves the Skeletal Phenotype of Phospho1−/− Mice

Abstract

PHOSPHO1 and tissue-nonspecific alkaline phosphatase (TNAP) have non-redundant functions during skeletal mineralization. While TNAP deficiency (Alpl(-/-) mice) leads to hypophosphatasia, caused by accumulation of the mineralization inhibitor inorganic pyrophosphate (PPi ), comparably elevated levels of PPi in Phospho1(-/-) mice do not explain their stunted growth, spontaneous fractures, bowed long bones, osteomalacia, and scoliosis. We have previously shown that elevated PPi in Alpl(-/-) mice is accompanied by elevated osteopontin (OPN), another potent mineralization inhibitor, and that the amount of OPN correlates with the severity of hypophosphatasia in mice. Here we demonstrate that plasma OPN is elevated and OPN expression is upregulated in the skeleton, particularly in the vertebrae, of Phospho1(-/-) mice. Liquid chromatography/tandem mass spectrometry showed an increased proportion of phosphorylated OPN (p-OPN) peptides in Phospho1(-/-) mice, suggesting that accumulation of p-OPN causes the skeletal abnormalities in Phospho1(-/-) mice. We also show that ablation of the OPN gene, Spp1, leads to improvements in the skeletal phenotype in Phospho1(-/-) as they age. In particular, their scoliosis is ameliorated at 1 month of age and is completely rescued at 3 months of age. There is also improvement in the long bone defects characteristic of Phospho1(-/-) mice at 3 months of age. Mineralization assays comparing [Phospho1(-/-) ; Spp1(-/-) ], Phospho1(-/-) and Spp1(-/-) chondrocytes display corrected mineralization by the double knockout cells. Expression of chondrocyte differentiation markers was also normalized in the [Phospho1(-/-) ; Spp1(-/-) ] mice. Thus, while Alpl and Phospho1 deficiencies lead to similar skeletal phenotypes and comparable changes in the expression levels of PPi and OPN, there is a clear dissociation in the hierarchical roles of these potent inhibitors of mineralization, with elevated PPi and elevated p-OPN levels causing the respective skeletal phenotypes in Alpl(-/-) and Phospho1(-/-) mice. © 2014 American Society for Bone and Mineral Research.
