However, functional capacity is sometimes not comparable with functional performance in clinical settings. We hypothesized that specific personality traits are related to this capacity-performance discrepancy of functional outcome.Methods: The measures in this study were the UCSD Performance-based Skills Assessment Brief (UPSA-B) for functional capacity, the Social Functioning Scale (SFS) for functional performance, and the Temperament and Character Inventory-140 (TCI-140) for personality traits. A total of 94 stable outpatients with schizophrenia were divided into four groups based on combinations of their UPSA-B and SFS cut-off points, as follows: a high capacity and high performance (HH) group, a high capacity but low performance (HL) group, a low capacity but high performance (LH) group, and a low capacity and low performance (LL) group.Results: There were no significant differences in demographics among the four groups. The LH group showed a significantly lower harm avoidance level than the LL group. The HL group showed a significantly lower persistence level than the HH group.Conclusion: Lower harm avoidance may enhance functional performance, and lower persistence can limit functional performance, regardless of the primary levels of functional capacity in patients with schizophrenia. Novel and individualized psychosocial treatments considering the individual’s personality traits seem to be helpful for maximizing their functional performance in the community.Keywords: functional capacity, performance-based assessment, real-world functioning, social functioning, Temperament and Character Inventory

Abstract

Background: Excessive alcohol intake has been shown to be associated with cardiovascular disease via metabolic pathways. However, the relationship between alcohol intake and obesity has not been fully elucidated. We aimed to examine the association of alcohol consumption with fat deposition and anthropometric measures. Methods: From 2006–2008, we conducted a cross-sectional study in a population-based sample of Japanese men aged 40 through 79 years. Areas of abdominal visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) were calculated using computed tomography imaging. Based on a questionnaire, we classified participants into five groups according to weekly alcohol consumption, excluding former drinkers: non-drinkers (0 g/week), 0.1–160.9, 161–321.9, 322–482.9, and ≥483 g/week. Multivariable linear regression was used to estimate adjusted means of obesity indices for each group. Results: We analyzed 998 men (mean age and body mass index [BMI], 63.8 years and 23.6 kg/m2, respectively). Abstract

Aims: Calcification in the coronary and aortic arteries has been linked to cardiovascular morbidity and mortality. The pathophysiological influence of aortic artery calcification (AAC) differs from that of coronary artery calcification (CAC). We aimed to compare the relationships between CAC and AAC and cardiovascular disease (CVD) risk factors. Methods: We examined a random sample of 1035 Japanese men aged 40–79 years. CAC and AAC were measured by computed tomography and scored according to the Agatston method. Using a logistic regression, we calculated the odds ratio (OR) as being in the highest quintile (Q5) of the calcification score compared to the lower quintiles (Q1–Q4) per 1 standard deviation higher CVD risk factor. Models were simultaneously adjusted for age, body mass index (BMI), systolic blood pressure, smoking (pack-year), alcohol intake, hemoglobin A1c, uric acid, estimated glomerular filtration rate (eGFR), serum lipids, and C-reactive protein. Differences in ORs were investigated using a generalized estimating equation. We performed a multiple linear regression using log-transformed CAC and AAC values as dependent variables. Results: CAC and AAC were independently associated with age (OR, 95% CI: 2.30 [1.77–2.98] for CAC and 3.48 [2.57–4.73] for AAC), p for difference < 0.001), systolic blood pressure (1.29 [1.08–1.53] and 1.28 [1.07–1.54], p for difference = 0.270), and smoking (1.22, [1.04–1.43] and 1.34 [1.13–1.58]) p for difference = 0.071). Alcohol correlated with AAC only (1.17 [0.97–1.41] for CAC and 1.42 [1.16–1.73] for AAC, p for difference = 0.020). Conclusions: CAC and AAC were associated with similar CVD risk factors. The strength of association slightly differed between CAC and AAC.

Abstract

AIMS: We aimed to evaluate the validity of the Academic Research Consortium for High Bleeding Risk (ARC-HBR) criteria for East Asian patients undergoing contemporary percutaneous coronary intervention (PCI) from the PENDULUM registry. METHODS AND RESULTS: This post hoc analysis included 6,267 Japanese patients undergoing PCI between December 2015 and June 2017 enrolled in PENDULUM. The primary endpoint was the incidence of major bleeding at 12 months post index PCI. In total, 3,185 (50.8%) and 3,082 (49.2%) patients were stratified to the ARC-HBR and non-ARC-HBR groups, respectively, and almost all patients had overlapping criteria. Abstract

The molecular orientation of steroidal side chains at rubbed polyimide (PI) surfaces is studied by sum frequency generation (SFG) vibrational spectroscopy. The main objective is to find a correlation between the molecular structure of the PI film and the liquid crystal alignment on the polymer. Analysis of the SFG spectra shows that rubbing of the polymer film appears to cause conformational changes in the methyl group of the polymer side chain near the steroidal structure. However, rubbing does not significantly influence the orientation of the isopropyl group at the end of the polymer side chain. Abstract

Objectives: Three subcategories of high‐risk flat and depressed lesions (FDLs), laterally spreading tumors non‐granular type (LST‐NG), depressed lesions, and large sessile serrated lesions (SSLs), are highly attributable to post‐colonoscopy colorectal cancer (CRC). Efficient and organized educational programs on detecting high‐risk FDLs are lacking. We aimed to explore whether a web‐based educational intervention with training on FIND clues (fold deformation, intensive stool/mucus attachment, no vessel visibility, and demarcated reddish area) may improve the ability to detect high‐risk FDLs. Methods: This was an international web‐based randomized control trial that enrolled non‐expert endoscopists in 13 Asian countries. The participants were randomized into either education or non‐education group. All participants took the pre‐test and post‐test to read 60 endoscopic images (40 high‐risk FDLs, five polypoid, 15 no lesions) and answered whether there was a lesion. Only the education group received a self‐education program (video and training questions and answers) between the tests. The primary outcome was a detection rate of high‐risk FDLs. Results: In total, 284 participants were randomized. After excluding non‐responders, the final data analyses were based on 139 participants in the education group and 130 in the non‐education group. The detection rate of high‐risk FDLs in the education group significantly improved by 14.7% (66.6–81.3%) compared with −0.8% (70.8–70.0%) in the non‐education group. Similarly, the detection rate of LST‐NG, depressed lesions, and large SSLs significantly increased only in the education group by 12.7%, 12.0%, and 21.6%, respectively. Abstract

Contribution of main chains and side chains of unrubbed polyimide films with steroidal structure side chains to second harmonic generation (SHG) response was studied by varying their molar fraction of side-chain diamine. The SHG intensity in P_/P_ polarization (P-polarization input and P-polarization output) configuration does not depend strongly on the molar fraction. This indicates that both the main chains and side chains at the surface contribute similarly to the SHG intensity. The SHG intensity in S_/P_ polarization configuration normalized by that of P_/P_ polarization configuration was the smallest for the molar fraction of side-chain diamine of 0%. This fact indicates that the microscopic nonlinear dipoles are standing nearly upright at the surface of the film consisting only of the main chains, while their orientational spread is wider when the side chains are involved., identifier:https://dspace.jaist.ac.jp/dspace/handle/10119/16247

Abstract

Background and Aim Right colon polyps can especially be overlooked when they are located on the backs of haustral folds. Previous studies have reported that repeated forward‐view examinations in the right colon were effective in reducing adenoma miss rates. The aim of this study was to clarify the impact of retroflexion in the right colon after repeated forward‐view examinations. Methods This multicenter, prospective, observational study was conducted at three institutions in Kumamoto, Japan, between February 2014 and December 2015. Subjects who were over 40 years old and scheduled for colonoscopy were recruited. For the forward view, after cecal intubation, the colonoscope was withdrawn to the hepatic flexure. The colonoscope was sequentially reinserted to the cecum and then withdrawn to the hepatic flexure. For the retroflexion view (RV), the colonoscope was reinserted to the cecum, retroflexed, and then withdrawn to the hepatic flexure. All polyps were resected at the time of detection. The primary outcome of this study was the adenoma miss rate for the repeated forward‐view examinations. Results Of the 777 enrolled participants, retroflexion was successful in 730 (94.0%). The repeated forward‐view withdrawal technique detected 291 adenomas, while the third withdrawal in the RV detected 53. The adenoma miss rate for the repeated forward‐view withdrawal was 15.4%. No severe adverse events occurred during retroflexion. Conclusion Because adenomas located on potential blind spots can be missed when only using forward‐view examinations, retroflexion in the right colon after repeated forward‐view examinations might improve colonoscopy detection rates.
