107. Clinical outcomes of carbon‐ion radiotherapy for locally advanced non‐small‐cell lung cancer

Abstract

Efficacy and safety of carbon-ion radiotherapy (CIRT) for locally advanced non-small-cell lung cancer (LA-NSCLC) remain unclear. We reported the clinical outcomes of CIRT for LA-NSCLC. Data of 141 eligible patients who received CIRT between 1995 and 2015 were retrospectively analyzed. Local control (LC), locoregional control (LRC), progression-free survival (PFS), and OS were calculated using Kaplan-Meier method. The median age was 75.0 years. Overall, 21, (14.9%), 57 (40.4%), 43 (30.5%), and 20 (14.2%) patients had T1, T2, T3, and T4 disease, respectively. Moreover, 51 (36.2%), 45 (31.9%), 40 (28.4%), and 5 (3.5%) patients had N0, N1, N2, and N3 disease, respectively. Furthermore, 34 (24.1%), 42 (29.8%), 45 (31.9%), and 20 (14.2%) patients had stages IIA, IIB, IIIA, and ΙΙΙB disease, respectively. Overall, 62 (44.0%), 60 (42.6%), 8 (5.7%), and 11 (7.8%) patients had adenocarcinoma, squamous cell carcinoma, large cell carcinoma, and others, respectively. The median dose was 72.0 Gy (relative biological effectiveness). No patient received concurrent chemotherapy. Median follow-up periods were 29.3 (1.6-207.7) and 40.0 (10.7-207.7) months for all patients and survivors, respectively. Two-year LC, PFS, and OS rates were 80.3%, 40.2%, and 58.7%, respectively. Overall, 1 (0.7%), 5 (3.5%), and 1 (0.7%) patient developed Grades 4 (mediastinal hemorrhage), 3 (radiation pneumonitis), and 3 (bronchial fistula) toxicities, respectively. Multivariate analysis showed adenocarcinoma and N2/3 classification as significant poor prognosticators of PFS. CIRT is an effective treatment with acceptable toxicity for LA-NSCLC, especially for elderly patients or patients with severe comorbidities who cannot be treated with surgery or chemoradiotherapy. Abstract

「グレーデッドアプローチに基づく合理的な安全確保検討グループ」は、原子力科学研究部門、安全・核セキュリティ統括部、原子力施設管理部署、安全研究・防災支援部門の関係者約10名で構成され、機構の施設管理や規制対応に関する効果的なグレーデッドアプローチ(安全上の重要度に基づく方法)の実現を目的としたグループである。本グループは、2019年の9月に活動を開始し、以降、2020年9月末までに、10回の会合を開催するとともに、メール等も利用し議論を行ってきた。会合では、グレーデッドアプローチの基本的考え方、各施設での新規制基準等への対応状況、新検査制度等についての議論を行なうとともに、各施設での独自の検討内容の共有等を行っている。本活動状況報告書は、本活動の内容を広く機構内外で共有することにより、原子力施設におけるグレーデッドアプローチに基づく合理的で効果的な安全管理の促進に役立つことを期待し取りまとめるものである。, A group named as "The group for investigation of reasonable safety assurance based on graded approach", which consists of about 10 staffs from Sector of Nuclear Science Research, Safety and Nuclear Security Administration Department, departments for management of nuclear facility, Sector of Nuclear Safety Research and Emergency Preparedness, aims to realize effective graded approach (GA) about management of facilities and regulatory compliance of JAEA. The group started its activities in September, 2019 and has had discussions through 10 meetings and email communications. In the meetings, basic ideas of GA, status of compliance with new regulatory standards at each facility, new inspection system, etc were discussed, while individual investigation at each facility were shared among the members. This report is compiled with expectation that it will help promote rational and effective safety management based on GA by sharing contents of the activity widely inside and outside JAEA.

Abstract

Poly (ADP-ribose) polymerase inhibitors (PARPi) are known to induce radiosensitization. However, the exact mechanisms of radiosensitization remain unclear. We previously reported that PARPi may have a unique radiosensitizing effect to enhance β-components of the linear-quadratic model. The aim of this study was to evaluate PARPi in combination with high-dose-per-fraction radiotherapy and to elucidate the underlying mechanisms of its radiosensitization. Radiosensitizing effects of PARPi PJ34, olaparib, and veliparib were measured using a colony-forming assay in the human cancer cell lines, HCT116, NCI-H460, and HT29. Six different radiation dose fractionation schedules were examined by tumor regrowth assay using three-dimensional multicellular spheroids of HCT116, NCI-H460, SW620, and HCT15. The mechanisms of radiosensitization were analyzed by measuring DNA double-strand breaks (DSB), DNA damage responses, chromosomal translocations, cellular senescence, and cell cycle analysis. Olaparib and PJ34 were found to show radiosensitization preferentially at higher radiation doses per fraction. Similar results were obtained using a mouse model bearing human tumor xenografts. A kinetic analysis of DNA damage responses and repairs showed that olaparib and PJ34 reduced the homologous recombination activity. However, a neutral comet assay showed that PJ34 treatment did not affect the physical rejoining of DNA-DSBs induced by ionizing radiation. Cell cycle analysis revealed that olaparib and PJ34 strikingly increased G1 tetraploid cells following irradiation, leading to premature senescence. The C-banding analysis of metaphase spreads showed that olaparib and PJ34 significantly increased ionizing radiation-induced dicentric chromosomes. The data suggests that PARPi olaparib and PJ34 altered the choice of DNA-DSB repair pathways rather than reducing the total amount of DNA-DSB repair, which resulted in increased repair errors. Increased quadratic misrepair was one of the mechanisms of PARP-mediated radiosensitization, preferentially at the higher dose range compared to the lower dose range. 134. Feasibility of carbon-ion radiotherapy for re-irradiation of locoregionally recurrent, metastatic, or secondary lung tumors

Abstract

Intrathoracic recurrence after carbon-ion radiotherapy for primary or metastatic lung tumors remains a major cause of cancer-related deaths. However, treatment options are limited. Herein, we report on the toxicity and efficacy of re-irradiation with carbon-ion radiotherapy for locoregionally recurrent, metastatic, or secondary lung tumors. Data of 95 patients with prior intrathoracic carbon-ion radiotherapy who were treated with re-irradiation with carbon-ion radiotherapy at our institution between 2006 and 2016 were retrospectively analyzed. Seventy-three patients (76.8%) had primary lung tumors and 22 patients (23.2%) had metastatic lung tumors. The median dose of initial carbon-ion radiotherapy was 52.8Gy (relative biological effectiveness) and the median dose of re-irradiation was 66.0Gy (relative biological effectiveness). None of the patients received concurrent chemotherapy. The median follow-up period after re-irradiation was 18months. In terms of grade ≥3 toxicities, one patient experienced each of the following: grade 5 bronchopleural fistula, grade 4 radiation pneumonitis, grade 3 chest pain, and grade 3 radiation pneumonitis. The 2-year local control and overall survival rates were 54.0% and 61.9%, respectively. In conclusion, re-irradiation with carbon-ion radiotherapy was associated with relatively low toxicity and moderate efficacy. Re-irradiation with carbon-ion radiotherapy might be an effective treatment option for patients with locoregionally recurrent, metastatic, or secondary lung tumors.

Abstract

This article describes the ongoing structure of radiation oncology in Japan in terms of equipment, personnel, patient load and geographic distribution to identify and overcome any existing limitations. From May 2016 to August 2018, the Japanese Society for Radiation Oncology conducted a questionnaire based on the Japanese national structure survey of radiation oncology in 2015. Data were analyzed based on the institutional stratification by the annual number of new patients treated with radiotherapy per institution. The estimated annual numbers of new and total (new plus repeat) patients treated with radiation were 225 000 and 271 000, respectively. Additionally, the estimated cancer incidence was 891 445 cases with approximately 25.2% of all newly diagnosed patients being treated with radiation. The types and numbers of treatment devices actually used included linear accelerator (linac; n  = 936), Gamma Knife (n  = 43), 60Co remote afterloading system (RALS; n  = 21), and 192Ir RALS (n  = 129). The linac system used dual-energy functions in 754 units, 3D conformal radiotherapy functions in 867, and intensity-modulated radiotherapy (IMRT) functions in 628. There were 899 Japan Radiological Society/Japanese Society for Radiation Oncology-certified radiation oncologists (RO), 1213.9 full-time equivalent (FTE) ROs, 2394.2 FTE radiotherapy technologists (RTT), 295.7 FTE medical physicists, 210.2 FTE radiotherapy quality managers, and 906.1 FTE nurses. The frequency of IMRT use significantly increased during this time.
