101. Tissue-type plasminogen activator predicts endocrine responsiveness of human pancreatic carcinoma cells

Abstract

Background. Estrogen receptor (ER) has been found in human pancreatic carcinoma, but the potential benefit of endocrine therapy never has been assessed adequately. The aim of this study was to determine whether the presence of ER can be used as an indicator of hormone responsiveness, and whether modulation of tissue-type plasminogen activator (t-PA) by ER can identify hormone-responsive pancreatic carcinomas. Methods. The authors investigated ER status and hormonal regulation of t-PA in nine human pancreatic carcinoma cell lines, AsPC-1, BxPC-3, Capan-1, Capan-2, Hs-700T, Hs-766T, MiaPaCa-2, PANC-1, and SUIT-2. Furthermore, to examine whether estrogen dependency of t-PA production in pancreatic carcinoma cells correlated with responsiveness to endocrine therapy, the in vivo effects of various endocrine agents on the growth of the nine pancreatic cell lines transplanted into nude mice were examined. Results. In a 17beta-estradiol ([E.sub.2])-binding assay, three of the nine pancreatic carcinoma cell lines (BxPC-3, Capan-2, and MiaPaCa-2) contained measurable levels of estradiol binding sites in vitro and in vivo using tumors transplanted into nude mice. Although t-PA was present in the culture medium in eight of the nine pancreatic carcinoma cell lines (not in Hs-700T), t-PA production was regulated by estrogen via an ER system in vitro only in the Capan-2 cell line. [E.sub.2] injection into tumor-bearing mice showed acceleration of tumor growth only in Capan-2 tumors. Administration of a competitive ER antagonist, toremifene, and a luteinizing hormone-releasing hormone analogue, leuprorelin acetate (LEU), to Capan-bearing 2-mice significantly reduced the rate of tumor growth, although there was no actual shrinkage of tumor mass. These agents failed to exert any antitumor effect on the other eight pancreatic cell lines. Although aromatase inhibitors, CGS 20267 and vorozole did not modify the in vivo growth of the nine pancreatic carcinoma cell lines significantly, the combined use of aromatase inhibitors with LEU exhibited a synergistic antitumor effect on Capan-2-bearing mice. Medroxyprogesterone acetate treatment significantly reduced the tumor volume of Capan-2 and also caused delayed growth in two other cell lines, AsPC-1 and MiaPaCa-2. Conclusions. The estrogen dependency of t-PA production may clarify the functional state of ER in human pancreatic carcinoma cells. Abstract

Perioperative changes in neutrophil attachment level and neutrophil elastase-releasing capacity were examined in patients who underwent surgery for either esophageal or gastric cancer. The neutrophil attachment level was significantly increased in both groups to approximately three times that of the preoperative value. The elastase-releasing capacity of nonstimulated neutrophils was not significantly changed, but it was significantly increased in neutrophils stimulated by N-formyl-L-methionyl-L-leucyl-L-phenylalanine (FMLP). Moreover, both neutrophil attachment and elastase-releasing capacity of FMLP-stimulated neutrophils were more enhanced in patients with postoperative complications than in patients without complications. Peak levels of serum interleukin 6, serun [alpha]1 proteinase inhibitor, and plasma neutrophil elastase were also significantly higher in patients with postoperative complications than in patients without. These results suggest that during the postoperative period, neutrophils may be primed and activated in response to inflammatory mediators such as cytokines, and that such an alteration of neutrophil functions may reflect the extent of inflammation and may, at times, be implicated in postoperative complications.

103. Increased expression of membrane-associated phospholipase A2 shows malignant potential of human breast cancer cells

Abstract

Background. Recently, the authors reported that membrane-associated phospholipase [A.sub.2] (M-[PLA.sub.2]) was one of the acute phase reactants and increased in serum of patients with various malignant tumors. Methods. M-[PLA.sub.2] concentrations in tissue specimens from 78 breast cancers, 16 benign breast tumors, and 10 normal breast tissues were determined by a specific radioimmunoassay recently developed. Immunohistochemical staining was performed on all specimens by the avidin-biotin-peroxidase method. Results. Tissue levels of M-[PLA.sub.2] concentration were significantly higher in breast cancer than in benign breast tumor or normal breast tissue (P < 0.01). Correlation analyses between the tissue concentration of M-[PLA.sub.2] and clinicopathologic factors showed that tissue M-[PLA.sub.2] levels were significantly higher in patients with skin or muscle invasion, vessel involvement, and distant metastasis than in those without. In addition, this enzyme concentration was significantly greater in scirrhous carcinoma than in papillotubular or solid-tubular carcinoma. No association was found between M-[PLA.sub.2] concentration and steroid hormone receptor status. Immunohistochemically, M-[PLA.sub.2] was preferentially stained in the invading zone of breast cancer tissues, especially in scirrhous carcinoma. Patients with breast cancer with low levels of M-[PLA.sub.2] showed significantly longer overall survival and disease-free survival compared with those with high levels of this enzyme at the cutoff point of 50 ng/100 mg protein. The combination of estrogen receptor status with M-[PLA.sub.] concentration could be a powerful prognostic factor in predicting such survival rates. Conclusions. M-[PLA.sub.2] is closely related to the malignant potential of breast cancers, and the M-[PLA.sub.2] contents in breast cancer tissues could be a new valuable prognostic factor, other than the hormone receptor, in delineating the status of human breast cancer. 104. A new approach to chemoembolization for unresectable hepatocellular carcinoma using aclarubicin microspheres in combination with cisplatin suspended in iodized oil

Abstract

Hepatocellular carcinoma, a common form of primary liver cancer, may be curable when the tumor is surgically removable. However, the fraction of cases that are surgically treatable is not high, ranging from 10 to 60 percent. Even when the tumor is surgically resectable, the chance for recurrence in the remaining liver tissue is still high. One of the possible treatment methods for unresectable liver cancer is transcatheter embolization. When this method was first developed, a gelatin sponge was simply inserted into the arterial branch bringing blood to the cancer, blocking it and temporarily starving the tumor of oxygen. This method has enjoyed several improvements, however, since it was first developed and is now often combined with chemotherapeutic attacks on the cancer. In the present study, tiny microspheres composed of aclarubicin, a newer chemotherapeutic drug, was administered in combination with cisplatin by chemoembolization. The drugs were suspended in iodized oil, which has been found to preferentially collect in the new blood vessels feeding a cancer as well as in the tissue spaces surrounding such new blood vessels. A total of 62 patients treated with the method could be evaluated for their responses to treatment. Thirty-one patients had partial responses to treatment. In 17 cases, only minor responses were observed, and in 13 patients there were no observable responses, but the progression of the tumor was temporarily halted. In one case, disease progression continued despite treatment. There were 1, 10, 26, and 29 patients with cancer Stages I through IV, respectively. The one-year survival rate was 81 percent, and the three-year survival rate for these patients was 51 percent. These results are superior to those that have been obtained either by aclarubicin microspheres alone or chemotherapeutic agents in iodize oil alone. Abstract

Pancreatic secretory trypsin inhibitor (PSTI) has been suggested to be an acute-phase reactant in humans and to be induced by inflammatory cytokines such as the interleukins IL-1 and IL-6. We report that PSTI is synthesized in hepatoma cells and that the gene expression is augmented by IL-6. The start points (tsp) for basal and augmented transcription are exactly the same as the tsp in normal pancreas. Analysis of the PSTI gene revealed that a 40-bp DNA fragment located between kb −3.84 and −3.80 carries the element responsible for both basal transcriptional activity and IL-6-induced gene expression. This 40-bp fragment contains TGTNNGNAAGT, the consensus sequence for the NF-IL6-binding site, which is also known as the IL-6-responsive element that is conserved among various acute-phase genes. The basal activity was augmented by another sequence that lies between kb −4.0 and −3.9.

A rare case of leiomyosarcoma of the ascending colon in a 26-day-old boy, presenting with tarry stools and an abdominal mass, is reported, and the literature pertaining to childhood colorectal leiomyosarcoma is reviewed., 生後26日目の男児, 下血, 腹部腫瘤を主訴に当科紹介受診. 注腸造影, CTスキャン等にて上行結腸腫瘍を疑い, 開腹術施行. 腫瘍は上行結腸より発生しているのが認められ, 右半結腸切除術を行った. 腫瘍の大きさ 4. 7×4. 5×2. 1 cm, 病理組織学的検査にて平滑筋肉腫と診断された. 大腸の平滑筋肉腫は小児には極めてまれであり, 今までに15例の報告をみるにすぎない. 今回, 文献的考察を加えて報告した.
