Abstract

Afforestation programs as a strategy to address the climate crisis are on the rise worldwide. Although concerns exist about the effects of afforesting treeless habitats on their biodiversity and ecosystem services, potential impacts on areas beyond afforestation boundaries have been largely overlooked. Cork oak (Quercus suber) woodlands in southern Spain are regarded as a successful example of sustainability. However, the afforestation of their neighbouring treeless habitats may compromise their productivity through indirect effects that are not fully understood. Using a multi-method approach, we studied the effects of the afforestation of Mediterranean heathland (herriza) areas with pine tree species on the productivity of neighbouring cork oak woodlands downslope over a 37-year period. The differences in Normalized Difference Vegetation Index (NDVI) and Basal Area Increment (BAI) between cork oak woodlands below open herriza and below afforested herriza became apparent approximately ten years after pine afforestation. Specifically, NDVI was significantly reduced in cork oak stands below afforested herriza areas for the remaining years, while a substantial decrease in BAI of cork oak trees was also associated with afforested herriza areas upslope. Moreover, the NDVI and BAI trends of cork oak stands below afforested herriza reached the minimum levels of the time series following an extreme drought event. Our results reveal that the ecological impacts of afforestation of naturally treeless habitats can expand beyond the plantation area. We propose that the restoration of treeless habitats, such as the herriza, in areas where pine plantations are no longer productive, may enhance their ecological services, particularly in the context of climate change. Therefore, afforestation programs worldwide should carefully consider the trade-offs between carbon mitigation and other ecosystem services at the landscape scale. Abstract

BACKGROUND Patients who are hospitalized for medical illness remain at risk for venous thromboembolism after discharge, but the role of extended thromboprophylaxis in the treatment of such patients is a subject of controversy. METHODS In this randomized, double-blind trial, medically ill patients who were at increased risk for venous thromboembolism on the basis of a modified International Medical Prevention Registry on Venous Thromboembolism (IMPROVE) score of 4 or higher (scores range from 0 to 10, with higher scores indicating a higher risk of venous thromboembolism) or a score of 2 or 3 plus a plasma d-dimer level of more than twice the upper limit of the normal range (defined according to local laboratory criteria) were assigned at hospital discharge to either once-daily rivaroxaban at a dose of 10 mg (with the dose adjusted for renal insufficiency) or placebo for 45 days. The primary efficacy outcome was a composite of symptomatic venous thromboembolism or death due to venous thromboembolism. The principal safety outcome was major bleeding. RESULTS Of the 12,024 patients who underwent randomization, 12,019 were included in the intention-to-treat analysis. The primary efficacy outcome occurred in 50 of 6007 patients (0.83%) who were given rivaroxaban and in 66 of 6012 patients (1.10%) who were given placebo (hazard ratio, 0.76; 95% confidence interval [CI], 0.52 to 1.09; P = 0.14). The prespecified secondary outcome of symptomatic nonfatal venous thromboembolism occurred in 0.18% of patients in the rivaroxaban group and 0.42% of patients in the placebo group (hazard ratio, 0.44; 95% CI, 0.22 to 0.89). Major bleeding occurred in 17 of 5982 patients (0.28%) in the rivaroxaban group and in 9 of 5980 patients (0.15%) in the placebo group (hazard ratio, 1.88; 95% CI, 0.84 to 4.23). CONCLUSIONS Rivaroxaban, given to medical patients for 45 days after hospital discharge, was not associated with a significantly lower risk of symptomatic venous thromboembolism and death

METHODS In this randomized, double-blind trial, medically ill patients who were at increased risk for venous thromboembolism on the basis of a modified International Medical Prevention Registry on Venous Thromboembolism (IMPROVE) score of 4 or higher (scores range from 0 to 10, with higher scores indicating a higher risk of venous thromboembolism) or a score of 2 or 3 plus a plasma n-dimer level of more than twice the upper limit of the normal range (defined according to local laboratory criteria) were assigned at hospital discharge to either once-daily rivaroxaban at a dose of 10 mg (with the dose adjusted for renal insufficiency) or placebo for 45 days. The primary efficacy outcome was a composite of symptomatic venous thromboembolism or death due to venous thromboembolism. The principal safety outcome was major bleeding. RESULTS Of the 12,024 patients who underwent randomization, 12,019 were included in the intention-to-treat analysis. The primary efficacy outcome occurred in 50 of 6007 patients (0.83%) who were given rivaroxaban and in 66 of 6012 patients (1.10%) who were given placebo (hazard ratio, 0.76; 95% confidence interval [CI], 0.52 to 1.09; P=0.14). The prespecified secondary outcome of symptomatic nonfatal venous thromboembolism occurred in 0.18% of patients in the rivaroxaban group and 0.42% of patients in the placebo group (hazard ratio, 0.44; 95% CI, 0.22 to 0.89). Major bleeding occurred in 17 of 5982 patients (0.28%) in the rivaroxaban group and in 9 of 5980 patients (0.15%) in the placebo group (hazard ratio, 1.88; 95% CI, 0.84 to 4.23). CONCLUSIONS Rivaroxaban, given to medical patients for 45 days after hospital discharge, was not associated with a significantly lower risk of symptomatic venous thromboembolism and death due to venous thromboembolism than placebo. 121. Morphometric Analysis of Bite Mark Patterns Caused by Domestic Dogs ( Canis lupus familiaris ) Using Dental Wax Registers

Abstract

SUMMARY: Dog bites are a known public health problem involving physical, mental and emotional traumas. From a forensic point of view, it has been stated that their morphological characters, and the intercanine and interincisive measurements, could allow a taxonomic and specific identification of the implicated animal. The aim of this study was to differentiate and identify the biological profile of a potential aggressor dog by analysing eight morphometric bite patterns belonging to three different dog breeds. The data obtained were analyzed following three categories: a) breeds; b) sexes among breeds; and c) sex within breed. Significant differences were detected among the variables (p≤ 0.05), but only the maximum maxillary intercanines width (MaxCW) allowed a breed differentiation. The other variables allowed a differentiation between two breeds or one breed over the others. The principal components analysis (PCA) allowed visualizing the degree of dispersion and relationship among the scores. It showed three well-defined and separated breed groups, and different degrees of dispersion within and among breeds. The most important variable for such a differentiation was MaxCW. When considering sex among breeds for males, it showed a statistically significant difference, but only the diastema located between the third left mandibular, incisive and the left mandibular canine (C-I-ManL) allowed breed differentiation. For females, only MaxCW allowed a differentiation among breeds. The multivariate analysis permitted with a 95 % confidence interval, a breed and sex differentiation. Besides, the PCA models allowed classifying, identifying, separating and graphically showing the relationship among the variables. This made it possible to differentiate between breeds and sexes. Due to the large range of dog breeds around the world, this multivariate analysis could also help determining the dog’s weight and size, narrowing down towards an approximate number of offending dogs, focussing on a certain kind of dog breed, and pinpointing any suspect dog. RESUMEN: Las mordeduras de perros son un conocido problema de salud pública que involucra traumas físicos, mentales y emocionales. En la faz forense, se ha establecido que características morfológicas, así como las medidas intercanina e interincisiva, permitirían una identificación taxonómica y especifica del animal involucrado. El objetivo fue diferenciar e identificar el perfil biológico de un potencial perro agresor analizando ocho patrones morfológicos de mordeduras pertenecientes a tres diferentes razas caninas. Los datos fueron analizados considerando tres categorías: a) razas; b) sexo entre razas; y c) sexo en cada raza. Se observaron diferencias entre las variables (p≤0,05), pero sólo la distancia máxima entre caninos maxilares (MaxCW), permitió una diferenciación entre razas. Las otras variables permitieron una diferenciación entre dos razas o de una raza sobre las otras. El análisis de componentes principales (PCA) permitió visualizar el grado de dispersión y la relación entre las puntuaciones (dentro y entre razas). Se observaron tres grupos (razas) bien definidos y separados con diferentes grados de dispersión dentro y entre razas. La variable más importante para tal diferenciación fue la MaxCW. Al considerar sexo (machos) entre razas, se encontró diferencia estadísticamente significativa, pero sólo la medida del diastema localizado entre el tercer incisivo y canino izquierdo mandibulares (C-I-ManL) permitió la diferenciación entre razas. Para hembras, sólo la MaxCW permitió una diferenciación entre razas. El análisis multivariante permitió, con un intervalo de confianza del 95 %, diferenciar raza y sexo. El modelo PCA permitió además clasificar, identificar, separar y mostrar gráficamente la relación entre las variables. Esto posibilitó diferenciar entre razas y sexos. Debido a la gran variedad de razas de perros en el mundo, este análisis multivariado permitiría estimar peso y tamaño del animal, indicando un número aproximado de perros atacantes, centrándose en ciertos tipos de raza, y localizar a cualquier cánido sospechoso.

128. An enigmatic carnivorous plant: ancient divergence of Drosophyllaceae but recent differentiation of Drosophyllum lusitanicum across the Strait of Gibraltar

Abstract

Drosophyllaceae is the only plant family endemic to the Mediterranean Floristic Region. It includes a single species, the carnivorous Drosophyllum lusitanicum, which is hypothesised to be a relict plant. Our aim here is to reconstruct the evolutionary history of Drosophyllum to evaluate whether the African-European disjunction across the Strait of Gibraltar is the result of vicariance or long-distance dispersal, and to assess the role of the Strait of Gibraltar area in the long-term persistence of this species. A time-calibrated phylogeny based on nuclear ITS sequences estimated a mid-Eocene to mid-Miocene age (19–44 Mya) for the common ancestor of Drosophyllaceae and its sister clade (Ancistrocladaceae-Dioncophyllaceae). In contrast, differentiation of ITS lineages of Drosophyllum occurred in the Pleistocene, after the opening of the Strait of Gibraltar (ca. 5 Mya). A haplotype network built using two plastid DNA regions revealed six haplotypes in Drosophyllum, five of which occur in the Strait of Gibraltar area. Four of the six haplotypes were present in both European and African populations. Therefore, although Drosophyllaceae diverged from the closest relatives in ancient times, the current distribution of Drosophyllum is relatively recent (Pleistocene). Given that the Strait of Gibraltar was already open in the Pliocene, the disjunct distribution of this species between Europe and Africa appears to be the result of multiple events of long-distance dispersal. Species distribution modelling (SDM) was used to reconstruct the potential distribution of Drosophyllum throughout the late Quaternary (last interglacial, Last Glacial Maximum, mid-Holocene and present), which revealed long-term persistence around the Strait of Gibraltar and in the south-western Iberian Peninsula. The Strait of Gibraltar area and the south-western Iberian Peninsula may have shared similar climatic conditions, and thus acted as refugia for Drosophyllum throughout the late Quaternary.

Abstract

Objective To find out if cardiovascular alterations are present in pediatric patients with X-linked hypophosphatemia (XLH). Study design Multicentre prospective clinical study on pediatric patients included in the RenalTube database (www.renaltube.com) with genetically confirmed diagnosis of XLH by mutations in the PHEX gene. The study's protocol consisted of biochemical work-up, 24-h ambulatory blood pressure monitoring (ABPM), carotid ultrasonography, and echocardiogram. All patients were on chronic treatment with phosphate supplements and 1-hydroxy vitamin D metabolites. Results Twenty-four patients (17 females, from 1 to 17 years of age) were studied. Serum concentrations (X +/- SD) of phosphate and intact parathyroid hormone were 2.66 +/- 0.60 mg/dl and 58.3 +/- 26.8 pg/ml, respectively. Serum fibroblast growth factor 23 (FGF23) concentration was 278.18 +/- 294.45 pg/ml (normal < 60 pg/ml). Abnormally high carotid intima media thickness was found in one patient, who was obese and hypertensive as revealed by ABPM, which disclosed arterial hypertension in two other patients. Z scores for echocardiographic interventricular septum end diastole and left ventricular posterior wall end diastole were +0.77 +/- 0.77 and +0.94 +/- 0.86, respectively. Left ventricular mass index (LVMI) was 44.93 +/- 19.18 g/m(2.7), and four patients, in addition to the obese one, had values greater than 51 g/m(2.7), indicative of left ventricular hypertrophy. There was no correlation between these echocardiographic parameters and serum FGF23 concentrations. Conclusions XLH pediatric patients receiving conventional treatment have echocardiographic measurements of ventricular mass within normal reference values, but above the mean, and 18% have LVMI suggestive of left ventricular hypertrophy without correlation with serum FGF23 concentrations. This might indicate an increased risk of cardiovascular involvement in XLH.

Abstract

BackgroundData regarding high-sensitivity troponin concentrations in patients presenting to the emergency department with symptoms suggestive of myocardial infarction may be useful in determining the probability of myocardial infarction and subsequent 30-day outcomes. MethodsIn 15 international cohorts of patients presenting to the emergency department with symptoms suggestive of myocardial infarction, we determined the concentrations of high-sensitivity troponin I or high-sensitivity troponin T at presentation and after early or late serial sampling. The diagnostic and prognostic performance of multiple high-sensitivity troponin cutoff combinations was assessed with the use of a derivation-validation design. A risk-assessment tool that was based on these data was developed to estimate the risk of index myocardial infarction and of subsequent myocardial infarction or death at 30 days. ResultsAmong 22,651 patients (9604 in the derivation data set and 13,047 in the validation data set), the prevalence of myocardial infarction was 15.3%. Lower high-sensitivity troponin concentrations at presentation and smaller absolute changes during serial sampling were associated with a lower likelihood of myocardial infarction and a lower short-term risk of cardiovascular events. For example, high-sensitivity troponin I concentrations of less than 6 ng per liter and an absolute change of less than 4 ng per liter after 45 to 120 minutes (early serial sampling) resulted in a negative predictive value of 99.5% for myocardial infarction, with an associated 30-day risk of subsequent myocardial infarction or death of 0.2%; a total of 56.5% of the patients would be classified as being at low risk. Methods: In this risk-evaluation and risk-modelling study, we used Multinational Cardiovascular Risk Consortium data from 19 countries across Europe, Australia, and North America. Individuals without prevalent cardiovascular disease at baseline and with robust available data on cardiovascular disease outcomes were included. The primary composite endpoint of atherosclerotic cardiovascular disease was defined as the occurrence of the coronary heart disease event or ischaemic stroke. Sex-specific multivariable analyses were computed using non-HDL cholesterol categories according to the European guideline thresholds, adjusted for age, sex, cohort, and classical modifiable cardiovascular risk factors. In a derivation and validation design, we created a tool to estimate the probabilities of a cardiovascular disease event by the age of 75 years, dependent on age, sex, and risk factors, and the associated modelled risk reduction, assuming a 50% reduction of non-HDL cholesterol. Findings: Of the 524 444 individuals in the 44 cohorts in the Consortium database, we identified 398 846 individuals belonging to 38 cohorts (184 055 [48·7%] women; median age 51·0 years [IQR 40·7–59·7]). 199 415 individuals were included in the derivation cohort (91 786 [48·4%] women) and 199 431 (92 269 [49·1%] women) in the validation cohort. This work demonstrates an scheme in which a weak electric quadrupole (E2) transition with a controlled frequency detuning in a ladder configuration is used as an extremely non-perturbing probe for precisely identifying subtle details of the effects driven by AC fields coupling atomic states and the preparation of quantum states. For this purpose, we provide a precise experimental and theoretical study of the Autler Townes (AT) effect derived from a strong electric dipole transition stimulated by a resonant laser beam and probed by the E2 interaction. The experiment was carried out for a 87Rb atomic gas at room temperature in a velocity-selective scheme. The AT effect was monitored via the splitting of the fluorescence spectra associated with the spontaneous decay to the ground state. The theoretical description incorporates the modification of standard few-level schemes introduced by forbidden electric-dipole transitions selection rules. We develop an analytic ladder three-level scheme to approximate the cyclic 5S1/2F = 2 → 5P3/2F = 3 → 6P3/2F = 1, 2, 3 → 5S1/2F = 2 path. Other levels that could have effects on the fluorescence are included via a fourth level with effective parameters. Doppler effects and finite bandwidths of the laser beams are included in the theoretical model to closely reproduce the experimental results. [ABSTRACT FROM AUTHOR]
