109. Efficacy of Endoscopic and Surgical Treatments for Gastroesophageal Reflux Disease: A Systematic Review and Network Meta-Analysis.

Abstract

Although various endoscopic and surgical procedures are available for the treatment of gastroesophageal reflux disease (GERD), the comparative efficacy of these treatments has not been fully elucidated. This study aimed to comprehensively evaluate the efficacy of various endoscopic and surgical treatments for GERD. All relevant randomized controlled trials published through August 2021 that compared the efficacy of endoscopic and surgical GERD treatments, including radiofrequency energy delivery, endoscopic plication, reinforcement of the lower esophageal sphincter (LES), and surgical fundoplication, were searched. A network meta-analysis was performed to analyze treatment outcomes, including the requirement of proton pump inhibitor (PPI) continuation and GERD-health-related quality of life questionnaire score (GERD-HRQL). As such, 25 studies with 2854 patients were included in the analysis. Endoscopic plication, reinforcement of the LES, and surgical fundoplication were effective in reducing the requirement of PPI continuation compared to PPI therapy (pooled risk ratio (RR) (95% confidence interval [CI]): endoscopic plication, 0.34 (0.21–0.56); reinforcement of LES, 0.32 (0.16–0.63), and surgical fundoplication, 0.16 (0.06–0.42)). Radiofrequency energy delivery tended to reduce the requirement of PPI continuation compared to PPI therapy (RR (95% CI): 0.55 (0.25–1.18)). In terms of GERD-HRQL, all endoscopic and surgical treatments were superior to PPI therapy. In conclusion, all endoscopic or surgical treatments, except radiofrequency energy delivery, were effective for discontinuation of PPI medication, especially surgical fundoplication. Quality of life, measured by GERD-HRQL, also improved in patients who underwent endoscopic or surgical treatment compared to those who received PPI therapy. [ABSTRACT FROM AUTHOR]

Abstract

연소기기에서 연소반응과정이 일어날 때 연소로 내 고온의 온도 분위기에서 공기 중의 산소와 질소가 반응하여 질소산화물이 발생하게 된다. 발생한 질소산화물을 저감하기 위하여 화력발전소나 폐기물 소각로는 촉매를 이용한 탈질설비를 설치하고 있는데 이에 따른 설치와 유지비용이 많이 소요된다. 연소기기에서 질소산화물을 저감하기 위한 여러 가지 방법 중에 배기가스 재순환 방법이 널리 쓰이고 있다. 본 연구에서는 배기가스 재순환 배관에 코안다 노즐을 사용하여 배기가스를 재순환하는 재순환 버너에 대하여 전산유체해석을 통해 연구를 수행하였으며 냉간 유동에서 배기가스 재순환 유동 특성을 살펴보았고 코안다 노즐의 공기 측 간격 변화와 공기 유량 변화에 따른 배기가스 재순환 유량 특성을 살펴보았다. 본 연구의 버너 형상은 배기가스 재순환 흡입구와 출구는 원통버너의 중심을 향하지 않고 접선 방향으로 설치되어 있어서 버너 내부에서 선회 유동이 형성 되었으며 이에 따라 원통 내부의 중심부분에 역류가 일어나는 특성을 관찰하였다. 또한, 코안다 노즐의 공기 측 간격은 0.5mm일 때는 배기가스 재순환 유량이 공기량 보다 약 2.5배 이었고 1.0mm일 때는 약 1.5배로 나타났으며 같은 공기 측 간격에서 공기량을 증가하면 배기가스 재순환 유량은 약간 증가하는 것을 알 수 있었다.

Abstract

BACKGROUND : Bleeding after endoscopic submucosal dissection (ESD) is a severe adverse event. Several methods to prevent post-ESD bleeding (PEB) have been introduced; however, they have not been widely used because of technical difficulties. We aimed to investigate whether polysaccharide hemostatic powder (PHP), which is very easy to apply, can prevent early post-ESD bleeding, especially in patients with a high risk of post-ESD bleeding. METHODS : This was a prospective, multicenter, randomized, open-label, controlled trial. Patients with a high risk for post-ESD bleeding were enrolled. Patients with gastric neoplasms in whom the resected specimen size was expected to be > 40 mm and those who were regularly taking antithrombotic agents were defined as high risk patients. Patients were randomly assigned to the PHP or control groups. RESULTS:  Between May 2017 and September 2018, 143 patients were enrolled (PHP group, 73; control group, 70). The total post-ESD bleeding rate was 6.3 % (PHP group, 5.5 % vs. control group, 7.1 %; P = 0.74). There was no bleeding within 7 days after ESD in the PHP group. Continued antithrombotic use was an independent risk factor for post-ESD bleeding. In subgroup analysis excluding the patients who continued to take antithrombotic agents (n = 129) during ESD, the rate of post-ESD bleeding tended to be lower in the PHP group than in the control group (0 % vs. 6.3 %; P = 0.06). CONCLUSION : PHP did not demonstrate a significant effect on the prevention of post-ESD bleeding in this study. Further larger scale, randomized controlled trials are needed to confirm this.
