114. Acute Mechanical Complications of Coronary Arterial Disease: Surgical Correction

Abstract

Mechanical complications that cause heart failure after acute myocardial infarction may be corrected early by surgery. The development of effective cardiac assistance by intra-aortic balloon pumping (IABP) has proved invaluable in resuscitating patients in over-whelming cardiac failure, maintaining them during diagnostic evaluation, and affording long-term (up to two weeks) postoperative support. Four of five patients with acutely expanding ventricular aneurysms, two of six patients with acute mitral regurgitation, and one of two patients with acute ventricular septal defects are living and well. Ten of these 13 patients required IABP. Seven of the group, which was totally refractory to medical therapy, are long-term survivors as a result of this combined supportive, diagnostic, and surgical approach. Usually the heart can be repaired successfully. Patients are lost primarily from the ravages, prior to surgery, of over-whelming cardiac failure on other organs.

117. Thoracic Duct Injury: A Complication of Jugular Vein Catheterization

Abstract

Iatrogenic injury to the thoracic duct followed percutaneous cannulation of the left internal jugular vein. The case was managed successfully by supraclavicular ligation of the thoracic duct. We feel cannulation of the right internal jugular vein is preferable to cannulation of the left internal jugular vein.

Abstract

This article presents information on the book "The Social Psychology of George Herbert Mead," edited by Anselm Strauss. It is informed that in the introduction of the above mentioned book, the editor outlines the character of the biological, psychological, and philosophical thought which influenced Mead and to which he, in turn, contributed by extension, correction, and application. It is informed that Mead took issue with all who saw human behavior either in mechanistic, individualistic terms or in terms of irrational or non-rational determinants. Mead's central thought focuses upon the development and function of socially reflexive behavior and upon the problem of intelligent control of the physical and social environment. The bulk of the introduction briefly interprets the pivotal ideas of Mead's system in the order of their appearance in the selections. It is further opined that, grounded in pragmatism, Mead saw evolution as emergent and telic and as a process flowing through both species and social organizations.
