The purpose of this study was to demonstrate the accuracy of a novel, surgically useful guide that could place a drill or pin within the femoral head. The radiolucent guide for attachment to the lateral femoral cortex was tested for reproducibility on cadaveric specimens simulating a surgical procedure. The guide allows selection of the three-degree freedom angle of insertion of a drill or pin via two circular channels allowing anterior–posterior or superior–inferior adjustment. The guide can accurately place a pin tip within 2.5–3.8 mm of a desired location. We conclude this guide could be used to accurately place a fibular graft in an osteonecrotic lesion, or place pins or nails during many different surgical procedures such as fractures or slipped capital femoral epiphysis. While favorable tribological properties and allowance for larger femoral head sizes have made metal-on-metal (MoM) bearings an increasingly popular choice for total hip arthroplasty, concerns have mounted regarding adverse reactions to metal wear debris and ions. MoM cups differ from conventional polyethylene cups in terms of edge profile design and reductions from full hemisphericity, suggesting differences in loading at or near the cup edge, especially during subluxation events. Finite element analysis was used to investigate the effects of cup orientation and lip edge curvature on damage propensity for edge or near-edge loading during subluxation. Increased cup lip radius (resulting in reduced articular arc) had a detrimental effect upon subluxation-free hip range of motion and upon dislocation resistance. Contact stresses near the cup edge demonstrated complex relationships between edge radius and cup orientation, with peak stresses being influenced by both variables. The tendency for scraping wear at the egress site demonstrated similarly complex dependencies. Investigation of femoral head osteonecrosis would benefit from an animal model whose natural history includes progression to bony collapse of a segmental necrotic lesion. The bipedal emu holds attraction for systematic organ-level study of collapse mechanopathology. One established method of experimentally inducing segmental lesions is liquid nitrogen cryoinsult. Four cryoinsult parameters-hold temperature, freeze duration, freeze/thaw repetition, and thaw duration-were investigated to determine their individual and combined effects on resulting necrotic lesion morphology. 3D distributions of histologically apparent osteocyte necrosis from 24 emus receiving varying cryoinsults were used to develop univariate and multivariate linear regression models relating resulting necrotic lesion morphology to particular cryoinsult input parameters. These models were then applied to predict lesion size in four additional emus receiving differing input cryoinsults. The best multivariate regression model predicted lesion volumes that were accurate to better than 8% of overall emu femoral head volume. The hold temperature during cryoinsult was by far the most influential cryoinsult input parameter. The utility of this information is to enhance the consistency and predictability of cryoinsult-induced segmental lesion size for the purposes of systematic laboratory studies at the whole-organ level. © 2011 Orthopaedic Research Society Published by Wiley Periodicals, Inc. Articular cartilage degeneration in osteoarthritis has been linked to abnormal mechanical stresses that are known to cause chondrocyte apoptosis and metabolic derangement in in vitro models. Evidence implicating oxidative damage as the immediate cause of these harmful effects suggests that the antioxidant defenses of chondrocytes might influence their tolerance for mechanical injury. Based on evidence that antioxidant defenses in many cell types are stimulated by moderate oxidant exposure, we hypothesized that oxidant preconditioning would reduce acute chondrocyte death and proteoglycan depletion in cartilage explants after exposure to abnormal mechanical stresses. Porcine cartilage explants were treated every 48 h with tert-butyl hydrogen peroxide (tBHP) at nonlethal concentrations (25, 100, 250, and 500 µM) for a varying number of times (one, two, or four) prior to a bout of unconfined axial compression (5 MPa, 1 Hz, 1800 cycles). When compared with untreated controls, tBHP had significant positive effects on post-compression viability, lactate production, and proteoglycan losses. Overall, the most effective regime was 100 µM tBHP applied four times. RNA analysis revealed significant effects of 100 µM tBHP on gene expression. Catalase, hypoxia-inducible factor-1alpha (HIF-1α), and glyceraldehyde 6-phosphate dehydrogenase (GAPDH) were significantly increased relative to untreated controls in explants treated four times with 100 µM tBHP, a regime that also resulted in a significant decrease in matrix metalloproteinase-3 (MMP-3) expression. These findings demonstrate that repeated exposure of cartilage to sublethal concentrations of peroxide can moderate the acute effects of mechanical stress, a conclusion supported by evidence of peroxide-induced changes in gene expression that could render chondrocytes more resistant to oxidative damage. © 2010 Orthopaedic Research Society. Abstract: The emu is a large, (bipedal) flightless bird that potentially can be used to study various orthopaedic disorders in which load protection of the experimental limb is a limitation of quadrupedal models. An anatomy-based analysis of normal emu walking gait was undertaken to determine hip contact forces for comparison with human data. Kinematic and kinetic data captured for two laboratory-habituated emus were used to drive the model. Muscle attachment data were obtained by dissection, and bony geometries were obtained by CT scan. Inverse dynamics calculations at all major lower-limb joints were used in conjunction with optimization of muscle forces to determine hip contact forces. Like human walking gait, emu ground reaction forces showed a bimodal distribution over the course of the stance phase. Two-bird averaged maximum hip contact force was approximately 5.5 times body weight, directed nominally axially along the femur. This value is only modestly larger than optimization-based hip contact forces reported in literature for humans. The interspecies similarity in hip contact forces makes the emu a biomechanically attractive animal in which to model loading-dependent human orthopaedic hip disorders. [Copyright &y& Elsevier]

Abstract: A computational model was developed to identify the sites of third body particle embedment in a total hip acetabular component surface that are most problematic in terms of roughening the overpassing regions of the femoral head counterface, leading in turn to most severely accelerated polyethylene wear. The analytical approach used was to calculate loci of acetabular sites that, during the gait cycle, overpass previously documented regions of kinetically most critical femoral head roughening. Instantaneous local contact stress and sliding distance were postulated as factors contributing to the severity of the femoral head scratching/roughening which would be expected, due to otherwise-similar particles embedded along each such acetabular overpass locus. The computational results showed that the location of debris embedment was a potent determinant of the amount of polyethylene wear acceleration expected. The data also showed that the supero-lateral aspect of the acetabular cup is consistently and by far the most problematic area for third body particle embedment. [Copyright &y& Elsevier]

Abstract: Constrained acetabular liners are utilized to deal with the infrequent but devastating problem of recurrent dislocation. While an encouraging treatment of last resort, the clinical performance of contemporary constrained liners has been somewhat mixed. There are multiple factors contributing to this variability, one of which is the limited understanding of the intrinsic mechanical characteristics of these specialty devices. To address this issue, a three-dimensional, materially nonlinear, multi-surface contact finite element model of a representative constrained liner was created. The model was physically validated, and then used for parametric testing to explore the effects of individual design features. The model was exercised for both intra-operative assembly and lever-out dislocation. It was found that the coefficient of friction between the femoral head and the liner substantially affected both the force required to seat the femoral head into the liner during assembly, and the peak moment resisting dislocation (226% increase in assembly force for friction coefficients of 0.2 versus 0.0; 49% reduction in dislocation moment for friction coefficients of 0.013 versus 0.135). As expected, the cup opening radius also had a dominant effect on both maneuvers: decreasing the opening radius from 13.9 to 13.6mm increased assembly force by 506N and increased the dislocation moment by over 3.5N-m, whereas the influence of other design parameters was much more modest.
